Non-Randomized Confirmatory Trial of Laparoscopy-Assisted Total Gastrectomy and Proximal Gastrectomy with Nodal Dissection for Clinical Stage I Gastric Cancer: Japan Clinical Oncology Group Study JCOG1401

  • Several prospective studies on laparoscopy-assisted distal gastrectomy for early gastric cancer have been initiated, but no prospective study evaluating laparoscopy-assisted total gastrectomy or laparoscopy-assisted proximal gastrectomy has been completed to date. A non-randomized confirmatory trial was commenced in April 2015 to evaluate the safety of laparoscopy-assisted total gastrectomy and laparoscopy-assisted proximal gastrectomy for clinical stage I gastric cancer. A total of 245 patients will be accrued from 42 Japanese institutions over 3 years. The primary endpoint is the proportion of patients with anastomotic leakage. The secondary endpoints are overall survival, relapse-free survival, proportion of patients with completed laparoscopy-assisted total gastrectomy or laparoscopy-assisted proximal gastrectomy, proportion of patients with conversion to open surgery, adverse events, and short-term clinical outcomes. An Experimental Implementation of a Cross-Layer Approach for Improving TCP Performance over Cognitive Radio Networks

  • In cognitive radio networks (CRNs), the performance of the transmission control protocol (TCP) at the secondary user (SU) severely drops due to the mistrigger of congestion control. A long disruption is caused by the transmission of primary user, leading to the mistrigger. In this paper, we propose a cross-layer approach, called a CR-aware scheme that enhances TCP performance at the SU. The scheme is a sender side addition to the standard TCP (i.e., TCP-NewReno), and utilizes an explicit cross-layer signal delivered from a physical (or link) layer and the signal gives an indication of detecting the primary transmission (i.e., transmission of the primary user). We evaluated our scheme by implementing it onto a software radio platform, the Universal Software Radio Peripheral (USRP), where many parts of lower layer operations (i.e., operations in a link or physical layer) run as user processes. In our implementation, we ran our CR-aware scheme over IEEE 802.15.4. Furthermore, for the purpose of comparison, we implemented a selective ACK-based local recovery scheme that helps TCP isolate congestive loss from a random loss in a wireless section.

Are the Primary School Teachers of the Future Ready to Solve the Word Problems without Algebra?

  • The aim of this study is to investigate future teachers' skills that can make problem solving methods concrete for 7-11 year old students. For the students in the concrete operations level, solutions of word problems should also be taught by concreting. But most of teacher candidates can not solve the problems without algebra because they got used to solve the word problems with algebra during their high school and university education. In this study, whether the teacher candidates have the skills of solving the primary school level problems without using algebra or not are being observed. At the end of this observation it is determinated that primary level teacher candidates generally prefer using algebra operations because of their former habits. The results show that in the education of the primary level teacher candidates, there is the need of developing the solving skills using figures and diagrams without algebra rather than algebraic solutions in word problems.

Surgical Treatment of Discrete Subaortic Stenosis (대동맥판막하 막상협착증의 수술요법)

  • During the 4 year period from 1982 through 1985, twelve patients have undergone operations for discrete subaortic stenosis with good short-term clinical result at Department of Thoracic and cardiovascular Surgery, S.N.U.H. According to the cineangiographic and operative findings, nine of the 12 patients were classified as Deutsch type I, the other 3 as type II, and eleven of the 12 had one or more associated anomalies of the cardiovascular system such as PDA[5], VSD[5], left SVC[2], MS[1], COA[1], supramitral membrane[1], DORY[1], right aortic arch[1], DCRV[1], and TOF[1] [one with Shone`s complex], and three of them had secondary cardiac disorders such as aortic regurgitation[3],mitral regurgitation[2], and tunnel shaped dynamic obstruction of left ventricular outflow tract[2]. We have performed membrane resection via oblique aortotomy with retraction of the aortic cusps in 7 cases and via VSD from right cardiac chamber in 5 cases with large VSD and have also performed the operations on the correctable associated anomalies. There was only one operative death in patient with associated TOF due to neurologic complication and no other postoperative difficulties except in one patient with transient heart block resolved spontaneously on postoperative 3rd day. To our knowledge, this article is the first report of operation for discrete subaortic stenosis in Korean literature.

A study on pressurizer cutting scenario for radiation dose reduction for workers using VISIPLAN

  • The operations in the design lifecycle of a nuclear power plant targeted to be decommissioned lead to neutron activation. Operations in the decommissioning process include cutting, decontamination, disposal, and processing. Among these, cutting is done close to the target material, and thus workers are exposed to radiation. As there are only a few studies on pressurizers, there arises the need for further research to assess the radiation exposure dose. This study obtained the specifications of the AP1000 pressurizer of Westinghouse and the distribution of radionuclide inventory of a pressurizer in a pressurised water reactor for evaluation based on literature studies. A cutting scenario was created to develop an optimal method so that the cut pieces fill a radioactive solid waste drum with dimensions 0.571 m × 0.834 m. The estimated exposure dose, estimated using the tool VISIPLAN SW, in terms of the decontamination factor (DF) ranged from DF-0 to DF-100, indicating that DF-90 and DF-100 meet the ICRP recommendation on exposure dose 0.0057 mSv/h. At the end of the study, although flame cutting was considered the most efficient method in terms of cutting speed, laser cutting was the most reasonable one in terms of the financial aspects and secondary waste.

  • In this paper, the fault current limiting operations of three-phase transformer type superconducting fault current limiter (SFCL) using double quench, which consisted of E-I iron core with three legs wound by primary and secondary windings and two superconducting modules (SCMs), were analyzed according to three-phase ground fault types. To verify the effective operation of the three-phase transformer type SFCL using double quench, the test circuit for three-phase ground faults was constructed, and the fault current tests were carried out. Through analysis on the fault current test results, the different fault current limiting characteristics of three-phase transformer type SFCL using double quench from three-phase transformer type SFCL using three SCMs were discussed.

  • Jejunum and fasciocutaneous and myocutaneous flaps are theoretically recommended as esophageal substitutes in reconstruction of the esophagus after several occurrences of failed reconstruction. However, other esophageal substitutes should also be considered. Secondary esophageal reconstruction was successfully performed on a 24-year-old woman who had a history of cervical, thoracic, and abdominal operations and esophageal stricture secondary to several failed reconstructions for esophageal atresia and tracheo-esophageal fistula 21 years prior. The esophageal reconstruction was done subcutaneously by end-to-side anastomosis at the left cervical area using a deformed stomach graft. The patient was discharged with the ability to consume a regular diet after the operation.

  • Recently, parents and teachers consider physical education as a minor subject for students in elementary and secondary schools. Physical education performance has become increasingly significant as parents and schools pay more attention to physical schooling. The sports mining with distribution analysis model considers different factors, including the games, comments, conversations, and connection made on numerous sports interests. Using different machine learning/deep learning approach, children's athletic and academic interests can be tracked over the course of their academic lives. There have been a number of studies that have focused on predicting the success of students in higher education. Sports interest prediction research at the secondary level is uncommon, but the secondary level is often used as a benchmark to describe students' educational development at higher levels. An Automated Student Interest Prediction on Sports Mining using DL Based Bi-directional Long Short-Term Memory model (BiLSTM) is presented in this article. Pre-processing of data, interest classification, and parameter tweaking are all the essential operations of the proposed model. Initially, data augmentation is used to expand the dataset's size. Secondly, a BiLSTM model is used to predict and classify user interests. Adagrad optimizer is employed for hyperparameter optimization. In order to test the model's performance, a dataset is used and the results are analysed using precision, recall, accuracy and F-measure. The proposed model achieved 95% accuracy on 400th instances, where the existing techniques achieved 93.20% accuracy for the same. The proposed model achieved 95% of accuracy and precision for 60%-40% data, where the existing models achieved 93% for accuracy and precision.

Comparison of Clinical Outcomes in Patients Undergoing a Salvage Procedure for Thrombosed Hemodialysis Arteriovenous Grafts

  • Background: The major limitation of arteriovenous graft access is the high incidence of thrombotic occlusion. This study investigated the outcomes of our salvage strategy for thrombosed hemodialysis arteriovenous grafts (including surgical thrombectomy with balloon angioplasty) and evaluated the efficacy of intragraft curettage. Methods: Salvage operations were performed for 290 thrombotic occluded arteriovenous grafts with clinical stenotic lesions from 2010 to 2018. Of these, 117 grafts received surgical thrombectomy and balloon angioplasty from 2010 to 2012 (group A), and 173 grafts received surgical thrombectomy and balloon angioplasty, with an additional salvage procedure using a curette and a graft thrombectomy catheter, from 2013 to 2018 (group B). Outcomes were described in terms of post-intervention primary patency and secondary patency rates. Results: The post-intervention primary patency rates in groups A and B were 44.2% and 66.1% at 6 months and 23.0% and 38.3% at 12 months, respectively (p=0.003). The post-intervention secondary patency rates were 87.6% and 92.6% at 6 months and 79.7% and 85.0% at 12 months, respectively (p=0.623). Multivariate Cox regression analysis demonstrated that intragraft curettage was a positive predictor of post-intervention primary patency (hazard ratio, 0.700; 95% confidence interval, 0.519-0.943; p=0.019). Conclusion: Surgical thrombectomy and balloon angioplasty showed acceptable outcomes concerning post-intervention primary and secondary patency rates. Additionally, intragraft curettage may offer better patency to salvage thrombotic occluded arteriovenous grafts with intragraft stenosis.

STSAT-3 Operations Concept (과학기술위성 3호 운영개념)

  • The Science and Technology Satellite-3 (STSAT-3) is based on the KITSAT-1, 2, 3 and STSAT-1, 2 which were Korea micro-satellites for the mission of space and earth science. The objectives of the STSAT-3 are to support earth and space sciences in parallel with the demonstration of spacecraft technology. The STSAT-3 carries an infrared (IR) camera for space & earth observation and an imaging spectrometer for earth observation. The IR payload instrument of the STSAT-3, Multi-purpose Infrared Imaging System (MIRIS), will observe the Galactic plane and North/South Ecliptic poles to research the origin of universe. The secondary payload instrument, Compact Imaging Spectrometer (COMIS), images the Earth's surface. The data acquired from COMIS are expected to be used for various application fields such as monitoring of disaster management, water quality studies, and farmland assessment. In this paper we present the operations concept of STSAT-3 which will be launched into a sun-synchronous orbit at a nominal altitude of 600km in late 2012.
