  • 1. Objective. The aim of this study was to investigate whether dental implant exposure to the maxillary sinus cavity increases the risk of maxillary sinus complications. 2. Study design. An implant was placed bilaterally in the maxillary sinus of eight adult female mongrel dogs in a way that it penetrated the bone and mucous membrane of the maxillary sinus floor to the extent of 2 mm, 4 mm, or 8 mm. The implants were left in place for six months. 3. Results. Radiographic and histologic examinations did not show any signs of pathologic findings in the maxillary sinus of the eight dogs. 4. Conclusion. This study indicates that implant protrusion into the maxillary sinus cavity is not related to the development of sinus complications.

  • Aim: The aim of this study was to investigate the arterial arcade of maxillary sinus by determining the distance between the alveolar crest and the inferior border of the vessels in Korean population. Materials and methods: 76 patients and 87 available sinuses were determined by CT scan (Implagraphy, Vatech, Seoul, Korea). The distance between alveolar crest and inferior border of the vessel was determined considering the tooth area of posterior maxilla. Results: The lowest distance was observed on first molar area and the average distance is 16.9 mm. Discussions: it is important in sinus lift surgery that the vessels not to be damaged and that the anastomosis is situated in the lateral wall of maxillary sinus, which has to be exposed to gain access to the maxillary sinus. The knowledge of arterial arcade of maxillary sinus is important to perform the sinus floor elevation procedure effectively.

  • Pyriform sinus fistula is a rare anomaly arising from 3rd or 4th branchial apparatus and has been recognized as one cause of acute suppurative thyroiditis or acute deep neck infection. Pyriform sinus fistula must be considered when a clinician is encountered recurrent left lower neck abscess and a history of repeated incision and drainage. The confirmation of the diagnosis is made when the fistula tract is identified on a barium swallow study and when the internal orifice of the fistula is found at the apex of pyriform sinus on laryngoscopic examination. A complete excision of the fistula tract has been proposed as a treatment of choice. However, in some cases it is very difficult to resect the tract completely because of severe inflammation and repeated drainage procedure. We present three cases of pyriform sinus fistula which are successfully treated by laryngomicroscopic chemocauterization using synthetic fibrin and $AgNO_3$.

Floating septum technique: easy and safe method maxillary sinus septa in sinus lifting procedure

  • Background: The presence of septa increases the risk of Schneiderian membrane perforation during sinus lift procedure, and therefore, the chance of graft failure increases. We present a safe method of managing septa and, in particular, overcoming small and palatally located septa. Methods: After the elevation of the flap and the creation of a small bony window positioned anterior to the septum, the Schneiderian membrane is lifted carefully. A thin and narrow osteotome is then placed at the indentation created at the base of the septum, and mobilization of the septum is achieved by gentle malleting. The membrane is again carefully lifted up behind the septum. Results: There was one small membrane perforation case in all 16 cases, and none of these patients showed postoperative complications such as implant failure, infection, or maxillary sinusitis. Conclusions: This technique is useful for overcoming the problem of maxillary sinus septa hindering the sinus floor elevation procedure, leading to fewer complications.

청소년기에 발견된 상구순 누공의 치험례 (Congenital Upper Lip Sinus Found in Adolescent Patient: A Case Report)

  • Purpose: Congenital sinus of the upper lip is extremely rare and only 3 cases have been reported domestically. We report a case of congenital sinus of midline upper lip, which was found in an adolescent patient. Methods: A 14-year-old girl presented with a small pit on midline of the upper lip, which was visible at birth. The patient had never been treated for the congenital sinus because it was asymptomatic. Surgical excision under local anesthesia was performed. Results: The sinus had a tract extending into 5 mm posteroinferior and had not penetrated the oral cavity. Histological examination showed a fistulous tract lined by keratinized squamous epithelium. After complete excision, there was no recurrence and we obtained a satisfactory cosmetic result. Conclusion: Congenital sinus of the midline upper lip is extremely rare. This is a special case that is reported because it did not cause symptoms for the patient until she reached adolescence.

심실중격결손을 합병한 Valsalva's 동 동맥류 파열의 치험예 (Surgical Treatment for Aneurysm of Sinus of Valsalva Combined with Ventricular Septal Defect)

  • This is a case report of surgically treated rupture of Valsalva Sinus aneurysm combined with VSD. He has been relatively healthy until about one month before admission, when during bath, he felt abruptly palpitation, left chest pain and exertional dyspnea. These symptoms have progressed. On admission, thrill was palpable and continuous machinery murmur was audible on 2nd and 3rd intercostal space along the left sternal border. A rupture of Valsalva`s sinus aneurysm was confirmed by aortography and echocardiography but a small VSD was found by cardiotomy in open heart surgery. On 11th Sep. 1978, open heart surgery was performed. Valsalva`s sinus aneurysm came out from right coronary aortic sinus and ruptured into the right ventricle. It sized 1.2X1.5X1.5 cm. Ruptured opening was noted on apex of aneurysm [0.8X0.8cm], VSD [1. 0X0. 3cm in size] was just below the aortic annulus. The aneurysmal sac was removed on neck. After that, VSD and aneurysmal orifice were closed together with interrupted mattress sutures on same plane. The postoperative course was uneventful and discharged three weeks after open heart surgery.

Valsalva형 동뇌류파열의 외과적 치료 (Surgical treatment of sinus valsalva rupture -Surgical considerations and results of operation-)

  • Over the past 11 years, from Jan. 1975 through Nov. 1985, 27 consecutive patients with congenital aneurysms of sinus of Valsalva underwent corrective surgery in our department of Thoracic Surgery. 26 were suggested to arise from right coronary sinus and 1 from noncoronary sinus: among 26, 18 ruptured into right ventricle, 1 into right atrium, 1 into pulmonary artery. And 1 from noncoronary sinus into right atrium. Among 18 cases of preoperatively diagnosed as ruptured aneurysm, the fistula was repaired through the aorta in 12, although the chamber of termination was also opened in some. Recurrent fistula occurred in 2 cases in which the ruptured aneurysm was repaired through right ventricle. It appears that repair of the fistula itself is best carried out through an aortotomy, after cross-clamping.

3차원 컴퓨터단층촬영 영상을 이용한 전두동 발달의 연구 (Developmental Study of Korean Frontal Sinus by using 3D CT Scan)

  • Purpose: Although there are many reports on the development of paranasal sinuses, few studies were performed with respect to the frontal sinus with 3D CT, especially in Korean children. In this study, we evaluated the normal development of frontal sinus in Korean children with 3D CT imaging. Methods: A total of 301 patients under the age of 20 were reviewed retrospectively. Maximal AP (anteroposterior) length, height and width were measured with the aid of computer device. The volume of frontal sinus was also evaluated. Results: At the age of more than 4, the pneumatization of frontal sinus was detected. Frontal sinus developed continuously and was observed in almost every children at the age of more than 17. The AP length, height, width and volume of frontal sinus were enlarged continuously until the age of 20. And the maximal growth of frontal sinus was observed at puberty. The width and volume of frontal sinus showed high correlation coefficient (r=0.8) relative to height or AP distance. Conclusion: The results of this study may be helpful in the comprehension of normal development of frontal sinus in Korean children.

Functional Endoscopic Sinus Surgery for a Patient with Maxillary Sinusitis Occurring after Implant Placement

  • Maxillary sinus membrane elevation and bone graft have been performed routinely in alveolar bone with insufficient residual bone height. There are a number of causes for development of maxillary sinusitis after these procedures. When maxillary sinusitis is caused by sinus membrane elevation, bone graft, and implant placement, various treatment such as medication, incision and drainage (I&D), implant removal, and the Caldwell-Luc procedure can be considered. Removal of an implant or the Caldwell-Luc procedure can be harmful if inflammation is not present in the oral cavity and survival of grafted bone and implant osseointegration can be expected despite the presence of maxillary sinusitis. In this case, functional endoscopic sinus surgery, which was often used in the otorhinolaryngology department, was performed without removal of the implant for a patient with maxillary sinusitis after one month following implant placement. Thus, we report on this case with a review of the literature.

전두동 골절 양상에 따른 치료 (Treatment of Frontal Sinus Fractures According to Fracture Patterns)

  • Purpose: Frontal sinus fractures are relatively less common than other facial bone fractures. They are commonly concomitant with other facial bone fractures. They can cause severe complications but the optimal treatment of frontal sinus fractures remains controversial. Currently, many principles of treatment were introduced variously. The authors present valid and simplified protocols of treatment for frontal sinus fractures based on fracture pattern, nasofrontal duct injury, and complications. Methods: A retrospective chart review was performed on 36 cases of frontal sinus fractures between January, 2004 and January, 2009. The average age of patients was 33.7 years. Fracture patterns were classified by displacement of anterior and posterior wall, comminution, nasofrontal duct injury. These fractures were classified in 4 groups: I. anterior wall linear fractures; II. anterior wall displaced fractures; III. anterior wall displaced and posterior wall linear fractures; IV. anterior wall and posterior wall displaced fractures. Also, assessment of nasofrontal duct injury was conducted with preoperative coronal section computed tomographic scan and intraoperative findings. Patients were treated with various procedures including open reduction and internal fixation, obliteration, galeal frontalis flap and cranialization. Results: 12 patients are group I (33.3 percent), 14 patient were group II (38.8 percent), group III, IV were 5 each (13.9 percent). Frontal sinus fractures were commonly associated with zygomatic fractures (21.8 percent). 9 patients had nasofrontal duct injury. The complication rate was 25 percent (9 patients), including hypoesthesia, slight forehead irregularity, transient cerebrospinal fluid leakage. Conclusion: The critical element of successful frontal sinus fracture repair is precise diagnosis of the fracture pattern and nasofrontal duct injury. The main goal of management is the restoration of the sinus function and aesthetic preservation.
