Calcific Tendinitis of the Shoulder by Oriental Medical Treatments : 4 Cases Report (한방치료로 호전된 견관절의 석회화 건염 4례 보고)

  • This study was performed to report the effect of oriental medical treatments on the 4 patients with calcific tendinitis of the shoulder. The 4 patients were treated by cupping, acupuncture, pharmacopuncture on tender points of the greater tuberosity of humerus. The state of patients was measured by visual analog scale(VAS) and shoulder range of motion(ROM). After mean 3.25 times of treatments, the significant recovery of shoulder ROM was measured and the significant pain relief was seen as the average of VAS changed from 8.5 to 1. These results suggest that oriental medical treatments including cupping, acupuncture, pharmacopuncture are effective to improve the calcific tendinitis of the shoulder. However more rigorous study should be followed.

Effects of Rehabilitation Programs with Couching Exercise or Home Exercise on Shoulder Pain, Muscle Strength, and Daily Living Functions in Patients with Shoulder Impingement Syndrome (재활 프로그램을 적용한 코칭 운동과 자가 운동이 견관절 충돌증후군 환자의 견부통증, 근력 및 일상생활기능에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of rehabilitation programs with coaching exercise (CE) or home exercise (HE) on shoulder pain, muscle strength, and daily living functions (DLF) in patients with shoulder impingement syndrome. Methods: With quasi-experimental design, thirty patients (15 CE and 15 HE) participated in rehabilitation programs for 12 weeks. At first, data were collected for pain and DLF using questionnaires with testing muscle strength and range of motion. Then, subjects were allowed to receive training on rehabilitation with structured training materials. CE group visited sports center three times a week at P hospital and HE group did self-exercise at home. Data were analyzed using SPSS program. Results: After 12 weeks, both groups showed statistically significant changes in improving pain, muscle strength, and DLF. Compare to HE group, CE group showed a statistically significant improvement in shoulder pain (p=.021), muscle strength in internal rotation (p=.001) and abduction (p=.013) and DLF (p=.012). Conclusion: Rehabilitation programs for patients with shoulder impingement syndrome were effective to improve pain, muscle strength, and DLF. Although CE would bring the better effects, it is possible to apply HE through structured education to patients who are difficult to visit hospitals.

  • Objectives : This study was performed to evaluate the effects of thoracic vertebral region's Du mai(督脈) and Jia ji xue(夾脊穴) intervention on active range of motion(ROM) restricted shoulder pain. Methods : 1. Measurement : The unpleasantness of pain was measured by visual analogue scale(VAS), and ROM was measured by using Goniometer and scratch test. 2. Intervention : Use Acupuncture and bee venom injection in thoracic vertebral region's Du mai(督脈) and Jia ji xue(夾脊穴) that display main tender points. Results : The pain and ROM were improved after thoracic vertebral region intervention. Conclusion : Thoracic vertebral region intervention was effective to active ROM restricted shoulder pain, to reduce the pain and to improve ROM. And this study shows that active ROM restricted shoulder pain and thoracic vertebral region's tender points are connected to each other. Also, region that suggest main tender points previews region of thoracic vertebrae 5-8.

  • This study is to report the effectivenes of Meridian-Test (M-test) in 2 cases on shoulder diseases. The M-test was applied to one patient diagnosed with as impingement syndrome and one patient diagnosed with tear of supraspinatus tendon. The changes in numeral rating scale and range of motion were identified to assess the progress of the treatment. After three times treatments, In example 1, the pain decreased from 6 to 0.5 and the range of flexion and abduction increased from $150^{\circ}$ to $180^{\circ}$. In example 2, the pain decreased from 6.5 to 1, and the range of shoulder flexion increased from $120^{\circ}$ to $170^{\circ}$, while the range of abduction increased from $90^{\circ}$ to $170^{\circ}$. Based on the above results, it would be effective to apply M-test to treat shoulder diseases in these two cases.

  • Objectives The objective of this study is to report the improved case of Korean medicine treatment with Shinbaro3 pharmacopuncture for supraspinatus tendinitis and subdeltoid bursitis. Methods We used Shinbaro3 pharmacopuncture with other Korean medicine treatment (acupuncture, herbal medicine, cupping and chuna) for this patient. This case is measured and assessed by Numerical Rating Scale (NRS), shoulder range of motion (ROM) and shoulder physical examination. Results The NRS decreased from 10 to 2 and the patient showed nearly full shoulder range of motion. Also physical examination of shoulder was improved. Conclusions Korean medicine treatment with Shinbaro3 pharmacopuncture is proved to be helpful to relieve pain and recover function on shoulder joint.

Musculoskeletal Ultrasound Findings of Shoulder Pain Patients: A Retrospective Observational Study (근골격계 초음파 진단기기를 활용한 견관절 통증 환자의 초음파 소견: 후향적 관찰 연구)

  • Objectives The aim of this study is to retrospectively evaluate the shoulder lesions in patients experiencing shoulder pain through the use of musculoskeletal ultrasound during Korean medicine intervention treatments. Methods A total of 20 cases were collected, including biceps tendinitis (n=4), calcification (n=3), SASD bursitis (n=4), partial tear (n=2), full-thickness tear (n=1), tendinopathy (n=5), and impingement syndrome (n=1). Musculoskeletal ultrasound was used by Korean medicine doctors to perform real-time scanning and to explain the patient's condition during treatment sessions. Results The use of musculoskeletal ultrasound allowed Korean medicine doctors to perform treatments such as pharmacoacupuncture, acupotomy, and acupuncture more safely and effectively. Patients were able to better understand their conditions through real-time imaging and explanations provided by the doctors. Conclusions Musculoskeletal ultrasound has the potential to enhance the safety and efficacy of Korean medicine intervention treatments for patients with shoulder pain. Institutional or governmental support is needed to further promote the use of medical devices by Korean medicine doctors, ultimately leading to an increase in cases and clinical evidence.

The Effect of Scapular Girdle Taping on Hemiplegic Shoulder Pain; A Clinical Study (테이핑이 편마비로 인한 어깨 통증에 미치는 영향)

  • OBJECTIVES: In order to study the effect of scapular girdle taping on hemiplegic shoulder pain, a clinical study was performed. . METHODS: Forty patients with hemiplegic shoulder pain were randomized into two groups. Twenty patients underwent a standard acupuncture treatment for shoulder pain (named control group). The other twenty patients underwent the same acupuncture treatment, but scapular girdle taping was added after acpuncture treatment (named sample group). Tapes were applied to the supraspinatus, infraspinatus, deltoid and pectoris major muscles. Shoulder pain was measured with a visual analog scale. The acupuncture treatment was performed daily for 3 weeks, and the scapular girdle taping was performed for 3 weeks in the frequency of 2 times a week. RESULTS: In terms of improvement of pain, the sample group showed statistically meaningful decrease after 3 times treatment compared with control group (P<0.05). In terms of improvement of pain with shoulder subluxation, the sample group showed a statistically meaningful decrease after 3 treatments compared with the control group (P<0.05). CONCLUSIONS: These results support that scapular girdle taping is significantly effective in reducing hemiplegic shoulder pain.

A Systematic Review and Meta-Analysis of Herbal Medicine for Frozen Shoulder (동결견의 한약 치료에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives The objective of this study is to provide the evidence of the effectivness of herbal medicine for frozen shoulder. Methods 2 Korean medical on-line databases (Oriental Medicine Advanced Searching Integrated System, Korean Traditional Knowledge), and 3 foreign databases (Pubmed, Cochrane library, China National Knowledge Infrastructure) were searched to find articles concerning herbal medicine for frozen shoulder. We selected randomized controlled trials (RCTs). Several repeated articles and those not relevant to the topic were excluded, as well as review articles and commentaries. The methodological quality of RCTs were evaluated using the Cochrane risk of bias tool and meta-analyes were perfomed. Results Total 474 studies were founded and 22 RCTs were selected for systematic review. Efficiency rate was used as the primary evaluation method. Almost studies reported that herbal medicine has significant effect on pain reduction, increasing shoulder function on frozen shoulder. 3 studies comparing herbal medicine and chuna with chuna and 3 studies comparing herbal medicine with ibuprofen were included in the meta-analysis. At herbal medicine and chuna with chuna meta-analysis, it showed positive results of herbal medicine for efficiency rate (risk ratio: 1.18, 95% confidence interval: 1.09-1.27, p<0.0001). At herbal medicine with ibuprofen medta-analysis, it showed positive results of herbal medicine for efficiency rate (risk ratio: 1.15, 95% confidence interval: 1.05-1.27, p=0.003). Conclusions The studies showed that herbal medicine can significantly effective on frozen shoulder. However, the risk of bias in RCTs were evaluated as uncertain. In the future, further well-designed RCTs are needed to prove the effectiveness of herbal medicine for frozen shoulder and reduce the risk of bias.

Changes in Range of Motion after Intra-Articular Corticosteroid Injection in Frozen Shoulder: A Retrospective 3-Month Follow-Up Study (동결견 환자에서 관절강내 스테로이드 주사 후 관절가동범위의 변화)

  • Objective: To examine (1) the degree of reduction of passive range of motion (PROM) on the affected side compared to that on the unaffected side and (2) the degree of increase in PROM following intra-articular corticosteroid injection (IACI) in patients with frozen shoulder. Method: The medical records of 120 patients with frozen shoulder were retrospectively reviewed. PROM of the unaffected and affected shoulder (flexion, extension, abduction, internal rotation, external rotation) was compared, and changes in PROM of the affected shoulder after a single IACI (triamcinolone 20 mg) were evaluated after 12 weeks. Results: At the time of diagnosis, PROM of the affected shoulder was most limited in external rotation, followed by internal rotation, abduction, extension, and flexion, compared to that of the unaffected shoulder. Compared to before IACI, PROM of external rotation demonstrated the greatest increase compared to all the other movements after IACI. Conclusion: Limitation in PROM of the frozen shoulder at the time of diagnosis was greatest for external rotation. Moreover, external rotation experienced the greatest improvement after IACI. Our findings should help to further clarify the clinical characteristics of frozen shoulder, aid in its diagnosis, and allow the prediction of the effects of IACI.

Early and Delayed Postoperative Rehabilitation after Arthroscopic Rotator Cuff Repair: A Comparative Study of Clinical Outcomes

  • Background: The duration of immobilization after arthroscopic rotator cuff repair and the optimal time to commence rehabilitation are still the subject of ongoing debates. This study was undertaken to evaluate the functional outcome and rotator cuff healing status after arthroscopic rotator cuff repair by comparing early and delayed rehabilitation. Methods: Totally, 76 patients with small, medium, and large sized rotator cuff tears underwent arthroscopic repair using the suturebridge technique. In early rehabilitation group, 38 patients commenced passive range of motion at postoperative day 2 whereas 38 patients assigned to the delayed rehabilitation group commenced passive range of motion at postoperative week 3. At the end of the study period, clinical and functional evaluations (Constant score, the University of California, Los Angeles [UCLA] shoulder score) were carried out, subsequent to measuring the range of motion, visual analogue scale for pain, and isokinetic dynamometer test. Rotator cuff healing was confirmed by magnetic resonance imaging at least 6 months after surgery. Results: No significant difference was obtained in range of motion and visual analogue scale between both groups. Functional outcomes showed similar improvements in the Constant score (early: 67.0-88.0; delayed: 66.9-91.0; p<0.001) and the UCLA shoulder score (early: 20.3-32.3; delayed: 20.4-32.4; p<0.001). Furthermore, rotator cuff healing showed no significant differences between the groups (range, 6-15 months; average, 10.4 months). Conclusions: Delayed passive rehabilitation does not bring about superior outcomes. Therefore, early rehabilitation would be useful to help patients resume their daily lives.
