Background: CC chemokine receptor (CCR) 7 and cognate CCR7 ligands, CCL21 (formerly secondary lymphoid tissue chemokine [SLC]) and CCL19 (formerly Epstein-Barr virus-induced molecule 1 ligand chemokine [ELC]), were known to establish microenvironment for the initiation of immune responses in secondary lymphoid tissue. As described previously, coadministration of DNA vaccine with CCR7 ligand-encoding plasmid DNA elicited enhanced humoral and cellular immunity via increasing the number of dendritic cells (DC) in secondary lymphoid tissue. The author hypothesized here that CCR7 ligand DNA could effectively expand memory CD4+ T cells to protect from viral infection likely via increasing DC number. Methods: To evaluate the effect of CCR7 ligand DNA on the expansion of memory CD4+ T cells, DO11.10.BALB/c transgenic (Tg)-mice, which have highly frequent ovalbumin $(OVA)_{323-339}$ peptide-specific CD4+ T cells, were used. Tg-mice were previously injected with CCR7 ligand DNA, then immunized with $OVA_{323-339}$ peptide plus complete Freund's adjuvant. Subsequently, memory CD4+ T cells in peripheral blood lymphocytes (PBL) were analyzed by FACS analysis for memory phenotype ($CD44^{high}$ and CD62 $L^{low}$) at memory stage. Memory CD4+ T cells recruited into inflammatory site induced with OVA-expressing virus were also analyzed. Finally, the protective efficacy against viral infection was evaluated. Results: CCR7 ligand DNA-treated Tg-mice showed more expanded $CD44^{high}$ memory CD4+ T cells in PBL than control vector-treated animals. The increased number of memory CD4+ T cells recruited into inflammatory site was also observed in CCR7 ligand DNA-treated Tg-mice. Such effectively expanded memory CD4+ T cell population increased the protective immunity against virulent viral infection. Conclusion: These results document that CCR7 and its cognate ligands play an important role in intracellular infection through establishing optimal memory T cell. Moreover, CCR7 ligand could be useful as modulator in DNA vaccination against viral infection as well as cancer.
Mycoplasma pneumoniae (MP) is the most common cause of childhood bacterial pneumonia. Although macrolide is known to be effective as a first-line therapy, the proportion of macrolide resistance in MP pneumonia has strikingly increased during recent 2 decades in East Asia. This is challenging to physicians since they have to decide more often whether to use secondary treatment. Diagnostic methods to detect macrolide-resistance of MP are currently not available in Korean hospitals. Even in the diagnosis of MP infection, both serologic and molecular test have limitation: inability to differentiate current illness from carriage or asymptomatic infection. Combining these 2 diagnostic methods and excluding infection caused by other respiratory pathogens allow a more reliable diagnosis. This effort is even more demanding in recent years to keep children from unnecessary exposure to secondary antibiotics. Although several observational studies have reported that tetracycline and fluoroquinolone, which are considered in the treatment of refractory MP pneumonia, have efficacy of shortening the duration of fever and respiratory symptoms, those findings need to be proven by well-designed prospective studies. The use of tetracycline and fluoroquinolone in children is generally tolerable, as supported by many observational data. However, since concerns about side effects still remain, careful consideration about benefits and risks is needed to decide their use.
We experienced 48 operations in 46 surgical patients of bronchiectasis admitted to the Department of Thoracic and Cardiovascular Surgery of Jeonbug National University Hospital from January, 1975 to August, 1982. Among 46 patients, 27 patients [59%] was age group between 21 to 30 years. Common symptoms were cough with sputum, hemoptysis, dyspnea, fever and chilliness, and chest pain. The duration of the symptoms was variable between below one year and above 10 years. The most frequent associated disease, probably the cause of the bronchiectasis, was secondary bacterial infection after viral infection. The left lower lobe and lingular segment was involved most frequently, and the most frequent pathologic type was mixed type [40%]. Single lobectomy, and combined lobectomy and segmentectomy were performed in 77% of the patients. Bilateral resection was performed in three patients with good result. In those patients, the isolated pulmonary function test on each side of the lung performed 2 month later primary lung resection could make them be prevented from pulmonary insufficiency after secondary lung resection. The results were good except two patients who developed pulmonary insufficiency and chronic empyema with bronchopleural fistula.
Background: It is generally accepted that gastric carcinomas are preceded by a sequential multistage process that includes chronic gastritis, gastric atrophy, usually with intestinal metaplasia (IM), and dysplasia. This series of changes in gastric carcinogenesis is often initiated by Helicobacter pylori (H pylori) infection. The aim of the present study was determination of gastric histopathologic changes in IM patients after at least one year in Guilan province, Iran. Materials and Methods: This case-series study was conducted in Guilan Gastrointestinal and Liver Disease Research Center (GLDRC) during 2010 to 2011. Gastric biopsy was performed for all 71 known cases of IM and precanceric lesions including gastric atrophy, IM, dysplasia and H pylori infection were determined after at least one year. Results: Of the total of 71 patients with established IM who were enrolled, 50 had complete-type IM and 21 had incomplete-type IM. Fifty two people had H pylori infection. H pylori eradication was achieved in 39 patients (75%). Secondary pathology findings of patients with IM were complete metaplasia (39.4%), incomplete metaplasia (32.4%), dysplasia (23.9%) and other precanceric lesions (4.2%). Dysplasia (20%vs 33%) occurred in patients who had complete and incomplete IM at baseline respectively (p>0.05). Age, gender, family history of gastric cancer(GC); smoking habits and NSAIDs use were not associated with gastric premalignant lesions in initial and secondary pathologies (p>0.05). The difference became statistically significant between H pylori infection in patients with more than 3 years diagnostic intervals (p<0.05). Statistical difference between eradicators and non-eradicators was not significant. Conclusions: We found that incomplete IM increased the risk of subsequent dysplasia in this study.

It has been known that the eczema herpeticum is the one of most severe skin manifestation of external herpetic infection. It is developed by skin infection of Herpes simplex Virus(HSV) that superimposed on children with atopic dermatitis, and this could be fatal. The secondary bacterial infection happens frequently in eczema herpeticum. Staphylococcus aureus is the most frequently isolated aerobic strain. The important point of treatment of eczema herpeticum is the immediate start of systemic antiviral treatment and use of antibiotics to decrease the secondary infection. We report two cases of severe eczema herpeticum after herb medical treatment in atopic dermatitis. They have been improved after systemic acyclovir and antibiotic therapy with burn dressing.
To observe the effect of Demodex canis infection on the cellular immune response and hematological profile, 8 Doberman pinschers experimentally infected with D cains and 4 uninfected control dogs were sensitized with 2, 4-dinitro-chlorobenzene(DNCB) on the skin and were challenged with DNCB 14 days after the initial sensitization to elicit allergic contact dermatitis. Histological and hematological changes of these dogs were then observed. Macroscopic changes of skin challenged with DNCB in D canis-infected dogs included significantly reduced area of allergic reaction(p<0.05) than in uninfected control group. Infiltration of inflammatory cells in the D canis-infected group was also significantly reduced(p<0.05) than in the uninfected control group. These changes indicated that the cell-mediated immune response of the animals was suppressed by the infection with D canis. Total white blood cell count in dogs infected with D canis was increased when dogs were sensitized with DNCB (p<0.01). The result appeared to be caused by stress due to D canis infection, secondary bacterial infection and decreased efficacy of general body defense system. Blood eosionophils were increased in D canis-infected dogs which appreared to be caused by the allergic contact dermatitis. Blood chemistry analysis revealed that total protein and globulin were increased(p<0.05), while albumin level was decreased. This result appeared to be caused by secondary bacterial infection.

Background Postoperative dehiscence and surgical site infection after spinal surgery can carry serious morbidity. Multidisciplinary involvement of plastic surgery is essential to minimizing morbidity and achieving definitive closure. However, a standardized approach is lacking. The aim of this study was to identify effective reconstructive interventions for the basis of an evidence-based management protocol. Methods A retrospective review was performed at a single tertiary institution for 45 patients who required 53 reconstruction procedures with plastic surgery for wounds secondary to spinal surgery from 2010 to 2019. Statistical analysis was performed for demographics, comorbidities, and treatment methods. Primary outcomes were postoperative complications, including dehiscence, seroma, and infection. The secondary outcome was time to healing. Results The overall complication rate was 32%, with dehiscence occurring in 17%, seroma in 15% and infection in 11% of cases. Median follow-up was 10 months (interquartile range, 4-23). Use of antibiotic beads did not affect rate of infection occurrence after wound closure (P=0.146). Use of incisional negative pressure wound therapy (iNPWT) was significant for reduced time to healing (P=0.001). Patients treated without iNPWT healed at median of 67.5 days while the patients who received iNPWT healed in 33 days. Demographics and comorbidities between these two groups were similar. Conclusions This data provides groundwork for an evidence-based approach to soft tissue reconstruction and management of dehiscence after spinal surgery. Timely involvement of plastic surgery in high-risk patients and utilization of evidence-based interventions such as iNPWT are essential for improving outcomes in this population.
Recently new technologies for the establishment of high health herds are becoming efficient tools in the control of PRRS virus and secondary infections. Medicated early weaning(MEW) and nursery depopulation(ND) have shown to be one of the most successful procedures in the eradication and control of pathogens. Indirect evidence of the role of PRRSV in precipitating secondary infection comes from successful improvement in growth and in decreasing mortality on farms that have eliminated PRRSV through ND. Hence the present experiments were conducted in an effort to compare ND with MEW procedures as a means of eliminating PRRSV controlling secondary pathogens and improving performance of pigs in endemically infected swine herds. Following MEW and ND procedures practiced in the farms, some benefits obtained were as follows: 1. A decrease in PRRSV circulation in the nursery, but no entire elimination. 2. Decrease in the frequency of secondary bacteria and in the use of antibiotics. 3. Mycoplasma hyopneumoniae infection was prevented during the nursery stage. 4. ND protocol had a lower cost and management changes than MEW techniques. 5. Nursery performance was improved after the depopulation, cleaning and disinfection procedures, even though PRRSV still being cycled in the old nursery rooms. These studies revealed that the MEW and ND protocols are not always successful for PRRS virus elimination but it's great effect on control of secondary pathogens and improvement of performance make MEW and ND an efficient tools for the establishment of healthier and more efficient herds.
