Effectiveness of e-health systems in improving hypertension management and awareness: a systematic review

  • Recent studies have focused on self-management of hypertension using smart devices (cellular phones, tablets, watches). It has proven to be an effective tool for early detection and control of high Blood Pressure (BP) without affecting patients' daily routines. This systematic review surveys the existing self-monitoring systems, evaluate their effectiveness and compares the different approaches. We investigated the current systems in terms of various attributes, including methods used, sample size, type of investigation, inputs/ outputs, rate of success in controlling BP, group of users with higher response rate and beneficiaries, acceptability, and adherence to the system. We identified some limitations, shortcomings, and gaps in the research conducted recently studying the impact of mobile technology on managing hypertension. These shortcomings can generate future research opportunities and enable it to become more realistic and adaptive. We recommended including more observable factors and human behaviors that affect BP. Furthermore, we suggested that vital monitoring/logging and medication tuning are insufficient to improve hypertension control. There is also a need to observe and alter patient behavior and lifestyles.

Optimal endoscopic drainage strategy for unresectable malignant hilar biliary obstruction

  • Endoscopic biliary drainage strategies for managing unresectable malignant hilar biliary obstruction differ in terms of stent type, drainage area, and deployment method. However, the optimal endoscopic drainage strategy remains unclear. Uncovered self-expandable metal stents (SEMS) are the preferred type because of their higher functional success rate, longer time to recurrent biliary obstruction (RBO), and fewer cases of reintervention than plastic stents (PS). Other PS subtypes and covered SEMS, which feature a longer time to RBO than PS, can be removed during reintervention for RBO. Bilateral SEMS placement is associated with a longer time to RBO and a longer survival time than unilateral SEMS placement. Unilateral drainage is acceptable if a drainage volume of greater than 50% of the total liver volume can be achieved. In terms of deployment method, no differences were observed in clinical outcomes between side-by-side (SBS) and stent-in-stent deployment. Simultaneous SBS boasts a shorter procedure time and higher technical success rate than sequential SBS. This review of previous studies aimed to clarify the optimal endoscopic biliary drainage strategy for unresectable malignant hilar biliary obstruction.

Predicting Factors of Self-esteem in Patients with Hemodialysis (혈액투석환자의 자아존중감 예측요인)

  • The purpose of this study was to examine the predictive relationships between the predictors and self-esteem among hemodialysis(HD) patients. Predictive correlational design, participants 154 HD patients in the Seoul. SPSS for Window 22.0 used to perform descriptive statistics, Pearson's Correlation Coefficient and Stepwise Regression analysis. The strongest predictor was uncertainty (${\beta}=-.33$, p<.001), economy (${\beta}=.18$, p<.05), occupation (${\beta}=.17$, p<.05) and health status (${\beta}=.16$, p<.05) were followed. A total explained variance was 30.0% of self-esteem (Adjust $R^2=.30$). Managing or preventing uncertainty, providing vocational education in consideration of the dialysis environment, providing employment, increasing economic level, and maintaining good health status are factors that enhance self-esteem. Conducted to investigate the self-esteem of HD patients considering the machine-dependent characteristics, future, provide the data of the study to improve the self-esteem of HD patients. Also, nursing intervention should be developed based on the findings.

Convergence effects of husband's help on self-efficacy and postpartum depression in mother (남편의 도움이 산모의 자기효능감과 산후우울에 미치는 융합적 영향)

  • This study is to find the convergence effect of a husband's help on self-efficacy and postpartum depression in mother. Each husband did mother and baby care until postpartum 6th week after completing three sessions of education. The data was collected in the 1st and 6th week postpartum, and analyzed using SPSS 20.0. Husbands's practice rate was 80%. Factors affecting to mothers' self-efficacy were parity, antepartum admission and husband's job, and to their postpartum depression were mothers' education, monthly income and antepartum depression. Prevalence of postpartum depression in the 1st and 6th week were 33.3% and 1.7% each. There were significant differences in increasing self-efficacy and in decreasing postpartum depression(p<.001), and there was a negative correlation between two variables(r=-.62, p=.01). Husbands' help brought convergence effects on increasing mothers' self-efficacy and decreasing postpartum depression(p<.001). This results could be used in developing intervention programs in managing mothers' self-efficacy and postpartum depression after repeated studies with wider range of subjects.

Mediating Effect of Self-Esteem on the Relationship between Perceived Discrimination Occupational Stress of School Nurse (보건교사의 차별인식과 직무스트레스와의 관계에서 자아존중감의 매개효과)

  • Objectives: This study examined the relationship between discrimination perception and occupational stress of school nurses to verify the mediating effects of self-esteem on the relationship between the perception of discrimination and occupational stress. Methods: This study was a secondary analysis of the data collected by the Public Health Nurses Association of D city from regular school nurses working in elementary, middle, and high schools in September 2017. The data were analyzed using the SPSS 25.0 Statistics Program, and multiple regression analysis was used to analyze the effects of self-esteem on the relationship between the perception of discrimination and occupational stress. Results: The occupational stress of school nurses was higher with higher discrimination and lower self-esteem, and the moderating effects of self-esteem (β=-.152, p=.016) was confirmed between the perception of discrimination and occupational stress (F=30.117, p<.001). Conclusions: Based on the results of this study, it is necessary to prepare a method for managing occupational stress due to job demands of school nurses as well as organizational measures to reduce discrimination in organizations. In addition, efforts to develop various programs focusing on improving the self-esteem of school nurses are required.

The Influence of Mothers' Transformational Leadership in the Family on the Psychological Well-Being and Career Decision-Making Self-Efficacy of Adolescents (어머니의 가정내 변혁적리더십이 청소년자녀의 심리적복지감 및 진로결정자기효능감에 미치는 영향)

  • The purpose of this study was is to investigate the relationship between transformational leadership in families and psychological well-being and career decision-making self-efficacy. The samples were randomly selected from high school students studying liberal arts in Seoul. 400 survey questionnaires were distributed March 11 through 18, 2010. Among them, 386 completed surveys were utilized as the data for the statistic analyses. SPSS were applied. The main results of the study are as follows. First, adolescent children of mothers with leadership have higher psychological well-being than those from families of mothers without leadership. This has been determined as a result of reviewing the psychological well-being of adolescent children according to their mothers' transformational leadership in the family. Second, the adolescent group with mothers who show transformational leadership in managing the home have higher career decision-making self-efficacy than the group without such mothers. This was determined as a result of reviewing adolescent children's career decision-making self-efficacy according to mother's transformational leadership level in the family. Third, career decision-making self-efficacy according to adolescents' psychological well-being show positive relations by group. That is, adolescents living table lives are more positive about their future. Lastly, when comparing the relative contribution of the mother's transformational leadership in the family and psychological well-being on career decision-making self-efficacy, it has gained more credibility as stage ads. Based on the results, when mothers nurture children with a affection-based autonomy, children gain higher psychological well-being, which leads to firm beliefs about constructive futures. Therefore, for successful and systematic career development, CEO mothers' transformational leadership should serve as its foundation.

A Study on Understanding of Time Constraints and Time Management of Farmers (농업인의 시간제약과 시간관리에 관한 인식 분석)

  • This study aimed to provide an efficient time management scheme for farmers by analysing how they experience time constraints in spending their time and understand time management. Firstly, farmers found that they didn't have enough time for farm keeping as a result of house work obligations. It was caused by excessive time for agricultural work. Thus, 93.7% of respondents recognized that ‘they were pressed for time' because there is relatively less time to spend for socio-cultural and physiological matters. In addition, 83.0% responded that they felt the pressure of something. Respondents who were in forties reduced their sleeping hours due to lack of time. The majority of people who were in the farmhouse doing controlled horticulture cannot enjoy their own time for leisure owing to a lack of time. Secondly, respondents under thirty-nine years of age were solving the problems of time constraints as prioritizing matters, planning handling time and utilizing household instruments. On the other hand, respondents who were in forties found the solution by reflecting on their conduct toward a time consumption plan, self-evaluation and received help from their family and/or neighbors. A group of respondents who had already graduated from college or higher education often reflected on their conduct and self-evaluated. Respondants who participatied in farmhouse controlled horticulture, generally took turns in managing their house work. For reducing farmers' time constraints and managing their time efficiently, they should standardize matters done repeatedly and also manage time by dividing work among family members. Furthermore, if a long-term time scheme such as more than a month is established and practiced, time constraints may be somewhat solved.

Evaluation of the Effects of Self-Managed Percussion Therapy Using Video Education: A One-Group Pretest-Posttest Pilot Study for Burn Patients' Pruritus Management (화상환자의 소양증 관리를 위한 경타법 적용 효과)

  • Purpose: Post-burn pruritus is one of the most prevalent complications experienced by burn patients. Though medications are prescribed for pruritus, managing this condition can prove challenging. The aim of this research was to develop and disseminate non-pharmacological self-management educational resources, and subsequently evaluate the effectiveness of implementing a percussion therapy approach. Methods: A one-group pretest-posttest design was employed to evaluate the effect of percussion therapy on one hundred patients, all of whom had suffered deep second-degree burns and undergone skin graft surgery. Variables such as the severity of pruritus, distress caused by pruritus, and satisfaction with pruritus treatment were scrutinized. A paired t-test was conducted to compare these variables before and after the intervention. Results: The majority of participants were diagnosed with third-degree burns, and the average Total Body Surface Area (TBSA) affected was 25.2%. Post-test results revealed a marked reduction in both severity and distress caused by pruritus. The intervention led to significant decreases in average severity and pain due to itchiness, and an increase in the satisfaction level following the application of percussion therapy. Conclusions: The implementation of the percussion was identified as effective in diminishing post-burn pruritus, alleviating pruritus-associated pain, and enhancing satisfaction with pruritus treatment. The percussion approach presents itself as a non-invasive, non-pharmacological strategy for managing pruritus in burn patients.

The Influence of Job Stress on Job Attitude and Moderating Effects of Organizational Support among Hospital Nurses (임상간호사의 직무스트레스와 직무태도의 관계에 있어 조직지원인식의 조절효과)

  • The purpose of this study is to analyze the effects of job stress on job attitude among nurse practitioners, determine moderating effects of perceived organizational support in this process, and provide preliminary data to devise a scheme for managing job attitude effectively in nursing manpower. The research model was composed of 7 sub-variables of job stress, which was a independent variable; job attitude, which was a dependent variable, was divided into job satisfaction and organizational commitment; and variables of moderating effects included perceived organizational support. The data for empirical analysis of the model were collected from 300 nurses working in one university hospital and two general hospitals in Busan by using structured self-administered questionnaires. The main results of empirical analysis were as follows: Perceived organizational support was effective in preventing job stress from worsening job attitude. Moderating effects of perceived organizational support were found to exist in relations between job stress and organizational commitment. These results imply that a sense of organizational support has both direct and moderating effects on job attitude and can be a good means of managing job attitude.

An Exploratory Study of Strata Residential Properties Problems in Peninsular Malaysia and How They are Resolved (말레이시아 주택의 구분소유권에 관한 탐색연구)

  • This study identifies the common problems faced by the owners of strata titles, the Management Corporation, the Joint Management Body, the Commissioner of Building (COB) as well as the Managing Agent being the stakeholders in managing strata properties. The methods employed are qualitative in nature. Analysis is done based on reports published by the relevant authorities dealing with strata residential properties as well as the case law as reported in the two leading journals in Malaysia such as the Malayan Law Journal and the Current Law Journal. The types and nature of problems are derived from the annual reports. The extent of the problems is determined based on the figures and supported by observation and interviews with the COB, being the agency directly involved in overseeing and monitoring strata properties management. The findings show that a substantial number of problems exist in the management of strata properties despite a law that allows the owners to self-managed their own properties. Having stated the status quo concerning the problems, the study also looks at the various means of resolving disputes as exist under the Malaysian housing system. The study proposes that alternative dispute resolution (ADR) modes are more appropriate in ensuring the sustainability of strata living and management.
