A Systematic Review of Moxibustion for Frozen Shoulder (동결견에 대한 뜸치료 효과의 체계적 문헌고찰)

  • Objectives The purpose of this research is to evaluate the effectiveness of moxibustion theraphy for frozen shoulder through systematic review and meta-analysis. Methods The effect of moxibustion for frozen shoulder was constructed as a key question to search randomized controlled trials (RCTs) in 16 databases without type of publication restriction. Results A total of 790 studies were searched and 22 RCTs were finally selected through primary and secondary selection/exclusion process. In most of the evaluation indexes used in each study, moxibustion treatment for shoulder pain was statistically significant compared with other treatments. A meta-analysis of effective rate in 10 studies comparing moxibustion with acupuncture and acupuncture showed a statistically significant effect of moxibustion treatment. And there was a significant effect on the improvement of the shoulder mobility, but the effect size of shoulder pain was no significant effect for shoulder pain reduction. In addition in the result of meta-analysis of 4 studies' effective rate comparing moxibustion with electronic acupuncture, the effect of moxibustion was significantly higher than that of electronic acupuncture in frozen shoulder. Conclusions Moxibustion therapy was effective in symptoms in patients with frozen shoulder. However because of the high risk of bias in the quality of the included studies, the applicability of this study seems to be limited. It is thought that future randomized controlled trials will need to be designed and performed by reviewing ways to minimize bias.

Area Classification of Hazardous Gas Facility According to KGS GC101 Code (KGS GC101을 통한 가스시설 폭발위험장소의 설정)

  • Technical practice code, KGS GC101 2018, for explosion hazard area selection and distance calculation of gas facility was enacted and implemented from July 12, 2018. This code includes whole contents of IEC60079-10-1 2015 (Explosive atmospheres Part 10-1: Classification of areas - Explosive gas atmospheres), and clarifies the interpretation of ambiguous standards or adds guidelines for standards. KGS GC101 is a method for classifying explosion hazard place types: (1) Determination of leak grade (2) Determination of leakage hole size (3) Determination of leakage flow (4) Determination of dilution class (5) Determination of ventilation effectiveness, finally (6) Determination of danger place (7) Explosion The range of dangerous places can be estimated. In order to easily calculate this process, the program (KGS-HAC v1.14, C-2018-020632) composed by Visual Basic for Application (Excel) language was produced by Korea Gas Safety Corporation. We will discuss how to use codes and programs to select and set up explosion hazard zones for field users.

  • Unlike benign tumors, malignant tumors are capable of metastasis, easy to relapse, poor survival, and low quality of life. In Korea, here is a tendency to treat the tumors collectively according to the General Principles of Cancer Chemotherapy(GPCC) of the Health Insurance Review & Assessment Service (HIRA). But recently, companion diagnostics(CDx) is recommended rather than unilateral medication because biomarker-based molecular diagnostics is possible to predict the drug response of patients before drug treatment. Not only domestic but also overseas Food and Drug Administratio (FDA) recommends the development of the CDx system at the stage of drug development to ensure the responsiveness and safety of medicines. In this study, I focused on the necessity of CDx development direction as well as CDx development status through literature review. Furthermore I also discussed CDx types according to the molecular diagnostic technology such as immunohistochemistry (IHC), polymerase chain reaction (PCR), in situ hybridization (ISH), and next-generation sequencing (NGS) not only in the approved CDx but also in the developing one by US FDA. And I suggested the technology issue of CDx development process such as a selection of molecular diagnostics at the time of release, a clear understanding of the CDx mechanism, and a convergence of drug with CDx development. The necessity of social insurance system also was proposed for CDx development.

A Preliminary Study on the Evaluation of Internal Exposure Effect by Radioactive Aerosol Generated During Decommissioning of NPPs by Using BiDAS (BiDAS를 적용한 원전 해체 공정 시 발생되는 방사성 에어로졸의 내부피폭 영향평가 사전 연구)

  • Radioactive aerosol generated in cutting and melting work during the NPP decommissioning process can cause internal exposure to body through workers' breath. Thus, it is necessary to assess worker internal exposure due to the radioactive aerosol during decommissioning. The actually measured value of the working environment is needed for accurate assessment of internal exposure, but if it is difficult to actually measure that value, the internal exposure dose can be estimated through recommended values such as the fraction of amount of intake and the size of particles suggested by the International Committee on Radiological Protection (ICRP). As for the selection of particle size, this study applied a value of $5{\mu}m$, which is the size of particles considering the worker recommended by the ICRP. As for the amount of generation, the amount of intake was estimated using data on the mass of aerosol generated in a melting facility at a site in Kozloduy, Bulgaria. In addition, using these data, this study calculated the level of radioactivity in the worker's body and stool and conducted an assessment of internal exposure using the BiDAS computer code. The internal exposure dose of Type M was 0.0341 mSv, that of Type S was 0.0909 mSv. The two types of absorption showed levels that were 0.17% and 0.45% of the domestic annual dose limit, respectively.

An Analysis on Determinants of the Capesize Freight Rate and Forecasting Models (케이프선 시장 운임의 결정요인 및 운임예측 모형 분석)

  • In recent years, research on shipping market forecasting with the employment of non-linear AI models has attracted significant interest. In previous studies, input variables were selected with reference to past papers or by relying on the intuitions of the researchers. This paper attempts to address this issue by applying the stepwise regression model and the random forest model to the Cape-size bulk carrier market. The Cape market was selected due to the simplicity of its supply and demand structure. The preliminary selection of the determinants resulted in 16 variables. In the next stage, 8 features from the stepwise regression model and 10 features from the random forest model were screened as important determinants. The chosen variables were used to test both models. Based on the analysis of the models, it was observed that the random forest model outperforms the stepwise regression model. This research is significant because it provides a scientific basis which can be used to find the determinants in shipping market forecasting, and utilize a machine-learning model in the process. The results of this research can be used to enhance the decisions of chartering desks by offering a guideline for market analysis.

Environmental Impact and Water Foot Print Assessment of Pot Bearing Using Life Cycle Assessment (LCA) (LCA를 이용한 교량용 포트받침 환경영향 및 물발자국 분석)

  • In this research, LCA analysis of the manufacturing process of pot bearing for fixed, movable in all directions, movable in one direction was carried out to analyze the environmental load using the LCA methodology. Especially, the water footprint that has been and issue in recent years was analyzed. As a result of LCA, it was analyzed that the contribution of the plate was more than 64.2% in all of the six impact categories in the case of fixed pot bearing base, and more than 94% in the category of resource depletion and photochemical oxidant creation. In the case of all direction pot bearing and one direction pot bearing, the contribution of PTFE was the highest in the global warming and stratospheric ozone depletion, and the contribution by the plate was higher in the other impact categories. The water footprint of each type of pot bearing was analyzed as $22.4m^3\;H_2O\;eq/kg$ for one direction pot bearing, $17.1m^3\;H_2O\;eq/kg$ for fixed pot bearing, and $14.1m^3\;H_2O\;eq/kg$ for all direction pot bearing. As a result of life cycle analysis, the contribution of water use in manufacturing was more than 65% in all three types. The results of this study can be used as basic data for decision making in construction method and material selection of bridges in the future.

A Review on Clinical Research Trends in the Treatment of Post Traumatic Stress Disorder (PTSD) in Korean Medicine (외상후 스트레스장애 치료에 대한 한의학 임상연구 동향)

  • Objectives: The purpose of this study was to review the clinical research trends in the treatment of post traumatic stress disorder (PTSD) in Korean medicine (KM). Methods: We searched MEDLINE, CENTRAL, EMBASE, Google Scholar and five Korean databases through May 2019, for studies on KM to treat PTSD. Clinical research that conducted KM treatment of PTSD patients were included. Two researchers independently conducted study selection and data extraction process. Results: Totally, eight studies were included in this review. Types of traumatic events that patients experienced included physical violence/threatening, traffic accidents, sexual violence and personal tragic events. KM interventions performed included acupuncture, moxibustion, herbal medicine, physical therapy, and KM-based psychotherapy. Treatment duration varied from two days to more than five months. Follow-up began at least one week to three months after the end of treatments. It was reported that the major psychological and/or somatic symptoms of PTSD, such as anxiety, depression, insomnia, and musculoskeletal pain, subjectively improved, as well as other objective outcomes: Impact Event Scale-Revised Korean version (IES-R-K), Beck's Depression Inventory (BDI), State-Trait Anxiety Inventory, Hwabyung Symptoms/characters, Electroencephalography (EEG) change, etc. Statistical studies were conducted in three studies only. Outcomes such as Visual Analogue Scale (VAS), BDI, and IES-R-K showed statistically significant improvement after KM treatments. There was no study reporting adverse events during or after the interventions. Conclusions: According to this review, diverse types of KM treatments have been used among PTSD patients in eight studies. The KM treatments effectively improved psychological and somatic symptoms of PTSD patients. However, the lack of high quality research as well as the lack of standardization of KM treatments for PTSD are limitations. Further methodologically robust clinical trials should be performed, and the standardization of KM treatments for PTSD should be sought.

A Study on the Designation of Scenic Sites Considering Visual Perception Intensity (시지각강도를 고려한 명승 구역설정에 관한 연구)

  • This study applied the index called Visual Perception Intensity (VPI) which quantitatively deals with landscape values and viewpoints to designate the cultural heritage areas in the Scenic Sites. The results of the study are as follows. First, a VPI selection index was presented for designating the cultural heritage areas in the Scenic Sites. The index was applied in consideration of the distance from the viewing point to the object and its incident angle. In addition, the process of the VPI analysis was implemented with GIS and the analysis algorithm was constructed. Second, the possibility of VPI was examined by comparing the simple frequency of the cumulative visibility with the results of the VPI. The VPI was analyzed to be more influenced by the incidence angle than the distance between the viewpoint and the object within a 4.74 km area. Third, a field survey was performed to investigate the effectiveness of the VPI classification. The survey was implemented based on the results of the investigation into the VPI to examine whether human visual perception was fully reflected. It was confirmed through the field survey that an area with high VPI was also an important area. Fourth, a plan for the cultural heritage area adjustment was constructed by applying the VPI to the areas already designated as Scenic Sites. As a result of classifying the VPI into three classes, it was found that the areas with the second class or higher were needed to be designated as cultural heritage areas and the areas with the third class as the Historical and Cultural Environments Preservation Area.

Introduction to Current Status and Researches for Rock Engineering of Finnish Geological Disposal of Spent Fuel (핀란드의 사용후핵연료 지층처분 현황 및 암반공학 관련 연구소개)

  • This technical note describes the current status of Finnish radioactive waste disposal project which started to construct the repository for spent nuclear waste for the first time in the world. Finland started operating nuclear power plant in 1977 and is currently operating four nuclear power plants. After detailed site surveys started in 1993, Olkiluoto was finally selected by the parliament of Finland as the site for geological disposal in 2001 followed by a construction license in 2015. If the operating license is approved by the government in the 2020s, it would be the world's first case of geological disposal. In ONKALO, a site-specific underground research facility at the site of Olkiluoto, various studies were conducted to verify the safety of the repository. Finland uses the KBS-3 disposal concept, and Korea considers a similar disposal concept because of similar rock formations. The entire process in Finland including the operation status of intermediate and low-level waste disposal, site investigation and selection stages, and the latest rock mechanics and hydrogeological studies in ONKALO are presented. Suggestions for the radioactive waste disposal in Korea is given based on the Finnish case.

Effects of types of bridge decks on competitive relationships between aerostatic and flutter stability for a super long cable-stayed bridge

  • Aerodynamic configurations of bridge decks have significant effects on the aerostatic torsional divergence and flutter forsuper long-span bridges, which are onset for selection of suitable bridge decksfor those bridges. Based on a cable-stayed bridge with double main spans of 1500 m, considering typical twin-box, stiffening truss and closed-box section, which are the most commonly used form of bridge decks and assumed that the rigidity of those section is completely equivalent, are utilized to investigate the effects of aerodynamic configurations of bridge decks on aerodynamic instability performance comprised of the aerostatic torsional divergence and flutter, by means of wind tunnel tests and numerical calculations, including three-dimensional (3D) multimode flutter analysis and nonlinear aerostatic analysis. Regarding the aerostatic torsional divergence, the results obtained in this study show twin-box section is the best, closed-box section the second-best, and the stiffening truss section the worst. Regarding the flutter, the flutter stability of the twin-box section is far better than that of the stiffening truss and closed-box section. Furthermore, wind-resistance design depends on the torsional divergence for the twin-box and stiffening truss section. However, there are obvious competitive relationships between the aerostatic torsional divergence and flutter for the closed-box section. Flutter occur before aerostatic instability at initial attack angle of $+3^{\circ}$ and $0^{\circ}$, while the aerostatic torsional divergence occur before flutter at initial attack angle of $-3^{\circ}$. The twin-box section is the best in terms of both aerostatic and flutter stability among those bridge decks. Then mechanisms of aerostatic torsional divergence are revealed by tracking the cable forces synchronous with deformation of the bridge decksin the instability process. It was also found that the onset wind velocities of these bridge decks are very similar at attack angle of $-3^{\circ}$. This indicatesthat a stable triangular structure made up of the cable planes, the tower, and the bridge deck greatly improves the aerostatic stability of the structure, while the aerodynamic effects associated with the aerodynamic configurations of the bridge decks have little effects on the aerostatic stability at initial attack angle of $-3^{\circ}$. In addition, instability patterns of the bridge depend on both the initial attack angles and aerodynamic configurations of the bridge decks. This study is helpful in determining bridge decksfor super long-span bridges in future.
