코로나19 전·후 응급실로 내원한 소아청소년 호흡기계 환자의 유사점과 차이점 (Similarities and Differences in Patients under Aged 18 with Respiratory Disease on Emergency Departments: Before and after COVID-19 Outbreak)

  • Background: The purpose of this study was to the impact of the coronavirus disease 2019 (COVID-19) outbreak on emergency departments (EDs) in patients under the age of 18 years with respiratory disease. Also, we analyzed similarities and differences in patients including revisit before and after the COVID-19 outbreak. Methods: This study population was respiratory patients under the age of 18 years who visited all 403 EDs in Korea between January 1st, 2019 and December 31st, 2020, using the National Emergency Department Information System Database. The primary outcome was the number of respiratory patients according to age, sex, the type of EDs, season, Korean Triage and Acuity Scale (KTAS) levels, the result of ED, and length of stay. The secondary outcome was the number of revisit respiratory patients within 72 hours. We calculated the risk-adjusted revisit rates according to the KTAS level using a multiple logistic regression model. Results: The number of ED visits decreased from 274,526 in 2019 to 79,007 in 2020; this number was 71.2% lower than that before COVID-19. In spring 2020, this number was 90.1% lower than during the same period in 2019. For the revisit rate in the study population, the adjusted odds ratio (95% confidence interval) was 1.22 (1.05-1.41) in 2019 and 1.39 (1.07-1.81) in 2020. Conclusion: Implementing appropriate emergency care policies in severe respiratory patients would have contributed to improving the safety of reducing in revisit rate.

Dietary intake and food sources of essential fatty acids among Korean adolescents: a cross-sectional study based on the 2016-2021 KNHANES data

  • Objectives: This study evaluated dietary intake and food sources of essential fatty acids in Korean adolescents. Methods: This study was comprised of 3,932 adolescents (9-18 years) who participated in the 2016-2021 Korea National Health and Nutrition Examination Surveys. Dietary intake and food sources of essential fatty acids, including alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), and linoleic acid (LA) were evaluated using data obtained from one-day 24-hour dietary recall. The proportions of adolescents consuming ALA, EPA + DHA, and LA above or below the adequate intake (AI) of the 2020 Dietary Reference Intakes for Koreans were calculated. All statistical analyses accounted for the complex sampling design effect and appropriate sample weights. Results: The mean intakes of ALA, EPA, DHA, and LA among Korean adolescents were 1.29 g/day, 69.6 mg/day, 166.0 mg/day, and 11.1 g/day, respectively. Boys had higher intakes of all essential fatty acids compared to girls. By age group, adolescents aged 15-18 years showed lower intakes of EPA and DHA compared to adolescents in younger age groups. The 9-11-year-old adolescents had lower intakes of ALA and LA than older adolescents. The proportions of adolescents who consumed more than AI were 35.7% for ALA, 30.4% for EPA + DHA, and 41.5% for LA. Adherence to the AI for ALA did not differ by sex or age group, although boys showed a lower adherence to the AI for EPA + DHA than girls. Major food sources for ALA and LA were plant-based oils, mayonnaise, pork, and eggs. Mackerel was the most significant contributor to EPA and DHA intake (EPA, 22.6%; DHA, 22.2%), followed by laver, squid, and anchovy. Conclusions: The proportion of Korean adolescents who consumed EPA + DHA more than AI was low. Our findings highlight that nutrition education emphasizing an intake of essential fatty acids from healthy food sources is needed among Korean adolescents.

  • Although excessive intake of sodium is known to cause various diseases and threaten public health and policies for reducing overconsumption of sodium are being implemented, the theoretical research on sodium intake is incomplete. This study attempted to provide a policy basis by examining whether or not planned behavior theory, which is a typical health communication theory used to describe health-related behaviors, can be applied to explain sodium intake behavior. Especially, sodium ingestion behaviors were compared using the theory of actions planned for men and women as well as young people, middle-aged, and elderly persons, who are predicted to show differences in eating habits. In this study, questionnaires were prepared to measure the daily eating habits of subjects and subordinate factors of planned behavior theory: behavioral beliefs, outcome expectations, normative beliefs, compliance motivation, and control beliefs. Questionnaires were given to 93 college students and their families. As a result of the difference analysis and path analysis of the questionnaire responses, the following suggestions were obtained. First, age difference is more obvious than sex difference in terms of low salt intentions. For example, older people are healthier than younger people in terms of eating habits and health concerns. Second, the elderly are more likely to practice the low-salt formula when they are provided information on the benefits and effects of the low-salt formula compared to younger ones. Third, systematic efforts are needed to promote a culture that emphasizes the health benefits of a low salt diet rather than providing piecemeal information on the advantages and effects of a low salt diet.

COVID-19 Vaccination and Clinical Outcomes at a Secondary Referral Hospital During the Delta Variant-dominant Period in West Sumatra, Indonesia

  • Objectives: The second wave of coronavirus disease 2019 (COVID-19) cases in Indonesia, during which the Delta variant predominated, took place after a vaccination program had been initiated in the country. This study was conducted to assess the impact of COVID-19 vaccination on unfavorable clinical outcomes including hospitalization, severe COVID-19, intensive care unit (ICU) admission, and death using a real-world model. Methods: This single-center retrospective cohort study involved patients with COVID-19 aged ≥18 years who presented to the COVID-19 emergency room at a secondary referral teaching hospital between June 1, 2021 and August 31, 2021. We used a binary logistic regression model to assess the effect of COVID-19 vaccination on unfavorable clinical outcomes, with age, sex, and comorbidities as confounding variables. Results: A total of 716 patients were included, 32.1% of whom were vaccinated. The elderly participants (≥65 years) had the lowest vaccine coverage among age groups. Vaccination had an effectiveness of 50% (95% confidence interval [CI], 25 to 66) for preventing hospitalization, 97% (95% CI, 77 to 99) for preventing severe COVID-19, 95% (95% CI, 56 to 99) for preventing ICU admission, and 90% (95% CI, 22 to 99) for preventing death. Interestingly, patients with type 2 diabetes had a 2-fold to 4-fold elevated risk of unfavorable outcomes. Conclusions: Among adults, COVID-19 vaccination has a moderate preventive impact on hospitalization but a high preventive impact on severe COVID-19, ICU admission, and death. The authors suggest that relevant parties increase COVID-19 vaccination coverage, especially in the elderly population.

당뇨병 유발에 영향을 미치는 식이 요인 (The Influence of Dietary Factors on the Incidence of Non-Insulin-Dependent Diabetes Mellitus)

  • The purpose of this study was to identify dietary factors related to the incidence of diabetes mellitus in Korea. The study consisted of 165 diabetic patients, male and female, aged 30 to 70 years and 198 healthy persons as controls. Diabetic patients who had been diagnosed with diabetes mellitus for less than five years before the study period were recruited from eight different hospitals located in Seoul, Korea. Socioeconomic status, state of illness, physical activity, food habits and food intake were assessed. Food intake was assessed by food frequency questionnaire method using a 105-food frequency questionnaire developed for diabetic patients. The stress and activity indices of diabetic patients were not significantly different from control, but alcohol consumption and smoking levels were higher in diabetics than controls. Other diseases of male diabetic patients included liver diseases, digestive system diseases, and hypertension, while those of female diabetics were hypertension, neuralgia, arthritis and digestive system diseases. These disease patterns are different from Western countries whose most common complications are hypertension and hyperlipidemia. More irregular and less varied meals were found in the diabetic group compared to the control group, suggesting that diabetic patients have generally undesirable food habits. Otherwise, food and nutrient intake of diabetics did not differ greatly from the control group. It was found that diabetic patients consumed more cereals and less fruit than the control group, and also that male diabetics consumed more alcohol. The carbohydrate : protein : fat energy distribution ratio was 61.7 : 15.8 : 22.5 in male patients and 65.1 : 14.9 : 19.7 in female patients. Discriminant analysis showed that diabetes risk factors differed with sex. In male patients, the important factors were body mass index(BMI), vitamin C intake, family diabetic history and vegetable intake, while in female patients they were BMI, cereals intake, carbohydrate intake, vitamin C intake, stress, food habits and Ca intake. These results show that excessive intake of energy and fat are not the major causal factors in Korean NIDDM. Therefore, the diabetes risk factors of Western countries may not directly apply to Koreans. Mors study is needed to clarify the risk factors of Korean NIDDM.

일개지역 구급활동 중 병원 전 심정지 및 약물중독 환자 분석 - 노인대 비노인의 차이를 중심으로 - (The Analysis on Pre-hospital Cases of Cardiac Arrest and Drug Intoxication during Local Emergency Activities - Based on Differences between Elderly Group and Non-Elderly Group -)

  • Purpose: The purpose of this study is to determine potential differences in pre-hospital cases of cardiac arrest and drug Intoxication between elderly group and non-elderly group on local emergency activity sites of rescue 119 team for those cases, so that it can provide useful reference materials for a system of corresponding emergency medical services. Methods: Patients with cardiac arrest and drug intoxication in the elderly and the non-elderly group were analyzed by analyzing the Ambulance Run Report for 3 years from January 2007 to December 2009. Results: According to analysis on potential differences between elderly and the non-elderly group, it was found that there was no significant difference between elderly cases (evacuated to hospital due to cardiac arrest and drug poisoning) and non-elderly cases in year of onset (p = .247), quarter of onset (p = .813), sex (p = .235), consciousness state (p = .126), place of onset (p = .215) and number of first aid services (applied to emergency cases) respectively, but there were significant differences between elderly cases and non-elderly cases in guardian availability (p = .042), time zone of onset (p = .050), distance from the site of onset (p = .278), type of onset (p = .000), number of first aid services depending on distance of evacuation (p = .008) and effectiveness of emergency care (p = .003) on statistical basis. Conclusion: It is important to establish a system of early emergency case reports for rational emergency case management with lower mortality; shorten distance from the site of onset at each time zone of onset in emergency cases; employ more emergency team members; facilitate firsthand / secondhand medical instructions for emergency teams in specialized emergency care depending on distance of evacuation for each kind of onset (elderly group vs. non-elderly group); and improve rate of resuscitated emergency cases by extending the scope of works for emergency medical technicians into wider applications, so that it will be possible to take timely and appropriate measures for emergency settings of ever-increasing aged population in near future.

고혈압 관리수준에 따른 효율적인 관리 방안 (An Efficient Management by Analyzing Control Level of Hypertension)

  • 본 연구는 고혈압 관리지표에 영향을 미치는 여러 요인을 포괄적으로 분석함으로써 효율적인 고혈압 관리사업을 수행하기 위한 기초자료를 제공하고자 국민건강영양조사 2007년, 2008년도 자료를 활용하여 성인 남녀 고혈압 유병자 2,580명을 대상으로 심층 분석하였다. 주요 결과로 첫째, 고혈압 환자의 인지 및 치료에 영향을 미치는 요인으로 남녀 모두 연령이 증가할수록, 주관적 건강상태가 나쁜 경우, 건강검진을 받은 경우에 인지 및 치료 확률이 유의하게 높았다. 둘째, 고혈압 환자의 조절에 영향을 미치는 요인으로 남녀 모두 연령이 증가할수록, 건강검진을 실시한 경우 조절 확률이 유의하게 높았고, 반면 평소 음주자인 경우 유의하게 낮았다. 상기의 결과를 토대로 고혈압 관리수준에 영향을 미치는 건강행태 요인을 중심으로 성별과 연령대에 맞는 체계적인 프로그램이 실시된다면 좀 더 효율적인 고혈압 관리 사업을 추진할 수 있을 것이다.

Effect of a 5% naproxen patch on reducing pain caused by separators prior to fixed orthodontic treatment

  • Background: The pain involved in orthodontic treatments may involve inflammatory processes. This study evaluated the effect of using a naproxen patch for pain reduction in the separating stage of fixed orthodontic treatment. Methods: In this double-blind, randomized, controlled clinical trial of 35 orthodontic patients (age: 14-19 years) who had pain during separator placement, each patient randomly placed naproxen and placebo patches in the first permanent molar region, in opposite quadrants of the same jaw. Patches were replaced every 8 hours until 3 days after separator placement. Patients recorded their pain perception at 2, 6, and 24 hours, and on days 2 (6 PM), 3 (10 AM and 6 PM), and 7 (10 AM and 6 PM), using a visual analog scale. Mean pain scores were compared for the two patches, and effects of sex and age thereon determined. Results: Data from 29 patients (21 girls, eight boys) were analyzed. Mean pain values decreased over time for both patches (P < 0.001). Recorded pain did not differ significantly between the sexes (P = 0.059) or between those aged <16 and those ${\geq}16years$ (P = 0.106). Mean pain recorded with naproxen patches was statistically significantly less than that with placebo patches at all time points (P = 0.004). Conclusion: The naproxen patch was more efficient than the placebo patch for reducing pain at all time points. The highest pain score was recorded at 6 hours, and the least pain was recorded at the $7^{th}$ day after separator placement.

혈중 납이 비흡연자들의 혈압에 미치는 영향 (The Effects of Blood Lead on Blood Pressure Among Non-smokers)

  • Objective: The effect of lead on blood pressure remains controversial in spite of the numerous studies which have been conducted in the recent years. The aim of this study was to evaluate the effects of exposure to lead on blood pressure among non-smokers. Methods: In this cross sectional study, 1416 male and female non-smokers were enrolled, aged 20 years or older, from the Korean National Health and Nutrition Examination Survey 2008. Blood pressure, blood lead levels (BLLs), height, weight, and cotinine level were measured for all subjects. Results: Geometric mean BLLs of the participants was 2.20 ${\mu}g$/dl. BLLs were higher in the older, male, and lower education groups than the younger, female and higher education groups. After adjusting for age, sex, education and BMI through multiple regression analysis, a significant positive association between systolic blood pressure (p = 0.0357), diastolic blood pressure (p = 0.0111) and BLLs. Also, among the normal BMI group (18.5 kg/$m^2$ < BMI < 25 kg/$m^2$), we also found a significant positive association between diastolic blood pressure and BLLs (p = 0.0370). Conclusion: The present study showed that blood lead serves as a good predictor of blood pressure changes and that there was a statistically significant association between blood lead and blood pressure, especially diastolic blood pressure.

Prevalence and Risk Factors of Functional Gastrointestinal Disorders in Infants in Indonesia

  • Purpose: Information regarding functional gastrointestinal disorders (FGIDs) in infants is currently lacking in Indonesia. This study aimed to describe the prevalence and risk factors of FGIDs in infants aged 6 weeks to 4 months in Indonesia. Methods: This cross-sectional study of 433 infants was conducted between September 2018 and February 2020. Information on FGIDs was collected using the Infant Gastrointestinal Symptom Questionnaire and the Feeding Practice and Gut Comfort Questionnaire. Adapted Rome IV criteria were used to define the FGIDs. Results: The prevalence of regurgitation was 26.3%; 16.8% of the infants presented cryingrelated symptoms and 5.5% exhibited constipation. The statistical analyses revealed that constipation was associated with sex (odds ratio [OR], 2.74; 95% confidence interval [CI], 1.07-7.71; p=0.043), employment of the father (OR, 0.3; 95% CI, 0.12-0.77; p=0.01), and education of the mother (OR, 1.92; 95% CI, 1.07-3.51; p=0.031). Length at birth (OR, 0.74; 95% CI, 0.55-0.99; p=0.042) was associated with constipation. Length at visit (OR, 0.83; 95% CI, 0.76-0.91; p<0.001) was associated with regurgitation, and the weight at visit (OR, 0.58; 95% CI, 0.35-0.96; p=0.038) was associated with crying and/or colic. A history of parental FGIDs was associated with crying-related symptoms (OR, 2.12; 95% CI, 1.23-3.68; p=0.007). Conclusion: Regurgitation, crying, and constipation are common FGIDs in infants. Some parental and infant characteristics may be predictors for FGIDs. Further investigations are needed to evaluate the clinical relevance of our findings. Understanding the determinants of FGIDs will benefit healthcare professionals and parents to improve infant's quality of life and better manage these condition.
