Background: The Purpose of this study was to evaluate the value of Spencer technique on the range of motion (ROM), Pain, function in patients with shoulder adhesive capsulitis. Methods: subjects consisted of 30patients who were diagnosed shoulder adhesive capsulitis. All subjects are randomly assigned to 2groups: Spencer technique (ST) group (n=15), self assistive ROM exercise(S-A ROM E) group (n=15). The subjects performed an intervention program 30 minuets per day and was repeated 3 times a week for 4 weeks a total of 12 times. ROM of flexion, abduction, external rotation, internal rotation were measured using a goniometer. The visual analog scale (VAS), Shoulder pain and disability index (SPADI) were used to measure pain, functional ability. Results: In the intergroup comparisons after the intervention, ROM of flexion, abduction, internal rotation, VAS, SPADI were significantly different(p<.05). Spencer technique was more effective for improving ROM, pain, functional ability than self assistive ROM exercise. Conclusions: Our study suggest that considering Spencer technique for the patient with shoulder adhesive capsulitis. Further studies on Spencer technique are needed in the future.
The purpose of this study was to compare EMG activity for pectoralis major muscle during shoulder movement with various abduction angle and rotation position in supine position. Fifteen healthy subjects were recruited for this study. All subjects performed shoulder horizontal adduction holding a 2 kg dumbbell in shoulder abduction $40^{\circ}C$, $70^{\circ}C$, $90^{\circ}C$, $130^{\circ}C$, $160^{\circ}C$ with shoulder neutral, internal rotation (IR), and external rotation (ER). Surface EMG activity was recorded from pectoralis major clavicle part and pectoralis major sternum part for 5 seconds and EMG activity was normalized to the value of maximal voluntary isometric contraction (%MVIC). Dependent variables were examined with 3 (Neutral, IR, ER) ${\times}$ 5 ($40^{\circ}C$, $70^{\circ}C$, $90^{\circ}C$, $130^{\circ}C$, $160^{\circ}C$) analysis of variance with repeated measures. The EMG activity of pectoralis major muscle was significantly different between shoulder abduction angles and between shoulder rotation positions (p<.05). The highest value of EMG activity of pectoralis major clavicle part among shoulder abduction angles was in $70^{\circ}C$ and, $90^{\circ}C$ in that order. The highest value of EMG activity of pectoralis major sternum part among shoulder abduction angles was in $130^{\circ}C$ and, $90^{\circ}C$ in that order. According to the rotation degree, shoulder ER showed the highest value and IR showed the lowest value in both muscle parts. These results suggest that shoulder abduction $70^{\circ}C$, $90^{\circ}C$, $130^{\circ}C$ will be effective during manual muscle testing (MMT) and strengthening exercise for pectoralis major muscle. It is also supposed that shoulder ER is the efficient posture for strengthening of pectoralis major muscle.

PURPOSE: The aim of this study was to identify the effects of joint mobilization intervention combined with exercise on range of motion (ROM), pain intensity, and functional performance in adolescent baseball players with internal impingement syndrome of the shoulder. METHODS: The subjects were 30 adolescent baseball players diagnosed with internal impingement. Ten subjects were randomly assigned to each of 3 groups: Group 1 (exercise only), Group 2 (joint mobilization combined with exercise), and Group 3 (rest-only control group). Three weekly interventions were given for 4 weeks (the control group rested for 4 weeks). The main outcome measures were goniometer, visual analogue scale (VAS), and Korean Kerlan Jobe shoulder-elbow (K-KJOC) scores. The ROM (external and internal rotation), pain intensity (at the moment of throwing), and functional performance were compared among the groups. RESULTS: No significant difference was observed among the groups in the range of external rotation of the shoulder before and after the intervention, but the range of internal rotation was significantly increased in Group 2 compared to Groups 1 and 3. Pain intensity was significantly lower in Group 2 than in Group 1 and 3, and functional performance was significantly increased in Group 2 compared to Group 1 and 3. CONCLUSION: An intervention that combined joint mobilization with exercise was more effective than resting or exercise alone for rapid recovery from sports injury and improvement in athletic performance.
Purpose: This study aimed to investigate the effect of angular joint mobilization (AJM) on the shoulder pain, range of motion, and functional improvement in a patient with shoulder adhesive capsulitis. Methods: The patient diagnosed with right shoulder adhesive capsulitis by an orthopedic surgeon was a 60-year-old male, right hand/arm dominant, with a height of 175 cm and weight of 75 kg. The patient received 12 sessions of AJM once or twice per week for eight weeks. AJM was applied for 5 min each of flexion, abduction, external rotation, internal rotation, for a total of 20 min per session. The visual analog scale, the goniometer, and the Oxford shoulder score were used to measure pain, range of motion, and shoulder pain & disability index, respectively. Results: After all the treatments, the pain decreased from 6 to 2 points. The range of motion increased in flexion by $54.3^{\circ}$ from $125^{\circ}$ to $179.3^{\circ}$, abduction by $38^{\circ}$ from $140^{\circ}$ to $178^{\circ}$, external rotation by $54.4^{\circ}$ from $30.3^{\circ}$ to $84.7^{\circ}$, and internal rotation by $25^{\circ}$ from $45^{\circ}$ to $70^{\circ}$. The shoulder disability index decreased from 33 points to 17 points. Conclusion: This study found that AJM has a positive effect on the improvement of shoulder pain, range of motion, and function in a patient with shoulder adhesive capsulitis. Further studies on AJM are needed in the future.

Purpose: The present study investigated the effects of 12-week aquatic exercise training on isokinetic muscle function of the shoulder in adolescents with cerebral palsy. Methods: The study included four male and four female adolescents with cerebral palsy. Isokinetic muscle function was measured at an angular velocity of $60^{\circ}/s$, using Biodex System VI Pro. The peak torques of internal rotation and external rotation were measured before and after training. Aquatic exercise training was performed once a day for 120 min, 4 times a week for 12 weeks. Results: The peak torque of external rotation according to body weight and mean power of internal rotation were significantly higher after training (p < 0.05). Conclusion: Our findings suggest that 12-week aquatic training for adolescents with cerebral palsy can improve isokinetic muscle function of the shoulder. Future studies should analyze the changes in isokinetic muscle function of the shoulder in more detail using various aquatic exercise programs to investigate their effects on individuals with cerebral palsy.

The purpose of this study was to comparatively analyze the active stability of shoulder joints according to the frequency of overhead motions, such as serving and spiking, engaged in by female professional volleyball athletes who play different positions, and to provide the results as the basic data for developing exercise programs to prevent shoulder joint injuries. The subjects of this study were 50 Korean female professional volleyball players and positions were as follows: left and right attackers, centers, setters, and liberos. The external rotation and internal rotation muscle strength and muscle strength ratios of the dominant and non-dominant arms of all subjects were measured using Biodex. The results of this study are as follows: Frist, no significant differences were found in the internal and external rotation muscle strength of the dominant and non-dominant arms between positions. Second, for the shoulder joint muscle strength ratio of the dominant arm, by position, the setter showed significantly greater stability compared to the other positions. Third, for the shoulder joint muscle strength ratio of the non-dominant arm, by position, no significant difference in stability between positions was found. Fourth, it was found that the dominant arm had significantly greater instability of the shoulder joint than the non-dominant arm for attackers and centers, but no significant difference was found for setters and liberos. This study comparatively analyzed the muscle strength ratios of the external/internal rotations and dominant/non-dominant arms, which can determine the stability of the shoulder joints between female professional volleyball playing positions that engage in jumps and spikes using only the dominant hand and positions that do not.
Purpose: The purpose of this study was to investigate the validity and reliability of the measurement of shoulder joint motions using an inertial measurement unit (IMU). Methods: For this study, 33 participants (32 females and 1 male) were recruited. The subjects were passively positioned with the shoulder placed at specific angles using a goniometer (shoulder flexion $0^{\circ}-170^{\circ}$, abduction $0^{\circ}-170^{\circ}$, external rotation $0^{\circ}-90^{\circ}$, and internal rotation $0^{\circ}-60^{\circ}$ angles). Kinematic data on the shoulder joints were simultaneously obtained using IMU three-dimensional (3D) angular measurement (MyoMotion) and photographic measurement. Test-retest reliability and concurrent validity were examined. Results: The MyoMotion system provided good to very good relative reliability with small standard error of measurement (SEM) and minimal detectable change (MDC) values from all three planes. It also presented acceptable validity, except for some of shoulder flexion, shoulder external rotation, and shoulder abduction. There was a trend for the shoulder joint measurements to be underestimated using the IMU 3D angular measurement system compared to the goniometer and photo methods in all planes. Conclusion: The IMU 3D angular measurement provided a reliable measurement and presented acceptable validity. However, it showed relatively low accuracy in some shoulder positions. Therefore, using the MyoMotion measurement system to assess shoulder joint angles would be recommended only with careful consideration and supervision in all situations.

The purpose of this study was to analyze the shoulder joint torque and muscle pattern performing as Tennis serve by Isokinetic motions on Isomed 2000. The subject, who was 5 males Korean national high school tennis players. By analyzing the flexion/extension of the shoulder, the peak torque of the internal/external rotation motion(at 60,180 and 300 degree/sec) at peak torque degree, the weight, peak torque, and power. combined with the timing of the electrode of the attached trapezius and posterior deltoid at the three part. From the data analysis & discussion the following conclusions were drawn. When doing a shoulder extension, the peak torque can be widely seen at 60degree per second. However the degree may be different depending on angular velocity. When doing an internal rotation at 90degree abduction, peak torque per weight was seen at 60degree per second. The degree of peak torque was at 31.6-44.2 and peak power was faster when angular velocity was increased. The aspect of muscle pattern was seen more at the internal rotation in the 90degree abduction rather than the shoulder extension. However the angular velocity was not influenced by muscle mobilization(in order of anterior deltoid, posterior deltoid and trapezius. To properly apply the above conclusion, when tennis players serve it is better the elbow be impacted by the extension. when doing isokinetic motion it is better to increase angular velocity and improve muscle power. also the anterior deltoid amongst the shoulder muscle should be improved to develop serve speed.
This study used a three-dimensional motion analysis system for 15 elite tennis players (male 8, female7) to identify the relevance of scapula movement to shoulder pain. During the flat serve, the angular velocity and joint moment of scapula anterior/posterior tilt, downward/upward rotation, internal/external rotation were calculated and this was compared between groups. As a result, the maximum angular velocity for the anterior and posterior tilt tended to be higher in control group(CG) than in the shoulder pain group(SPG), and the maximum angular velocity for internal and external rotation in all phases except the follow-through phase was higher than that of CG. The maximum moment for the anterior and posterior tilt in the late coking phase was statistically significantly higher than that of SPG, the joint moment for the downward and upward rotation of the coking phase was statistically significantly lower than that of CG, and the moment for the internal and external rotation, the SPG was found to be lower than that of CG in the whole phases.
Background: Reverse total shoulder arthroplasty (rTSA) has gained popularity in recent years and is indicated for a wide variety of shoulder pathologies. However, use of rTSA in patients with "weight-bearing" shoulders that support wheelchair use or crutches has higher risk. The aim of this study was to assess the results of rTSA in such patients. Methods: Between 2005 and 2014, 24 patients (30 shoulders) with weight-bearing shoulders were treated with rTSA at our unit. Patients had cuff arthropathy (n=21), rheumatoid arthritis (n=3), osteoarthritis (n=1), acute fracture (n=3), or fracture sequela (n=2). Postoperatively, patients were advised not to push themselves up and out of their wheelchair for 6 weeks. This study was performed in 2016, and 21 patients (27 shoulders) were available for a mean follow-up of 5.6 years (range, 2-10 years). The mean age at surgery was 78 years (range, 54-90 years). Results: Constant-Murley score improved from 9.4 preoperatively to 59.8 at the final follow-up (P=0.001). Pain score improved from 2/15 to 13.8/15 (P=0.001). Patient satisfaction (Subjective Shoulder Value) improved from 0.6/10 to 8.7/10 (P=0.001). Significant improvement in mean range of motion from 46° to 130° of elevation, 14° to 35° of external rotation, and 29° to 78° internal rotation was recorded (P=0.001). Final mean Activities of Daily Living External and Internal Rotation was 32.4/36. Only three patients showed Sirveaux-Nerot grade-1 (10%) glenoid notching and three grade 2 (10%). Conclusions: rTSA can be used for treatment of patients with weight-bearing shoulders. Such patients reported pain free movement, resumed daily activities, and high satisfaction rates.
