폐결핵 환자의 지지체계와 낙인감 및 자가간호 수행도 (Support System, Stigma and Self-Care Behaviors in Patients with Pulmonary Tuberculosis)

  • Purpose: The purpose of this descriptive study was to identify factors influencing pulmonary tuberculosis patients' self-care behaviors. Methods: The patients were 125 adults over the age of 19 who were diagnosed with pulmonary tuberculosis, receiving care at the National tuberculosis hospital. The survey questions measured family support, multidimensional scale of perceived social support (MSPSS), stigma and self-care behaviors. Results: The average score for self-care behaviors was 3.91, ranging from 2.22 to 5.00. There was a significant positive correlation between self-care behaviors and support system. There was a significant negative correlation between self-care behaviors and stigma. The significant variables influencing self-care behaviors were family support (β = .33, P< .001), smoking (β= -.23, p= .002), age (β= .22, p= .005) and experience of stopping treatment (β= -.17, p= .040). These factors explain 36% of pulmonary tuberculosis patients' self-care behaviors. Conclusion: Family support is very important to improve selfcare for tuberculosis patients. However, those in their fifties, men, and those who do not have a job and have underlying diseases have low family support, so they need active support at the national level. The study results suggest that a educational and nursing programs must be designed to reduce stigma and promote support system in order to enhance self-care behaviors.

아동의 방과 후 자기보호와 문제행동과의 관계에서 어머니 지지 및 지역사회 환경의 중재효과 (Moderating Effects of Mother's Support and Community Environment on Relationships Between After School Self-Care and Problem Behavior)

  • This study examined moderating effects of mother's support and community environment on relationships between after school self-care and problem behaviors. Subjects were 579 3rd and 6th grade elementary school children. Major findings were positive relationships between after school self-care and problem behaviors. Neither gender differences nor grade differences were found in the relationships between after school self-care and problem behaviors. Moderating effects of care by relatives or neighbors on mother's support was shown in the relationships between after school self-care and internal problem behaviors. Moderating effects of proximity to harmful facilities were found in relationships between after school self-care and external problem behaviors.

  • Purpose: This study was to compare the differences in self-care behaviors between the group with fighting spirit and the group with helplessness. Methods: This study utilized a cross-sectional descriptive design. A total of 97 ambulatory cancer patients completed the Mental Adjustment to Cancer (MAC) scale by Watson et al. (1988) and Self Care Behaviors Scale by Oh et al. (1997). The data was analysed using frequency, percentage, t-test, ANOVA, Pearson's correlation and stepwise multiple regression. Results: Fighting spirit group were found to conduct self-care behaviors more compared to the helplessness group (t=3.346, p<.0001). The score of the self-care behaviors in fighting spirit group was 4.42 out of a total score of 7 and that of helplessness group was 3.85. There was a significant difference (p<.0001) in self-care behaviors according to the level of faith and performance status. Fighting spirit ($R^2$=0.120, p<.0001), performance status ($R^2$=0.078, p=.001) were predictive of self-care behaviors. Conclusion: Cancer patients' mental adjustment is correlated with the degree of self-care behaviors. Having fighting spirit is one of the most adaptive techniques that cancer patients could use in adjustment to the life with cancer.

심부전 입원 환자의 자가관리 행위 이행에 영향을 미치는 요인 (Factors Affecting to Adherence to Self-care Behaviors among Inpatients with Heart Failure in Korea)

  • Purpose: This study was to analyze adherence to self-care behaviors and identify factors affecting the adherence behaviors among inpatients with heart failure. Methods: A total 94 hospitalized inpatients from three hospitals participated in a survey. Data were collected using structured self-reported questionnaire from November 28, 2011 to March 31, 2013 and analyzed using frequency, t-test, ANOVA, Pearson's correlation coefficients and stepwise multiple regression. Results: The score of adherence to self-care behaviors among inpatients with heart failure was $26.02({\pm}8.84)$. Factors related to the adherence to self-care behaviors were living with spouse (t=-2.47, p=.019), functional state (t=2.18, p=.034), heart failure knowledge (r=-.49, p<.001), social support (r=-.35, p<.001), self-control (r=-.25, p=.016), and self-care confidence (r=-.24, p=.019). The factors affecting adherence to self-care behaviors were heart failure knowledge, self-care confidence, and social support. These factors explained 32% of the variance in adherence to self-care behaviors. Conclusion: The adherence to self-care behaviors with heart failure can be improved if heart failure knowledge, self-care confidence, and social support are improved. Therefore, developing a nursing intervention program for patient with heart failure that is considered these factors leads to improve quality of life and prevent readmission.

분위회귀분석법을 이용한 노인 고혈압 환자의 자가간호에 따른 분위별 영향 요인 (Predictors of Self-care Behaviors among Elderly with Hypertension using Quantile Regression Method)

  • Purpose: The objective of this study was to identify the predictors of self-care behaviors among elderly patients with hypertension using quantile regression method. Methods: A total of 253 elderly patients diagnosed with hypertension was recruited via 3 different medical clinics for the study. The quantile regression and a liner regression was conducted using Stata 12.0 program by analyzing predictors of self-care behaviors. Results: In the ordinary least square, self-efficacy, period of disease, and education level explained 42% of the variance in self-care activities. In the quantile regression, affecting predictors of self-care behaviors were self-efficacy for all quantiles, the period of disease for from 60% quantile to 90% quantile, education level for 20%, 30%, and 50% quantiles, economic status for 10%, 50%, and 60% quantiles, age for 10%, 70% quantiles, fatigue for 10% quantile, knowledge about hypertension for 10% and 20% quantiles, and depression for 30% and 40% quantiles. Conclusion: The affecting predictors of self-care behaviors among elderly with hypertension were different from the level of self-care behaviors. These results indicated the significance in assessing predictors according to the level of self-care behaviors when clinical nurses examine the patients' health behaviors and plan any intervention strategies. Specially, education level and knowledge about hypertension were the significant predictors of self-care activities for low quantiles. Clinical nurses may promote self-care activities of the given population though health education programs.

노인 당뇨병환자의 인슐린요법 관련 자가간호행위에 대한 영향요인 (Factors Influencing Self-Care Behaviors Related to Insulin Therapy in Elders with Diabetes Mellitus)

  • Purpose: Self-care behavior is a key dimension of healthcare quality among elderlys with diabetes mellitus. The purpose of this study was to identify factors associated with self-care behaviors related to insulin therapy among elders with diabetes mellitus. Methods: The research was a cross-sectional, descriptive design using questionnaires. The participants were 147 older adult patients with diabetes mellitus receiving insulin therapy. Data were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with SPSS statistics 19.0 program. Results: Participants' self-care behaviors related to insulin therapy were not adequate. Stepwise multiple regression analysis for self-care behaviors related to insulin therapy revealed that the most powerful predictor was self-efficacy. Self-efficacy, family support and frequency of blood sugar test explained 54.4% of the variance in self-care behaviors related to insulin therapy. Conclusion: The results indicate a need to promote self-efficacy, family support and frequency of blood sugar testing with these participants. In addition, strategies to improve self-care behaviors among elderly patients should be considered by health professionals, family members and the elderly patients themselves.

치열교정환자의 투명 교정장치에 대한 지식 및 자가관리행태 (Knowledge and self care behaviors of orthodontic patients using clear aligner)

  • Objectives : The purpose of the study is to investigate the knowledge and self care behaviors of orthodontic patients using clear aligner. Methods : A total of 181 orthodontic patients using clear aligner in Daegu filled out the self-reported questionnaire from September 6 to October 18, 2010. and 162 subjects completed orthodontic treatment. Results : There existed a significant difference in knowledge and self care behaviors between gender and wearing duration of clear aligner(p<0.01). Treatment duration influenced significantly on the score of self care behaviors(p<0.01) and the high score of self care behavior showed in short duration of treatment. Conclusions : Self care behaviors were closely correlated with treatment duration. An information for the clear aligner will be helpful to improve the knowledge and self care behaviors.

  • Purpose: This study aimed to compare self-care behaviors and depressive symptoms between the young old (65-74 yr) and the old-old (75-84 yr) in low-income women with hypertension. Methods: This study used a descriptive research design. The subjects of this study were 136 elderly women over 65 yr living in D city. Data was collected from September to December 2007 through personal interviews using a questionnaire. The collected data was analyzed using the SPSS WIN 12.0 Program. Results: Self-care behaviors, functional status, and number of medications showed a significant difference between young elderly and middle elderly. There was a negative correlation between self-care behaviors and depressive symptoms. 52.2% of variance in self-care behaviors of young elderly and 76.8% of variance in self-care behaviors of middle elderly were explained by depressive symptoms, number of medications, and functional status. Conclusion: It is necessary to manage depressive symptoms to improve the self-care behaviors of low-income elderly with hypertension. Depressive symptoms need to be considered in planning hypertension programs for low-income elderly women.

당뇨환자의 자기관리행위 및 자기효능감에 대한 추후 전화상담의 효과 (The Effects of Telephone Consulting Follow-up on Self care behaviors and Self-efficacy in Diabetic Patients)

  • This study was conducted to identify the effects of telephone consulting follow- up on self care behaviors and self efficacy in diabetic patients after discharge. The subjects for this study were consisted of 39 patients and twenty subjects assigned to the experimental group received the telephone consulting follow-up after discharge once a week during 4 weeks with diabetic education before discharge and nineteen subjects assigned to the control group received diabetic education before discharge without telephone consulting follow-up. The results of this study are as follows: 1. The experimental group which received the telephone consulting had higher self care behaviors scores than the control group (t=4.92, p=.00). 2. The experimental group which received the telephone consulting had higher self efficacy scores than the control group (t=4.71, p=.00). 3. The relationship between self efficacy and self care behaviors showed a significant correlation in the subjects. In conclusion, the telephone consulting follow-up improved self care behaviors and self efficacy, therefore this intervention can be effective in promoting the care of diabetic patients

지역사회 노인 고혈압 환자의 건강정보이해능력이 고혈압 자가관리행위에 미치는 영향 (The impact of health literacy on self-care behaviors among hypertensive elderly)

  • Objectives: This study thus set out to examine the health literacy and self-care behaviors of community-dwelling elderly patients with hypertension and investigate their relations and influential factors, thus providing basic data to develop a nursing intervention program to promote self-care behaviors among hypertension patients. Methods: The subjects include 180 hypertension patients using community health centers, public senior centers, and welfare centers in Jeju. Results: The subjects scored a mean of 49.8 (${\pm}12.4$) points on health literacy and 55.8 (${\pm}6.7$) points on self-care behaviors. In the results of hierarchical regression analysis, health literacy independently predicted self-care behaviors (${\beta}=.12$, p=.050) after controlling for general characteristics, hypertension-related knowledge and self-efficacy. Conclusions: These results suggest that interventions for improving health literacy are important to enhance elder's ability of self-care behaviors. It is necessary to understand the health literacy of elderly patients with hypertension before providing them with educational intervention and information and to develop appropriate educational materials and intervention programs.
