The Effectiveness of Early Tracheostomy (within at least 10 Days) in Cervical Spinal Cord Injury Patients

  • Objective : This study aimed to determine the optimal time for tracheostomy by evaluating the benefits and safety of early versus late tracheostomy in spinal cord injury (SCI) patients. Methods : We retrospectively reviewed a total of 254 patients with spinal cord injury. Of them, we selected 21 spinal cord injury patients who required tracheostomy due to long-term mechanical ventilation and analyzed their medical records. The patients were categorized into two groups. Early tracheostomy was performed day 1-10 from intubation in 10 patients and the late tracheostomy was performed after day 10 in 11 cases. We also evaluated the duration of mechanical ventilation, stay in the ICU and complications related to tracheostomy, the injury level of and clinical severity. All data was analyzed using SPSS 18.0/WIN. Results : The early tracheostomy offered clear advantages for shortening the total ICU stay (20.8 day vs. 38.0 day, p=0.010). There was also statistically significant reduction in the total length of time on mechanical ventilation (5.2 day vs. 29.2 day, p=0.009). However, the reductions in the incidence of pneumonia (40% vs. 82%) and the length of ICU stay post to tracheostomy (6 day vs. 15 day) were found to be statistically not significant. There were also no statistically significant differences in the injury level and clinical severity between the groups. Conclusion : We concluded that the early tracheostomy (at least 10 days) is beneficial for SCI patients who are likely to require prolonged mechanical ventilation.

Histopathological Effects of Tissue Adhesives on Experimental Peripheral Nerve Transection Model in Rats

  • Objective : Our aim was to evaluate the histopathological effects of tissue adhesives on peripheral nerve regeneration after experimental sciatic nerve transection in rats and to search whether these tissue adhesives may possess a therapeutic potential in peripheral nerve injuries. Methods : This experimental study was performed using 42 female Wistar-Albino rats distributed in 6 groups subsequent to transection of right sciatic nerves. Group I underwent external circumferential neurolysis; Group II received suture repair; Group III had local polymeric hydrogel based tissue adhesive administration; Group IV received suture repair and polymeric hydrogel based tissue adhesive application together; Group V had gelatin based tissue adhesive application and Group VI had suture repair and gelatin based tissue adhesive together. After a 6-week follow-up period, biopsies were obtained from site of neural injury and groups were compared with respect to histopathological scoring based on inflammatory, degenerative, necrotic and fibrotic changes. Results : There were remarkable differences between control group and study groups with respect to inflammation (p=0.001), degeneration (p=0.002), necrosis (p=0.007), fibrosis (p<0.001) and vascularity (p=0.001). Histopathological scores were similar between study groups and the only noteworthy difference was that Group V displayed a lower score for necrosis and higher score in terms of vascularization. Conclusion : Our results imply that tissue adhesives can be useful in repair of peripheral nerve injuries by decreasing the surgical trauma and shortening the duration of intervention. Results with gelatin based tissue adhesive are especially promising since more intense vascularity was observed in tissue after application. However, trials on larger series with longer durations of follow-up are essential for reaching more reliable conclusions.

  • For treatment of full thickness skin defects caused by trauma or infections, skin grafts or flaps have been the treatment of choice to date. However, in patients who are not candidates for surgery, either due to his general conditions or refusal to receive treatment, supportive methods have been the only means of care, which inherently caused psychological trauma to the patient due to uncertainties of ultimate outcome and the length of treatment. This study aimed to heal full thickness skin defects through application of topical epidermal growth factor in patients who have received 2 to 3 weeks of conservative management using medifoam $B^{(R)}$ without improvement. Six patients from March 2002 to July 2004 were enrolled. The mean size of defects was $5.4{\times}4.6cm$ in 4 patients with carcinoma and $6.4{\times}4.1cm$ in 2 patients with osmidrosis. Commercially available 0.005% EGFR solution was used, and dressing was performed once daily. All patients benefitted from the use of EGF, with closure of skin defects taking an average of 28 days in cancer patients and 22.5 days in osmidrosis cases. EGF can be used as a supportive mean of treatment in the inoperable patients with skin defects, with resultant hastening of healing shortening duration of treatment.

A study of the color reproducibility and color fastness of digital textile printing for nylon sublimation transfer (나일론 승화전사 디지털 프린팅의 컬러 재현성 및 견뢰도에 관한 연구)

  • This study examined the color reproducibility and color fastness of digital textile printing for nylon sublimation transfer. After measuring the temperature and time suited to nylon sublimation transfer, the researchers conducted various tests for comparison and analysis including polyester transfer paper on polyester fabric to check dyeing characteristics, color change, sharpness, and the rubbing fastness of the dyeing samples for nylon sublimation transfer. These tests produced the following results. At $185^{\circ}C$ and $187^{\circ}C$, the sublimation transfer dyeing characteristics of nylon were similar to those of polyester and the researchers even observed superior color development in some colors; at a low temperature of $180^{\circ}C$, the sample that was worked on had the lowest level of color development. The examination of color difference (${\Delta}E$), which compared $L^*a^*b^*$ values, showed that the ${\Delta}E$ value of magenta was 10.34, that of yellow was 24.70, and that of black was 15.28. These results highlight the important role of heat treatment temperature and time on color development in nylon sublimation transfer. Concerning sharpness, the samples subjected to higher temperature heat treatment exhibited fewer color spreading phenomena around lines. Thus, dyeing properties and fastness can be enhanced by elongating time at low temperatures and shortening time at high temperatures; however, considering production time constraints as well as the need to produce industrially marketable quantities, the findings of this study suggest that the heat treatment temperature most suitable for nylon sublimation transfer is $187^{\circ}C$ for a duration of 50 seconds.

  • The earth volume which is the basis of all the construction has gone through great development so far with the use of construction machine; however, systematic studies on the related area is in need since the appropriate compound engineering method of earth volume equipments which is a key factor for shortening the project duration and cost reduction is not systematically established and it is dependent on experience. Reasonable mechanical earth volume should take into consideration of performance and characteristics of the equipment, the kind of project, scale and conditions in advance. Also, the optimum compound engineering should be planned by selecting several available scales of equipment. In this study, the earth volume estimate model is established for optimum compound engineering of earth volume equipment for mechanized earth volume equipment loading and moving stage among many stages of earth volume task using system dynamics technique. The optimum compound engineering model of the earth volume equipment produced as a result of this is expected to make reasonable decisions in the shortest time in selecting earth volume facility.

Degree of Rancidity and Sensory Characteristics of Frying Oils with Reuse and Storage at Home (가정에서의 튀김유지 재사용과 보관에 따른 산패도 및 관능적 특성 평가)

  • The purpose of this study was to determine the effects of the duration of frying and storage periods on physicochemical characteristics of various oils using at home. The materials used for the study consisted of four kinds of vegetable oils: soybean, canola, extra virgin olive and pure olive oils, and shortening. Chicken breasts were fried in oils heated at $180^{\circ}C$. The oils were stored with or without filtering and reused 3 times, during the 10 day period. The extra virgin and pure olive oils showed higher acid, peroxide value and yellowness than the other oils (p<0.05), but soybean oil showed the highest increase in acid, peroxide value and yellowness with reusing and storage. In sensory evaluation, the chicken breast fried with soybean oils remarkably decreased the overall acceptance. These results suggested that all frying oils are available because acid and peroxide values of the oils are lower than the standard level. However, reusing soybean oil should be noted with caution in that it is very easy to reduce rancidity, and extra virgin olive oil is not appropriate for frying.

Redesigning nozzle propeller of trawl vessel for improving towing speed (트롤어선의 예망속도 향상을 위한 추진기 구조개선)

  • Fishing efficiency of a trawl vessel can be enhanced by increasing the swept area per unit time, which can be attained either by increasing the mouth size of the net, or by increasing the towing speed. To improve fishing and fuel efficiency of trawl vessels targeting fishes of greater mobility, in which the towing speed is more critical in determining fishing efficiency, we conducted a series of model tests to evaluate the performance of the newly-designed nozzle propeller before installing it in a trawl vessel to verify its towing speed and fuel efficiency in the sea. By conducting further model tests in the experimental basin, we redesigned the propeller of stern trawler to improve the resistance and propulsion performance. Through actual fishing operations, we evaluated the improvement in fuel and fishing efficiency by installing the new nozzle propeller. The trawling speed increased by 0.6kts at the same engine power (RPM), while the engine margin increased by more than 20%. The increased towing speed by installing the redesigned propeller is expected to enhance fishing performance through increasing the number of hauling- and casting operations per unit times, while shortening the towing duration. Analysis of the Catch-Per-Unit-Effort (CPUE) data indicated that the mean CPUE of trawl fishery increased from 3.04kg/m in year 2007 to 6.15kg/m in year 2008, confirming enhanced fishing efficiency by adopting the redesigned propeller.

A Case Study on the Practical Use of PDA for Panel Survey (패널조사에서 PDA 활용 사례연구)

  • A survey method in Korea has being developed rapidly since a survey company was established in 1968. In latest, the CATI, internet survey, Notebook, PDA survey methods are being utilized in accordance with communication development. This article focuses on the practical use and the strength and weakness of the PDA survey solution which employs 'KISS' system designed by KRIVET itself. The survey using PDA has, on the one hand, such strengths as follows : 1) efficient field control, 2) reduction of time for verifying data and generation of proper data 3) shortening survey duration from intial survey to data completion, 4) cutting down survey personnel and cost. On the other hand, survey using PDA has the following weakness: 1) limitation of questionnaire expression, 2) increasing time for survey, 3) communication barrier in some regions.

Effect of Gamiheichumhwan Extract on the GABAergic Neurotransmission (가미희첨환(加味稀僉丸)이 GABA성(性) 신경전달에 미치는 영향에 관한 연구)

  • Objective: The aim of this study is to evaluate the anticonvulsive effects of Gamiheichumhwan extract and to explain its action in GABAergic neuromodulation of the rat brain. Method: The extracts of Gamiheichumhwan were investigated for their inhibitory effect on GABA transaminase activity, their influence on brain GABA and glutamate levels, their agonistic activity on GABA/benzodiazepine receptor and anticonvulsive action using in vitro and in vivo assays. Results: 1. The extract inhibited dose-dependently GABA transaminase (GABA-T) activity by 4.6% and 18.9%, respectively at dosages of 250 mg/kg and 500mg/kg mouse (p.o.). 2. Brian GABA level was increased to 72.0% and brain glutamate level was decreased to 9.6% at a dosage of 500 mg/kg mouse (p.o.). 3. The extract suppressed [3H]Ro15-1788 binding to rat cerebral cortical membrane by $81.4{\pm}0.8%$ at a dosage of 3.2 mg, suggesting its agonistic activity on GABA/benzodiazepin receptor. 4. The extract showed anticonvulsive effect by lengthening the onset time of convulsion, shortening the convulsion duration and diminishing the lethality. Conclusion : It is suggested that Gamiheichumhwan can be used to somnipathy and adapted to treatment and prevention of epilepsy or convulsion.

Endothelin Receptor Overexpression Alters Diastolic Function in Cultured Rat Ventricular Myocytes

  • The endothelin (ET) signaling pathway controls many physiological processes in myocardium and often becomes upregulated in heart diseases. The aim of the present study was to investigate the effects of ET receptor upregulation on the contractile function of adult ventricular myocytes. Primary cultured adult rat ventricular myocytes were used as a model system of ET receptor overexpression in the heart. Endothelin receptor type A ($ET_A$) or type B ($ET_B$) was overexpressed by Adenoviral infection, and the twitch responses of infected ventricular myocytes were measured after ET-1 stimulation. Overexpression of $ET_A$ exaggerated positive inotropic effect (PIE) and diastolic shortening of ET-1, and induced a new twitch response including twitch broadening. On the contrary, overexpression of $ET_B$ increased PIE of ET-1, but did not affect other two twitch responses. Control myocytes expressing endogenous receptors showed a parallel increase in twitch amplitude and systolic $Ca^{2+}$ in response to ET-1. However, intracellular $Ca^{2+}$ did not change in proportion to the changes in contractility in myocytes overexpressing $ET_A$. Overexpression of $ET_A$ enhanced both systolic and diastolic contractility without parallel changes in $Ca^{2+}$. Differential regulation of this nature indicates that upregulation of $ET_A$ may contribute to diastolic myocardial dysfunction by selectively targeting myofilament proteins that regulate resting cell length, twitch duration and responsiveness to prevailing $Ca^{2+}$.
