To meet demand of big capacity and high speed rotation for washing machine, more stress from bending and twisting are complexly loaded onto the shaft supporting the horizontal drum, causing problems in fracture strength and fatigue life. Also, Vibration occurs due to the frequency of the rotating parts. But, shaft has various design factors such as diameter and distance between bearings according to configuration of shaft, the optimal values can't be easily determined. Using a design of experiment (DOE) based on the FEM (Finite Element Method), which has several advantages such as less computing, high accuracy performance and usefulness, this study was performed investigating the interaction effect between the various design factor as well as the main effect of the each design factor under bending, twist and vibration and proposed optimum design using center composition method among response surface derived from regression equation of simulation-based DOE.

목적: 쇄골 간부 골절에서 경피적 골수강내 금속핀 고정술을 시행 후 결과를 보고하고자 한다. 대상 및 방법: 쇄골 간부 골절로 내원한 환자 중 2004년 1월부터 2014년 6월까지 경피적 골수강내 금속핀 고정술을 시행한 135예를 대상으로 하였다. 평균 추시 기간은 15개월(12-24개월)이었다. 기능적 평가로 Disabilities of the Arm, Shoulder and Hand(DASH) score와 Constant score, 임상적 평가로 쇄골의 단축, 수술 반흔의 길이, 수술 시간 및 Kang's criteria에 등에 의하여 분석하였다. 결과: 방사선적 골유합 기간은 평균 11.6주(8-16주), DASH score는 평균 11.8점, Constant score는 평균 91.2점이었다. 임상적 결과로 쇄골의 단축은 평균 20 mm 이하였으며, 수술 반흔의 길이는 평균 1.2 cm (0.7-1.5 cm), 수술 시간은 평균 18분(10-35분)이었으며 Kang's criteria를 기준으로 한 임상적 치료 결과는 우수 이상이 총 131예(97.0%)로 만족할 만한 치료 결과를 보였다. 합병증으로는 3예의 핀 전이, 그 중 2예에서 불유합이 관찰되었다. 결론: 쇄골 간부 골절에서 시도한 경피적 골수강내 금속핀 고정술은 유용한 수술법이라 생각된다.

Purpose: The causes of twelve cases of the mid-shaft clavicle nonunion and the results of internal fixation with plate and bone graft were investigated. Materials and method: From August 1997 to March 2003, twelve cases of the mid-shaft clavicle nonunion were operated with internal fixation with plate and bone graft. The duration of follow-up was average 13 months. Results: The causes of the mid-shaft clavicle nonunion included severe associated injury, severe initial displacement of the fracture fragments and insecure fixation. All cases were operated with internal fixation with plate and bone graft. According to the factor for evaluations of results, using a rating scale of excellent (no apparent factors), good (one factor), fair (two factors), poor (more than three factors), the results showed 10 excellent, 1 good and 1 poor. Screw loosening was developed in only one case . Conclusion: The internal fixation with plate and bone graft of the mid-shaft clavicle nonunion after failed conservative treatment achieved excellent results and seemed to be the procedure of choice for mid-shaft clavicle nonunion.

1. 저자들은 1986년 11월부터 1988년 2월까지 경골간부 골절 8례를 나사못 맞물림법을 이용한 골수강내 금속정을 사용하여 치료하였다. 2. 술 후 방사선 소견상 골유합 기간은 평균 약 15주였다. 3. 환자의 입원기간은 평균 5.5주로서 비교적 짧은 입원기간을 보여주었으며 아울러 사회복귀 및 적응이 빨랐다. 4. 술 후 조기운동이 가능하였고 평균 4.3주 경과 후 목발보행을 시작하였다. 5. 술 후 관절강직, 근위축등의 합병증은 거의 없었다. Hamza등에 의한 술후 평가방법에 따르면 Excellent 7례, Good 1례로 결과는 만족스러웠다.

상완골 골절에 동반된 요골 신경 마비는 전체 상완골 골절의 약 10%에서 발생하는 것으로 보고되고 있으며, 상완골 골절 수술과 관련하여 발생하는 의인성 요골 신경 마비는 10~20%로 알려져 있어 임상에서 흔히 경험하게 된다. 상완골 골절에 동반된 원발성 요골 신경 마비나 의인성 요골 신경 마비가 발생한 경우 요골 신경이 단열된 것인지 아니면 주위 구조물에 압박된 것인지 등의 요골 신경의 해부학적 상태의 평가는 자연회복을 기다릴 것인지 아니면 수술적 탐색술을 실시할 것인지 여부를 결정하는데 중요하다. 저자들은 상완골 골절에 동반된 원발성 요골 신경 마비 1예와 수술과 관련한 의인성 요골 신경 마비 1예를 초음파 검사로 신경의 해부학적 상태를 확인한 후 신경의 단열이 있었던 1예는 비복신경 이식술을 시행하였고, 신경유착이 있었던 1예는 신경박리술을 시행하여 만족할 만한 임상결과를 얻었기에 초음파를 이용한 요골 신경 검사를 중심으로 문헌 고찰과 함께 보고하고자 한다.

Midshaft clavicle fractures are the most common fracture of the clavicle accounting for 80% of all clavicle fractures. Traditionally, midshaft clavicle fractures are treated with conservative treatment even when prominent displacement is observed; however, recent studies revealed that nonunion or malunion rate may be higher with conservative treatment. Moreover, recent studies have shown better functional results and patient satisfaction with surgical treatment. This review article provides a review of clavicle anatomy, describes the current clavicle fracture classification system, and outlines various treatment options including current surgical options for clavicle fracture in adults.

Purpose: The purpose of this study was to evaluate the effects of mobilization application for the lower extremity after fibula Pilon fracture operation patient. Methods: The subjects was 62 years old male who was injury of Lt. fibula shaft Fx., 3 cuneiform & cuboid Fx., 2.3.4 metatarsal bone Fx., We were compared to result of physical therapy between pre and post exercise for 2weeks. Results: The results of this study were summarized below; The mobilization application of the Lt. lower extremity was significantly differences of the ROM at pre and post therapy after 2 weeks, especially in knee flexion ($40^{\circ}$). The increased of accessary movement was evaluated to increased of the physiologic movement about the joints of the lower extremity. Conclusion: We consider that factors of therapy result were not only fracture types, operation and reduction methods for the fibula Pilon fracture but also the ability of physical therapist's manual techniques.

Antegrade interlocking intramedullary nailing (AIIN) for the humeral shaft fracture can induce shoulder pain and decrease of shoulder function postoperatively. The purpose of this study was to estimate the outcome of the shoulder functions after AIIN through the rotator interval between the subscapularis and the supraspinatus to decrease the shoulder pain. Out of consecutive 43 cases that underwent AIIN 42 had been followed for two years or more. Among them we analysized 40 cases of 39 Patients excluding two cases of Pathologic fractures. The average was 47 years. There were 17 men and 23 women. The average follow-up was 34 months. Open nailing was performed in 26 fractures and closed nailing in 14. Bone graft was done in 7 fractures with open nailing. With a single operation, all but two patients achieved osseous union. Average pain score with visual analog scale was one (range; 0∼4) postoperatively By the Neer's score 37 patients received a excellent or satisfactory results while 3 patients' unsatisfactory or failed results. By the functional score of ASES (American Shoulder and Elbow Society) 6 cases received the fair or poor results. Except three cases with persistent nerve palsy and one case of technique failure with protruded nail over humeral head, all patients could achieved satisfactory results with Neer's score and 35 cases (94%) satisfactory results with functional score of ASES. An insertion of antegrade nail to the rotator interval was recommended for better shoulder functions and less pain postoperatively.
