Development of a Psychological Insulin Resistance Scale for Korean Patients with Diabetes (심리적 인슐린저항성 측정도구 개발)

  • Purpose: This study was done to develop and validate a measure to evaluate the Korean version of psychological insulin resistance (K-PIR) in patients with diabetes in Korea. Methods: Items were initially generated from literature reviews and interviews with 19 patients with diabetes. The content validity of the items was evaluated by experts. Participants were 424 patients with diabetes recruited through convenience sampling. A cross-sectional survey was designed for item-analysis, exploratory factor analysis with principal axis factoring, and confirmatory factor analysis. Cronbach's alpha was calculated to measure the internal consistency. Results: For the 24 items of the Korean version of psychological insulin resistance, six items were eliminated because of low correlation with the other items. Exploratory factor analysis with 18-item showed that two factors (psycho-cognitive factor and supportive factor) explained 41.8% of the variance, and the factor structure of K-PIR model had a good fit. Internal consistency of K-PIR with 18 items revealed good reliability. Conclusion: The findings show that the K-PIR is reliable for measuring the psychological resistance to insulin therapy for Korean patients with diabetes. However, further study is needed to evaluate the validation because the proportion of variation of K-PIR was low in this study.

Developement of the reinforced acrylic-based hybrid denture composite resin with vinyloligosilsesquioxane (POSS)

  • The mainly used polymeric material for the denture is PMMA because of its cost and easiness to handle. So it was widely used material among dentists for past decades. But the acrylic-based denture materials have several common weak points such as shrinkage after curing and lack of strength. In order to solve these problems, we adapted one of hybrid system using acrylic polymer and vinyloligosilsesquioxane(POSS). POSS, which is a well known expandable monomer during polymerization process, may eventually suppress volumetric shrinkage. And the hybrid system makes it possible for the polymer to be stable in various severe conditions. Eight different kinds of samples were designed and synthesized. Each samples were characterized with dynamic mechanical analyser(DMA) to confirm their thermodynamic properties, fractured to analyze the cross-sectional morphology of the samples. And elongation, flexural and impact tests were also executed to evaluate the mechanical properties of the samples. From the results, hybrid composites had well defined crosslinked network structure compared to the widely used denture materials, and the mechanical strength improved without changing any surface condition as increment with POSS ratio in hybrid system. Fractured morphology showed homogeneous surfaces in spite of mutli component system, therefore we can conclude that the adoption of the POSS brought the reinforcement of the denture resin.

Sampling Efficiency of Organic Vapor Passive Samplers by Diffusive Length (확산길이에 따른 수동식 유기용제 시료채취기의 시료채취성능에 관한 연구)

  • Passive samplers have been used for many years for the sampling of organic vapors in work environment atmospheres. Currently, all passive samplers used in domestic occupational monitoring are foreign products. This study was performed to evaluate variable parameters for the development of passive organic samplers, which include the geometry of the device and diffusive length for the sampler design. Four prototype diffusive lengths; A-1(4.5 mm), A-2(7.0 mm), A-3(9.5 mm), A-4(12.0 mm) were tested for adsorption performances to a chemical mixture (benzene, toluene, trichloroethylene, and n-hexane) according to the US-OSHA's evaluation protocol. A dynamic vapor exposure chamber developed and verified by related research was used for this study. The results of study are as follows. The results in terms of sampling rate and recommended sampling time test indicate that the most suitable model was A-3 (9.5 mm diffusive lengths on both sides) for passive sampler design in time weighted average (TWA) assessment. Sampling rates of this A-3 model were 45.8, 41.5, 41.4, and 40.3 ml/min for benzene, toluene, trichloroethylene, and n-hexane, respectively. The A-3 models were tested on reverse diffusion and conditions of low humidity air (35% RH) and low concentrations (0.2 times of TLV). These conditions had no affect on the diffusion capacity of samplers. In conclusion, the most suitable design parameters of passive sampler are: 1) Geometry and structure - 25 mm diameter and 490 $mm^2$ cross sectional area of diffusion face with cylindrical form of two-sided opposite diffusion direction; 2) Diffusive length - 9.5 mm in both faces; 3) Amount of adsorbent - 300 mg of coconut shell charcoal; 4) Wind screen - using nylon net filters (11 ${\mu}m$ pore size).

  • The reliability analysis for web frame of tanker is carried out by the probabilistic finite element method combined with the classical reliability method such as MVFOSM and AFOSM which can be used for calculating the probability of failure for the complicated structures in which the limit state equation is implicitly expressed. As random variables external load, elastic modulus, sectional moment of inertia and field stress are chosen and Parkinson's iteration algorithm in AFOSM is used for reliability analysis. By adding only the covariance data of the random variables to the input data set required for conventional finite element method, the present method can easily calculate the probability of failure at every element end as well as the covariances of structural reponses such as displacements at every element end and member forces at every element, even for the complicated ship structure.

An Exploration about Current Nursing Handover Practice in Korean Hospitals (국내 병원에서 이루어지고 있는 인수인계 현황에 대한 조사연구)

  • Purpose: Examining current status of nursing intershift handover practice would provide opportunity to identify areas in need of improvement. The aim of this study was to describe nursing intershift handover practice in Korean hospitals. Methods: A cross-sectional survey using questionnaire was used to collect data. A total of 788 nurses working in general hospitals had participated. Results: More than half of the nurses participated responded that they used kardex for verbal handover (51.8%). The average time took for handover was 40.2 minutes (SD=14.7). Only 5.8% of nurses were satisfied with intershift handover practice. 72.9% of respondents did not have manuals for handover and 86% of nurses considered intershift handover manuals were necessary. Most nurses considered intershift handover pratice as an important component for patient safety (99.2%). The causes of handover error were interruption during handover, high work load, and stress/fatigue of nurses. Conclusion: The findings of this study indicated a need to develop handover practice guidelines which outline pertinent details. Intershift handover practice guidelines could provide consistency and structure to the information that is handed over.

  • Penetrating injuries in the head and neck region are not common but can pose difficult situations to manage properly. In small cross-sectional area, the neck housed many vital structures, such as carotid artery, internal jugular vein, cervical spines, esophagus, laryngotracheal complex and nerves. Because each vital structure is located within the fascial compartments, bleeding into these closed spaces can give rise to compression of surrounding structures, which may result in compromised airway. Therefore, management of the penetrating injuries should be based on the fully understanding of anatomical relationships, accurate clinical examinations, a careful history taking and the proper treatment planning. We present two cases of penetrating injuries in the head and neck region and discuss on the clinical considerations for the proper management with the literature review.

  • This study is the research appling the representative Displacement-Based Design which is the basic concept of Direct Displacement Based Design proposed by Chopra and Goel to original Reinforced Concrete structure and determining the thickness of retrofit Steel Jacket about the Maximum design ground acceleration, and developing the more improved Algorithm as well as program by the Retrofit Design method and Nonlinear analysis by the Performance design method before and after reinforcement appling the determined retrofit thickness. To predict the target displacement of retrofitted columns, a nonlinear analysis model of reinforced concrete columns has been developed to be based on the nonlinear fiber cross-sectional and segmental analysis model, and the seismic displacement level of retrofitted columns is estimated by two procedures, the direct displacement-based design method and the displacement coefficient method. In examples of seismic retrofit design, the current seismic improved design method gives good results in improvements of displacement levels and displacement ductilities of retrofitted columns.

A Comparative Study on the Institutional Complementarities in Coordination of the sphere of Distribution and Production: Focus on the Determinants of Income Inequality (분배조정과 생산조정의 제도적 상보성에 관한 비교사회정책연구: 소득 불평등에 미친 영향을 중심으로)

  • This paper examines the impact of the institutional complementarity between welfare regimes and production regimes on income inequality. Using comparative welfare data by various sources for 14 OECD nations from 1980 to 1997, this paper attempts to answer two questions. First, is there a institutional complementary in regulatory process between distribution and production? Second, if it is correct, what kind of causal structure do we predict? Panel Corrected Standard Errors(PCSE) model, a data analysis method in pooled cross-sectional time-series, is employed to examine the interaction effects between the two variables: coordination in the sphere of distribution; coordination in the sphere of production The evidence suggest that there are powerful interaction effects between distributive coordination and production coordination and that the institutional complementary has effects on income inequality. First, the income inequality effect of coordination in the sphere of distribution becomes less positive(more negative) as coordination in the sphere of production increases. Second, the income inequality effect of coordination in the sphere of production becomes less positive(more negative) as coordination in the sphere of distribution increases.

  • The evaluation methods of river naturalness were reviewed, and one of the best methods was applied to three sample rivers-Narinchun, Bokhachun, Anyangchun. As a result, Narinchun with well preserved state, Bokhachun with being to restoration stage from the damaged, Anyangchun with severe damaged state showed 2$^{nd}$ , 3$^{rd}$ and 4$^{th}$ Grade. The application result shows the method is reliable for evaluating the damaged habitat in river. The method was improved on the evaluation items to apply lager river systems. This suggested method have 14 evaluation items with two category-river shape and river environment and five different Grades for evaluation degree according to state of natural closeness. The every 2,000-3,000m along the river system is recommended as the interval of evaluation unit. According to calssification of evaluation items, this method can supply various information related to river environment and ecology such as ecological deformation and diversity, cross sectional structure and continuity of river shapes..

Study on the formation of Ta-silicides and the behavior of dopants implanted in the poly-Si substrates (Dopant가 주입된 poly-Si 기판에서 Ta-silicides의 형성 및 dopant 의 거동에 관한 연구)

  • Trantalum thin films have been prepared by DC sputtering onto As, P, and $BF_2$-implanted ($5{\times}10^15cm^-2$) poly-silicon. The heat treatments by rapid thermal annealing(RTA) have been applied to these samples for the formation of silicides. We have studied the application possibility of Ta-silicide as gate electrode and bit line. The silicide formation and the dopant diffusion after the heat treatment were investigated by various methods, such as four-point probe, X-ray, SEM cross sectional views, ${\alpha}$-step, and SIMS, The tantalum disilicide($TaSi_2$) are formed in the temperature above $800^{\circ}C$, and grown in colummar structure. $TaSi_2$ has a good surface roughness, having range from $80{\AA}\;to\;120{\AA}$, and implanted dopants are incoporated into the $TaSi_2$ layer during the RTA temperature.
