New Algorithm of Determining the Floating and Sinking Pulse with a Pulse Diagnosis Instrument (맥진기를 이용한 새로운 부침맥 판단 방법)

  • The pulse diagnosis is an important and universally used method in Oriental Medicine. Since the traditional method of palpating the pulse relies on the subjective sense in the fingers of an Oriental Medical Doctor(OMD), there has been continued need for more objective method for pulse diagnosis. Recently, various pulse analyzers have been developed to meet such objective palpation and interpretation. However, most of these attempts were not successful to replace OMD's own palpation by fingers. To improve the performance of the pulse analyzers, one should develop machine-appropriate interpretations for the pulse images in the literature, in addition to the improvement in the repeatability and reproducibility. One of such widely-used pulse images to be interpreted is the floating and sinking pulse. The floating and sinking pulses are the two representative pulse images informing us how strong pressure one should apply to obtain the maximal pulse strength. A previous study suggested a convenient and unified measure for the floating and sinking pulses by defining the coefficient of the floating-sinking pulse(CFS). We found the original definition of the CFS could be erroneous under some situations. To improve the performance, we introduce new CFS algorithm for determining the floating and sinking pulse with a pulse analyzers(3-D MAC). To test the performance of the newly suggested algorithm, we conducted a clinical study comparing the agreement ratio with the floating and sinking pulse diagnosis by the OMDs. We found that, among the subjects who are diagnosed with having either the floating pulse or sinking pulse, the new CFS algorithm showed 55.3% diagnosis rate and 73.0% concordance rate, which are about 3% and 6% improvement in the diagnosis rate and agreement rate, respectively, compared to the original CFS algorithm.

  • STATEMENT OF PROBLEM. Unlike screw-retention type, fixture-abutment retention in Locking taper connection depends on frictional force so it has possibility of abutment to sink. PURPOSE. In this study, Bicon$^{(R)}$ Implant System, one of the conical internal connection implant system, was used with applying loading force to the abutments connected to the fixture. Then the amount of sinking was measured. MATERIAL AND METHODS. 10 Bicon$^{(R)}$ implant fixtures were used. First, the abutment was connected to the fixture with finger force. Then it was tapped with a mallet for 3 times and loads of 20 kg corresponding to masticatory force using loading application instrument were applied successively. The abutment state, slightly connected to the fixture without pressure was considered as a reference length, and every new abutment length was measured after each load's step was added. The amount of abutment sinking (mm) was gained by subtracting the length of abutment-fixture under each loading condition from reference length. RESULTS. It was evident, that the amount of abutment sinking in Bicon$^{(R)}$ Implant System increased as loads were added. When loads of 20 kg were applied more than 5 - 7 times, sinking stopped at $0.45{\pm}0.09\;mm$. CONCLUSION. Even though locking taper connection type implant shows good adaption to occlusal force, it has potential for abutment sinking as loads are given. When locking taper connection type implant is used, satisfactory loads are recommended for precise abutment location.

A Study of 'uplift yang-qi to raise sinking' efficacy in Radix Bupleuri (시호(柴胡)의 승약작용에 관한 연구)

  • Objectives : Radix Bupleuri is from the dried root of the herb of the Perennial herbaceous plant, Bulpleurum falcatum L. or Bulpleurum chinense DC. or Bulpleurum. scorzonerifolium Willd., family Umbelliferae. Medicinal Properties are bitter and pungent in flavor, slightly cold in nature and attributed to the liver and gallbladder meridians. Actions in modern Materia Medica are regulate the functional relation of internal organs to relieve fever, disperse the stagnated liver-qi and uplift yang-qi to raise sinking. The ‘uplift yang-qi to raise sinking’ efficacy, out of three efficacies (regulate the functional relation of internal organs to relieve fever, disperse the stagnated liver-qi and uplift yang-qi to raise sinking), has been disputed in the medical profession for a long period. Hereupon, this study ascertained the reason why it has been disputed. Methods : With respect to this medicinal herb, the efficacies of regulate the functional relation of internal organs to relieve fever, disperse the stagnated liver-qi and uplift yang-qi to raise sinking were described as to what was written in ‘Shen Nong's Herbal’ from Chin and Han dynasties until Jin and Yuan dynasties. Results : The beginning of Jin and Yuan dynasties, it began to deal with the ‘uplift yang-qi to raise sinking’ efficacy and so it has been carried on modern textbooks and medical books. The reason why it was added is that it was influenced by the theory of ‘Raise Sinking’ advocated by Zhang jieku who lived in the period of Jin and Yuan dynasties. Since then, the properties of ‘Radix Bupleuri’ have been wrongly known to the public. Additionally, ‘Radix Stellariae Seu Gypsophilae’, which was begun to be introduced from the Four Cities since the Five Dynasties, has been combined with the best stuffs of ‘Radix Bupleuri’ produced from Yin Zhou. Consequently, its original properties were remarkably disordered. Likewise, respective medical schools’ theories were changed by the influence of ‘Bulpleurum. scorzonerifolium Willd’ begun to be used since Tang dynasty. Conclusion : it is considered that the current ‘Raise Sinking’ efficacy of Radix Bupleuri is unreasonable to be applied to the efficacy of the whole Radix Bupleuri because it is limited to certain species.

Clinical Study of the Floating-Sinking Pulse Quantification Analysis on Ages, Left/Right, and Palpation Positions (20/60대 여성을 중심으로 살펴본 좌우 촌관척 부/침맥 정량화 임상연구)

  • Pulse diagnosis is a central diagnosis method used in traditional Oriental medicine. To standardize and modernize the pulse diagnosis method, it is essential to develop an instrument-based reinterpretation of the clinically used pulse images in terms of the physical quantities such as the strength, period, width, length, and depth of the pulse. As a step towards such standardization, we conducted a clinical study on the floating/sinking pulses based on an automated palpation instrument (3D-MAC, Daeyo Medi, Korea) for 213 female subjects in their 20s and 174 female subjects in their 60s. The floating/sinking pulses are the two representative pulse images depending only on the depth of the pulse, and can be conveniently scaled by the coefficient of the floating-sinking pulse ($C_{fs}{\in}(0,1)$), which represents how strong one should apply the hold-down pressure to obtain the maximal pulse strength. As a result, primarily we found that it tends to appear more floating-like pulse ($C_{fs}{\rightarrow}0$) at Gwan and more sinking-like pulse ($C_{fs}{\rightarrow}1$) at Cheek, at both age groups and at both wrists. This result is consistent with a previous study on the geometrical structure of the blood vessel by an ultrasonograph. Second, the pulse tends to be more sinking-like in the age group of 60s than 20s. Finally, the pulses at the right palpation positions were found to be more sinking-like than the left, at both age groups.

A Study on Transient Operation Characteristics of 30kW Scale CVCF Inverter based Micro-grid (30kW급 CVCF 인버터 기반 Micro-grid의 구현 및 과도상태 운용특성에 관한 연구)

  • Recently, micro-grids based on a CVCF inverter have been replacing diesel generators to reduce CO2 emissions in island areas with the increase of renewable energy sources. Stable operation methods are also being researched. These micro-grids may cause energy sinking if the total output of renewable energy sources is larger than the total customer loads. In the case of energy sinking, the voltage of a CVCF battery could rapidly increase according to the condition of SOC, and blackout could occur in the micro-grid due to the operation of a protection device in a CVCF inverter. Therefore, this paper analyzes the operation characteristics of a CVCF-inverter-based micro-grid when energy sinking occurs and proposes a transient operation strategy to prevent shut-down of the CVCF inverter. A test device of a 30-kW CVCF-inverter-based micro-grid was implemented, and the transient operation characteristics for the energy sinking phenomenon are presented. The simulation results confirm that blackout can be properly prevented according to the conditions of SOC and voltage in a CVCF battery.

The piling-up/sinking-in response of elasto-plastic materials in nano-indentation using sharp indenter (나노 인덴테이션 시험에서의 탄소성 재료의 파일업/싱크인 특성)

  • Over the past decade, many computational researches have been performed to investigate quantitative relationships between load-displacement and material properties. But piling-up which causes errors to estimate mechanical material properties remains the most significant unresolved issue in nano-indentation test. This study has estimated quantitative aspects of the effects of material properties, especially work hardening exponent, on piling up/sinking in response of various materials. Using FE Analysis, piling up/sinking in response when material is indented by sharp indenter is investigated to evaluate the effects of material properties. From the FE analysis result, quantitative relationships between piling up/sinking in height and material properties is assessed using dimensional analysis which is used to define scaling variables and universal functions. And nano-indentaion test is performed to verify this relation on various materials. From the result of comparison with prediction from dimensional function and experiment, the work hardening exponent was found to have greater influence on the piling up/sinking in height during the nano-indentation than other material properties, such as elastic modulus and yield stress.

A Philological study on Submersion Property of Aquilariae Lignum (침향(沈香)의 침수성(沈水性)에 관한 문헌적 연구)

  • Objectives : The purpose of this study is searching for grounds of new regulations about submerging properties of Aquilariae Lignum described in the Korean Herbal Pharmacopoeia. Methods : Through the investigation of the various Chinese histories and herbal classics, the definitions of Aquilariae Lignum and documents on actual trade and discrimination methods by sinking under water. Results : In Jiaozhouyiwuzhi(交州異物志) written in the early 2nd century, Aquilariae Lignum was defined as heart wood part or knots sinking under water due to hard and dark deposition of resins. Also, which are not sinking were defined as zhan xiang(棧香), and which are floating were named as qian xiang(槧香). The definitions of Aquilariae Lignum had not been changed till Ching Dynasty era, but the classification and the names about not-sinking and floating Lignum materials were a little changed. Conclusions : These results indicate that it is reasonable to make a regulation that "Aquilariae Lignum sinks under water" in the property description of The Korean Herbal Pharmacopoeia.

Carotenoid Pigments from Suspended and Sinking Particulate Matter in Prydz Bay, Antarctica

  • Suspended and sinking particles were collected during ODP Leg 119 to the Indian Ocean sector of the Antarctic Ocean. Field work was carried out at four sampling sites in Prydz Bay. Two of these sites were located in the Outer Bay, and two in the Inner Bay. At the four locations, a total of ten deployments of a sediment trap array were made. The concentrations of carotenoids both in suspended and sinking particulate matter in Prydz Bay were analyzed using HPLC. Fucoxanthin was the dominant carotenoid pigments both in suspended and sinking particles. The present study also indicates that 19'-hexanoyoxyfucoxanthin-containing prymesiophytes (Phaeocystis spp.) was abundant in the study area. The flux rates of carotenoids were generally highest at 50 m, and approximately double the flux rates at deeper horizons, however, at Inner Bay sites, the mean flux rates of carotenoids were greatest at 200 m, and 3 times greater than that of 50 m. Such anomalous high fluxes at 200 m imply that grazers were locally abundant between 100 m and 200 m at these sites close to land, and this hypothesis is supported by visual evidence of lots of fecal pellets in the 200 m trap. Integrates standing stocks versus sinking pigments data support that particulate material in Prydz Bay was not recycled rapidly.

"Syndrome of the Sinking Skin-Flap" Secondary to the Ventriculoperitoneal Shunt after Craniectomy

  • The syndrome of the sinking skin flap was introduced to explain the phenomenon of neurological deterioration after decompressive craniectomy. A 37-year-old man was admitted with acute subdural hematoma and traumatic intraparenchymal hematoma. After decompressive craniectomy, the patient suffered from hydrocephalus for which a ventriculoperitoneal (V-P) shunt was inserted. Following this procedure, the depression of the skin flap became remarkable and his mentation was deteriorated. The patient recovered uneventfully after temporary elevating of valve pressure and cranioplasty. We present a patient who was successfully managed with elevation of valve pressure and cranioplasty for the syndrome of the sinking scalp flap with review of a pertinent literature.

Study on the Selection of Representative Pulse Wave

  • Objectives : The purpose of this study is to develop the method of selecting representative pulse wave. Methods : The pulse waves were acquired at the right and the left Guan point(關部) with 1420 people who were apparently healthy. The shape agreement of right and left pulse wave and the floating-sinking ratio were compared with three cases, which were the pulse height based method, the pulse area based method, and the pulse time based method. Results : In the point of the shape accordance, the pulse time based method was the best, and the pulse area based method was the worst. In the point of the floating-sinking ratio, the pulse height based method was the worst, and the pulse time based method was the best. Conclusions : So, the pulse time based method was recommended for selecting the representative pulse wave. This study compared the selection methods of representative pulse using the physiological characteristics of pulse wave. Further studies are required, because the representative pulse wave is the main factor of determining the shape and the floating-sinking characteristic of the pulse wave.
