Microleakage and characteristics of resin-tooth tissues interface of a self-etch and an etch-and-rinse adhesive systems

  • Objectives: This study was conducted to compare the microleakage and characteristics of the resin-tooth tissue interface between self-etch and etch-and-rinse adhesive systems after 48 hours and 3 months. Materials and Methods: 40 extracted premolar teeth were randomly divided into 2 groups: 1-step self-etch adhesive system - OptibondTM All-In-One, and 2-step etch-and-rinse adhesive system - AdperTM Single Bond 2. Both groups were subjected to 500 thermocycles (5℃-55℃) before scanning electron microscope (SEM) analysis or microleakage trial at 48-hour and 3-month time periods. Results: SEM images showed the hybrid layer thickness, diameter, and length of resin tags of the self-etch adhesive (0.42 ± 0.14 ㎛; 1.49 ± 0.45 ㎛; 16.35 ± 14.26 ㎛) were smaller than those of the etch-and-rinse adhesive (4.39 ± 1.52 ㎛; 3.49 ± 1 ㎛; 52.81 ± 35.81 ㎛). In dentin, the microleakage scores of the 2 adhesives were not different in both time periods (48 hours/3 months). However, the microleakage score of etch-and-rinse adhesive increased significantly after 3 months (0.8 ± 0.63 and 1.9 ± 0.88, p < 0.05). Conclusions: The self-etch adhesive exhibited better long-term sealing ability in dentin when compared to that of the etch-and-rinse adhesive. The greater hybrid layer thickness and dimensions of resin tags did not guarantee reliable, long-lasting sealing in the bonding area.

The effect of different adhesive system applications on push-out bond strengths of glass fiber posts

  • PURPOSE. Over the past years, the adhesion of fiber posts luted with simplified adhesive systems has been a matter of great interest. The aim of this study was to assess the post retentive potential of a self-adhesive resin cement using different adhesive systems to compare the push-out bond strengths of fiber posts. MATERIALS AND METHODS. The post spaces of 56 mandibular premolar roots were prepared and divided into 4 experimental groups and further divided into 2 subgroups according to testing time (n=7). The fiber posts (Rely X Fiber Post) were luted with a self-adhesive resin cement (RelyX Unicem) and one of the following adhesive systems: no adhesive, a total-etch adhesive resin (Single Bond), a two-step self-etch adhesive resin (Clearfil SE Bond) and a one-step self-etch adhesive resin (Clearfil S3 Bond). Each root was cut horizontally, and 1.5 mm thick six root segments were prepared. Push-out tests were performed after one week or three months (0.5 mm/min). Statistical analysis were performed with three-way ANOVA (${\alpha}$=.05). RESULTS. Cervical root segments showed higher bond strength values than middle segments. Adhesive application increased the bond strength. For one week group, the total-etch adhesive resin Single Bond showed higher bond strength than the self-adhesive resin cement RelyX Unicem applied without adhesive resin at middle region. For 3 months group, the two-step self-etch adhesive resin Clearfil SE Bond showed the highest bond strength for both regions. Regarding the time considered, Clearfil SE Bond 3 months group showed higher bond strength values than one week group. CONCLUSION. Using the adhesive resins in combination with the self-adhesive resin cement improves the bond strengths. The bond strength values of two-step self-etch adhesive resin Clearfil SE Bond improved as time passes.

Adhesion of 10-MDP containing resin cements to dentin with and without the etch-and-rinse technique

  • PURPOSE. This study evaluated the adhesion of 10-MDP containing self-etch and self-adhesive resin cements to dentin with and without the use of etch-and-rinse technique. MATERIALS AND METHODS. Human third molars (N=180) were randomly divided into 6 groups (n=30 per group). Conventional (Panavia F2.0, Kuraray-PAN) and self-adhesive resin cements (Clearfil SA, Kuraray-CSA) were bonded to dentin surfaces either after application of 3-step etch-and-rinse (35% $H_3PO_4$ + ED Primer) or two-step self-etch adhesive resin (Clearfil SE Bond). Specimens were subjected to shear bond strength test using the universal testing machine (0.5 mm/min). The failure types were analyzed using a stereomicroscope and quality of hybrid layer was observed under a scanning electron microscope. The data (MPa) were analyzed using two-way ANOVA and Tukey's tests (${\alpha}$=.05). RESULTS. Overall, PAN adhesive cement showed significantly higher mean bond strength ($12.5{\pm}2.3-14.1{\pm}2.4$ MPa) than CSA cement ($9.3{\pm}1.4-13.9{\pm}1.9$ MPa) (P<.001). Adhesive failures were more frequent in CSA cement groups when used in conjunction with two-step self-adhesive (68%) or no adhesive at all (66%). Hybrid layer quality was inferior in CSA compared to PAN cement in all conditions. CONCLUSION. In clinical situations where bonding to dentin substrate is crucial, both conventional and self-adhesive resin cements based on 10-MDP can benefit from etch-and-rinse technique to achieve better quality of adhesion in the early clinical period.

Effect of chlorhexidine application on the bond strength of resin core to axial dentin in endodontic cavity

  • Objectives: This study evaluated the influence of chlorhexidine (CHX) on the microtensile bonds strength (${\mu}TBS$) of resin core with two adhesive systems to dentin in endodontic cavities. Materials and Methods: Flat dentinal surfaces in 40 molar endodontic cavities were treated with self-etch adhesive system, Contax (DMG) and total-etch adhesive system, Adper Single Bond 2 (3M ESPE) after the following surface treatments: (1) Priming only (Contax), (2) CHX for 15 sec + rinsing + priming (Contax), (3) Etching with priming (Adper Single Bond 2), (4) Etching + CHX for 15 sec + rinsing + priming (Adper Single Bond 2). Resin composite build-ups were made with LuxaCore (DMG) using a bulk method and polymerized for 40 sec. For each condition, half of specimens were submitted to ${\mu}TBS$ after 24 hr storage and half of them were submitted to thermocycling of 10,000 cycles between $5^{\circ}C$ and $55^{\circ}C$ before testing. The data were analyzed using ANOVA and independent t-test at a significance level of 95%. Results: CHX pre-treatment did not affect the bond strength of specimens tested at the immediate testing period, regardless of dentin surface treatments. However, after 10,000 thermocycling, all groups showed reduced bond strength. The amount of reduction was greater in groups without CHX treatments than groups with CHX treatment. These characteristics were the same in both self-etch adhesive system and total-etch adhesive system. Conclusions: 2% CHX application for 15 sec proved to alleviate the decrease of bond strength of dentin bonding systems. No significant difference was shown in ${\mu}TBS$ between total-etching system and self-etching system.

Interface between calcium silicate cement and adhesive systems according to adhesive families and cement maturation

  • Objectives: This study aimed to evaluate the interface between a calcium silicate cement (CSC), Biodentine and dental adhesives in terms of sealing ability. Materials and Methods: Microleakage test: 160 standardized class II cavities were prepared on 80 extracted human molars. The cavities were filled with Biodentine and then divided into 2 experimental groups according to the time of restoration: composite resin obturation 15 minutes after Biodentine handling (D0); restoration after 7 days (D7). Each group was then divided into 8 subgroups (n = 5) according to the adhesive system used: etch-and-rinse adhesive (Prime & Bond); self-etch adhesive 2 steps (Optibond XTR and Clearfil SE Bond); self-etch adhesive 1 step (Xeno III, G-aenial Bond, and Clearfil Tri-S Bond); and universal used as etch-and-rinse or self-etch (ScotchBond Universal ER or SE). After thermocycling, the teeth were immersed in a silver nitrate solution, stained, longitudinally sectioned, and the Biodentine/adhesive percolation was quantified. Scanning electron microscopic observations: Biodentine/adhesive interfaces were observed. Results: A tendency towards less microleakage was observed when Biodentine was etched (2.47%) and when restorations were done without delay (D0: 4.31%, D7: 6.78%), but this was not significant. The adhesives containing 10-methacryloyloxydecyl dihydrogen phosphate monomer showed the most stable results at both times studied. All Biodentine/adhesive interfaces were homogeneous and regular. Conclusions: The good sealing of the CSC/adhesive interface is not a function of the system adhesive family used or the cement maturation before restoration. Biodentine can be used as a dentine substitute.

Effects of endodontic tri-antibiotic paste on bond strengths of dentin adhesives to coronal dentin

  • Objectives: The aim of this study was to evaluate the effects of tri-antibiotic paste (TAP) on microtensile bond strengths (MTBS) of dental adhesives to dentin. Materials and Methods: Sixty extracted molars had their occlusal surfaces flattened to expose dentin. They were divided into two groups, i.e., control group with no dentin treatment and experimental group with dentin treatment with TAP. After 10 days, specimens were bonded using self-etch (Filtek P90 adhesive) or etch-and-rinse (Adper Single Bond Plus) adhesives and restored with composite resin. Teeth were sectioned into beams, and the specimens were subjected to MTBS test. Data were analyzed using two-way ANOVA and post hoc Tukey tests. Results: There was a statistically significant interaction between dentin treatment and adhesive on MTBS to coronal dentin (p = 0.003). Despite a trend towards worse MTBS being noticed in the experimental groups, TAP application showed no significant effect on MTBS (p = 0.064). Conclusions: The etch-and-rinse adhesive Adper Single Bond Plus presented higher mean bond strengths than the self-etch adhesive Filtek P90, irrespective of the group. The superior bond performance for Adper Single Bond when compared to Filtek P90 adhesive was confirmed by a fewer number of adhesive failures. The influence of TAP in bond strength is insignificant.

산부식 전처리에 따른 2단계 자가부식 접착제의 연마 법랑질에 대한 미세인장결합강도 (The micro-tensile bond strength of two-step self-etch adhesive to ground enamel with and without prior acid-etching)

  • 자가부식 접착제는 사용하기 쉽고, 술식 민감성이 적은 장점이 있으나 특히 산도가 약한 자가부식 접착제의 법랑질에 대한 결합력은 논란이 되고 있다. 본 연구에서는 2단계 자가부식 접착제인 Clearfil SE Bond (Kuraray, Okayama, Japan)의 연마 법랑질에 대한 미세인장 결합강도를 측정하여 3단계 산부식수세 접착제인 Scotchbond Multi-Purpose (3M ESPE, St. Paul, MN, USA) 및 1단계 자가부식 접착제인 iBond (Heraeus Kulzer Gmbh, Hanau, Germany)의 결합강도와 비교하고자 하였고, 2단계 자가부식 접착제에 산부식 전처리를 시행하는 것이 법랑질에 대한 결합강도를 높일 수 있는지 알아 보고자 하였다. 실험군은 2단계 자가부식 접착제인 Clearfil SE Bond만 사용한 비산부식 군과 35% 인산 (Scotchbond Etchant, 3M ESPE)으로 산부식 후 Clearfil SE Bond를 사용한 산부식 군, 그리고 1단계 자가부식 접착제인 iBond를 사용한 군으로 나누었다. 대조군은 3단계 산부식수세 접착제인 Scotchbond Multi-Purpose를 사용한 군으로 정하였다. Bovine 전치의 순면을 십자가형으로 4등분하여 각 군으로 무작위로 배분하였다. 각 치아의 순면을 800-grit 실리콘 카바이드 지로 연마한 후 삭제된 법랑질면에 제조사의 설명서에 따라 각 군의 접착제를 적용하고 Light-Core (Bisco)로 적층 충전하였다. 시편은 $37i{\acute{E}}$, 증류수에 일주일 동안 보관한 후 low speed precicion diamond saw (TOPMENT Metsaw-LS, R&B, Daejeon, Korea)를 이용하여 약 $0.8{\times}0.8mm$ 단면이 되도록 시편을 절단하여 미세시편을 제작하였다. 일주일마다 증류수를 교환하면서 한 달, 세 달 동안 $37i{\acute{E}}$, 증류수에 미세시 편을 보관한 후 각각의 미세인장결합강도를 측정하였다. 미세인장결합강도 (MPa)는 파절 시에 가해진 힘 (N)을 접착면적 ($mm^2$)으로 나누어 계산하였다. 접착계면에서의 파절 양상은 실물현미경 (Microscope-B nocular, Nikon)을 이용하여 분류하였다. 미세인장결합강도에 대한 통계분석은 one-way ANOVA를 이용하여 유의수준 5%에서 검정하였고, 사후감정은 Least Significant Difference 방법을 이용하였다. 중합 후 1개월 뒤 측정된 각각의 접착제의 평균 미세인장결합강도는 유의수준 5%에서 모든 접착제 간에 유의한 차이가 없었다. 3개월 뒤 측정된 각각의 접착제의 평균 미세인장결합강도는 유의수준 5%에서 iBond 와 Clearfil SE Bond 비산부식 군과 Scotchbond Multi- Purpose 간에는 각각 유의한 차이가 없었다. 본 연구에서는 2단계 자가부식 접착제인 Clearfil SE Bond의 연마 법랑질에 대한 미세인장결합강도가 3단계 산부식수세 접착제인 Scotchbond Multi-Purpose 와 비교하여 유의한 차이가 없었으며 (P>0.05), 3개월 후의 결과에서 Clearfil SE Bond 비산부식 군의 미세인장결합강도가 Clearfil SE Bond 산부식 군보다 유의하게 낮았다 (P<0.05). 즉 35% 인산으로 산부식 전처리를 시행한결과 Clearfil SE Bond 의 법랑질에 대한 결합강도가 증가하였다.

Do universal adhesives promote bonding to dentin? A systematic review and meta-analysis

  • Objectives: The aims of this study were to conduct a systematic review of the microtensile bond strength (${\mu}TBS$) of multi-mode adhesives to dentin and to perform a meta-analysis to assess the significance of differences in the ${\mu}TBS$ of one of the most commonly used universal adhesives (Scotchbond Universal, 3M ESPE) depending on whether the etch-and-rinse or self-etch mode was used. Materials and Methods: An electronic search was performed of MEDLINE/PubMed, ScienceDirect, and EBSCOhost. Laboratory studies that evaluated the ${\mu}TBS$ of multi-mode adhesives to dentin using either the etch-and-rinse or self-etch mode were selected. A meta-analysis was conducted of the reviewed studies to quantify the differences in the ${\mu}TBS$ of Scotchbond Universal adhesive. Results: Only 10 studies fulfilled the inclusion criteria for the systematic review. Extensive variation was found in the restorative materials, testing methodologies, and failure mode in the reviewed articles. Furthermore, variation was also observed in the dimensions of the microtensile testing beams. The meta-analysis showed no statistically significant difference between the etch-and-rinse and self-etch modes for Scotchbond Universal adhesive (p > 0.05). Conclusions: Multi-mode 'universal' adhesives can achieve substantial bonding to dentin, regardless of the used modes (either etch-and-rinse or self-etch).

접착제의 다층적용이 복합레진의 전단결합강도에 미치는 영향 (INFLUENCE OF MULTIPLE ADHESIVE COATINGS ON THE SHEAR BONDING STRENGTH OF COMPOSITE RESIN)

  • 본 연구는 자가산부식 접착시스템과 자가산부식 프라이머 접착시스템의 다층적용이 혼성층 및 adhesive layer의 형성 양상, 그리고 전단결합강도에 미치는 영향 등을 비교 평가하기 위하여 시행되었다. 영구 구치 140개의 치아를 근원심으로 분할하여 협면 혹은 설면의 상아질을 이용하였다. 각 접착시스템을 1회, 2회, 3회 도포, 그리고 1회 도포 후 광중합한 후 1회의 추가 도포한 군으로 분류하였다. 전단결합강도와 혼성층 및 adhesive layer 두께를 측정하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 모든 접착제에서 도포횟수가 증가함에 따라 adhesive layer의 두께가 증가하였으며 (p<0.05), Adpor Prompt L-Pop과 Xeno III에서는 자가산부식 프라이머 시스템에서보다 작게 나타났다(p<0.05). 2. Adpor Prompt L-Pop에서 혼성층은 도포횟수에 따라 혼성층의 두께도 증가하였다(p<0.05). 3. Unifill Bond와 Clearfill SE Bond의 전단결합강도는 Scotchbond Multi-Purpose Plus와 Adpor Prompt L-Pop에서보다 높게 나타났으며 (p<0.05), Xeno III와는 유사하였다. 4. Adpor Prompt L-Pop은 접착제의 도포횟수가 증가함에 따라 전단결합강도는 유의하게 증가되었으며(p<0.05), AdheSE에서는 3회 도포한 경우 전단결합강도가 감소하는 경향을 보였다(p>0.05). 5. Adpor Prompt L-Pop과 Xeno III에서 첫 층을 중합한 후 두 번째 층을 추가 도포한 경우 전단결합강도는 감소하였다(p<0.05).

The effect of saliva decontamination procedures on dentin bond strength after universal adhesive curing

  • Objectives: The purpose of this study was to investigate the effectiveness of multiple decontamination procedures for salivary contamination after curing of a universal adhesive on dentin bond strength according to its etch modes. Materials and Methods: Forty-two extracted bovine incisors were trimmed by exposing the labial dentin surfaces and embedded in cylindrical molds. A universal adhesive (All-Bond Universal, Bisco) was used. The teeth were randomly divided into groups according to etch mode and decontamination procedure. The adhesive was applied according to the manufacturer's instructions for a given etch mode. With the exception of the control groups, the cured adhesive was contaminated with saliva for 20 sec. In the self-etch group, the teeth were divided into three groups: control, decontamination with rinsing and drying, and decontamination with rinsing, drying, and adhesive. In the etch-and-rinse group, the teeth were divided into four groups: control, decontamination with rinsing and drying, decontamination with rinsing, drying, and adhesive, and decontamination with rinsing, drying, re-etching, and reapplication of adhesive. A composite resin (Filtek Z350XT, 3M ESPE) was used for filling and was cured on the treated surfaces. Shear bond strength was measured, and failure modes were evaluated. The data were subjected to one-way analysis of variation and Tukey's HSD test. Results: The etch-and-rinse subgroup that was decontaminated by rinse, drying, re-etching, and reapplication of adhesive showed a significantly higher bond strength. Conclusions: When salivary contamination occurs after curing of the universal adhesive, additional etching improves the bond strength to dentin.
