The Effect of Proprioceptive Neuromuscular Facilitation Rehabilitation Exercise on Range of Motion, Pain, and Function of Breast Cancer Patients after Surgery (고유수용성신경근촉진법 재활운동이 수술 후 유방암 환자의 관절운동범위와 통증 및 기능에 미치는 영향)

  • Purpose: This study examined the effect of proprioceptive neuromuscular facilitation (PNF) rehabilitation exercise on postoperative range of motion (ROM), pain, and function of breast cancer patients. The purpose of this study was to provide fundamental data regarding the use of PNF rehabilitation exercise for patients with breast cancer. Methods: Fourteen patients who underwent surgery due to breast cancer were randomly divided into an experimental group (n=7) that performed PNF rehabilitation exercise and a control group (n=7) that performed general rehabilitation exercise. Both groups performed the respective exercises for 30 minutes, five times a week for 4 weeks. For the measurement of ROM, the range of shoulder abduction was measured using a clinometer smartphone application. The shoulder pain and disability index (SPADI) was used to measure the level of pain and functional activity. A paired t-test was conducted to compare within-group changes before and after the PNF rehabilitation exercise. Differences between the experimental group and control group were analyzed by an independent t-test. For all tests, the level of statistical significance was set at ${\alpha}=0.05$. Results: After the exercise, there was a significant within-group change in the ROM and SPADI in both the experimental group and control group (p<0.01). There was also a significant between-group difference in the ROM and SPADI after the intervention (p<0.05). Conclusion: Rehabilitation exercise is generally applied as a treatment for patients with breast cancer after surgery and is relatively effective. The application of PNF rehabilitation exercise may be useful in such patients, considering its effects on ROM improvement, pain reduction, and functional enhancement.

  • Tje purpose of this study is to understand body as a whole concept through examining effects of distant shoulder and neck with GCM ankle joint therapy. The twenty three subject with ankle joint therapy and comparison groups without ankle joint therapy were authorized the score of before and after therapy by correspondence T-test using pump lines and tapes as instruments. The results are as follows. 1. The test group had change of shoulder in measurement 1, 2, 3 and were regarded statistically 1st measurement on left, 2nd on right.(p<.05) 2. The test group had change of neck in measurement 1, 2, 3 and were regarded statistically 2nd on left on right.(p<.05) 3. In the comparison group the comparison of shoulder height was not regarded statistically all at 1, 2 comparison on left and regarded statistically all at 1, 2 comparison on right.(p<.05) 4. In the comparison group the comparison of neck height was not regarded statistically both left and right at 1, 2 comparison.(p<.05)

The Effect of Neural Mobilization on the Grip Strength (견갑대 안정화 운동이 상지 근력에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate effects of shoulder girdle stability exercise on upper extremity muscle strength. Methods : Subjects were consist of 20 people who had no disorder of upper extremity from 20 to 27 years of age during 6 weeks from April 3, 2006 to May 14. Ten of all subjects exercised muscles which stabilize to shoulder girdle for 30 minutes during 6 weeks. Biodex was used to measure upper extremity muscle strength. Results : There are increasing of shoulder flexion and extension on average torque but no statistically significant difference between pre-exercise and post-exercise. Conclusion : There are no severe difference between exercise group and non-exercise group on muscle strength.

  • Objectives : The purpose of this report is to prove the effect of The Daoyin exercise program combined with Korean medicine treatment on supraspinatus tendon tear. Methods : Two patients, diagnosed with Supraspinatus tendon tear by shoulder MRI, were treated with The Daoyin exercise program and Korean medicine treatment. VAS(Visual analog scale), ROM(Range of motion) and Physical examination were used to evaluate patients. Results of application : The Daoyin exercise program combined with Korean medicine treatment reduced shoulder pain and improved shoulder ROM of the two patients significantly. Conclusions : The Daoyin exercise program combined with Korean medicine treatment is proved to be effective treatment for reducing pain and improving shoulder ROM of supraspinatus tendon tear patients.

  • The purpose is paper was to discuss current concepts related to anatomic stabilizing structures of the shoulder joint complex and their clinical relevance to shoulder instability. The clinical syndrome of shoulder instability represents a wide spectrum of symtoms and signs which may produce various levels of dysfunctions, from subtle subluxations to gross joint instability. The glenohumeral joint attains functional stability through a delicate and intricate interaction between the passive and active stabilizing structures. The passive constraints include the bony geometry, glenoid labrum, and the glenohumeral joint capsuloligaments structure. Conversely, the active constraints, also referred to as active mechanism, include the shoulder complex musculature, the projprioceptive system, and the musculoligamentous relationship. The interaction of the active and passive mechanism which provide passive and active glenohumeral joint stability will be throughtly discussed in this paper

  • Objectives : This study was performed to evaluate the effects of oriental medical treatments and Chuna treatment on patients with pain and limited range of motion(LOM) about shoulder subsequent to scapular fracture. Methods : Two patients suffered from pain and LOM about shoulder after scapular fracture, one is scapular body fracture with multiple rib fractures, the other is only intraglenoid fracture were treated with Chuna therapy, acupuncture, herbal medicine, physical therapy and measured by VNRS(Verbal numerical rating scale) and ROM (Range of motion). Results : After oriental medical treatments and Chuna treatment, we found out a recovery from two patients suffered from pain and LOM about shoulder subsequent to scapular fracture. Conclusions : Through this study, we suggest that oriental medical treatments and Chuna treatment were effective to cure patients with pain and LOM subsequent to scapular fracture.

The Shoulder Pain after Stroke and the relationship with Motor Function, and Quality of Life (뇌졸중 환자의 견관절 통증과 운동 기능 및 삶의 만족도와의 관계)

  • Purpose : To assess the relationship between post-stroke shoulder pain, motor function, and pain-related quality of life(QOL) Methods : Volunteer sample of 62 chronic stroke survivors with post-stroke shoulder pain and glenohumeral subluxation. The patients answered the question in shoulder pain with the Brief Pain Inventory question 12 (BP1-12), Pain-related Quality of life(BPI-23). Therapists measured the performance of combined upper-limb movement including the hand-behind-neck(HBN), hand-behind-beck(HBB) maneuver, added passive pain-free shoulder external rotation range of motion, and Modified Ashworth Scale(MAS) score of the elbow flexors. Physical performance assessments were used to measure basic activity daily living(Modified Barthel Index-self care, MBI-S/C), motor function of upper limb(Fugl-Meyer Upper/Lower Extremity, FM-U/E). Results : Stepwise regression analyses indicated that post-stroke shoulder pain is associated with the BPI 23, but not with the FM-U/E, MBI-S/C. Thus, the presence of shoulder pain is more important predicting pain-related QOL than its degree in predicting motor function of upper limb and basic activity daily living. Conclusion : Post-stroke shoulder pain was associated with reduced quality of life related to pain. The pain was not associated with the motor function of upper limb and basic activity daily living. The result imply that management of shoulder pain & anatomical position of shoulder joint after stroke should be emphasized. This provides a further incentive to develop effective rehabilitation prevention and treatment strategies for post-stroke shoulder pain.

The Impact of Shoulder Flexion Angle on Hand Grip Strength in Male and Female Undergraduate Students (견관절 굴곡 각도가 남·녀 대학생의 악력 변화에 미치는 영향)

  • Purpose : This study's purpose is consideration about change of the hand grip strength according to different posture and shoulder flexion angle. The shoulder joint permits the greatest mobility and carries out the important function of stabilization for hand use. Hand grip activity is important to evaluate while assessing loads of shoulder in hand mobilities. Methods : Thirty(15 male, 15 female) college students with unknown shoulder dysfunction participated subject in five different positions of elbow extension with sitting and standing posture, different positions is followed : (1) shoulder $0^{\circ}$ flexion (2) shoulder $45^{\circ}$ flexion (3) shoulder $90^{\circ}$ flexion (4) shoulder $135^{\circ}$ flexion (5) shoulder $180^{\circ}$ flexion. Results : On the average, in the hand grip strength, the standing posture is higher than sitting posture. Sitting posture showed a most high level at the man's $0^{\circ}$ and woman's $135^{\circ}$. And standing posture showed a most high level at the man's $135^{\circ}$ and woman's $90^{\circ}$. Conclusion : The paired t-test was used to determine the different in grip strength between sitting and standing posture by shoulder angle change. There was no significant difference between the five position by sitting and standing posture. In man, correlation analysis revealed significant connection for all five position by sitting and standing posture. And in woman, correlation analysis revealed connection for all five position by sitting and standing posture.

  • The purpose of the study was to investigate the effectiveness of ankle mobilization on neck and shoulder position. One hundred volunteers, aged between 18 and 26 years (mean age 22), were recruited and each subject was divided into four mobilization groups by their body type, which is based on the concept from 'general coordinative manipulation' and a control group. Ankle mobilization was applied based on their body type and no mobilization was applied on those of the control group. The positions of shoulders and neck were measured in comfortable standing posture. All measurements were taken before and after ankle mobilization from each subject in mobilization groups and those of control group was measured twice between ten minutes by a different tester on three different occasions. Twenty subjects were in each group basis of their body type and a control. In the position of neck and shoulder, all subjects regardless group showed significantly changed their neck

  • Objectives : This study was designed to analyze the pattern of asymmetrical alignment. Methods : This study was carried out with the data from comprehensive medical testing. 91 subjects aged 30-39 were evaluated by full spine AP X-ray. For pelvis, innominate measurement(IM), off centering measurement(OCM), ilium shadow measurement(ISM), major axis of obturator foramen(MaF), minor axis of obturator foramen(MiF) were analyzed. Spinal curvature and height of shoulder girdle were analyzed. Results : 1. In pelvis, It. posterior-inferior and it. inflare combination pattern was 38 cases(42.8%). 2. In spinal curvature, "reverse S" curve was 45 cases(49.4%) and "reverse C" curve was 30 cases(33%). 3. In shoulder girdle, It. superior pattern was 42 cases(46.1 %) and It. superior pattern was 39 cases(42.9%). 4. In whole body analysis, It. posterior-inferior and It. inflare pelvis, "reverse S" spinal curvature and It. superior shoulder girdle combination patten was 11 cases(12.1 %). This pattern is similar to Kendall's right handedness pattern and Zink's common compensatory pattern. Conclusions : Results from this investigation showed asymmetrical alignment in 30-39 years-old adults. This results are expected to contribute to classifying the alignment pattern in clinic and systemic treatment.
