Leukocyte platelet-rich fibrin in endodontic microsurgery: a report of 2 cases

  • Endodontic microsurgery is a predictable treatment option when orthograde treatment or retreatment is unsuccessful or unfeasible. However, when there is a gross compromise of periapical bone, achievement of bone regeneration after the surgical procedure may be hampered. In such cases, the application of guided tissue regeneration principles, with adjunctive use of leukocyte platelet-rich fibrin to fill the bone defect as a bone substitute and as a membrane to cover the site, provides a cost-effective solution with the benefits of accelerated physiological healing and reduced post-surgical pain and discomfort. This case report presents 2 cases of endodontic microsurgery of the upper lateral incisors with loss of buccal cortical plate, where platelet-rich fibrin was successfully applied.

  • Chronic nonbacterial osteomyelitis(CNO) is histologically characterized by nonspecific osteitis. This inflammatory disorder, which lacks an infectious origin, typically presents with chronic pain and swelling at the affected site that can persist for months or even years. However, it is rare for CNO to affect the mandible. A 10-year-old girl presented with a primary complaint of pain in her left mandible. She had no significant medical or dental history. On examination, swelling was visible on the left buccal side, and imaging revealed radiolucent bone deterioration within the left mandible. This case report presents the radiological changes observed over a 12-year follow-up period. Variations in radiopacity, radiolucency, and periosteal reactions were noted periodically. This case highlights the radiological characteristics and findings that are crucial for the diagnosis of CNO, a condition for which no clear diagnostic criteria are currently available.

  • Cubital tunnel syndrome refers to compression neuropathy caused by pressure on the ulnar nerve pathway around the elbow. A 63-year-old male patient visited the clinic complaining of decreased sensation and weakness in his left ring finger and little finger, stating that the symptoms first began 6 months prior. He had undergone surgery to remove a ganglion cyst from his left elbow joint about 5 years prior in Mongolia. Magnetic resonance imaging revealed a cystic mass located at the previous surgical site, which was compressing the ulnar nerve within the cubital tunnel. Ulnar nerve decompression and anterior transposition were performed, and the cystic mass was excised. Upon pathological examination, the mass was diagnosed as a ganglion cyst. The patient's symptoms including sensory dysfunction and weakness improved over the 1-year follow-up period. This report describes a rare case of ganglion cyst recurrence compressing the ulnar nerve in the cubital tunnel after previous ganglion cyst excision.

  • The hamstring muscle injury is a quite common sports injury which has a wide spectrum of treatment option that simple muscle strain can be treated conservatively otherwise total rupture of muscle or musculotendinous junction must be treated surgically. Among them, rupture of proximal hamstring muscle near at its origin site is quite uncommon and it should be treated surgically because it could cause several complications if not treated appropriately. In this case report, the authors report successful surgical treatment of acute rupture of proximal hamstring muscle and also would like to review the clinical presentation of this type of injury.

  • Isolated lateral collateral ligament injury has been reported as avulsion fracture of attachment site or intrasubstance tear. The treatment of avulsion fracture of lateral collateral ligament was primary repair with anchor or staple. There are some reports about loosening or migration of staple which used in repairing meniscus of knee. There is, however, no report about loosening or migration of staple which was used for lateral collateral ligament repair. We report a case of migrated staple that was used for lateral collateral ligament repair, which was migrated to intraarticular posterolateral corner of the knee.

  • Hemangioma are not rare tumors. They can be found in almost any of the vascular structures of the body. Hemangiomas involving the deep structures of the extremities may produce extremely difficult therapeutic problems for the orthopedic surgeon. Pigmented villonodular synovitis (PVNS) is a rare proliferative disorder that affects synovium, tendon sheath and bursa. Although the condition can present in any joint, knee joint is the most commonly affected site and only 2.5% of cases occur in foot and ankle joint. We have experienced a patient who has of foot and report an optimal method of surgical treatment. Authors report the result of hemangioma in mid-foot which arise from removal of a pigmented villonodular synovitis that has low out break rate of benign tumor in mid-foot with literature review.

  • Ameloblastic fibrosarcoma(AFS) is a rare malignant odontogenic tumor presented as painful swelling and intraosseous mass with occasional ulceration. The most frequent site is the mandible body. AFS of the jaw generally occurs in all ages($3{\sim}83$ years old), with the average age of 27.3. AFS was associated with high local recurrence rate of 37% in the areas of gingiva, floor of mouth and neck. Although metastasis is not a special feature of this lesion, 20% have died within 3 months to 19 years, due to locally aggressive tumor growth. This report describes an ameloblastic fibrosarcoma occurring in the mandible of a twenty-five year old male. The tumor was treated by partial mandibulectomy and reconstructed with a fibular flap. The patient has shown no signs of recurrence or complications during 18 months postoperatively. In this study, we report our case with a review of literatures.

Recovery of Acute Ophthalmoplegia after Hyaluronic Acid Filler Injections to the Temples: A Case Report and Review of the Literature

  • Soft tissue filler injection is the second most common nonsurgical cosmetic procedure. Despite the safety of fillers, as use has grown, so has the number of patients affected by adverse events. Ophthalmoplegia following cosmetic filler injection is a rare complication, mostly occurring after injection to the glabella, nasolabial fold, periorbital, and lateral nasal site. In all cases where ophthalmoplegia has been reported following fillers, patients have simultaneously experienced vision loss and other ocular symptoms. We report a case of isolated acute ophthalmoplegia following hyaluronic acid injection solely in the temple region. A 40-year-old woman, 3 hours after the procedure, presented to our hospital with left eye ophthalmoplegia, ptosis, and hypotropia. Treatment started with hyaluronidase, steroids, and anticoagulants. After 4 weeks, left eye ophthalmoplegia remained unchanged, and through a 10-week follow-up, all left ocular movements improved, and only mild hypotropia and ptosis persisted. This case report shows that ophthalmoplegia may also happen with temple region filler injections. We also review available prevention techniques and treatments to avoid such complications when performing soft tissue fillers for gaunt appearance correction.

  • Poor water-proofing integrity is related to many factors : site procedure, workmanship, weather. substrate conditions, installation(including asphalt application) conditions. The evaluation of adhesive properties tested by concrete surface moisture and asphalt application was based on laboratory test and observations. This report describes the pull-off test results of waterproofing systems currently used for installing and reparing for concrete bridge decks. The test results showed that the moisture condition fully affected the adhesive properties of sheet membranes and sealer but partially for liquid membranes.

  • The purpose of this report is to study the traffic opening day of concrete pavement. For this purpose this paper studies on the propeties of various cement types which include the newly developed cement for the cement pavement regarding the resistance to the chemical attack caused by de-icing salt and the durability of the concrete pavement. Especially, traffic opening day of concrete pavement are discussed on site.
