Comparison of Denture Satisfaction by Burdened Cost Types (비용부담형태에 따른 의치 만족도 비교)

  • Purpose: The purpose of this study is to analyze denture satisfaction by burdened cost types. Methods: All 588 subjects in Gyeongsangnam-do, Korea was surveyed randomly. The study was conducted for about 24 days from March 28th to April 20th, 2014. The objects who participated in the survey were offered self-report questionnaires for this study. Results: Satisfaction with Half Price of the National Health Insurance Support Denture patients appears the highest point of $2.98{\pm}0.22$, and The Governor Pledge Free Denture appears $2.90{\pm}0.17$ the lowest. Also Pay Denture appears $2.96{\pm}0.21$. Provincial authority regulated medical carelessness inefficiently and at all during the procedure, without having to pay the cost of individual. also provincial authority support low budget. This is the result from the lowest satisfaction of The Governor Pledge Free Denture patient. Conclusion: According to the survey results, The Governor Pledge Free Denture and The Total Pay Denture's satisfaction are higher than The National Health Insurance Support Denture. and The National Health Insurance Support Denture are more uncomfortable than pay denture to use.

  • Purpose: This study aims to suggest political alternatives for nursing care costs for PCAs to provide qualitative medical benefit for patients with occupational disease by investigating present situation and problems of the nursing care cost system of Korea's Industrial Accident Compensation Insurance. Methods: Data was collected from 6 workers' accident medical corporation and 275 nurses affiliated with Korea labor welfare corporation using self reported questionnaire. Result: Research results were as follows; Character of nursing care cost of the Korea's Industrial accident Compensation Insurance changed as if it aims to support for living expenses for the family; As possible problems which could be caused under current system, administrative problems, decrease of service quality were made as objects of criticism. Some patients did not make every effort in rehabilitation to be beneficiaries continuously. Some patients were supplied with whole one PCA or all-night PCA even though they did not need as much caring as such. Conclusion: The research suggested that PCAs payment system improvement is necessary, and the presented nurses' opinion for the improvement method could be applied for policy making.

A Study for Exploring the Prevalence and Associated Factors of Unmet Health Care Needs due to Reduced Mobility: Evidence for Estimating Subjects of Visiting Health Care (거동불편 사유로 인한 미충족 의료의 규모와 관련 요인 탐색 연구: 방문의료 대상자 추계를 위한 근거)

  • Background: This study was designed to examine regional proportions for people who experienced unmet health care needs due to reduced mobility or unhealthiness and factors associated with experience of unmet health care needs by them. Methods: A total of 11,620 people were retrieved from the Korea Health Panel data (2014-2018). Regional proportions for people who experienced unmet health care needs due to reduced mobility or unhealthiness were estimated using cross-sectional weights and the factors associated with them were analyzed using generalized estimating equation. Results: The number of people who experienced unmet health care needs due to reduced mobility or unhealthiness was estimated as 278,083 in 2018. Women, the aged (65+), below elementary school, single as marital status, low income, bad self-rated health, people with disabilities, and long-term insurance beneficiaries were statistically significantly associated with experience of unmet health care needs due to reduced mobility or unhealthiness. Conclusion: Given high and dispersed demand for visiting health care, government need to expand the infrastructure and finance to facilitate visiting health care.

A Study on Nurses' Self-Leadership, Self-Esteem, and Organizational Effectiveness (간호사의 셀프리더십과 자아존중감 및 조직 유효성에 관한 연구)

  • Purpose: This study was conducted to define nurses' level of self-leadership, self-esteem, and organizational effectiveness, and to examine relationships among above variables. Methods: The subjects of this study were 313 nurses from two general hospitals located in Seoul Metropolitan area in Korea. The data was collected by self-reported instruments: Self-leadership scale, Self-esteem scale, Job satisfaction scale, Organizational commitment scale, and Turnover intention scale, Data was analyzed by descriptive statistics, Pearson correlation coefficient, and stepwise multiple reg res don u dng SPSS-PC program, Results: The average scores of variables were; self-leadership, 3,72/5,00 self-esteem, 3,76/5,00 Job satisfaction, 3,36/5.00 organizational commitment, 3.29/5.00 and turnover intentioncri 2.94/5,00, Self-leadership was dgnificantly correlated to self-esteem, job satisfaction, organizational commitment, and turnover intention. Self-leadership, job satisfaction, and turnover intention explained 52.3% of organizational commitment. Self-leadership and organizational commitment explained 43% of turnover intention. Conclusions: It is recommended to develop standardized self-leadership curriculum in the nursing school to build a foundation of self-leadership from undergraduate nursing students, Educational programs are needed that promote nurses' self-leadership as regular continuing education. Organizational effectiveness would be improved by providing programs to enhance nurses' self-esteem, educational motivation, and organizational commitment, which boost self-leadership.

The Influence of Self-care Behaviors, Empowerment and Social Support on Glycosylated Hemoglobin in Patients with Type 2 Diabetes (제2형 당뇨병 환자의 자가간호행위, 임파워먼트 및 사회적 지지가 당화혈색소에 미치는 영향)

  • Purpose: The purpose of the study was to analyze the effects of self-care behavior, empowerment, and social support on glycosylated hemoglobin in patients with type 2 diabetes. Methods: The data were collected during the period of July 1 to July 31, 2016. In total, 172 participants were recruited from outpatients who had been diagnosed with type 2 diabetes at a health care center, a health promotion center at National Health Insurance Corporation, and a tertiary hospital. Statistical data were analyzed with SPSS 20.0 using frequency analysis, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients, and hierarchical regression analysis. Results: The study results showed that self-care behavior (${\beta}=-.34$, p<.001), empowerment (${\beta}=-.34$, p<.001), and social support (${\beta}=-.20$, p=.018) were found to be influential factors affecting glycosylated hemoglobin, with an overall descriptive power of 69%. Conclusion: Self-care behavior, empowerment, and social support are considered to be important factors in blood glucose management for the patients with type 2 diabetes. Therefore, self-caring blood glucose programs and internal synchronizing education through social support and empowerment need to be improved.

  • Frauds detection is a difficult problem, requiring huge computer resources and complicated search activities Researchers have struggled with the problem. Even though a fee research approaches have claimed that their solution is much better than others, research community has not found 'the best solution'well fitting every fraud. Because of the evolving nature of the frauds. a novel and self-adapting method should be devised. In this research a new approach is suggested to solving frauds in insurance claims credit card transaction. Based on evolutionary computing approach, the method is itself self-adjusting and evolving enough to generate a new self of decision-makin rules. We believe that this new approach will provide a promising alternative to conventional ones, in terms of computation performance and classification accuracy.

  • Hawkes process is a self-exciting simple point process with clustering effect whose jump rate depends on its entire past history and has been widely applied in insurance, finance, queueing theory, statistics, and many other fields. Seol [27] proposed the inverse Markovian Hawkes processes and studied some asymptotic behaviors. In this paper, we consider an extended inverse Markovian Hawkes process which combines a Markovian Hawkes process and inverse Markovian Hawkes process with features of several existing models of self-exciting processes. We study the limit theorems for an extended inverse Markovian Hawkes process. In particular, we obtain a law of large number and central limit theorems.

Change of Medical Utilization Claims in Self-employees before and aster the Economic Crisis in Korea (IMF 경제위기 전.후 지역의료보험가입자들의 진료비 청구내용의 변화)

  • Objectives : To investigate the changing pattern of medical utilization claims following the economic crisis in Korea. Methods : The original data consisted of the claims of the 'Medical insurance program of self-employees' between 1997 and 1998. The data was selected by medical treatment day ranging between 8 January and 30 June. Medical utilizations were calculated each year by the frequency of claims, visit days for outpatients, length of stay for inpatients, total days of medication, and the sum of expenses. Results : The length of stay as an inpatient in 1998 was decreased 4.7 percent in comparison to 1997. However, inpatient expenses in 1998 increased 10.8 percent as compared to 1997. Inpatient hospital claims in 1998 increased 6.2 percent over 1997, although general hospital inpatient claims in 1998 decreased 3.3 percent in comparison to 1997. The outpatient claim frequency decreased 7.3 in 1998 percent as compared to 1997 Outpatient visit days of in 1998 were decreased 8.5 percent in comparison to that recorded in 1997. Outpatient claim frequencies of 'gu region' in 1998 decreased 10.5 percent comparison to that in 1997, but 'city and gun region' decreased less than 'gu region'. Conclusions : Medical utilization in 1998 deceased in relation to 1997 Medical utilization by outpatients decreased more than that of inpatients. Medical utilization by 'gu region' decreased mere than the other regions.

A Study on the Recognition about National Health Insurance Coverage of Denture, Implant of Elderly People (일부 노인층의 틀니, 임플란트 건강보험에 대한 인식도 연구)

  • The purpose of this study was to investigate the recognition on the national health insurance of denture, implant among the elderly. This survey was performed on 238 of the elderly aged over 60 years in Daejeon. The research was performed using a self-reported questionnaire and interview method from June to July, 2014. The collected data was analyzed using chi-square test, multiple response frequencies by PASW Statistics ver. 18.0. Recognition of national health insurance denture coverage was 76.9%. Channel of information awareness is higher in the media (61.8%). Awareness of application time (36.4%), medical expense by insurance (43.2%) is generally low. And awareness of denture follow up management is significantly low (18.6%). Time of denture and implant coverage needs were over 60 and 65 years old respectively. The respondents want the national health insurance to help medical expenses over 50%. Period of implant and denture re-production required unlimitedness 32.0% and 47.8%, participation to oral hygiene (dentures) management by dental hygienist was 94.1%. In conclusion, denture and implant coverage was higher awareness, but details were not recognized. Therefore, we should provide more detailed information. To increase the efficiency of national health insurance should be considered to lower the coverage age.

Policy Implications of the Long-term Care Insurance System Revitalization Process for Establishing Integrated Care System in Japan (일본의 지역포괄케어시스템 구축과 개호보험제도 개정과정의 정책적 시사점)

  • Recently, the Ministry of Health and Welfare has emphasized the necessity to build community care, and the interest of health care- long-term care-social welfare system continues. In order to examine the future vision of long-term care for elderly people, which can be a core system in establishing community models for elderly people in the future, we will implement aging in place as a national policy project. It is meaningful to analyze in depth the case of Japan's policy implementation through the amendment process of long-term care insurance system. The purpose of this study is to examine the concept definition, operating system, and major promotion process of the Integrated Care System in Japan and to suggest policy implications for the future Korea long-term care insurance system. As a result of reviewing the operation status and revision process of the long-term care insurance system for establishing the Integrated care system in Japan, it is necessary to clarify the basic principles and construction of community care system, diversification of residence type and upgrading work, establishment of linkage network of health care-long term care-social welfare, expansion of support for family and self-mind.
