The Study of Gait Parameter and Plantar Foot Pressure during Walking in Adults with Down Syndrome (다운증후군 성인의 보행 특성과 족저압에 관한 연구)

  • Purpose : The purpose of this study was to examine the gait parameter and plantar foot pressure of adults with Down syndrome(DS) during walking in order to provide data for developing evidence-based deficit or common rehabilitation strategies. Method : 15 participants with DS(12 men, 3 women; age $26.06{\pm}4.47$) and 15 healthy subjects(12 men, 3 women; age $25.33{\pm}3.43$) were matched age. They walked at self selected speeds on a GAITRite system and RS-scan system, and had the following measurements done: cadence, stride length, step width, foot angle, percent stance, percent double support, and plantar foot pressure in 10 areas of the foot. Results : In comparison of gait parameter(cadence, stride length, step width, foot angle, percent stance, and percent double support) between adults with DS and healthy subjects, there was significant differences(p<.05). Regarding plantar foot pressure during gait with or without DS, there were statisically significant differences in the area of Toes 1-5, Metatasal 1-4, Midfoot, and Heel(Medial and lateral)(p<.05). Conclusion : Our data show that DS walk with a less physiolosical gait pattern and plantar foot pressure than healthy subjects. Based on our results, DS patients need targeted rehabilitation and exercise strategies.

A Clinical report on psychogenic torticollis (심인성 사경증 환자 치험 1례)

  • Objectives : The purpose of this study was to investigate the clinical application of Korean Traditional treatment to psychogenic torticollis patient who has panic disorder as underlying disease. Methods : Herb medicine used in this study are Heosu-san(Huishou-san) which is previously used for torticollis in several studies, Guibi-tang(Guipi-tang) and Gamiondam-tang(Jiaweiwendan-tang) which are frequently used for phychological disease. Also, physical therapy and aroma therapy were practiced which basically stretches and relaxes muscles around cervical muscles. Self exercise of cervical muscles was carried out during acupuncture. Results : After the comprehensive treatment, the clinical symptoms of the patient were improved. Conclusions : It is suggested that oriental medical treatment are effective in psychogenic torticollis.

Comparison of Gait Patterns on Pregnant's Kinematic Factors and Lower-Limb Joint Moments During Pregnant Period (임신 기간에 따른 임산부 보행의 운동학적 요인과 하지 관절모멘트 패턴 비료)

  • The purpose of this study was to compare gait patterns during pregnancy. Because of the changes in hormone levels and anatomical changes such as body mass, body-mass distribution, joint laxity, and musculotendinous strength that result from pregnancy, it was possible that there would be certain gait deviations associated with these changes. Three-dimensional gait analyses were performed from a self-selected pace, and six subjects(height : $163{\pm}5.3cm$, mass : $61.3{\pm}3.80kg$, $65.3{\pm}5.14kg$, $70.2{\pm}4.98kg$) participated in the three times(the early, middle and last years). 7 cameras(Proreflex MCU-240, Qualisys) and 2 force plates (Type 9286AA, Kistler) were used to acquire raw data. The parameters were calculated and analyzed with Visual-3D and Joint moments computed using inverse dynamics. In conclusion, pregnant women's gait patterns were changed during pregnancy period because pregnancy makes them physical changes. The main changes were joint moments and kinematic factors during pregnancy period. The pregnancy transformed normal gait pattern Into toe out position. Therefore, exercise programs to improve muscle activity were necessary where joint moments were small. The development of simulator should be studied for pregnant women's tailored shoes and accessories in future.

The Effects of a Comprehensive Life Style Modification Program on Glycemic Control and Stress Response in Type 2 Diabetes (스트레스 중재를 포함한 포괄적인 생활습관개선 프로그램이 제2형 당뇨병환자의 당조절과 스트레스 반응에 미치는 효과)

  • Purpose: This study was to evaluate the effects of a comprehensive life style modification program on glycemic control and stress response in type 2 diabetes. Method: The participants(n=34) with type 2 diabetes were divided into either a usual care(control) or treatment(experimental) group. The experimental group(n=21) received a program that was based on a comprehensive life style modification protocol at a weekly meeting for 16 weeks. They also participated in individually prescribed exercise and diet along with stress management and self monitoring. The participants were followed for 6 months, during which postprandial glucose, HbA$_{1C]$, and stress response inventory were measured. Result: The experimental group showed a significant lower postprandial glucose and stress response compared to those of the control group. However, there was no significant change in the HbA$_{1C]$ value in either group. Conclusions: These results suggest that a type 2 diabetes comprehensive lifestyle modification program may lead to clinical improvement in glycemic control and reduce the stress response.

Analysis of the Nursing Interventions Performed by Public Health Nurses in Health Centers Using the NIC (보건소 간호사의 간호중재 분석 - 간호중재분류[NIC]의 적용 -)

  • Purpose: The purpose of this study was to identify nursing interventions performed by public health nurses in health centers. Method: Data was collected by the taxonomy of Nursing Intervention Classification(NIC 3rd: 486 nursing interventions) from 131 public health nurses in health centers and analyzed using descriptive statistics. Result: As its result, more than 50% of public health nurses performed 137 nursing interventions at least monthly. The most frequently used intervention class was 'activity and exercise management', followed by 'physical comfort promotion', 'community health promotion', 'life span care', 'coping assistance', 'Self care facilitation', 'information management', 'nutrition support', 'community risk management' and 'patient education'. One hundred twenty nursing interventions were rarely performed by 90% or more of the nurses. Most of them were the physical complex domain. Conclusion: In conclusion, 137 interventions were performed by public health nurses at least monthly. NIC is helpful to build a standardized language for public health nursing.

The Relationship between Physical Activity and Insulin Resistance in the Middle-Aged Adults (중년기 성인의 신체활동과 인슐린 저항성의 관계)

  • Purpose: This study was designed to find the correlations between physical activity and insulin resistance of the middle-aged adults. Methods: One hundred thirty one subjects participated in this study were age 40-60 from Y university's center for physical exercise in W city. The data were collected from August 5 to October 5, 2009. To measure physical activity, the contracted Korean version of the Self-Report of Physical Activity Questionnaires of IPAQ was used. Insulin resistance was measured using fasting glucose levels, serum insulin levels, and HOMA method (serum insulin${\times}$fasting glucose/22.5). Results: The continuous physical activity overall in this study was on average $1,792.30{\pm}2,216.81$ MET (min/week), and as a result of categorical classification: no activity was 66 subjects (50.4%); minimum activity, 41 (31.3%); and health-improving activity, 24 (18.3%), respectively. The overall degree of insulin resistance in these subjects was $2.20{\pm}2.62$(0.28-12.74). There was negative correlation between moderate intensity activity and insulin resistance (r= -.189, p<.05). Conclusion: These results revealed that promoting moderate-intensity physical activity is important in preventing and improving insulin resistance and possibly other metabolic risk factors in the middle-aged adults.

  • 1. Korean archery is very important in body-mind therapy because it has excellent effect in physical and spiritual culture. 2. Korean archery culture has physical effect that reinforce eum-gyeong(陰經) muscles, thus it much affect on reinforcement of ha-cho(下焦) and ha-dan-jeon(下丹田). 3. Korea archery has spiritual effect : etiquette, concentration, a state of balance, discipline, self-control, modesty, courage. 4. Author present model of korean archery culture and think enforcement the model will affect therapy and prevention Korean archery therapy is on elementary level, lack clinical datas. If the data of therapy which is proved objectively is more larger, korean archery therapy will be more popular by exercise therapies.

Influencing Factors of stress in clinical practice in dental hygiene students (치위생과 학생의 임상실습 스트레스 영향 요인)

  • Objectives: Identifying influencing factors of stress in clinical practice of dental hygiene students through controlling the amount of stress in clinical practice. Methods: A self-reported questionnaire was filled out by 226 dental hygiene students in Daejeon, Cheongju and Jeolla province from March 2 to 30, 2016. The contents of the questionnaire included general characteristics (6 items), clinical practice characteristics (4 items), clinical practice stress (16 items), quality of life (24 items), health promotion behavior (20 items), OHIP (14 items), and critical thinking disposition (15 items). Data was analyzed using SPSS 19.0. for one way ANOVA, scheffe' posthoc test, Pearson correlation coefficients, and multiple regression analysis. Cronbach's alpha of clinical practice stress, quality of life, health promotion behavior, OHIP, and critical thinking disposition were 0.827, 0.913, 0.896, 0.921 and 0.778, respectively. Results: Clinical practice stress was 3.38 points, quality of life was 3.40 points, health promotion behavior was 3.21 points, OHIP was 4.21 points and critical thinking disposition was 3.17 points. Conclusions: Factors that had impact on clinical practice stress include sleeping state, satisfaction of clinical practice, practice number, and the quality of life, and they appeared statistically significant (p<0.05). This exercise showed the necessary manuals and system implementations in clinical practice, which can reduce the amount of stress in dental hygiene students.

The Correlation between Musculoskeletal Pain and Level of Work Stress of Prosthetists and Orthotists (의지.보조기 기사의 근골격계 통증과 직무스트레스 수준의 관련성)

  • PURPOSE: It is known that work stress was linked to WRMDs in workplace. The purpose of this study was to identify the characteristics of work related musculoskeletal disorders (WRMDs) related to job stress among prosthetists and orthotists. METHODS: Eighty-six self-reported symptoms of discomfort (SRSD) and job stress were collected by questionnaire and analyzed. RESULTS: The results were as follows: Over 70% of respondents had WRMDs. The shoulder was the most painful region and each of "within 15 minutes", "once a week" and "severe moving or doing exercise" items were main characteristics in the SRSD. Finally, the "repetition of simple work" item in job stress ranked as resulting in "often discomfort" in over 43% of respondents. Additionally, there was a statistically significant correlation between job stress and pain intensity (p<.05, $r_s$=.36). CONCLUSION: Further studies are needed to identify the factors affecting musculoskeletal symptoms and job stress of prosthetists and orthotists in Korea.

A Survey of Characteristics and Management of Low Back Pain in Postpartum Women (산모의 요통 실태와 관리에 대한 조사 연구)

  • Many pregnant women have experienced low back pain (LBP) during pregnancy and after delivery, and it has been an important component in women health. This study was designed to investigate the characteristics and management of the LBP in postpartum women. Eighty-five postpartum women were participated in this survey. Mean age of 85 women was 28.1 years. Of 85 postpartum women, 55.3% (n=47) had LBP after pregnancy. Thirty of 47 women had pain on lumbar region, 17 postpartum women had pain on sacroilium region. Of 85 postpartum women, 74% (n=54) had LBP before pregnancy and 71.8% (n=61) had LBP during pregnancy. Of 47 postpartum women who had LBP, 83% (n=39) had not received medical management for LBP, 12.8% (n=6) took medication, and 4.3% (n=2) performed self-exercise. None of postpartum women had received physical therapy during pregnancy and after delivery for treatment low back pain. The pain in SI region was more severe than in lumbar region after pregnancy according to VAS (visual analog scale) (p<.05). However, there was no significant difference in VAS scores between SI pain and lumbar pain before and during pregnancy (p>.05). Pain region after delivery was related to pain region of pre-pregnancy and during pregnancy (p<.01). Pain level after delivery was related to the pain and night pain level during pregnancy (p<.01).
