Large Dose Dexmedetomidine in a Patient during Sedation for Invasive Oral Procedure

  • Certain oral procedures require a sedated patient who is responsive to allow for the mouth opening and position change. Dexmedetomidine is a relatively selective alpha2-adrenoceptor agonist with sedative, analgesic, amnestic, and anesthetic-sparing effects. Large dose dexmedetomidine is suitable as a single agent for sedation and anxiolysis for plate removal in a patient with bilateral sagittal split osteotomy and Lefort 1 osteotomy with genioplasty.

  • Pharmacological activities of two sesquiterpenes, curzerenone(I), and curcumenol(II) isolated from the rhizomes of Curcuma zedoaria being used as an aromatic stomachic in Chinese medicines were evaluated in rats and mice. Curzerenone (I), at 100mg/kg, p.o. showed a potent protective effect against HCl-ethanol induced gastric lesion in rats, and curcumenol(II), at the same dose level showed a CNS depressant action characterized by a potentiation of hexobarbital(HB)-induced narcosis. Both compounds showed a moderate analgesic action but did not exhibit a hypothermic action even at 200mg/kg dose level.

  • In order to investigate pharmacological actions of combined preparations of crude drugs, 'Jakyakgamcho-Tang' were studied. The 'Jakyakgamcho-Tang' using abdominal pain and convulsion of pediatric diseases are composed of Glycyrrhizae Radix and Paeoniae Radix alba. This study was undertaken to investigate the effect of 'Jakyakgamcho-Tang'(S-I) and parched 'Jakyakgamcho-Tang'(S-II) on anticonvulsive, analgesic, antipyretic and sedative actions. The results of this study were summarized as follows; Both 'Jakyakgamcho-Tang' and parched 'Jakyakgamcho-Tang' showed anticonvulsive effects against convulsions induced by strychnine, picrotoxine and caffeine. 'Jakyakgamcho-Tang' and parched 'Jakyakgamcho-Tang' also showed analgesic and antipyretic effects. Parched 'Jakyakgamcho-Tang' only prolonged the duration of hypnosis induced by sodium pentobarbital.

Ethanol Extract of Polygalae Radix Augments Pentobarbital-Induced Sleeping Behaviors through $GABA_Aergic$ Systems

  • Polygalae radix (PR) has traditionally been used as a sedative and anti-stress agent in oriental countries for a long time. PR which contains many ingredients is especially rich in saponins. This study was performed to investigate whether ethanol extract of PR enhances pentobarbital-induced sleep behaviors. In addition, possible mechanisms also were investigated. PR inhibited locomotor activity in mice. PR increased sleep rate and sleep time by concomitant administration with sub-hypnotic dose of pentobarbital (28 mg/kg). PR prolonged total sleeping time, and shortened sleep latency induced by pentobarbital (42 mg/kg). In addition, PR increased intracellular chloride concentration in primary cultured neuronal cells. The expression level of glutamic acid decarboxylase (GAD) were increased, and ${\gamma}$-aminobutyric acid $(GABA)_A$ receptors subunits were modulated by PR, especially increasing ${\gamma}$-subunit expression. In conclusion, PR augments penobarbital-induced sleep behaviors through activation of $GABA_A$ receptors and chloride channel complex.

  • The effect of Ruthenium Red on the antinociceptive action of capsaicinoids was investigated using tail-flick test in mice. Capsaicin and KR-25018, when administered subcutaneously, had a potent antinociceptive effect against noxious heat stimulus. Ruthenium Red which is known to block the calcium channel coupled to the capsaicin receptor, when injected intraperitoneally more than 5 mg/kg, showed severe sedation and apparent antinociceptive effect against noxious heat stimulus. The 2.5 mg/kg Ruthenium Red, at which dose any significant sedative effect was not shown, had no effect on the antinociceptive effects of capsaicin and KR-25018. Considering this result, the antinociceptive effect of capsaicinoid may not be related to the Ruthenium Red sensitive calcium channel which is activated by capsaicin.

An Acute Postoperative Intractable Hyperventilation after an Endoscopic Third Ventriculostomy

  • This report describes a rare case of postoperative hyperventilation attack after an endoscopic third ventriculostomy in a 46-year-old woman. About 60 min after the termination of the operation, an intractable hyperventilation started with respiratory rate of 65 breaths/min and $EtCO_2$, 16.3 mm Hg. Sedation with benzodiazepine, thiopental sodium, fentanyl, and propofol/remifentanil infusion was tried under a rebreathing mask at a 4 L/min of oxygen. With aggressive sedative challenges, ventilation pattern was gradually returned to normal during the 22 hrs of time after the surgery. A central neurogenic hyperventilation was suspected due to the stimulating central respiratory center by cold acidic irrigation solution during the neuroendoscopic procedure.

  • The acute and subacute toxicities and pharmacological actions of two preparations of Ssanghwa Tang prescriptions have been evaluated in mice, rats and rabbits. The two prescriptions were found to be safe drugs because those preparations exhibited almost no acute and subacute toxicities even at a high dosage level. The two prescriptions elicited CNS depressant activities characterized by potentiation of hexobarbital-induced narcosis, antipyretic activity in typhoid-vaccinated rabbits and a significant antifatigue effect against cold immobilized stress, the activities of prescription B being more potent.

  • Angelica flaccida Kom. is a herbal plant growing wild in the marsh of valley in Korea. It has been used for sedative and analgesic as a folk medicine, but its constituents have not been clarified yet. Five known coumarins such as isoimperatorin, decursidin, (-) anomalin, decursin, decursinol and two sterols such as ${\beta}-sitosterol$ and stigmasterol were isolated from the root of this plant, Their structures were identified by the UV, IR, NMR, mass data and the physico-chemical properties.

  • A growing number of people are concerned about their sleep. There are many people with chronic sleep disorders. As there are various causes in a disease, proper treatment according to each cause is necessary for a more effective treatment. In general, insomnia is classified into five categories of physical, physiological, psychological, psychiatric and pharmacological aspects. Sedative-hypnotics including benzodiazepine and non-benzodiazepine have widely been used in chronic insomniacs. However, most hypnotics including non-benzodiazepine cause some of dependence, tolerance, impaired daytime function and rebound insomnia. Therefore, we are looking forward to proposing an effective oriental treatment for insomnia. A 71-year-old male who had suffered from cerebral infarction was admitted to our department for oriental treatment of stroke and insomnia. Initial treatment modalities with administration of paroxetine were not effective. However administration of oriental medicine' Gochimmuwoo-san(Gaozhenwuyou-san)' achieved a desirable effect.

The Effect of Mugwort Extracts on the Benzo(a)pyrene-induced Hepatotoxicity in Rats (Benzo(a)pyrene에 의해 유도된 간기능 장해에 미치는 쑥의 효과)

  • Mugwort has been used as a Korean folk medicine in treating liver diseases acting as an analgesics, sedative, diuresis, choleretics. This study was perfomed to evaluate the effect of mugwort extracts on the changes of enzyme activities, lipid accumulation of the serum and liver, when hepatotoxicity was induced by benzo(a)pyrene. The results are as follows: 1. Mugwort water extract administration prevented the increase of serum and liver AST, ALT, LDH, ${\gamma}$-GTP, liver ALP activities and bilirubin content caused by B(a)P injection. 2. The increase of serum and liver ALT, LDH, ${\gamma}$-GTP, serum AST activities and liver bilirubin contents in B(a)P treated group were decreased by mugwort methanol extract treatment. 3. Serum and liver total cholesterol, phospholipid, triglyceride level and serum HDL-cholesterol level were increased by B(a)P treatment. After combined treatment of mugwort water and methanul extracts, these lipid content were significantly decreased. 4. The hepatotropic effect of mugwort water extract and after-treatment against B(a)P induced hepatotoxicity was superior to that of methanol extract and pretreatment.
