Comparison of Skin Condition Before and After Use of Scrub Cosmetics and Microscopic Characteristics of Microbeads (스크럽 화장품의 사용 전과 후의 피부상태 비교와 이에 첨가된 마이크로비드의 현미경적 특성)

  • In this study, the change of skin condition was analyzed by skin analysis equipment using scrub cosmetics, and the morphological characteristics of microbead were observed by dermascope and scanning electron microscope. In the dermascope observation, during the process of cleansing, the microbeads attached to the skin existed in close contact with each other or dispersed. The skin after scrubbing was clean and smooth and the fine wrinkles between epidermal keratinocytes were reduced. In the scanning electron microscopic observation, the microbead surface did not have severe bending or rough surface. The skin moisture and oil content were higher than the scrub skin before the scrub, and there was no significant difference in the pH.

Comparison of the Awareness and Knowledge of Scrub Typhus between Case and Control Groups (쯔쯔가무시증 환자군과 대조군의 인지도와 지식 비교)

  • Objectives: To survey the awareness of patient to scrub typhus to provide data for education and communication concerning scrub typhus. Methods: Patients with scrub typhus (case group, n=299) and people without scrub typhus within the previous 2 years (control group, n=598) were matched for age (within 5 years), gender, and occupation (farmer or non-farmer). The participants were recruited from 15 study areas between October and December 2006. Results: The awareness rate of scrub typhus was 75.1%, and was significantly higher than in the case group (79.4% vs. 66.6%, respectively; p<0.01). The major routes of awareness were from 'past history of scrub typhus in family members or neighbors' (54.9%), 'television' (28.3%), and their past history of scrub typhus (5.5%). The average correct rate of scrub typhus was 48.4%, and the correct response rate of cases was significantly higher than controls (p<0.01). Especially, the correct rate of etiology, incubation period, route of transmission, and acquired immunity was <40%. Through conditional logistic regression test, the factor significantly associated with awareness in case group was age (odds ratio [OR], 0.96; 95% confidence interval [CI], 0.94-0.98). And the factors associated with awareness in control group were female (OR, 1.56; 95% CI, 1.03-2.36) age (OR, 0.98; 95% CI, 0.96-0.99), family history of scrub typhus (OR, 10.18; 95% CI, 1.37-75.99), history of receiving prevention education (OR, 8.47; 95% CI, 1.14-63.00). Conclusions: The rate of awareness was relatively low in study population. Thus, effective working guidelines and educational program to prevent scrub typhus must be developed, and publicity activities about the prevention of scrub typhus are needed for high-risk groups.

Chemokine Gene Expression in Mice during Orientia tsutsugamushi Infection

  • Orientia tsutsugamushi, an obligate intracellular bacterium, is the causative agent of scrub typhus which is histopathologically characterized by inflammatory manifestations. To understand the pathogenesis of scrub typhus, chemokine gene expression in mice after infection with O. tsutsugamushi was investigated. The mRNAs that were upregulated included macrophage inflammatory proteins 1${\alpha}$/${\beta}$ (MIP-1${\alpha}$/${\beta}$), MIP-2, monocyte chemoattractant protein 1, RANTES (regulated upon activation, normal T-cell expressed and secreted), and gamma-interferon-inducible protein 10. Peak expression of these chemokines was observed six days after infection. These responses returned to or approached baseline preinfection levels by eight days after infection. Chemokine profiles in infected mice were well correlated with the kinetics of inflammatory cell infiltration. Thus, O. tsutsugamushi appears to be a strong inducer of chemokines which may significantly contribute to the inflammation observed in scrub typhus by attracting and activating phagocytic leukocytes.

  • Rickettsial infections (Rickettsioses) are the causes of acute fever found in Thailand. It is classified as acute febrile illnesses transmitted by bloodsucking arthropod vectors (tick, flea, and chigger). This research investigated pathogens of scrub typhus in vectors from Bangkaew District, Phatthalung Province. A total of 303 pools of vector samples were ticks (Rhipicephalus sanguineus, R. microplus, and Haemaphysalis sp.), fleas (Ctenocephalides felis orientis, C. f. felis, and C. canis), and chiggers (Leptotrombidium deliense, Aschoschoengastia indica, Blankaartia acuscutellaris and Walchia disparunguis pingue) collected from reservoir hosts (dogs and rodents). The 17 and 56 kDa gene of Rickettsia causing scrub typhus were found in 29% of ticks and 98% of flea. DNA sequence analysis reveeled the detected strains were R. asembonensis and Rickettsia sp. cf1 and 5.The chiggers, 1%, were infected with Rickettsia strain TA763, a pathogen of scrub typhus.

  • Guillain-Barre syndrome (GBS) is usually characterized by acute areflexic ascending paralysis with minimal sensory involvement. Only a few cases of GBS associated with scrub typhus have been reported. Previous case reports focused on the laboratory findings, pathogenesis, and clinical manifestation. Unlike the previous case, neuropathic pain was a prominent symptom of GBS in our case. We report scrub-typhus-related GBS with a detailed description of the clinical manifestations, especially neuropathic pain, along with results of serial follow-up electrodiagnostic studies.

  • Tsutsugamushi disease (Scrub typhus) is an acute, febrile illness caused by Orientia tsutsugamushi, which is transmitted to humans through chigger bites. Leptospirosis, a febrile disease caused by various pathogenic Leptospira, and is acquired by exposure to contaminated water and soil. Both diseases have been the most common acute febrile diseases in the autumn in Korea for many years. Concomitant leptospirosis and scrub typhus is quite rare. We report a case of a coinfection with leptospirosis and scrub typhus in a 51-year-old male who presented with fever, abdominal pain and acute dyspnea. The patient was diagnosed with as acalculous cholecystitis, acute respiratory distress syndrome, and septic shock caused by the infection. This is the first case report of a coinfection with leptospirosis and scrub typhus in Korea.

Analysis of Biochemical Markers in Sera of Patients with Febrile Disease During the Fall

  • We studied a comparison of the concentration of biochemical markers in sera of patients hospitalized with high fever (n=296) in Jeonbuk province during the last 2 years (2008 to 2009). The patients were divided into three patient groups of viral hemorrhagic fever (VHF) patient group tested positive for Hantavirus (n=53), leptospirosis (LEP) patient group tested positive for Leptospira interrogans (n=137) and scrub typhus (TSU) patient group tested positive for Orientia tsutsugamushi (n=106). We analyzed the concentration of ALP, AST, ALT, blood urea nitrogen, creatinine and glucose and compared the mean levels of them to normal range, the first sample and last sample. The frequencies of abnormal patient elevated above the upper limit of normal for ALP, AST and ALT were 18~43.4%, 78~97% and 62.3~92.7% in patient groups, and 24.5~47.4% (total protein) and 13.2~50.0% (albumin) of patients in patient groups had decreased below the lower limit of normal. The patients showed higher abnormal levels of glucose in patient groups were 58.5% (viral hemorrhagic fever patient group), 66.4% (leptospirosis patient group), 71.7% (scrub typhus patient group) and 66.9% (total patient group). There were significant difference between the first sample and the last sample in the mean levels of AST (decreased 22.2% in viral hemorrhagic fever patient group, 30.2% in leptospirosis patient group, 20.4% in scrub typhus patient group and 24.1% in total patient group), BUN (43.0% in viral hemorrhagic fever patient group, 41.6% in leptospirosis patient group, 47.4% in scrub typhus patient group and 43.0% in total patient group) and glucose (20.2% viral hemorrhagic fever patient group, 17.9% in leptospirosis patient group, 18.6% in scrub typhus patient group and 18.9% in total patient group) in the first sample and the last sample. According to these results, those diseases may cause liver damage and have high concentration of ALP, AST, ALT and glucose in blood even though the patients get out of the hospital.

A Study of Behavioral Characteristic of Elders during Showering (고령자의 샤워 시 행동 특성 분석)

  • Analysis of elders' behavioral characteristic during showering provides important fundamental data when developing an automatic showering system. We recorded showering behavior of 37 elders' at the front, back, left and right view using four CCTV cameras. The photographic data were analyzed by counting the frequencies of body scrub and identifying zones where hands could reach. And we measured their body sizes, and conducted questionnaire of showering for 302 peoples. Analysis was done according to the analysis criteria established. The human body was divided into 14 zones from a body, and the numbers of body scrub for each zone were counted. The average total numbers of body scrub were 170.8 excluding the head, the face and the neck. The average numbers of body scrub with right hand were 70.2, left hand were 88.2, and both hands were 81.8. The percentages of body scrub frequencies were 16.3% (breast), 12.2%(crotch), 8.8% (abdomen), 7.5% (waist), 8.8% (shoulder), 7% (hips), 0.4% (back), 15.2% (arm), and 23.9% (leg). Elders scrubbed the breast, crotch and abdomen more frequently than the back and hips. According to the video analysis and questionnaire results, the distribution of shower heads (or nozzles) at the front side should be arranged at the ratio of 40% of the trunk(breast, shoulders, abdomen), 20% of the thighs, the calves, and the crotch, respectively. The distribution of shower heads (or nozzles) of the back side were arranged at the ratio of 20% of the waist and the arms, 20% of the back, the thighs, the calves, and the hip, respectively.

Clarithromycin Therapy for Scrub Typus (쯔쯔가무시(Tsutusgamushi)병에서 Clarithromycin의 치료 효과)

  • Purpose : Scrub typhus(tsutsugamushi disease) is a febrile disease characterized by fever, rash, eschar, lymphadenopathy. Therapy with tetracycline(doxycycline) or chloramphenicol is currently recommended for the treatment for scrub typhus. But there are limitations in usage a tetracycline(doxycycline) for scrub typhus in the children. Recently, there was a report that azithromycin, a macrolide antibiotic was used for scrub typhus in pregnant woman successfully. So we evaluated the effectiveness of the Clarithromycin, other a macrolide antibiotic, for scrub typhus. Methods : Seven patients with scrub typhus at department of internal medicine and three patients with scrub typhus at department of pediatrics Masan Fatima Hospital were involved for this study. A serologic diagnosis for scrub typhus were performed by use of passive hemagglutination test. Clarithromycin(Abbott Laboratories, North Chicago, IL, USA) was administrated orally in a daily dose of 500 mg for adult patients and 15 mg/kg/bid/day for pediatric patients. Results : There were 7 cases of adult patients, varying from 28 to 76 years of age and 3 cases of pediatirc patients, varying from 4 to 7 years of age with scrub typhus. All of cases had fever, myalgia, headache, rash, eschar. Seven cases had positive passive hemagglutination test and eight cases had abnormal liver function. Mean duration for the removal of fever after medication was 1.3 day(1~2 days) and all cases were recovered without complications. Conclusion : Our results suggest that Clarithromycin therapy may be effective for scrub typhus.

[ $^{99m}Tc$ ] HSA Scintigraphy; Intestinal Protein Loss in Scrub Typhus ($^{99m}Tc$ HSA 신티그램을 이용한 쯔쯔가무시병에서의 장내단백소실 진단)

  • Scrub typhus의 발현증상에는 저알부민혈증, 단백뇨, 전신 부종 등을 들 수 있다. 그러나, 저알부민혈증에 비해 단백뇨와 부종 등은 심하지 않은 경우가 많다. 본 연구에서는 저알부민혈증이 장내에서의 단백질 소실에 의한 것인지를 확인하고자 하였다. Scrub typhus가 의심되는 25명의 환자를 대상으로 (1) 저알부민혈증의 원인 장내 단백질 소실에 의한 것인지 여부와 (2) $^{99m}Tc$-HSA 신티그램의 진단적 유용성을 알아보고자 하였다. 신티그램은 혈청학적 검사상 Scrub typhus로 확인된 18명의 환자에서 (13예는 항생제 치료 개시후 1일에서 8일 사이에, 5예는 치료 전에 시행하였으며, 11예에서는 대변에서 alpha-1- antitrypsin (${\alpha}AT$)배설 여부를 확인하였다. 촬영방법은 $^{99m}Tc$-HSA 30mCi를 정맥 주사한 후, 2, 4, 6, 24시간에 복부 전면상을 얻었다. 판정은 소장과 대장의 주행 위치에서의 혈관외 방사능 소견을 장내 단백질 소실로 간주하였다. 13예에서 양성으로 판정하였는데, 이중 8예는 대변에서의 ${\alpha}AT$치도 높게 나왔다. 신티그램상 음성을 보인 5예중 2예에서는 대변중 ${\alpha}AT$치가 매우 높게 나왔는데 그 이유는 신티그램 검사는 치료 도중에 하였고, 대변 검사는 치료 전에 하였기 때문인 것으로 간주하였다. 결론적으로 전체 18예중 15예에서 신티그램이나 대변검사로 Scrub typhus로 진단하였다. 한편, 13예 (72%)에서 저알부민혈증을 보였는데 이중 4예는 장내단백질 소실과 단백뇨를, 5예는 장내단백질 소실만을, 3예에서는 단백뇨만을 보였고, 1예는 아무 소견이 없었다. 요약하면, 장내단백질 소실 소견은 Scrub typhus환자의 83%에서 보여, 혈중 알부민치가 저하되는 원인으로 추정된다. 따라서 $^{99m}Tc$-HSA 신티그램은 장내단백질 소실의 진단에 매우 유용한 검사방법이다. 또한, 검사방법이 다소 어렵지만 대변에서의 ${\alpha}AT$측정도 동위원소 검사상 음성 소견을 보이는 환자에게는 어느 정도 진단에 도움을 주리라 보여진다.
