  • Disorders of the shoulder in which conservative management is indicated as a primary treatment include idiopathic frozen shoulder, atraumatic instability. acute calcific tendinitis , subacromial impingement syndrome, partial thickness or small full thickness tear of the rotator cuff, and chronic rotator cuff tear in elderly population. Aside from medication and various physical modalities used for relief of pain, the mainstay of orthopaedic rehabilitation consists of stretching of the tendons and capsulologamentous complex, and strengtnening of rotator cuff muscles as well as surrounding muscles. However, orthopaedic rehabilitation in chronically painful conditions of the shoulder differs in detail according to the specific diseases or injuries, And it is important to make the rehabilitation program that can be undertaken by the patients rather than adhering to a fixed schedule. Also the program should be changed as the condition of the patient improves or deteriorates. Nevertheless there are basic rules of stretching and strengthening , which should always be observed in applying orthopaedic rehabilitation treatment.

  • In this paper, we developed a shoulder rehabilitation exercise device and monitoring system to remotely provide rehabilitation system for the ones who need shoulder exercises including the patients with rotator cuff rupture. In order to evaluate the severity of shoulder muscle injury, a total of 4 shoulder rehabilitation exercises ((3) shoulder abduction, (2) shoulder flexion, (3) shoulder abduction with elbow flexion, (4) shoulder extension with elbow flexion) were selected and instructed to be performed with a 3 kg dumbbell for 5 times. For EMG (electromyogram) signal analysis, each subject's maximum voluntary contraction (MVC) was measured. EMG signals reflect the activation level of contracting muscles during dynamic exercises. Six participants' muscle activation levels in posterior deltoid, middle deltoid, upper trapezius, and infraspinatus were measured and compared. The mean power spectrum values in the time and frequency domains were compared between two age-matched groups (20s and 50s). The results showed lower muscle activation in the elderly subjects (n=3) compared to that of the ones in their twenties (n=3).

  • PURPOSE: This study analyzed the trends and characteristics of shoulder rehabilitation research through keyword analysis, and their relationships were modeled using text mining techniques. METHODS: Abstract data of 10,121 articles in which abstracts were registered on the MEDLINE of PubMed with 'shoulder' and 'rehabilitation' as keywords were collected using python. By analyzing the frequency of words, 10 keywords were selected in the order of the highest frequency. Word-embedding was performed using the word2vec technique to analyze the similarity of words. In addition, the groups were classified and analyzed based on the distance (cosine similarity) through the t-SNE technique. RESULTS: The number of studies related to shoulder rehabilitation is increasing year after year, keywords most frequently used in relation to shoulder rehabilitation studies are 'patient', 'pain', and 'treatment'. The word2vec results showed that the words were highly correlated with 12 keywords from studies related to shoulder rehabilitation. Furthermore, through t-SNE, the keywords of the studies were divided into 5 groups. CONCLUSION: This study was the first study to model the keywords and their relationships that make up the abstracts of research in the MEDLINE of Pub Med related to 'shoulder' and 'rehabilitation' using text-mining techniques. The results of this study will help increase the diversifying research topics of shoulder rehabilitation studies to be conducted in the future.

  • Objective: This study aimed to elucidate the effects of thoracolumbar fascia release (TLFR) on the degree of pain and disability in patients with shoulder pain. Design: Randomized control trial. Methods: Thirty subjects with shoulder pain participated in this study. They were allocated to TLFR group (n=15) and manual physical therapy (MPT) group (n=15). Shoulder pain and disability index (SPADI) and the score on the visual analogue scale (VAS) were measured before and after TLFR. Results: In the TLFR group, the degree of shoulder pain as indicated by SPADI measured after the intervention significantly differed from that before the intervention (p<0.05); moreover, in the MPT group, the degree of shoulder pain was significantly lower (p<0.05). The data of the 2 groups before the intervention significantly differed from those after the intervention (p<0.05). SPADI significantly differed within the groups (p<0.05), but not between the groups. The sum of SPADI did not differ significantly between the groups. The VAS scores of shoulder pain measured before the intervention significantly differed from those measured after the intervention (p<0.05) in the both groups. After the intervention, shoulder pain decreased significantly in the TLFR group as compared to that in the MPT group. Conclusions: TLF release was effective in reducing shoulder pain. The results of this study can be applied in clinical practice for TLFR performed to reduce shoulder pain. Further studies will need to be performed to elucidate the effects of TLFR on functional recovery.

  • Objective: The forward head posture (FHP) is strongly related to the rounded shoulder posture (RSP), which is associated with shoulder pain. Design: Observational cross sectional study design Methods: A total of 37 were enrolled in the study, 22 individuals with FHP(experimental group) and 15 healthy adults(control group). Correlation with differences between groups was analysed through craniovertebral angle (CVA) representing FHP for both groups, neck disability index (NDI) indicating neck pain, disability of the arm, shoulder and hand (DASH) indicating shoulder pain. Results: There was a significant difference in the results of CVA, NDI, and DASH in FHP and healthy adults (p<0.05). Significant correlations were found between DASH and CVA in FHP participants (r = -0.656, p = 0.001). Also, in the regression analysis results of DASH and CVA, the regression model was found to be suitable and the variation in DASH could be explained by 43% (F = 15.118, p = 0.001). Conclusions: Shoulder pain and neck discomfort are potentially related, and an increase in shoulder pain can increase FHP.
