An Analysis of the Physician Productivity in General Hospitals (전국 종합병원 의료인력의 생산성분석)

  • The purpose of this paper is to identify factors affecting the optimum mix of required inputs and other relevant factors which account for the variation in physician's productivity in general hospitals, and to find out their implications for the efficient health planning and management. An extended version of Cobb-Douglas production function and cross sectional data of one day patient census from all general hospitals in Korea in 1988 were used in the analysis. Main results of the analysis and their implications could be summarized as follows : (1) The production function for physician's inpatient service shows the evidence of economies of scale, but the production function for physician's outpatient and adjusted-patient service, which combines both out- and in-patient service, shows that of dis-economies of scale. (2) The physician's role for production for all service is smaller than auxiliary personnel's, which imply that more intensive utilization of nurses, nursing aides and other auxiliary personnel is desirable for improving general hospital productivity (3) In case of physician's inpatient and adjusted-patient service, nurses' role is greater than nursing aides'. Therefore, more extensive utilization of nurses is recommended for the efficient operation of general hospitals. (4) The factor of hospital beds plays the leading role among required inputs in the production for physician's in- and adjusted-patient service. (5) The physician's productivity of general hospitals in rural area is lower than that in urban area. And the productivity of teaching hospitals is lower than that of the other hospitals. Further analysis was made in physician production function based upon the size of hospitals, namely those hospitals below 250 beds and those above. Explained variances by the factor of hospital beds was significantly increased in the case of those hospitals above 250. A more detailed and thorough investigation is needed for verifying factors influencing physician's productivity in general hospitals in Korea.

  • This paper aims to identify the characteristics and the current situation of the architectural characteristics, children's characters using facilities, and status of management and utilization by surveying 'Day Service for Children' in Tokyo-do. First, About the status of management and utilization, 'Day Service for Children' is operated by a local government, NPO corporate group and social welfare corporate. There are not big differences of the care program or running hours depending on operating principals but there are differences depending on staff organization, number of children and space constitution. Also, infants ratio at 'Day Service for Children' is high for the purpose of detecting disability early and supporting their development early. They have diverse disabilities but developmental disabilities' ratios high and most children are using both this facility and normal nursery centers together. Samples for proper child care based on status of space constitutions were installing lock, raising door knobs and using softer floor materials which are for children's safety. For the problem of space utilization, most comments were about lack of space which is not enough for teaching and training materials. Therefore, as an example, observation rooms for parents and teachers were sometimes used as storages and it changes teachers' moving direction, staying area and behavior zone. The programs of 'Day Service for Children' are divided to group teaching and private one. It will be the best if they have separate spaces such as group class room, private class room and play room. When they were operated by NPO corporate group or Social welfare corporate, there are many cases that their space was not properly zoned for each activity.

The Characteristics and Spatial Configuration for a Nurse's Office in an Elementary School in a Rural Area -Focused on Cheongwon Gun Area- (농촌지역 초등학교 보건실의 공간구성 및 특성 -청원군 지역을 중심으로-)

  • As the importance of health and school health grows, the purposes of this research are to investigate the differences between the characteristics and the utilization of the nurse's office of an elementary school in an urban area and the nurse's office of an elementary school in a rural area, and to provide the data for the planning of construction of a nurse's office convenient for students to use after grasping the regional characteristics. The methods of this research include the interview and the survey conducted to the heath teachers after visiting 20 nurse's offices in 13 eups and myeons, and examined and analyzed the differences between the characteristics and the utilization of the nurse's office of elementary schools in an urban area and the nurse's office of an elementary school. The nurse's offices of the elementary schools in Cheongwon, a rural area, are worse in access to health care institutions and in the connection to the placement than those of the elementary schools in urban areas, so most of eups and myeons were not allowed for easy access to medical service. Depending on the bad personal surroundings of each elementary student, mental stress and the lack of heal care of each student should be taken care of along with the educating of the parents and the children. Also because of the difficulties in access to the health care institutions, it is suggested that a lot of support and interest should be provided for the nurse's offices and that a lot of changes with functions should occur to the nurse's offices so that the nurse's office of an elementary school can play the role as a local health care institution. Besides, the health teachers also should be able to provide more professional medical services by taking in-service training courses.

  • In this paper, conventional STM(Synchronous Transfer Mode) supports another traffics by using idle channels Proposed mobile terminals are distributed independently at the BSA(Basic Service Area). So, MAC(Medium Access Control) specified on the wireless ATM networks, which is managing statistics multiplexing functions, idle channel's utilization is maximized and multimedia service characteristic for conventional wired networks based fixed assignment scheme(connection-oriented) and random assignment scheme(connectionless) improving on dynamic reservation based MAC protocol.

  • The research on the health promotion effect of the elderly through the health care service has been going on for a long time, but there is insufficient research to grasp the needs of elderly people in order to effectively provide health care services. In order to solve these problems, this study suggested the direction of health care service for elderly people by analyzing regional characteristics and demand among rural areas. To this end, the direction of improvement of customized healthcare service model was suggested through the analysis of the health - related program utilization status, health management method, health care service type, and contents demand of the elderly by urban area and rural area.

  • This paper examined the effects of private health insurance(PHI) on the health care utilization among the Korean. The used data was the three waves of Korea Health Panel (2008, 2009, 2010), and the number of subjects was 13,951 persons. Authors employed two-stage least square panel model where the instrument variables for controlling for endogeneity of PHI were number of insurance planner per 100,000 in resident area and whether subject worked on financial profession. The results showed that healthcare expenditure of outpatients who purchasing PHI was higher than that of outpatients without PHI, and there was no difference in admission between the two groups. This article recommended the Korean government to monitor the effects of PHI on the health care utilization in order to improve the efficiency of health care finance.

The Effects of Hospital Resources on the Service Uses: Hospital Service Area Approach (병원서비스지역 내 병원자원과 의료서비스 이용 간의 관련성 분석)

  • Background: This study explored the relationship between hospital resources and services uses in outpatient/inpatient-based hospital service area (HSA) in Korea. Methods: Study hospitals included all acute care hospitals except tertiary hospitals. Inpatient and outpatient hospital claims from the Korean National Health Insurance (NHI) program in 2010 were used to identify the service uses. Hospital resources and the degree of insurance premium in study areas were identified with the NHI corporation data. Study variables were computed by summing the service uses or hospital resources of study hospitals in each HSA. Service uses were represented by the total medical charges and number of visits/inpatient days. Hospital resources were measured by number of beds, number of doctors, and number of computed tomography (CT). The economic status of NHI enrollees in each HSA was controlled by the average monthly premium of NHI program per household in each HSA. The degree of using local hospitals was controlled with the localization index. Results: Analysis results showed that hospital resources such as beds, CT were statistically related to the service uses. And also localization index was found to have positive significant relationships with service uses. Conclusion: Hospital resources such as beds, CT had not only positive impacts on inpatient service uses, but also influences on the outpatient setting. Health policy makers will require monitoring and assessing the hospital resources in Korea.

Food Utilization Status and Perceived Performance of food Management of School Food Service in the Kyunggi Area (경기지역의 학교 급식 식재료 사용 현황 및 관리 직무 수행도에 관한 연구)

  • The perceived performance of food management and the food utilization status of school food service managers in nine sectors of management in the Kyunggido area were evaluated[ED highlight - please ensure this is correct]. The results of this study revealed that 84.5% of the service managers used domestic food and $20{\sim}40%$ used organic food, with an average of 80% using one of these types of foods. In addition, the average use of pre-handling food varied widely when compared to other food items[ED highlight - very confusing, please ensure my changes do not alter your intended meaning] Additionally, most respondents used frozen food (72.0%), processed food (83.9%), and substitute food (53.4%), for the average of 20%,[ED highlight - I cannot infer your intended meaning here, what is an average of 20%? Please clarify]; however, 40.6% did not use any food substitutes. The most common reason given for using pre-handled[ED highlight - do you mean pre-packaged or pre-prepared? Please clarify] food was to save time (32.2%), whereas frozen foods were most often used to help with menu organization (37.5%). Additionally, the respondents most common reason for using processed food was its high acceptability (47.8%), while substitute foods were most often used due to non suitable foods beingreturned (75.3%). Among the varieties of food that were used, those that are easily obtained and cooked were used the most. Furthermore, the mean score for the perceived performance of food sanitary management was $4.51{\pm}0.425$ (based on the 5-point Likert scale). PDO Packing Mechanism for Reducing CANopen Network Utilization (CANopen 네트워크 이용률 감소를 위한 PDO 패킹 메커니즘)

  • CANopen which is one of the in-vehicle network (IVN) protocols is adopted to solve the hardware dependency problem of the CAN-based application. CANopen makes different CAN devices interoperable each others. By the advantage of the device profiling concept, it can make the period of developing CAN-based application system shorten. The utilization of CANopen network must be reduced to improve the communication performance (e.g. worst-case response time). For reducing network utilization, messages need to be packed as many as possible so that message frame overhead can be decreased. In this paper, we suggested a PDO packing mechanism using object dictionary (OD) and process data object (PDO) communication service in CANopen. Through experiments, the performance of the mechanism is evaluated with SAE benchmark. As a result, network utilization is decreased about 10% compared to the result of the previous works.

A Survey on Utilization of Health Center and Health Service Demand of Residents in a Urban and Rural Unified Community (일개 도시·농촌 통합지역 주민의 보건기관 이용경험과 보건서비스 요구도 조사)

  • This survey was conducted to assess the utilization of health center including health sub-center and community health post and the health service demand of residents in a urban and rural unified community. Officials in Up·Myun·Dong offices visited randomly sampled 4,027 households(4.6% of total households in unified City) which included 3,337 households in urban area(4.9% of total households) and 690 households in rural area(3.7% of total households) and interviewed with heads or housewives of the households in September, 1995. There were significant differences in health-related demographic characteristics including age-sex distribution, educational level, period of residence in the community and medical insurance status of the interviewees between urban and rural areas. Of the respondents, 64.8% in urban area and 55.6% in rural replied that they had utilized the health center in the past. The most common purpose for visiting the health center was to get vaccination in urban area and to get outpatient care in rural area. The top priority health center activity that needs to be reinforced was communicable disease control and over 90% of the respondents preferred to have mobile clinic and home health care service in were also highly demanded. Eighty-six percent of the urban respondents replied that a health sub-center must be established in urban area. In the rural area, 90.3% of the respondents replied that they wanted to maintain the health sub-center and 88.3% wanted to maintain the community health post. Along with the improvement of facilities and equipments of the health center by Rural Health Service Improvement Project, new health service programs must be developed to meet the demand of the community.
