가정간호서비스의 비용효과분석;뇌혈관질환자를 중심으로 (Cost-Effectiveness Analysis of Home Care Service for Cerebrovascular Disease Patients)

  • Purpose: This study was designed to analyse economical efficiency of home care service by comparing a cost-effectiveness ratio(CER) between hospitalization and home care service. Method: The analytic framework of this study was constructed in 5 stages; perspective of the analysis, measurement of costs, measurement of effects, analysis of CER, and sensitivity analysis. The SAS program was utilized for the general characteristics of the subjects, descriptive statistics, homogeneous test, normality test and difference test. Result: The results were as follows; 1) CER was 35,248,256 of ADL, 7,996,026 of nursing satisfaction, 6,144,946,000 of QALY of patients in the hospital and 11,168,863 of ADL, 2,322,239 of nursing satisfaction, 3,674,556,000 of QALY of patients in the home care center. ICER was 438,067,932 of ADL, -190,044,176 of nursing satisfaction, 8,615,336,000 of QALY. 2) In the sensitivity analysis of sex, age and discount rate, the CER of patients in the home care center was lower than the CER of patients in the hospital. Conclusion: With these findings, it affirmed that home care service had an economical efficiency compared with hospitalization in cerebrovascular disease patients. Therefore, these results will be used to develop governmental policy or expansion of the home care service.

응급 환자 이송서비스의 적절성: 미충족 의료와 부적절한 이용 (Unmet Need and Inappropriate Use in Emergency Ambulance Service)

  • Background: The objective of this study was to evaluate the efficiency of an emergency ambulance system and to investigate socio-economic and clinical characteristics associated with emergency ambulance service. Methods: Based on 2011 Korea health panel, unmet need and inappropriate use of emergency ambulance service were measured by Gibson in 1977. Furthermore, the factors associated with unmet need and inappropriate use of emergency ambulance service were identified by Fisher's exact tests and multiple logistic regression models. Results: Unmet need, defined as the proportion of emergency patients who clinically need ambulance transportation but do not receive it, was found to be 59.8%. Inappropriate use, defined as the proportion of emergency patient receiving ambulance care who did not clinically need it, was found to be 37.2%. There were statistically significant differences between appropriate and inappropriate groups in overall variables of socio-economic and clinical characteristics. Specifically, gender, age, relationship to household, and reasons of visiting emergency department (accident/disease) were statistically significant factors associated with appropriate use of emergency ambulance service. Conclusion: Unmet ambulance need is a useful measure for patients needs assessment, and inappropriate ambulance use is a valid criteria in judging the efficiency of emergency ambulance system. To improve and understand emergency ambulance system, unmet need and inappropriate use of emergency ambulance service should be more concerned.

카메라 트래핑을 이용한 육상포유류 모니터링 효율성 평가 (Monitoring Efficiency Evaluation of Camera Trapping in Terrestrial Mammals)

  • The aim of this study was to evaluate the monitoring efficiency of camera trapping in wild animals and to determine ways to increase its utilization. Nineteen sensor cameras were installed in Sobaeksan National Park from October 2012 to September 2013. During the study period, a total of 1045 terrestrial mammal photos were secured and 15 species habitats were identified. Shooting frequency was higher for medium and large mammals, especially full images of carnivores accounted for approximately 83%. A comparison of track surveys revealed that camera trapping was highly efficient and helped in capturing real image of species. The supply of lure and bait stimulates the sense of smell in carnivores, which further enhances the capturing of images by camera trapping. The results of this study provide data on the ecological characteristics of mammals, which can aid in determining habitat use by these animals, and thereby facilitate prevention of crop damage by wildlife.

약제비 제외가 의원의 진료비 효율성 순위에 영향을 미치는가? (Does Omission of Pharmacy Cost Affect Cost-Efficiency Rankings in Medical Clinics?)

  • Background : If different cost efficiency indexes were informed to the same clinic depending on the inclusion or exclusion of pharmacy cost, it may impair the reliability of provider-profiling system. This study aimed to investigate whether the omission of pharmacy cost affects cost-efficiency rankings in medical clinics. Methods : Data for ambulatory care cost at 23,112 medical clinics were collected from the claims database, which was constructed after review by the Health Insurance Review and Assessment Service (HIRA) of Korea in April 2007. We calculated two types of cost efficiency indexes by inclusion or exclusion of pharmacy cost for a medical clinic. The agreement between the decile rankings of the two indexes was also assessed using the weighted kappa statistic of Landis and Koch. Results : When the cost efficiency index for total cost including pharmacy cost was compared with the index for total cost excluding it, the agreement between the two indexes was only 55%. The agreements between the two indexes were relatively low within specialties which have larger pharmacy volume of total cost and lower correlation between total cost with or without pharmacy cost included than the average level of all the specialties. Conclusion : These results suggest that the omission of pharmacy cost may result in contradictory outcomes that may be confusing to a medical institution and may impair the reliability of provider-profiling systems. It is very important to standardize profiling criteria for the reliability of provider profiling system.

  • As the IT service environment grows, it is critical in terms of IT service quality to minimize the occurrence of failures due to changes in applications and to diagnose and recover in a short period of time how failure will affect the business. Thus, the Defense Acquisition Program Administration (DAPA) has been building and operating ITSMs to implement IT service management in a leading manner. Information Technology Service Management (ITSM) is divided into events, obstacles, changes, versions and setup management to ensure flexibility and stability in service delivery. It is also operated separately from service level, availability, capacity, financial and IT service continuity management to ensure service quality and cost efficiency. Based on ITSM military service history, this study looks at the impact of quality of service on value, satisfaction, and trust. The results of the analysis are highly valuable for future ITSM implementation and operation.

The Influence of Personality Traits on Airline Untact Check-in: Focusing on Mobile Check-in User

  • Purpose: Year of 2020, COVID-19 has been changing the people's everyday life to ways never been thought of before all over the world. The IT and electronic industry, the methods of supplying goods and services have been changed from contact to un-tact environments based on un-tact systems very rapidly. COVID-19 has been striking the tourism and the travel industry, especially the airline and hotel industry of which services are provided by human. For the passenger service of airliner, automation has been propelled and un-tact style of service has become mainstreams except cabin service since 2000's. For passenger transportation, due to traffic regulations and exclusions etc., switching to new ways is not easy. However, under the new environment made by COVID-19, kiosk check-in, web check-in and mobile check-in has become more important. In this study, the characteristics of airline customers using mobile goods are investigated to find ways to raise the rate of utilizing mobile check-in and to increase the efficiency of boarding process. Research design, data, and methodology: Considering the COVID-19 environment, survey was done by online research company. The research model is designed to integrate the user characteristics and usage/purchase motive and technology acceptance theory. Especially considering infectious diseases prevention, concern of safety is adopted as one of the usage motive variable. Results: Extraversion or conscientiousness characteristics prefers counter check-in(contact service), while openness characteristics prefers mobile check-in(un-tact service). Concern of safety for infectious disease shows strong non-preference on counter check-in. Conclusions: Regarding service type regardless of type of the industry, automation and un-tact have been mainstreams due to high costs of labor, efficiency and standardization issue, etc., and COVID-19 has given impetus to them. For airliner, un-tact service including boarding process service has been more and more important. To raise the rate of un-tact service use, the characteristics of the user should be analyzed first. The study results indicate that for extraversion or conscientiousness, some kinds of methods to induce them to use un-tact service more are needed.

국내 발전정비산업의 효율성 및 경쟁력 결정요인 분석 (An Analysis on the Efficiency and the Determinants of International Competitiveness of Korean Power Plant Service Industry)

  • 본 연구는 국내 발전정비산업의 기업 자료와 설문조사를 통해 효율성과 국제경쟁력 결정요인 분석을 시도하였다. 특히 2005년에 국내 발전정비시장의 경쟁력 도입을 2009년으로 유보하였으며, 유보기간 동안 국내 발전정비업체의 경쟁력 강화 육성 정책을 시행하였다. 따라서 2005년의 경쟁도입 유보 정책에 따른 효율성 변화와 설문조사를 통해 국제 경쟁력 결정요인을 분석하였다. DEA를 이용하여 효율성 추정에 사용된 표본은 국내 6개 발전정비회사이며 분석기간은 2003년~2008년까지이다. 효율성 추정 결과에 의하면, 전반기(2003~2005)의 효율성 수준은 0.865인 반면에 후반기(2006~2008)에는 0.947로 전반적인 효율성 수준은 경쟁도입 유보 결정 이후에 개선되었으며 규모의 효율성도 전반기(0.890)에 비해 후반기(0.972)에 개선된 것으로 추정되었다. 한편, Porter의 다이아몬드 모형을 이용하여 국내 발전정비산업의 국제 경쟁력 결정요인을 설문조사를 통해 분석한 결과, 5개 결정요인 중에서 수요조건이 5점 만점에 4.22점을 획득하였으며, 정부부문(3.85), 요소조건 (3.63)의 순으로 높게 나타났다. 가장 취약한 요인은 기업의 전략 조직 경쟁 부문으로 2.94점을 받았다. 따라서 우리나라 발전정비 시장의 확대 가능성은 상당히 높게 평가되고 있으나 이를 충족할 수 있는 기업의 전략 조직 경쟁 부문은 취약한 것으로 볼 수 있기 때문에 이에 대한 기업의 개선 노력이 필요할 것으로 보인다.

게임 소프트웨어 및 서비스 산업의 경영효율성 분석 (Analysis of Management Efficiency of the Game Software and Service Industry)

  • 게임 소프트웨어 및 서비스 산업에 관한 연구가 학술적 정책적으로 의미있는 이유는 우리나라 문화산업의 핵심분야로 자리잡고 있기 때문이다. 그러나 치열한 경쟁력으로 인해 최근의 성장추세는 둔화되고 있다. 즉, 단기적인 수익창출에만 중점을 두었던 기업경영으로 인해 성장의 한계에 달했다는 평가이다. 이에 따라 효율적이고 혁신적인 기업경영의 도입과 중장기적 관점의 상품개발 및 R&D 전략수립의 필요성이 강조되고 있다. 이러한 문제의식에 따라 본 연구는 게임 소프트웨어 및 서비스 산업에 속하는 20개 기업의 경영효율성을 측정하기 위해 자료포락분석(DEA)을 실시하였다. 투입변수는 종업원수, 총자산, 투자자산이며, 산출변수는 총매출액과 기업가치로 하였다. 분석 결과는 다음과 같다. 첫째, 효율성에 격차가 있었다. 8개 기업이 비효율적이었다. 둘째, 비효율적인 기업의 경우 종업원수와 투자자산의 비중이 과다한 기업이 6개로 나타났다. 셋째, 규모수익불변인 DMU는 4개이며, 규모수확체증 5개 DMU는 투입요소의 규모를 늘려 효율성을 증가시켜야 하며, 그리고 규모수확체감 11개 DMU는 기업은 투입요소의 효율성 향상 방안을 수립해야 할 것으로 나타났다.

  • The purpose of this study is to examine the different preference as well as the factor for this preference between general internet shopping mall and feminine internet fashion shopping mall only which are subjected with women and know the satisfaction which follows hereupon. This research will help to establish the strategy to marketers who manage feminine internet fashion shopping mall. The results of this study as follows; 1. As the result of the analysis of preference factors for general internet shopping mall, it was classified with, convenient characteristic, economical efficiency, product and information characteristic, sales promotion, and customer service. Also differences among preference factors appear with following orders, economical efficiency, convenient characteristic, product and information characteristic, sales promotion, and customer service. 2. As the result of the analysis of preference factor for feminine internet fashion shopping mall, it was classified with, fashion information and style, purchase product, economical efficiency, product assortment, and customer service. Also differences among preference factors appear with following orders, fashion information style, product assortment, purchase product, economical efficiency, and customer service. 3. As the result of the analysis of preference factors, convenient characteristic and product information mostly effects to satisfy the customers who use general internet shopping mall. Also purchase product, product assortment, fashion information and style mostly effects to satisfy the customers who use feminine internet fashion shopping mall.
