Defects involving specialised areas with characteristic anatomical features, such as the nipple, upper eyelid, and lip, benefit greatly from the use of sharing procedures. The vulva, a complex 3-dimensional structure, can also be reconstructed through a sharing procedure drawing upon the contralateral vulva. In this report, we present the interesting case of a patient with chronic, massive, localised lymphedema of her left labia majora that was resected in 2011. Five years later, she presented with squamous cell carcinoma over the left vulva region, which is rarely associated with chronic lymphedema. To the best of our knowledge, our management of the radical vulvectomy defect with a labia majora sharing procedure is novel and has not been previously described. The labia major flap presented in this report is a shared flap; that is, a transposition flap based on the dorsal clitoral artery, which has consistent vascular anatomy, making this flap durable and reliable. This procedure epitomises the principle of replacing like with like, does not interfere with leg movement or patient positioning, has minimal donor site morbidity, and preserves other locoregional flap options for future reconstruction. One limitation is the need for a lax contralateral vulva. This labia majora sharing procedure is a viable option in carefully selected patients.
Pongamia pinnata Pierre is a tree legume, having potential in production of raw material for biodiesel. A protocol for in wk propagation of this plant was standardized using seedling explants. Growth regulators (GR) including gibberellic acid $(GA_3),\;N^6-benzylaminopurine(BA)$, thidiazuron (TDZ), and Adenine sulphate (Ads) were tested for optimum germination of seeds. Removal of seed coat prior to germination, controlled fungal growth partially but enhanced bacterial growth. Antibiotic cefotaxime was ineffective in controlling bacterial contamination. Seedling derived nodal explants and cotyledon nodes with attached cotyledons were excised and cultured for induction of shoots. Optimum sprouting and multiplication of shoot buds were obtained in MS medium supplemented with $8.88{\mu}M$ BA. These buds differentiated and rooted on medium devoid of GR. Optimum growth of Pongamia seedling was obtained in cotton plugged culture vessels. Reculturing of the cotyledon node explants produced more shoots from the same site. This process of removing shoots and reculturing of cotyledon node was followed for eight passages yielding 4 to 8 shoots in each cycle. The shoots (75%) rooted on half strength MS basal medium supplemented with 0.22% charcoal. All plants survived on transfer to soil. This is the first report on in vitro regeneration of Pongamia pinnata. This report demonstrates the possibility of coupling more than one parameter in single experiment to hasten the process of standardization. The process of cycling the nodal explant repeatedly for production of large number of shoots from single meristem may find application in genetic transformation experiments wherein meristems are used for transformation.
식도 재건술을 시행한 뒤에 발생하는 문합부의 협착은 비교적 드물지만 환자와 의료진에게 반갑지 않은 이다. 이에 대한 해결책으로 풍선확장술, 협착부에 대한 재수술 또는 심한 경우 식이용 장루술이 고려된다. 그러나 풍선확장술의 경우 단기간내 재발의 확률이 높고, 썩 효과적이지 않으며, 협착부에 대한 재수술은 수술적 접근이 쉽지 않고 재협착의 가능성 배제가 어려우며, 식이용 장루술의 경우 경구식이가 불가능하다는 어려운 점들이 있다. 이에 2001년 1월부터 2001년 12월까지 식도스텐트를 이용하여 문합부 협착의 완화를 시도하였다. 1예는 양성협착의 수술이었고 2예는 악성종양이었다 약 1년이상의 추적관찰을 하였고 연하곤란은 개선되었다. 이에 식도 스텐트를 이용한 술후 문합부 협착의 임상적 호전에 대하여 증례를 보고하고자 한다.
사용후핵연료의 효율적인 관리를 위하여 원자력연구소에서 개발중인 사용후핵연료 차세대관리 종합공정(ACP)은 공정타당성연구 단계를 마치고 이의 실증을 위한 $\alpha$-${\gamma}$ type핫셀 건설 단계에 이르렀다. 핫셀의 설계에 앞서 사용후핵 연료를 취급하게 되는 과정에서 발생할 수 있는 방사능에 대한 환경영향평가를 정상운전 시와 사고발생 시로 나누어 수행하였다. 평가에 필요한 자료들은 공정의 개념설계 보고서와 최근 연구소부지 기상 테이터 및 부지특성 자료를 바탕으로 하였으며 기존의 유사한 시설에 대한 평가방법을 참조하였다. 각 핵종별 발생량과 방출량을 계산하여 피폭선량을 계산하였으며 평가결과 원자력법관련 규제기준과 핫셀이 위치하게 되는 IMEF 건물의 안전성분석 기준보다 매우 안전한 결과를 얻어 시설 운영에 대한 안전성을 확보하였다.
The bronchogenic cysts result from an abnormal budding or branching of the tracheo-bronchial tree from the primitive foregut. Bronchogenic cysts may be pulmonary or mediastinal. The most common location for a mediastinal bronchogenic cyst is in relation to the carina. They are usually solitary. They usually are thin walled and contain fluid that is most often clear. The cyst is lined by ciliated columnar epithelium. The bronchogenic cyst i~ usually asymptomatic. They can cause pressure symptoms. When they become infected, symptoms do occur. We report 3 cases of bronchogenic cysts experienced at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. Case I, a man of 20 year-old, had a cyst at the site between aortic arch and left pulmonary artery. He complained cough and dyspnea. The cyst wasn`t communicated with tracheobronchial tree. Case II was 55-year-old male who had had hemoptysis. A huge cyst was located within the lower lobe of left lung and removed by pulmonary lobectomy. There were not any symptoms in Case III that was 6-year old girl. That cyst was located just next to the right main bronchus.
A 45 year old male officer was admitted due to upper substernal pain for 1 month, which was aggravated by swallowing. On past and family history, there was no specific history except heavy drinking. Simple chest x-ray revealed no specific abnormal findings. Preoperative esophagofiberscopy and Barium study showed midesophageal diverticulum, pulsion type, at about 2 cm below the left main bronchus. The opening of the diverticulum was located at the left posterolateral aspect of esophagus. Midesophageal false diverticulum, measuring 2 x 2 x 1 .S cm in size, was noted at about 5 cm under the aortic arch protruding through a slit-like muscular defect. After inversion of diverticular sac, interrupted sutures with 3-0 silks were done on muscular defect site, and mediastinal pleura was reinforced on the lesion with interrupted sutures. On 4th postoperative day, esophagography revealed no diverticulum or stenosis. Also esophagofiberscopy showed smooth mucosal tag without disturbance of passage. On 14th postoperative day, the patient was discharged uneventfully, and follow-up for 3 months after discharge revealed nothing abnormal symptoms. The authors report one case of midesophageal, pulsion type, false diverticulum.
Accidental high epidural block is a rare but serious complication. It can result from many factors, which include the volume and concentration of drug, posture, puncture site, age, pregnancy or intra-abdominal mass, and patients' height and weight. We had a case of accidental high epidural block recently. This is a case report which was confirmed by an epiduragram. A healthy 50-year-old woman with a huge uterine myoma was scheduled for a total abdominal hysterectomy under continous epidural analgesia. Epidural catheterization was carried out smoothly. However, an unexpected hypotension was noticed after an epidural injection of 2% lidocaine 25 ml. Thereafter, the patient was intubated and her respiration was controlled during the operation. Using the 5mg of ephedrine, her blood pressure and pulse were well maintained. The scheduled operation was carried out for one hour uneventfully, but after the operation, she felt paresthesia on her hands in the recovery room. To differentiate between the high epidural and the subdural blocks. We injected 5 ml of a water soluble Niopam 300 through the catheter postoperatively. It was observed on the epiduragram that the catheter was placed in the epidural space. It was suggested that the high epidural block was induced from the widespread diffusion through the narrowed epidural space due to the engorgement of the epidural venous plexus by the patient's huge uterine myoma.
지반조사자료는 건설공사의 계획부터 설계 및 시공, 유지보수에 이르기까지 건설전반에 걸쳐 공학적 판단근거를 제공하기 때문에, 모든 공사에는 필수적으로 지반조사를 실시하여 현재 방대한 양의 성과가 축적되었다. 그러나 조사 시 위치정보에 대한 인식 부족으로 지리정보시스템에서 이용하기에는 많은 제약이 있어 현행 지반조사체계에서 위치정보 확보에 대한 필요성이 제기되었다. 본 연구에서는 지반조사 과정 분석을 수행하여 입력항목 중 위치와 관련된 인자를 도출하고 데이터모델링에 반영하여 DB설계를 수행하였다. GPS, PDA, 무선통신 모듈을 이용하여 지반조사와 동시에 위치 및 속성정보를 실시간으로 GIS-DB화하는 지반정보취득 프로그램(PGeo)과 Web-DB를 이용하여 추가 지반조사자료를 입력하고, 레포팅할 수 있는 지반정보 레포팅 프로그램(GeoReport)을 개발하였다.
Gyong-gi province has been the political, social, economical center of Korea since the three Kingdom period, Koryo, Chosun dynasty. As a result, the area enjoys abundant local food culture. However, due to the truce line going through the province, it was divided into northern and southern region. Development of the northern Gyong-gi province has been restricted because of security and defence reasons. So, even though it has kept abundant local food heritage compared to other regions. local food products have not been fully developed. Furthermore, as bed towns of Seoul and new towns are constructed in this region, traditional and local culture has been vanished gradually and the regional uniqueness is disappearing. This report focuses on identifying unique local foods of northern Gyong-gj province where tourist site development is limited and finding out how to utilize these as international tourism resource in the 21$\^$th/ century, the era of reunified Korea. Now, we need to seek ways to overcome the current difficulties and utilize the heritage of food culture and develop It in a form that suits the taste of modern consumers.

Distraction osteogenesis is a biologic process in which new bone is formed between bone fragments being separated by a tractional force. This technique has the advantage of initiating new bone growth without bone transplantation and promoting the growth of soft tissue. Mandibular distraction osteogenesis has shown to be effective to treat congenital or acquired mandibular hypoplasias. On the basis of positive results with implant-supported prostheses, the use of implants in the distracted site can significantly help stabilize the prosthesis. We obtained good result in the patient with mandibular deficiency due to trauma, who have been reconstructed with distraction osteogenesis and implant. We report our experiences with literature view.
