  • The effect of dietary pectin, tangerine pulp meal, propionate, lactate or fumarate on cholesterol (C) and triacylglycerol(TG) levels in the serum and liver, and the effect of pectin on dietary C absorption were studied in a series of three experiments. Mature female Sprague Dawley rats were fed a control diet or diets containing 5% pectin, 5% tangerine pulp meal, 3% propionate, 3% lactate 3% fumarate, or 10% pectin. Serum total C levels were lower(p<0.05) in rats fed the diet containing 5% pectin than in control rats after a 4-week feeding period(93.8 vs 119.2mg/100mL). Serum HDL, LDL+VLDL C levels were not different among diet groups. Liver total C level was also lower(p<0.05) in rats fed the diet containing 5% pectin than in control rats, but liver TG level was not influenced by diet. Dietary propionate, lactate or fumarate did not reduce serum C, indicating that propionate is not a regulator of serum C. However, dietary pectin(10%) increased fecal excretion of dietary C(or its metabolites) more than 70% over a control value. Our data indicate that dietary pectin reduces serum and liver C levels by increased fecal secretion of dietary C, but not by its fermentation product propionate or other gluconeogenic substrates. • Purpose: Patients suffering from acute poisoning by different substances often visit the emergency department (ED) and receive various prognoses according to the toxic material and patients' condition. Hyperlactatemia, which is an increased blood lactate level that generally indicates tissue hypoperfusion, is commonly utilized as a prognostic marker in critically ill patients such as those with sepsis. This study was conducted to investigate the relationships between blood lactate and clinical prognosis in acute poisoned patients. Methods: This retrospective study was conducted from January 2013 to June 2014 at a single and regional-tertiary ED. We enrolled study patients who were examined for blood test with lactate among acute intoxicated patients. The toxic materials, patient demographics, laboratory data, and mortalities were also reviewed. Additionally, we analyzed variables including blood lactate to verify the correlation with patient mortality. Results: A total of 531 patients were enrolled, including 24 (4.5%) non-survivors. Patient age, Glasgow coma scale (GCS), serum creatinine (Cr), aspartate transaminase (AST), and serum lactate differed significantly between survivors and non-survivors in the binary logistic regression analysis. Among these variables, GCS, AST, and lactate differed significantly. The median serum lactate levels were 2.0 mmol/L among survivors and 6.9 mmol/L among non-survivors. The AUC with the ROC curve and odds ratio of the initial serum lactate were 0.881 and 3.06 (0.89-8.64), respectively. Conclusion: Serum lactate was correlated with fatalities of acute poisoning patients in the ED; therefore, it may be used as a clinical predictor to anticipate their prognoses.

  • OBJECTIVES : Schizandra chinensis is well known for its efficacy at liver function reinforcement, relieving thirst and recovery from fatigue. In this study, we examined the effects of sports drink containing Schizandra chinensis on serum metabolic substrate, electrolyte, stress indicators, related-enzyme and exercise performance, rectal temperature, and heat shock proteinb70 (HSP70). METHODS : Elite long-distance runners (male, 21.3yrs, n=16) were selected and divided into two groups; an experimental group (EXP, n=8) and a control group (CON, n=8). A beverage containing Schizandra chinensis was supplemented 3 times per day to EXP for 4 weeks. Serum biochemical elements (glucose, lactate, total cholesterol, triglyceride, high density lipoprotein cholesterol, glutamate oxaloacetate transaminase, glutamate pyruvate transaminase, creatinine, creatine phosphokinase, lactate dehydrogenase, blood urea nitrogen, Na, K, Cl) were analyzed by auto blood analyzer. Exercise performance was measured by treadmill exercise test, HSP70 was detected by electrophoresis and Western blotting, and rectal temperature was measured by rectal temperature probe. RESULTS : Administration of the beverage increased significantly the rest level of blood Na, Cl and glucose and decreased significantly lactate dehydrogenase, glutamate oxaloacetate transaminase. No difference was found in exercise performance, rectal temperature increment or HSP70 concentration between groups. CONCLUSIONS : Administration of a sports drink containing Schizandra chinensis altered blood glucose, lactate dehydrogenase, glutamate oxaloacetate transaminase, Na and Cl levels.

  • Objectives: We aimed to evaluate the effect of Cervi Cornu and coenzymeQ10 on exercise and endurance capacity in rats and mice. Methods: The extract of Cervi Cornu was manufactured by the pharmacy department of Kyung Hee Oriental Medical Hospital, and CoQ10 soft cap (Ildong Pharmaceutical) was used. In total, 24 rats and 30 mice were divided into 3 groups: Control (rat=8, mouse=10), CoQ10 alone (rat=8, mouse=10), Cervi Cornu extract, and CoQ10 (rat=8, mouse=10). Ergogenic effect was evaluated by administering the Cervi Cornu extract and coenzymeQ10 to rats and measuring the time to exhaustion during treadmill running; endurance capacity was assessed by measuring cold water swimming time, serum lactate level, and serum corticosterone level in each group. At 1 week from the end of treatment, we recalculated time to exhaustion during treadmill running in rats to investigate the long-term effect of the Cervi Cornu extract and coenzymeQ10. Results: Cervi Cornu extract has long-term benefits in that it preserves the ergogenic effect caused by exercise. Cervi Cornu and coenzymeQ10 have no effect on increasing cold water swimming time in ICR mice. CoenzymeQ10 decreases the serum corticosterone level in ICR mice performing cold water swimming test. Conclusions: Cervi Cornu seems to preserve the ergogenic effect caused by exercise, but a larger study is needed to investigate effect of Cervi Cornu and coenzymeQ10 on improving endurance capacity. CoenzymeQ10 decreases serum corticosterone level and it is related with the anti-psychological fatigue effect.

  • Stress response to the seawater acclimation in coho salmon (Oncorhynchus kisutch) smolt was investigated. Salt concentration of rearing water was gradually increased for 3 days from freshwater to seawater (30 ppt salt level). The changes of serum concentrations of cortisol as a primary stress indicator, and as secondary indicators, glucose (GLC), lactate (LAC), triglyce.ide (TG), cholesterol (CHOL), sodium ion $(Na^+)$, chloride ion $(Cl^-)$ and enzyme activities (alanine aminotransferase, ALT: aspartate aminotrasferase, AST; lactate dehydrogenase, LDH) were quantified during the acclimation experiment. Among them, cortisol, LAC, TG, CHOL, ALT, AST concentrations showed rapid increase at the first exposure to the 10ppt salt level (day 1), and began to decrease to the constant values after day 2 of adaptation at 20ppt salt level. However, LDH concentration tended to decrease during the whole experimental period. $Na^+\;and\;Cl^-$ showed slight decrease at day 1, and increased to a little bit higher values after day 2 rather than those in freshwater. All the fishes started on taking a food after day 4 of seawater adaptation. From these results, to reduce osmotic shock inducible stress to fish in seawater acclimation, gradual increase of salt levels is recommended.

  • Coal workers' pneumoconiosis (CWP) and chronic obstructive pulmonary disease (COPD) are characterized as chronic inflammation of the lung in miners exposed to coal mine dust. The aim of the present study was to compare the levels of serum lactate dehydrogenase (LD) and C-reactive protein (CRP) as the inflammatory indices between subjects with CWP and those with CWP and COPD (CWP+COPD), among a total of 97 subjects (27 control, 40 CWP, and 30 CWP+COPD patients). The mean levels of serum LD (165.7 vs 184.6 U/L, p=0.016) and CRP (0.08 vs 0.15 mg/dL, p=0.002) in subjects with CWP were higher than those of in subjects without CWP. The mean level of serum CRP (0.10 vs 0.19 mg/dL, p=0.008) in subjects with COPD was higher than that in subjects without COPD. In an analysis of covariance adjusted by age, the mean level of serum CRP showed statistical significance among the study groups, control, CWP, and CWP+COPD (0.07 vs 0.13 vs 0.19 mg/dL, p=0.005); the mean level of serum CRP in the CWP+COPD group was significantly higher than that of the control group (p=0.001). The results suggest that a high level of CRP in the serum may be associated with CWP and COPD in retired coal miners.

  • Purpose: Pediatric acute liver failure (PALF) is a serious condition; however, data on PALF in developing countries are sparse, particularly concerning molecular diagnosis and liver transplantation (LT). This study aimed to determine the causes, outcomes, and prognostic factors of PALF. Methods: We retrospectively reviewed the medical records of children (age <15 years) with PALF diagnosed using the American Association for the Study of Liver Diseases criteria at our center from 2011 to 2016. The collected data included laboratory results, complications, outcomes, and potential factors associated with death and LT. Results: We included a total of 27 patients, with a median age of 2 years (interquartile range, 3 months to 4 years). Viral infection was the most common etiology (n=8, 30%), predominantly dengue infection (n=4). A total of 16 patients (59%) died and 11 patients survived (3 patients with LT). The prognostic factors associated with death or LT requirement were grade IV hepatic encephalopathy (p<0.01), hypotension (p=0.02), gastrointestinal bleeding (p=0.03), increased intracranial pressure (p=0.04), and higher peak serum lactate level (p=0.01). Peak serum lactate ≥6 mmoL/L had a sensitivity of 79% and a specificity of 88% for predicting mortality or the necessity of LT. Conclusion: Viral infection was the most common cause of PALF. The mortality rate remained high, and a considerable number of patients required LT. In addition to several clinical factors, peak serum lactate could be a potential marker for predicting poor outcomes in PALF.

  • Objectives: Cultivated wild ginseng (cWG), called SanYangSanSam, has been used clinically in patients with chronic fatigue in Korea. Little is known about effects of the ginseng distilled (volatile) components produced during evaporizaiton. Recently, we first identified one major component from cWG distilled extract, panaxydol, by using mass spectrometry. However, functional properties of cWG distilled extract and panaxydol remains elusive. Therefore, the present study evaluated the effect of cWG distilled extract or panaxydol on exercise-induced fatigue in rats. Methods: Fatigue was induced by forced swimming and the immobility time was analyzed in male Sprague-Dawley rats. The animals received intraperitoneally either vehicle, cWG distilled extract, or panaxydol 10 min prior to beginning of the forced swimming test (FST) once daily for 5 days. After the FST on day 5, we also analyzed fatigue-related biochemical levels including blood urea nitrogen (BUN), lactate acid (LAC), and lactate dehydrogenase (LDH) in serum and levels of glycogen in liver and soleus muscle. Results: The forced swimming time in cWG distilled extract (0.6 mL/kg)-treated group was significantly longer than that of control group on day 4 and 5. Panaxydol (0.1 and 0.25 mg/kg)-treated groups showed significantly enhanced performance in the forced swimming, compared to control. In addition, a significant decrease in serum LDH level was found in panaxydol-treated group, while there were no alternations in levels of serum BUN and LAC and glycogen in liver or soleus muscle. Conclusion: The present study demonstrated cWG distilled extract and its active component panaxydol have a function of anti-fatigue.
