Serogroup and Antimicrobial Resistance of Streptococcus pneumoniae Isolated from Oropharynx in Children Attending Day Care Center (유아원 소아의 구인강에서 분리된 폐구균의 혈청군과 항균제 내성에 관한 연구)

  • Purpose : Penicillin- and multidrug-resistant S. pneumoniae poses a serious threat to clinicians because the rate of resistance of S. pneumoniae to penicillin in Korea has surged up to the world's highest level. This study was performed to assess the carriage rate, serogroups and antimicrobial susceptibility of S. pneumoniae isolated from oropharynx in children. Methods : From March to July 1998, 209 children under 5 years of age were recruited from five day care centers. The carriage rate for pneumococci was obtained. Antimicrobial susceptibilities were determined with the E-test and agar dilution methods. Serogrouping was performed on 48 of the pneumococcal isolates by the Quellung reaction. Results : The carriage rate of S. pneumoniae was 30.1%. Antimicrobial susceptibility profiles were available for 59 of the isolates. Sixty-six percent of isolates were not susceptible to penicillin, and multidrug-resistance was observed in 76.3% of the isolates. A high proportion of the penicillin-resistant strains showed associated resistance to trimethoprim-sulfamethoxazole, tetracycline, erythromycin, and oxacillin. The most prevalent oropharyngeal serogroups were 19, 6, 3, 23, and 29. Resistance of the pneumococcal isolates to penicillin was different according to the serogroups. All of the strains of serogroup 19, 23, and 29 was resistant to penicillin but 87.5% of serogroup 3 strains were susceptible to penicillin. Conclusion : The resistance rate of S. pneumoniae isolated from oropharynx in children was very high to penicillin and other antimicrobial agents. For the reduction of the drug-resistant rate of S. pneumoniae, clinicians should be required to be more judicious in their use of antimicrobial agents.

  • Background : Legionella pneumophila has been recognized as an important cause of community-acquired pneumonia(CAP) requiring hospitalization. However, epidemiological data on the occurrence of legionella-related pneumonia is unavailable in Korea. The purpose of this study was to evaluate the etiological importance of legionella pneumophila serogroup 1 in patients hospitalized with CAP. Method : The CAP patients over 16 year-old were recruited from July 1999 to June 2000 at the Chunchon Sacred Heart Hospital. Fifty four patients (male 29, female 25, average age $63.8{\pm}15.3$) were included in this study. A diagnosis of a legionella pneumophila infection was based on a urinary antigen test using the Binax Company enzyme immunoassay. The severity of pneumonia was assessed using the Fine's PORT scoring system. Result : The average Fine's PORT score was 99.7(${\pm}44.9$). According to the risk classification proposed by the Infectious Disease Society of America, the number of patients in each class(from class I to class V) were 6(11.1%), 13(24.1%), 9(16.7%), 14(25.8%), and 12(22.2%), respectively. Thirty two patients(59.3%) were initially admitted to the intensive care unit. The mortality rate was 16.7%(9 in 54). In all patients, urinary antigens to Legionella pnewnophila serogroup 1 were not detected. Conclusion : Legionella pnewnophila may play little role in causing adult CAP in Korea. Therefore, the routine use of macrolide in the empirical treatment of the CAP patients based upon the ATS guidelines(1993) in Korea should be reevaluated.

  • This study was conducted to investigate O group serotyping, antimicrobial drug resistance and distribution of extended spectrum ${\beta}$-lactamase of 203 Escherichia coli(E. coli) isolated from poultry in Korea during the period from April in 2003 to December 2005. The serogroup of 69.4% of isolates was determinated ; O 78(32.5%), O88(7.9%). O15(6.9%) and O141(6.4%) were the most common. These E. coli isolates showed resistance to nalidixic acid(92.6%), streptomycin(81.8%), ampicillin(77.3%), ciprofloxacin(70.9%), sulfisoxazole(66.5%) and trimethoprim(58.1%), respectively. The bla CTX$_{-M-3\;like}$(2 strains) and bla$\;_{CMY-2}$(2 strains) genes producing extended spectrum ${\beta}$ - lactamase(ESBL) were detected in four wild strains resistant to the third generation cephalosporin, respectively. The presence of the ESBL genes was confirmed in all transconjugants by PCR analysis with primers encoding CTX-M-3 like types or CMY-2.

Community-acquired Legionnaires' Disease in a Newly Constructed Apartment Building

  • Objectives: Legionnaires' disease (LD) is a severe type of pneumonia caused by inhalation of aerosols contaminated with Legionella. On September 22, 2016, a single case of LD was reported from a newly built apartment building in Gyeonggi province. This article describes an epidemiologic investigation of LD and identification of the possible source of infection. Methods: To identify the source of LD, we interviewed the patient's husband using a questionnaire based on the Legionella management guidelines from the Korea Centers for Disease Control and Prevention. Water samples from the site were collected and analyzed. An epidemiological investigation of the residents and visitors in the apartment building was conducted for 14 days before the index patient's symptoms first appeared to 14 days after the implementation of environmental control measures. Results: Legionella pneumophila serogroup 1 was isolated from the heated-water samples from the patient's residence and the basement of the apartment complex. Thirty-two suspected cases were reported from the apartment building during the surveillance period, yet all were confirmed negative based on urinary antigen tests. Conclusions: The likely source of infection was the building's potable water, particularly heated water. Further study of effective monitoring systems in heated potable water should be considered.

CTX Prophages in Vibrio cholerae O1 Strains

  • The classical biotype strains of the Vibrio cholerae O1 serogroup harbor the biotype-specific cholera-toxin encoding phage (CTX) $CTX^{cla}$, and the El Tor biotype strains contain CTX-1. Although the classical biotype strains have become extinct, a remnant of classical CTX phage is transferred to the El Tor biotype strains. The prototype El Tor strains, which produce the biotype-specific cholera toxin, are now being replaced by atypical El Tor variant strains producing classical biotype cholera toxin. The genome sequences of the CTX phages in atypical El Tor strains indicate that the CTX phages in atypical El Tor strains are a mosaic of $CTX^{cla}$ and CTX-1. Before the emergence of atypical El Tor stains in the early 1990s, unusual pre-seventh pandemic strains were isolated in the US Gulf Coast between 1973 and 1986. These strains have characteristics of atypical El Tor strains since they are El Tor biotype strains containing $CTX^{cla}$, yet the genome sequence of this CTX phage indicates that it is different from $CTX^{cla}$ and is therefore classified separately as $CTX^{US\;Gulf}$.

Isolation and Identification of Legionella pneumophila from Hot Water Spouts and Hot Water (溫水수도꼭지와 온수에서 Legionella pneumophila의 分離同定)

  • A study was carried out to find distribution of Legionella spp. in hot water supplying systems of buildings in Seoul. Water samples were taken from the hot water taps in three research institute buildings during the period from the 2nd to 27th February, 1987. And all of the three buildings were supplied with hot water from the same central hot water tank. GVPC BCYEa and blood agar were used for the isolation of Legionella pneurnophila and slide agglutination test (SAT) was performed to identify their strain types. Main findings were as follows 1. 44 samples taken from hot tap-water revealed 20% of positive culture and 44 samples taken from hot water spouts revealed 11% of positive culture. 2. The 78% of positive samples taken from hot water was obtained from 25.1$\circ$C~45.0$\circ$C temperature range. 3. Only, Legionella pneumophila serogroup 3 was identified and most of them has been than 2$\mu$m in length. 4. The densities of the Legionella pneurnophila isolated from hot water samples were within the range of $1.0\times 10^2$ to $1.1\times 10^3$ CFU/l and their densities isolated from hot water spouts were either 1 CFU/plate or 2 CFU/plate.

Characterization of the Serotyping and the Plasmid Profile of E. coli Isolated from Foods and Clinical Specimens

  • Characteristics of the food isolates and the clinical specimens isolates of E. coli harboring virulence factor and their correlations were analyzed. The predominant serogroup were 08 and 027 in the food isolates and 06 and 018 in the clinical isolates, respectively, showing the different patterns in serogrouping between them. In the test of antibiotic susceptibility, the food isolates were resistant to cephalothin, streptomycin, tetracycline and minocycline, and the clinical isolates were resistant to ampicillin, carbenicillin, streptomycin, cephalothin, trimethoprim/sulfamethoxazole, tetracyclino and minocycline, respectively. It shows that E.coli isolated from food sources and clinical specimens might be correlated. Plasmid profile in the food and clinical isolates showed wide diversity. Especially, large sized plasmid DNA such as 60 MDa, 90 MDa and 120 MDa were observed. The plasmid DNA (60 MDa) containing a gene encoding hemolysin was found in 43% of the food isolates and 35% of the clinical isolates. To study chromosomal homology, PFGE analysis was performed, showing different restriction patterns by Xbal. This result indicates that there were no genetic correlations between the foods and the clinical isolates.

Capsular serogrouping and antimicrobial drug susceptibility of Pasteurella multocida isolated from Youngnam swine herds (영남지방 돼지에서 분리한 Pasteurella multocida의 협막혈청형 및 항균제 감수성 조사)

  • The capsular serogroupes and drug susceptibility of 111 isolates of Pasteurella multocida from pigs with atrophic rhinitis and pneumonic lesions were investigated. Of the 111 P multocida isolates, 42 were from lung lesions, 47 from nasal turbinate lesions and the remaining 22 from the nasal swabs. P multocida isolates were typed for capsular serogroupes A by hyaluronidase inhibition of capsule and D by acriflavine auto-agglutination. Most isolates(64.9%) were type A, 23.4% were type D and the remaining 11.7% were untypable. Resistance to triple sulfa(97.3%) was most frequent, followed by resistance to tiamulin(71.2%), tylosin(56.8%), streptomycin(36.9%), and neomycin(36.0%). The majority of the organisms were susceptible in order of prevalence to baytril(100%), ampicillin(98.2%), linsmycin(97.3%), colistin(97.3%), cephalothin(94.6%), gentamicin(93.7%), amikacin(92.3%), tetracycline(91.9%), trimethoprim/sulfamethoxazole(91.0%), and kanamycin(90.1%). No differences in drug resistance in relation to capsular serogroupes of P multocida and the origin of lesions were noted. A high prevalence of multiple drug resistance was observed and the most common resistant patterns were Sss, Tm, Ty(12.6%) and Sm, Sss, Tm, Ty(8.1%) patterns.

A Survey of Puteurella Multocida Isolated from Pigs Affected with Pneumonia in Eastern Kangwon (강원 동해안 지역 돼지 폐렴에서 분리한 puteurella multocida 에 대한 조사)

  • The present study was conducted to investigate the incidence of pasteurella multocida(p. multocida) infection and some properties of the isolated organisms from the swine herds in Eastern Kangwon during the periods from March 1993 to November 1993. The results obtained were summerized as follows: 1. The lungs of 180 slaughtered pigs were sampled and p. multocida was isolated from 38 pigs (21.1%) and cultured positive. 2. The majority of biochemical md cultural properties of the p. multocida isolates were identical to those of the standard strains. 3. We investigated the capsular serogroup and drug susceptibility of 38 Isolates of p. multocida from pigs with pneumonic lesions 4. p. multocida isolateds were typed for capsular serogroupes A by hyaluronidase inhibition of capsule and D by acriflavin auto agglutination. Most isolates(55.3%) were type A, 15.8% were type D, and the remaining 28.9% were untypable 5. In antimicrobial susceptibility test these isolates of p. multocida were susceptible in order of colistin(94.7%), ampicillin(94.7%), cepalothin(92.1%). gentamicin(92.1%), amikacin(89.5%), but the majority of them were resistant in order of neomycin(26.3%), teracycline(23.7%), streptomycin( 15.8%)

  • The present study conducted to evaluate the isolation frequency of Salmonella sap., Escherichia O157:H7 and Listeria monocytogenes in fecal samples collected form 33 dairy cow feedlots in West Gyeong-nam province from tan. 179\ulcorner to Feb. 1999. Salmonella spp. were isolated from 8.7% of fecal samples and 57.5% of dairy cow feedlots participaiting in this study had at least one positive fecal 7ample. The isolation rates of Salmonella spp. in each herd size were 42.9% in ln less than 125 heads, 63.6% in 25% to 50 heads and 75% in more than 50 heads. Of the positive sample, the most common serogroup recovered was Bl(31.5%), followed by C1 and D(22.9%), C2(16.2%), A(4.2%) and E(2.1%), in order. However, E. coli O157:H7 and Listeria monocytogenes were net detected in the fecal samples collected from all feedlots.
