Effects of Upper Trapezius Inhibition Dynamic Taping on Pain, Function, Range of Motion, Psychosocial Status, and Posture of the Neck in Patients With Chronic Neck Pain (위등세모근 억제 다이나믹 테이핑이 만성 목 통증 환자들의 목 통증, 기능장애 수준, 관절가동범위, 심리사회적 수준, 목 자세정렬에 미치는 영향)

  • Background: Neck pain is a major health problem in developed countries and has a lifetime prevalence of 50%. Major problems include a reduced cervical range of motion, muscle stiffness, dysfunction, postural changes, and decrease in psychosocial level. Objects: This study aimed to investigate the effects of applying the upper trapezius inhibition dynamic taping to patients with chronic neck pain on their neck pain, functional level, cervical range of motion, psychosocial level, and neck posture. Methods: The study design was a randomized controlled trial. A total of 40 patients with neck pain participated in this study and were randomly assigned to a Dynamic Taping group (n = 20) or Sham Taping group (n = 20). In both groups, basic intervention cervical pain control therapy and shoulder stabilization exercise program were performed. In addition, dynamic taping and sham taping were applied to participants in the Dynamic Taping and Sham Taping groups to inhibit the trapezius muscle, respectively. All interventions were performed three times a week and a total of 12 times for 4 weeks, and the participants' neck pain, functional impairment level, cervical range of motion, psychosocial level, and neck posture were measured and compared before and after the intervention. Results: Both groups showed significant differences in neck pain, functional level, cervical range of motion, psychosocial level, and neck postural before and after the intervention (p < 0.05). Moreover, there were significant differences between the two groups regarding the functional level and neck posture (p < 0.05). Conclusion: Inhibition dynamic taping of the upper trapezius muscle suppression is an effective method with clinical significance in reducing pain in individuals with chronic neck pain and improving the functional level, cervical range of motion, psychosocial level, and neck posture.

A deep and multiscale network for pavement crack detection based on function-specific modules

  • Using 3D asphalt pavement surface data, a deep and multiscale network named CrackNet-M is proposed in this paper for pixel-level crack detection for improvements in both accuracy and robustness. The CrackNet-M consists of four function-specific architectural modules: a central branch net (CBN), a crack map enhancement (CME) module, three pooling feature pyramids (PFP), and an output layer. The CBN maintains crack boundaries using no pooling reductions throughout all convolutional layers. The CME applies a pooling layer to enhance potential thin cracks for better continuity, consuming no data loss and attenuation when working jointly with CBN. The PFP modules implement direct down-sampling and pyramidal up-sampling with multiscale contexts specifically for the detection of thick cracks and exclusion of non-crack patterns. Finally, the output layer is optimized with a skip layer supervision technique proposed to further improve the network performance. Compared with traditional supervisions, the skip layer supervision brings about not only significant performance gains with respect to both accuracy and robustness but a faster convergence rate. CrackNet-M was trained on a total of 2,500 pixel-wise annotated 3D pavement images and finely scaled with another 200 images with full considerations on accuracy and efficiency. CrackNet-M can potentially achieve crack detection in real-time with a processing speed of 40 ms/image. The experimental results on 500 testing images demonstrate that CrackNet-M can effectively detect both thick and thin cracks from various pavement surfaces with a high level of Precision (94.28%), Recall (93.89%), and F-measure (94.04%). In addition, the proposed CrackNet-M compares favorably to other well-developed networks with respect to the detection of thin cracks as well as the removal of shoulder drop-offs.

Molecular Structure and Gelatinization Properties of Turnip Starch (Brassica rapa L.)

  • Starch was isolated from turnip (Brassica rapa L.), and to elucidate the structure-function relationship its structural and physical properties were characterized. Morphological structure of the starch was analyzed by SEM (Scanning Electron Microscopy). Most of the starch granules were spherical in shape with diameter ranging from 0.5-10mm. Apart from larger granules ($<10\;{\mu}m$) which dominated the population size of turnip starch, significant amount of small ($0.5-2\;{\mu}m$) and mid-size granules (${\sim}\;{\mu}m$) were also detected. It was revealed that presumably, erosion damages occurred due to the attack of amylase-type enzymes on the surface of some granules. Branch chain-length distribution was analyzed by HPAEC (High-Performance Anion-Exchange Chromatography). The chain-length distribution of turnip starch revealed a peak at DP12 with obvious shoulder at DP18-21. The weight-average chain length ($CL_{avg}$) was 16.6, and a large proportion (11.8%) of very short chains (DP6-9) was also observed. The melting properties of starch were determined by DSC (Differential Scanning Calorimetry). The onset temperature ($T_o$) and the enthalpy change (${\Delta}H$) of starch gelatinization were $50.5^{\circ}C$ and 12.5 J/g, respectively. The ${\Delta}H$ of the retrograded turnip starch was 3.5 J/g, which indicates 28.2% of recrystallization. Larger proportion of short chains as well as smaller average chain-length can very well explain relatively lower degree of retrogradation in turnip starch.

  • As an idea of healing or fatigue recovery, a desire for massage therapy has increased for modern people who complain of pain such as knotted muscles or shoulder discomfort due to fatigue accumulation and stress. To resolve this, although a massage chair has been developed, it does not provide the most efficient massage as it is not suitable for Korean body types. In this paper, this research analyzed massage, assembly, massage intensity, Korean body types and functions of the massage for maximized efficiency to design critical functions. Also, we implemented the action control and algorithm of the massage chair for localization.

  • The fashion items of the "Rainy Look" has become fashion items that could be worn in any weather. Accordingly, there is a need for this study to research the fashion items of the Rainy Look as its influence is becoming big enough to impact modern fashion and expand into the high fashion and street fashion. First of all, the purpose of this study is to analyze the fashion items of the Rainy Look that were represented in the high fashion and street fashion of 21st century. And the analysis was performed using the following categories: the functional aspect, aesthetic aspect, and the formativeness of fashion items. Through this, the study provided fashion styling materials using the Rainy Look fashion items. This study examined the concepts and characteristics of the Rainy Look as a literature study and analyzed its characteristics, which appeared in collections and street fashions. Especially as attires coordinated with business casuals in the street fashion, they appeared often among commuting office workers. And the conveniences of having a string in the waistline or securing storage space or detachable function. On the contrary, traditional raincoat designs appeared often in high fashion and as feminine styles, which is characterized by a slimmed waist with a use of a belt. and a flared lower part, such as shoulder patch or strap.

The Effects of the Systematic Breathing Exercises Program on Recovery of Patients with Pneumothorax (체계적인 호흡운동 프로그램이 기흉환자의 회복에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of the systematic breathing exercise program on recovery of patients with pneumothorax. Methods: An nonequivalent interrupted time-series control group posttest design was used. Participants were 40 inpatients (Experimental Group; 20, Control Group; 20) at the one University Hospital in U city. The systematic breathing exercise program including education on deep breathing exercise using incentive spirometry, Range of motion (ROM) exercise in shoulder joint, walking exercise and feedback were provided to the experimental group, while the control group carried out deep breathing exercise using incentive spirometry. The duration of chest tube insertion, duration of hospitalization, and frequency of analgesics use were measured. The data were analysed by a SPSS/WIN program. Results: The duration of chest tube insertion and duration of hospitalization in the experimental group were significantly shorter than the control group. However, there is no difference of the frequency of analgesics use between the experimental group and control group. Conclusion: The result showed that the systematic breathing exercise program was effective to improve recovery of patients with pneumothorax. This program can be applied in hospitals for patients with pneumothorax as one of the nursing intervention modalities.

  • The purpose of the study was to investigate types of uniforms which were considered as appropriate, functional, and/or power look. Also, the possibility of acceptance of a pantsuit for uniforms was examined. For the study, a questionnaire was developed by the researcher. The subjects were asked to select appropriate, function Al, or power look among combinations of: 1) two types of jackets with different shoulder width (natural, wide) and two types bottoms(pants, skirt) : 2) blouses with three different collars (ribbon, convertible, tailored) and two types of bottoms (pants, skirt) . The majority (89%) of subjects were 185 male and 182 female white-collar workers. For the ststistical analysis, Chi-square, one-way ANOVA, and t-test were used. The results of the study were: 1) For a uniform, a skirted suit was considered as being appropriate and powerful, and pants were regarded as being functional. 2) A jacket with natural shoulders was viewed as an appropriate and functional uniform, and a jacket with wide shoulders as power look. 3) A blouse with a convertible collar was regarded as an appropriate and functioal uniform, and a tailored collared-blouse evaluated as power look.

  • Background and Purpose : Medical and Neurological complications in patients with acute stroke may affect their prognosis including death rate, function and admission period. Some of these complications may be preventable. But few data exist regarding complications occurring in the first weeks. So we sought to observe the type, timing, and frequency of complications in hospitalized patients after stroke. Methods : We retrospectively examined the case notes of patients admitted in Won Kwang Oriental Medical Hospital after stroke. Two observers inspected the case notes using predifined diagnostic criteria and recorded the type, timing, and frequency of complications that occurred during the inpatient period. Results : Complications were recorded in 43 patients(82.7%). The most common medical complications were constipation(25.0%) and shoulder pain(21.2%). The most frequent serious medical problems were pulmonary infection(9.6%) and UTI(7.7%). The most common neurological complications were insomnia(34.6%) and dysphagia(23.7%). The most frequent serious medical problem was mental deterioration(7.7%). Conclusion : Complications after acute stroke are common. There were more medical complications than neurological complications. So we should compile much knowledge about medical complications and treat them actively. The differences between our study and previous studies are attributable to the different methods including patient selection and diagnostic criteria.

Clinical Features of the TMD Patients with Degenerative Joint Disease (퇴행성 악관절장애환자의 임상양태에 관한 연구)

  • An Epidemiologic study was carried out on 77 TMD patients with degenerative joint disease who had visited the Orofacial Pain Clinic in Pusan National University Hospital. Al subjects were interviewed and examined clinically and radiologically using a standardized examination form. As related to gender and duration, subjective and objective sysmptoms in DJD patients were studied. The obtained results were as follows : 1. There were much more patients in the twenties or thirties, women and histories such as chronic duration and microtrauma. 2. Most patients responded positively more often to the questions of jaw function, unilateral chewing in habits, poot appetite and depression in behavioral response and shoulder pain in worsening prognosis 3. While the most common reasons for treatment were pain, noise, and limitation of opening, the associated symptoms such as headache, neckache, earache, jaw dysfunction, neck dysfunction, acute bite change and dizziness, ringing or fullness in the ears as secondary CNS excitatory effects were complained. 4. Opening the mouth in 25 to 40mm, soft end feel and deflective incisal pathway were seen and more tenderness to lateral or dorsal capsule of joint than intra or extra oral muscles were complained. 5. While there appeared no click, crepitus and single click in acute group, in chronic group, crepitus, single click and no click appeared in order of sequence. 6. Tomogram or bone scan revealed more bony changes than panorama and transcranial view.

Extracorporeal Shock Wave Therapy for Myofascial Pain Syndrome of Upper Trapezius (충격파치료를 이용한 상부 등세모근의 근막통증증후군 치료)

  • Purpose : The purpose of this study was to evaluate whether extracorporeal shock wave therapy (ESWT) is an effective treatment for myofascial pain syndrome (MPS) of upper trapezius. Methods : Study subject were 20 patients (7 male and 13 female). The patients were evaluated by assessing of pain and function using visual analog scale (VAS) and pressure pain threshold (PPT) and Constant-Murley scale (CMS). Statistical analysis was used paired T-test to know significance probability between pre-test and post-test. Results : There was no statistical significance in the change in PPT. But, VAS scores were significantly improved after ESWT (p<.05). Using the Constant-Murley score, this study showed a significant decrease in pain and a significant increase in range of motion of shoulder (p<.05). Conclusion : These results indicate that extracorporeal shock waves therapy could be considered as an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.
