A Study on Disease, Self-Esteem, Social Support and Health Management Behavior in Blind & Visually Handicapped Students (시각장애 학생들의 원인질환 및 사회적 지위, 자아존중감, 건강관리 행위에 관한 연구)

  • Blindness or visually handicaps are found to be an important factor destroying lifestyles as well as a physical problem. According to the data of handicapped person's(v.h.p) registered in Korea, the number of v.h.p. has not decreased. Especially, visual handicapped adolescents (v.h.a.) suffer from an insufficiency of social support under the current education system. There is a special education system for v.h.a. in Korea, and, as a result v.h.a.'s are educated separately from general students and communites. Compared with the self-conception of general students, v.h.a.'s have a more negative self-conception. Among former studies 51.7% of v.h.p's are not in good health. This, of course, means that their handicap impacts on other health problems. The first purpose of this study is to find out the causes of disease and the degree of social support, self-esteem, health management behavior provided and then to analyze the effect of self-esteem and social support on their behavior. To accomplish these purposes, visually handicapped the middle and high school students in Seoul city and Kyung-gi province were selected and 300 students sampled. The main results are as follows : 1) The highest specific handicap is impaired lenses (19.7%). 2. There were high correlations between health management behavior, self-esteem, and social support. 3. Social support is a more important variable than self-esteem on health management behavior. 4. Age of entry and length of stay in their special dormitories, and teacher's support account for 16% of psychological health management behavior 5. Age of entry in their special dormitory and total degree of social support account for 12% of preventive health behavior.

Comparative Studies on the Self Voice Assessment of Voice Disorder Patients and the Hearer Voice Assessment of a Comparative Group of normal subjects (음성장애인의 자가음성평가와 정상음성인의 청자음성평가 특성 비교)

  • This paper will discuss the difference between self assessment of voice disorders and the hearer voice assessment of a comparative group of normal subjects. The study was conducted on 25 voice disorder subjects and 32 hearers of a comparative group of normal subjects. The results are as follows. Firstly, in K-VHI and VHI-H, the hearers of the comparative group of normal subjects perceived more serious voice disorders than the voice disorder group in all sub-domains. Likewise, in K-VQOL and VRQOL-H, the hearers of the comparative group of normal subjects perceived more serious voice disorders than the voice disorder group in all sub-domains. Secondly, the hearer voice assessment of the comparative group of normal subjects showed no difference in gender regarding the perception of the severity of voice disorder issues. Thirdly, the hearer voice assessment of the comparative group of normal subjects states that in the emotional aspects of VHI-H, professional voice users perceive more serious voice disorders than others. Accordingly, in VRQOL-H, there was no difference in use of the voice between professionals and others.

Difference Between Self-Evaluation and Professional Evaluation in Child Care Centers with Less than 21 Children (21인 미만 보육시설에 대한 자체평가와 전문가평가의 차이 분석)

  • This study intended to provide basic materials for establishing a diversity of plans for assisting childcare centers, by examining differences between self-evaluation and professional evaluation in childcare centers with less than 21 children. Data were collected from 7 childcare centers to measure evaluation in five categories of accreditation. Significant differences were found between self-evaluation and professional evaluation in "child care curriculum" and in "interaction between teachers and children." These results point up the handicap of heavy dependence on self-evaluation as it leads to self-assistance, with limited access to new ideas. On the other hand, "circuit assistance" where a consortium of 2~3 different childcare centers advise and assist each other could effectively stimulate a more professional outlook.

Relationship among Parenting Stress, Self-Esteem and Social Support on Mental Retardation Child Mother's (정신지체아 어머니의 양육 스트레스, 자아존중감 및 사회적 지지와의 관계)

  • Purpose. The study verified the relationship between the mental retardation child mother's parenting stress, self-esteem and social supports. Methods. The subjects were 250 mothers who had their children with mental retardation educated and who experienced to receive social services in U City. They were asked to answer the questionnaire and 134 subjects resent the questionnaire. The survey information was analyzed by SPSS 18.0. Results. Sociodemographic characteristic effected on the parenting stress; In accordance with the severity of handicap, the level of the stress was different, self-esteem are the mother's educational attainment. The social supports in functional perspective are the religion. Conclusions. The relationship between the parenting stress and social support, self-esteem and parenting stress the both have negative relationship, More information support, emotional support, economic support, evaluating support get the less the parenting stress, and self-esteem have. The parenting stress and self esteem are highly related.

  • Purpose : Aim of the this study was conducted to determine whether hearing aids are responsible for the quality of life by comparing quality of life between the elderly who are suspected of difficulty in hearing selected by Hearing Handicap Inventory of Elderly (HHIE) without using hearing aids and who are using hearing aids. Methods : For the study, two groups of 33 people were analyzed for independent t-test based on whether they wore Hearing Aids (HA) or not whom wearing hearing aids (hearing aids users) and suspected difficulty in hearing. who are attending the Religious organization Senior Welfare Center in the OO region. Result : The quality of life of hearing aid users is generally observed to be higher than that of non-users in general, and by the section, psychological (p=0.019) and living environment (p=0.032) were appeared statistically signigicant. The elderly showed a significant difference. In the section, "Operation Satisfaction (p=0.038)" in the physical health area, "relationship of reason (p=0.018)" in the social relationship area, "Economic margin (p=0.021)" in the environmental area, "Information accessibility (p=0.020)", In the psychological area, hearing aid was used for "self satisfaction (p=0.033)", "concentration of work (p=0.045)". Conclusion : The use of hearing aids by hearing loss elderly improves the quality of life in the psychological health and living environment section.

A Study of depression symptom in patients with voice disorders (음성장애환자에게서의 우울감 연구)

  • The objectives of this study are to research the frequency of depression symptom in patients with voice disorders and to investigate parameters associated with depression from voice evaluation. A hundred ninety six patients(106 males and 90 females) who had been diagnosed with voice disorders first in their lifetime were selected. All the patients were examined by laryngeal stroboscopy. For depression and voice study, personal interview, acoustic and aerodynamic analysis, voice handicap index(VHI), reflux symptom index(RSI), and beck depression index(BDI) were done respectively. Mild to severe BDI were seen in 26.2%(52 patients) of the whole patients. A BDI mean score of female patients was $8.8{\pm}7.5$ which was higher than that of male patients($5.6{\pm}6.6$), the difference observed being statistically significant(p<0.001). In the acoustic analysis, the score of sent_duration parameter was increasing in the patients with depression, which was significantly higher than the score of the patients without depression(p<0.05). In the addition, the scores of VHI and RSI were higher in the patients with depression(p<0.001). Our findings suggest that the prevalence of depression in patients with voice disorders is related to female, speaking velocity, and self-questionnaire. This result can be used for psychologically based approach to therapy.

Rasch Analysis of the Clinimetric Properties of the Korean Dizziness Handicap Inventory in Patients with Parkinson Disease (파킨슨병 환자에서 한국어판 Dizziness Handicap Inventory의 라쉬 분석에 의한 임상측정 특성 평가)

  • Objectives: The Korean Dizziness Handicap Inventory (KDHI), which includes 25 patient-reported items, has been used to assess self-reported dizziness in Korean patients with Parkinson disease (PD). Nevertheless, few studies have examined the KDHI based on item-response theory within this population. The aim of our study was to address the feasibility and clinimetric properties of the KDHI instrument using polytomous Rasch measurement analysis. Methods: The unidimensionality, scale targeting, separation reliability, item difficulty (severity), and response category utility of the KDHI were statistically assessed based on the Andrich rating scale model. The utilities of the orderedresponse categories of the 3-point Likert scale were analyzed with reference to the probability curves of the response categories. The separation reliability of the KDHI was assessed based on person separation reliability (PSR), which is used to measure the capacity to discriminate among groups of patients with different levels of balance deficits. Results: Principal component analyses of residuals revealed that the KDHI had unidimensionality. The KHDI had satisfactory PSR and there were no disordered thresholds in the 3-point rating scale. However, the KDHI showed several issues for inappropriate scale targeting and misfit items (items 1 and 2) for Rasch model. Conclusions: The KDHI provide unidimensional measures of imbalance symptoms in patients with PD with adequate separation reliability. There was no statistical evidence of disorder in polytomous rating scales. The Rasch analysis results suggest that the KDHI is a reliable scale for measuring the imbalance symptoms in PD patients, and identified parts for possible amendments in order to further improve the linear metric scale.

Children's Parent-related Stress, Social Support and School Adjustment according to Self-esteem of Children with Handicap & Nonhandicap Parents (장애부모 아동과 일반아동의 자아존중감에 따른 부모관련 스트레스, 사회적 지원 및 학교적응)

  • This study investigates the differences in parent-related stress, social support, and school adjustment according to self-esteem of children with disabled and non-disabled parents. The data were collected from 176 students between 4th and 6th grade in Chungbuk area. For a data analysis, mean, frequency, percent, three-way ANOVA, and Pearson's correlation were used. The results show that children's perceived parent-related stress, social support, and school adjustment are varied according to their self-esteem. Those with higher self-esteem were found to have tusker perception level in social support and school adjustment. On the contrary, they were found to have lower level in parent-related stress. The findings indicate further that girls have a closer relationship with their teachers than boys and that children with disabled parents have a closer relationship with their teachers than those with non-disabled parents. However, as opposed to prediction, parental disability itself caused no difference in the level of parent-related stress.

Application of HHIE-S(Hearing Handicap Inventory for the Elderly-Screening version) to screening test of noise-induced hearing loss (소음성 난청 선별검사에 HHIE-S(Hearing Handicap Inventory for the Elderly-Screening version)의 적용)

  • The study was conducted from May to September in 1994 to investigate applicability of the Hearing Handicap Inventory for the Elderly-Screening version(HHIE-S) in parallel with the pure-tone audiometer to the initial screening test of noise-induced hearing loss(NIHL) in some noise-exposed workers. Subjects were selected by systemic sampling that took every 10th person from 6, 700 workers taking the annual occupational health examination by the department of Health Maintenance of Dongsan Hospital Keimyung University in Taegu. The authors administered the pure-tone audiometric test and self-reported questionnaire of HHIE-S including items of sociodemographic and job-related variables concurrently. The final subjects analysed were 1,019(488 males and 531 females) excluding fourteen persons who had many missing values in their questionnaires. The reliability coefficients of HHIE-S scale by Cronbach's alpha were 0.84. In the univariate analysis of hearing handicap measured by the HHIE-S, work duration, military service and the hearing threshold loss at 1KHz and 4KHz by the initial audiometer were significant in males while age, work duration and hearing threshold loss at 1KHz and 4KHz were significant in females. In the stepwise linear regression analysis, hearing threshold loss at 1KHz and 4KHz, was the only selected variable explaining the hearing handicap in males and hearing threshold loss at 1KHz and 4KHz, age, and work duration were selected in females. In ROC curves for HHIE-S scores against NIHL as gold standard which was defined by the follow-up audiogram as more than 30dB of the average of 0.5/1/2KHz and 50dB at 4KHz, the optimal cutoff for the parallel HHIE-S appeared to be 8. The results suggest that HHIE-S appeared to have some reliability and validity in this data and might be used in screening NIHL in parallel with pure-tone audiometer in noise-exposed workers.

Effectiveness of Self-efficacy Promoting Vestibular Rehabilitation Program for Patients with Vestibular Hypofunction (전정기능저하 어지럼 환자를 위한 자기효능증진 전정재활운동 교육의 효과)

  • Purpose: In this study an examination was done of the effect of self-efficacy promoting vestibular rehabilitation (S-VR) on dizziness, exercise self-efficacy, adherence to vestibular rehabilitation (VR), subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness in patients with vestibular hypofunction. Methods: This was a randomized controlled study. Data were collected 3 times at baseline, 4 and 8 weeks after beginning the intervention. Outcome measures were level of dizziness, exercise self-efficacy, and level of adherence to VR. Subjective and objective vestibular function, vestibular compensation and the recurrence of dizziness were also obtained. Data were analyzed using Windows SPSS 21.0 program. Results: After 4 weeks of S-VR, there was no difference between the groups for dizziness, subjective and objective vestibular functions. However, exercise self-efficacy and adherence to VR were higher in the experimental group than in the control group. After 8 weeks of S-VR, dizziness (p =.018) exercise self-efficacy (p <.001), adherence to VR (p <.001), total-dizziness handicap inventory (DHI) (p =.012), vision analysis ratio (p =.046) in the experimental group differ significantly from that of the control group. The number of patients with recurring dizziness were higher in the control group than in the experimental group (p <.001). Conclusion: The results indicate that continuous 8 weeks of S-VR is effective in reducing dizziness, and improving exercise self-efficacy, subjective vestibular function and adherence to VR. Objective vestibular function and vestibular compensation were also improved in the experimental group at the end of 8 weeks of S-VR.
