Trends in breast reconstruction: Implications for the National Health Insurance Service

  • Background Breast reconstruction has become more common as mastectomy has become more frequent. In Korea, the National Health Insurance Service (NHIS) began covering breast reconstruction in April 2015. This study aimed to investigate trends in mastectomy and breast reconstruction over the past 10 years and to evaluate the impact of NHIS coverage on breast reconstruction. Methods Nationwide data regarding mastectomy and breast reconstruction were collected from the Korean Breast Cancer Society registry database. Multiple variables were analyzed in the records of patients who underwent breast reconstruction from January 2005 to March 2017 at a single institution. Results At Seoul National University Hospital, the total number of reconstruction cases increased 13-fold from 2005 to 2016. The proportion of immediate breast reconstruction (IBR) cases out of all cases of total mastectomy increased from 4% in 2005 to 52.0% in 2016. The proportion of delayed breast reconstruction (DBR) cases out of all cases of breast reconstruction and the overall number of DBR cases increased from 8.8% (20 cases) in 2012 to 18.3% (76 cases) in 2016. After NHIS coverage was initiated, the proportions of IBR and DBR showed statistically significant increases (P<0.05). Among the IBR cases, the percentage of prosthesis-based reconstructions increased significantly (P<0.05), but this trend was not found with DBR. Total mastectomy became significantly more common after the expansion of NHIS coverage (P<0.05). Conclusions Over the last decade, there has been an increase in mastectomy and breast reconstruction, and the pace of increase accelerated after the expansion of NHIS coverage. It is expected that breast reconstruction will be a routine option for patients with breast cancer under the NHIS.

3D Coverage Analysis of LTE Network for UTM Services Considering Actual Terrain and Base Station Layouts (실제 지형과 기지국 배치를 고려한 UTM 통신을 위한 LTE 통신망 3차원 커버리지 분석)

  • Unmanned aircraft system traffic management (UTM) service for the safe operation of unmanned aerial vehicles (UAV) such as drones using commercial communication networks such as long-term evolution (LTE) and 5G in low-altitude areas of 150m or less is being studied in several countries. In this paper, whether it is possible to secure three-dimensional (3D) coverage for UTM service using the existing LTE cellular network for terrestrial usersis analyzed through simulations. The practicality in the real environment is confirmed by performing performance analysis in the actual topographical environment and the LTE base station layouts in Korea. According to the analysis results, as the altitude increases, the number of line-of-sight (LOS) interference base stations increases, resulting in a worse signal to interference plus noise ratio (SINR), but coverage is secured except for the limited areas within 150m. was confirmed to be possible. In addition, it is confirmed that a significant proportion of outage areas could be reduced by placing a small number of additional base stations for the outage area.

A Study on Application of DSS for enhancing 5G Coverage (5G 커버리지 개선을 위한 DSS 적용 방안 연구)

  • 5G service uses mid-band (n78) than existing mobile communication frequencies, so it is necessary to improve 5G coverage by utilizing low-band frequencies below 2 GHz. To this end, the application of Dynamic Spectrum Sharing technology of LTE and 5G-NR system using most of the low-band frequencies is required. In this paper, signaling overhead factors for DSS application and RF issues for terminal implementation are derived, and signaling overhead ratios from the respective perspectives of 5G-NR and LTE for the 1.8GHz band (50MHz width) that can utilize wide-bandwidth among low-band frequencies are estimated. Also handset RF issues were analyzed. Based on the analysis results, if DSS technology using low band is applied, it is expected that excellent 5G service quality can be provided due to 5G coverage improvement when LTE traffic quickly migrates to 5G-NR.

  • AIS(Automatic Identification System) is a radionavigation equipment for exchanging safety related information between ships as well as ship and shore station, introduced by SOLAS convention and widely used especially in vessel traffic service. However, in an area of broad communication coverage of coast station, various problems may appear in receiving AIS data from ships. In this paper, AIS problems that may happen in broad communication coverage of coast station are analyzed using received data.

  • Oil Field Geographical Information System (OFGIS) manages multiple spatial data, attribute data, and topographic data, which include almost every kind of ground info rmation and underground information. Subsystems managed by OFGIS include petroleum exploration subsystem (PESS), petroleum development and engineering subsystem (PDESS), petrochemical subsystem (PCSS), petroleum storage and transportation subsystem (PSTSS), petroleum sale subsystem (PSSS), etc. A basic OFGIS framework consists of oil field infrastructure coverage (OFIC), oil field specialized information coverage (OFSIC) and oil field synthesis and decision service coverage (OFSDSC). Basic function of OFGIS includes database management, geographic information management, spatial information processing and application.

  • The National Health Insurance Service(NHIS) has been selectively covering the elderly on the dental implant and removable denture treatment and gradually expanding the eligibility criteria. Every dentist needs to have a good understanding of the insurance policy to provide qualified dental service to the beneficiaries and to avoid confusions and complications. In order to help dentists understand the NHIS requirements, the definitions, categories, and procedures of dental implant and conventional denture treatment are suggested.

  • In this research the remaining service life of the concrete due to the steel corrosion was predicted by three cases; causing carbonation, using sea sand, using deicing salts. In case of deterioration by carbonation, effective carbonation depth, effective coverage depth and relative humidity are considered for predicting method. In case of using sea sand, predicting method is made of rust growth equation from polarization resistance method. In case of using deicing salts, predicting method is made of transformation of Fick's law. Three methods are very useful in predicting service life of concrete.

Staple Line Coverage with a Polyglycolic Acid Patch and Fibrin Glue without Pleural Abrasion after Thoracoscopic Bullectomy for Primary Spontaneous Pneumothorax

  • Background: This study was conducted to determine the efficacy of staple line coverage using a polyglycolic acid patch and fibrin glue without pleural abrasion to prevent recurrent postoperative pneumothorax. Methods: A retrospective analysis was carried out of 116 operations performed between January 2011 and April 2013. During this period, staple lines were covered with a polyglycolic acid patch and fibrin glue in 58 cases (group A), while 58 cases underwent thoracoscopic bullectomy only (group B). Results: The median follow-up period was 33 months (range, 22 to 55 months). The duration of chest tube drainage was shorter in group A (group A $2.7{\pm}1.2day$ vs. group B $3.9{\pm}2.3day$, p=0.001). Prolonged postoperative air leakage occurred more frequently in group B than in group A (43% vs. 19%, p=0.005). The postoperative recurrence rate of pneumothorax was significantly lower in group A (8.6%) than in group B (24.1%) (p=0.043). The total cost of treatment during the follow-up period, including the cost for the treatment of postoperative recurrent pneumothorax, was not significantly different between the two groups (p=0.43). Conclusion: Without pleural abrasion, staple line coverage with a medium-sized polyglycolic acid patch and fibrin glue after thoracoscopic bullectomy for primary spontaneous pneumothorax is a useful technique that can reduce the duration of postoperative pleural drainage and the postoperative recurrence rate of pneumothorax.

An Analysis on the Needs for the Mobile Internet Service (휴대인터넷 서비스에 대한 니즈[Needs]분석)

  • In this research, we have developed a model that can explain the potential customer's needs for the potable Internet service, a concept with 'QoS guaranteed lower-price high speed mobile Internet service. Based on the developed model, we have also derived some empirical implications for the business firms interest in the potable Internet market. The developed model is incorporating a survey result, answering of potential customer's attitude for the portable Internet service, from the subscribers to the Internet service, wireless LAN service, and mobile Internet service. As a result, we have found that a very innovative group, such as wireless LAN users and 20's age group using mobile Internet service, could be the most attractive market segment. Moreover, the aspects of the service coverage and the price competitiveness at the service launching stage could be the most critical success factors for the portable Internet service.

A Study on Drone's Liability on Flight in South Korea

  • Purpose - This paper analyzed the accident of drone and related insurance goods which can be coverable with regard to drone accident. The study range is limited in the owner's care, custody, and the control of drone in South Korea, but military area and law and regulation of the drone would be exceptional. Research design, data, and methodology - With regard to drone's flight, drone can make possible risks and can give severe damage to the people. To carry out this research, literature survey and review such as journal, thesis and publications were adopted. Results - As for the insurance coverage from drone's accident involved in the purpose of business, insurance coverage depends on 'commercial general liability insurance'. However, in case of personal hobby including leasure intention, insurance coverage depends on 'living liability insurance'. Conclusions - From a drone's accident, operator and owner of the drone may face the property damage to the drone itself, and then can give severe damage or loss to the people such as physical injuries and property damage. Peoples should be concerned about having the awareness of drone's accident with insurance coverage.
