Transtherapy for moderate to severe acne scar: a study of 2 cases

  • Objectives: The purpose of this study is to present effect of transtherapy which is treatment of Korean Medicine to acne scar patients. Methods: We used transtherapy to moderate and severe acne scar patients and evaluated by a Photo evaluation with 10-point score and Qualitative Global Acne Scarring Grading System. Results: After transtherapy treatment, mean of Qualitative Global Acne Scarring Grading System of patients changed from $3.5{\pm}0.71$ to $1.5{\pm}0.71$. Acne scar showed an improvement at least 8 up to 9 and mean of their acne scar improvement scores was $8.33{\pm}0.52$ on photo evaluation with 10-point score. Conclusions: This paper shows that transtherapy in the treatment of moderate to severe acne scars is beneficial for promoting skin regeneration.

Promotion Effects of Ultra-High Molecular Weight Poly-γ-Glutamic Acid on Wound Healing

  • We examined the in vivo efficacy of ultra-high molecular weight poly-γ-glutamic acid (UHMW γ-PGA) for wound healing. The wound area was measured by a ruler and documented by digital photography before the animals were sacrificed at days 8 and 16 post wounding. The areas of wounds treated with UHMW γ-PGA were significantly decreased on days 8 and 16, as compared with those receiving a control treatment, and more than 70% of the UHMW γ-PGAtreated area was repaired by day 8. Hematoxylin and eosin staining confirmed that the epidermis had regenerated in the UHMW γ-PGA-treated wounds. At 16 days post wounding, collagen pigmentation and cross-linking were increased as compared with the control groups, and greater regeneration of blood vessels had occurred in UHMW γ-PGA-treated groups. Increased levels of transforming growth factor-beta and β-catenin were also observed in skin samples collected from UHMW γ-PGA-treated animals on days 8 and 16 post incision. Taken together, these findings suggest that UHMW γ-PGA promotes wound healing in vivo.

  • The effect of silver sulfadiazine (SSD) on the proliferation of human dermal fibroblast (HDF) was studied to determine the impact of the drug on the wound healing process and dermal mechanical strength. Human dermal fibroblasts were cultured to 80% confluency using DMEM with 10% FBS and viability of the cell was estimated using neutral red assay. In addition, the $2^{nd}$ degree burn wound was prepared on the anterior part of rabbit ear skin and dressings containing SSD were applied for 96 h. Presence of inflammatory cells and degree of re-epithelialization were investigated in the wound. After 15 day of the induction of burn wounds, the treated area was excised and dermal mechanical strength was quantitatively measured with a constant speed tensiometer. SSD was found to be highly cyto-toxic in cultured HDF cells. The topical application of SSD (2%) could control the infection as evidenced by the lack of accumulation of inflammatory cells in histological evaluation. Therefore, these observations suggested that the impairment of dermal regeneration and decreased mechanical strength of dermal tissue was resulted from the cyto-toxic effect of SSD on dermal cells. Since the decreased mechanical strength may lead to reduction in resilience, toughness and maximum extension of the tissue, the identification of optimum dose for SSD that limits infection while minimizes the cyto-toxic effect may be clinically relevant.

Effect of Origanum vulgare Extracts on Hair Regeneration (오레가노 추출물의 모발성장 촉진효과)

  • This study was carried out to investigate the effect of Origanum vulgare extracts on cell proliferation of human hair dermal papilla cell (HHDPC) using sulforhodamine B (SRB) assay, antioxidant activity by 1,1-diphenyl-2-picryl hydrazyl (DPPH) method, expression of insulin-like growth factor-1 (IGF-1) by analyzing reverse transcriptase polymerase chain reaction (RT-PCR) and hair growth in a shaving animal model of C57BL/6 mice topically applying with an amount of 0.1 mL once a day for 3 weeks. The mice were divided into 4 groups including normal group (saline, N), negative control group (dimethyl sulfoxide, NC), positive control group (5 mg/mL minoxidil, PC), and experimental group (Origanum vulgare extracts, OV). Treatment of OV didn't show cytotoxicity in HHDPC up to 10 ${\mu}g/mL$ and exhibited antioxidant activity with $IC_{50}$ of 31.0 ${\mu}g/mL$. IGF-1 expression in the skin was significantly (p<0.05) increased in the PC and OV compared to the N or NC. PC and OV also showed a prominently promoted hair regrowth compared to the N or NC in hair growth observation. The hair regrowth of OV was significantly higher than that of PC (p<0.05). Therefore, these results indicate that O. vulgare extracts effectively stimulated hair growth in an animal model.

Treatment of Refractory Melasma with Microwave-generated, Atmospheric-pressure, Non-thermal Nitrogen Plasma

  • Periorbital melasma is often refractory to treatment and highly associated with rebound hyperpigmentation or mottled hypopigmentation after laser treatment in Asian patients. In this report, we describe 2 patients with cluster-1 periorbital melasma and 1 patient with cluster-2 periorbital melasma who experienced remarkable clinical improvements after microwave-generated, atmospheric-pressure, non-thermal nitrogen plasma treatments. All patients exhibited limited clinical responses after combination treatments with topical bleaching agents, systemic oral tranexamic acid, and low-fluenced Q-switched neodymium (Nd):yttrium-aluminum-garnet (YAG) lasers. Low-energy nitrogen plasma treatment at 0.75 J elicited remarkable clinical improvement in the periorbital melasma lesions without post-laser therapy rebound hyperpigmentation and mottled hypopigmentation. We deemed that a single pass of nitrogen plasma treatment at 0.75 J induces mild microscopic thermal tissue coagulation and modification within the epidermis while preserving the integrity of the basement membrane in patients with periorbital melasma. Accordingly, nitrogen plasma-induced dermal tissue regeneration could play a role in the treatment of melasma lesions.

Wound Healing Effect of Low Molecular PDRN on Experimental Surgical Excision Rat Model (저분자화된 Polydeoxynucleotide (PDRN)의 흰쥐에 대한 외과적 창상 치유 효과)

  • This study was performed to investigate the wound healing effect of skin regeneration cosmetics utilizing low molecular weight Polydeoxynucleotide (PDRN). High purity PDRN was prepared from salmon testes poly-deoxy-ribonucleotide through protein and toxin removal process and molecular weight reduction. In order to evaluate the wound healing effect of PDRN in SD rats, 4 sites of dorsal skin of each animal were excised by using biopsy punch and $500{\mu}L$ of test solution was topically applied once daily for 4 weeks. The tissue changes were observed for every week during the application periods. After applying the PDRN to the wound, the skin was cut flower and contraction of the wounds more quickly, and the coating of PDRN in the wound area was reduced significantly as compared to the positive control group $Fucidin^{(R)}$ applied. The microscopic observation of stained tissue showed that a positive control was most rapid in re-epithelialization ability followed by the PH group, PDRN group, HA group. In addition, transforming growth factor ($TGF-{\beta}$) and vascular endothelial growth factor (VEGF), such as in the growth factor was similar to the results of staining of tissue lesions. In conclusion, it is determined that the low molecular weight PDRN has the therapeutic effect to the wound, and could be used as a functional material of cosmetics and medical industries.

Effects of Daecheonglyong-tang on Atopic Dermatitis Induced by DNCB in Mice (대청룡탕(大靑龍湯)이 DNCB로 유발된 생쥐의 아토피피부염에 미치는 영향)

  • Objectives : This study was designed to examine the effects of Daecheonglyong-tang(DCL) on atopic dermatitis induced by DNCB in mice Methods : The Nc/Nga mice were divided into 5 groups, and four groups excluding the normal group were applied by 2,4-dinitrochlorobenzene(DNCB), to cause AD and were orally administered with distilled water(negative control), dexamethasone(positive control), and DCL 200 or 400mg/kg once a day for 4 weeks respectively. The visual changes on skin, changes in skin tissue thickness and eosinophil infiltration were observed. IgE, Histamine, Cytokines, immune cells and the amount of gene expression of filaggrin, VEGF, $TGF-{\beta}1$, EGF were measured. Results : Dermatitis score showed a gross improvement on all DCL groups, similar to or better than positive control. All DCL groups showed no significant change in the basophils, while neutrophils and eosinophils decreased. In only DCL 400 mg/kg groups, white blood cells and mononuclear cells were decreased and lymphocytes were increased. In particular, neutrophils had similar or better effects than the positive control. In all DCL groups, IgE, Histamine, $IL-1{\beta}$, IL-4, IL-5, IL-6 and $TNF-{\alpha}$ were decreased and IL-2 was increased. In only DCL 400 mg/kg groups, IL-10 decreased and $IFN-{\gamma}$ increased. In particular, $IL-1{\beta}$ and $IFN-{\gamma}$ showed a similar rate of increase and decrease comparing positive control in DCL 400 mg/kg. $TGF-{\beta}$1 was increased in all DCL groups, filaggrin and VEGF were increased in only DCL 400 mg/kg groups. EGF did not make any changes. Epidermis, dermis thickness and eosinophil infiltration were also decreased in all DCL groups. Conclusions : By increasing Th1 cytokine and decreasing Th2 cytokine, DCL extracts appear to be effective in controlling immune response imbalances, anti-inflammatory and skin regeneration and are likely to be available as a treatment for AD.

Effects of Anti-inflammation and Skin Barrier by Genistein Cyclodextrin Complex (제니스테인 Cyclodextrin 포접체의 항염 및 피부장벽에 미치는 영향)

  • Genistein is one of the representative isoflavone compounds isolated from soybeans and has been studied very well for its anti-aging and anti-inflammatory activity through previous studies. However, although genistein exhibits high solubility in organic solvents, it shows low bioavaility due to the low water solubility. In this study, we compared directly the functional difference between genistein and genistein cyclodextrin complex which has the improved water solubility and stability by cell based assay. Cell cytotoxicity experiment were carried out on RAW264.7 with CCK-8 assay and cytotoxicity was appeared from $10{\mu}g/mL$, thereby maximum concentration was set to $10{\mu}g/mL$ in all condition. We discovered that genistein CD complex suppressed NO production and iNOS expression as concentration dependent manner in the condition of LPS rather than genistein. Also, we could understand that genistein CD complex was able to down-regulate mRNA expression of anti-inflammatory cytokines such as $IL1-{\alpha}$, $IL1-{\beta}$, IL-6, and $TNF-{\alpha}$ as concentration dependent manner in the presence of LPS. In addition, we verified that genistein CD complex increased TEER of HaCaT human keratinocyte cells as concentration dependent pattern and stimulated cell division and migration rather than genistein in cell migration assay. Thus, it is expected that it can be used as an effective cosmetic raw material for improving atopic dermatitis or skin barrier if clinical studies on skin regeneration and skin barrier of the genistein CD complex are carried out.

Bio-Conjugated Polycaprolactone Membranes: A Novel Wound Dressing

  • Background The combination of polycaprolactone and hyaluronic acid creates an ideal environment for wound healing. Hyaluronic acid maintains a moist wound environment and accelerates the in-growth of granulation tissue. Polycaprolactone has excellent mechanical strength, limits inflammation and is biocompatible. This study evaluates the safety and efficacy of bio-conjugated polycaprolactone membranes (BPM) as a wound dressing. Methods 16 New Zealand white rabbits were sedated and local anaesthesia was administered. Two $3.0{\times}3.0cm$ full-thickness wounds were created on the dorsum of each rabbit, between the lowest rib and the pelvic bone. The wounds were dressed with either BPM (n=12) or Mepitel (n=12) (control), a polyamide-silicon wound dressing. These were evaluated macroscopically on the 7th, 14th, 21st, and 28th postoperative days for granulation, re-epithelialization, infection, and wound size, and histologically for epidermal and dermal regeneration. Results Both groups showed a comparable extent of granulation and re-epithelialization. No signs of infection were observed. There was no significant difference (P>0.05) in wound size between the two groups. BPM (n=6): $8.33cm^2$, $4.90cm^2$, $3.12cm^2$, $1.84cm^2$; Mepitel (n=6): $10.29cm^2$, $5.53cm^2$, $3.63cm^2$, $2.02cm^2$; at the 7th, 14th, 21st, and 28th postoperative days. The extents of epidermal and dermal regeneration were comparable between the two groups. Conclusions BPM is comparable to Mepitel as a safe and efficacious wound dressing.

Study on the Anti-Aging Activity of Chrysanthemum lucidum Exosomes (울릉국화 엑소좀의 항노화 활성 효과 연구)

  • Chrysanthemum lucidum (C. lucidum), a perennial herb in the Asteraceae family, is an endemic species found only on Ulleung island in Gyeongsangbuk-do, South Korea. Previous studies have reported that the extract of C. lucidum exhibits excellent antioxidant activity due to its high polyphenol and flavonoid content. However, the anti-aging effects of C. lucidum extract, such as wrinkle improvement and cell regeneration, are not well known, and there has been no research on the activity of C. lucidum-derived extracellular vesicles (ClDEVs). Therefore, this study aimed to verify the anti-aging effects of ClDEVs through in vitro and clinical analyses. In cell experiments, ClDEVs promoted cell regeneration, increased the expression of COL1A1, a gene involved in collagen synthesis, and enhanced the expression of FLG and LOR, a biomarker related to skin barrier improvement. Additionally, ClDEVs suppressed the expression of aging-related biomarkers, such as the CDKN2A (encodes p16) and TP53 (encodes p53) genes, in cells induced to age. In a human clinical trial, after using a cosmetic product containing ClDEVs for 4 weeks, significant improvement in wrinkles around the eyes and nasolabial folds was observed. In conclusion, ClDEVs have demonstrated high potential as a bio-cosmetic ingredient for wrinkle improvement and anti-aging.
