Cancer Screening Adherence of Asian Women According to Biochemically-verified Smoking Status: Korea National Health and Nutrition Examination Survey

  • Background: Men and women who smoke tend to show less compliance to screening guidelines than non-smokers. However, a recent study in Korea showed that self-reported female smokers constituted less than half of cotinine-verified smokers. Therefore, the aim of this study was to identify hidden smokers using cotinine-verified method and examine cancer screening behavior according to biochemically verified smoking status. Materials and Methods: Among 5,584 women aged 30 years and older who participated in the Fourth and Fifth Korea National Health and Nutrition Examination Survey (KNHANES), 372 (6.66%) hidden smokers were identified based on interview responses and verified by urinary cotinine levels. We compared cancer-screening behavior (cervical, breast, stomach, and colon cancer) of female hidden smokers to that of non-smokers and selfreported smokers by cross-sectional analysis. Results: Hidden female smokers had significantly lower adherence to breast cancer screening compared to non-smokers (aOR (adjusted odds ratio) [95% CI] = 0.71 [0.51-0.98]). Adherence to stomach cancer (aOR [95% CI] = 0.75 [0.54-1.03]) and cervical cancer (aOR [95% CI] = 0.85 [0.66-1.10]) screening was also lower among hidden female smokers compared to non-smokers. Self-reported (current) smokers showed lowest adherence to cervical cancer (aOR: 0.64, 95% CI0.47-0.87), breast cancer (0.47 [0.32-0.68]), stomach cancer (0.66[0.46-0.95]), and colon cancer (0.62 [0.38-1.01]) screening compared to non-smokers, followed by female hidden smokers, then non-smokers. These lower adherence rates of current smokers were attenuated after we incorporated hidden smokers into the current smoker group. Conclusions: Cancer screening adherence of female hidden smokers was lower than cotinine-verified non-smokers but higher than current smokers. Considering the risk of smoking-related cancer among women, identifying hidden smokers is important to encourage appropriate cancer screening.

  • 연구 배경: 자궁경부암은 조기검진을 통해 사망률과 의료비 부담을 감소시킬 수 있는 질환으로, 아직까지 우리나라에서는 자궁경부암이 여성암 중 높은 발생률을 보이고 있으나, Pap test를 통해 자궁경부암을 조기에 발견하고, 치료할 수 있는 것으로 알려져 있다. 연구 목적: 이 연구는 자궁경부암 조기검진 수검행동에 영향을 미치는 인지-행동적 요인을 고려하여 행동변화단계별 교육 프로그램을 개발하였고, 이를 40세 이상의 여성 주민에게 적용해봄으로써 행동변화단계별 맞춤형 교육 프로그램의 효과를 평가하고자 하였다. 연구 방법: 이 연구는 서울시 서초구에 거주하는 40세$\sim$59세 사이의 여성 주민을 대상으로 2003년 9월 1일부터 11월 14일까지 자료를 수집하였으며, 서초구의 5개 지역(동)에서 교육 중재군(162명)과 대조군(121명)을 선정하였다. 또한 행동변화단계별 교육 프로그램은 범이론적모형(TTM)을 활용하여 개발되었고, 교육 중재군의 교육내용 전달방법으로 인터넷 서비스와 우편 서비스가 활용되었다. 연구 결과: 조사대상자중 88.9%가 일생 중 1번 이상, 65.4%가 최근 2년 동안 1번 이상 자궁경부암 조기검진을 수검한 것으로 나타났다. 행동변화단계별 교육 프로그램의 효과를 살펴보면, 교육 후 인지 행동적 요인 중 태도와 변화의 과정이 통계적으로 유의하게 높아졌고, 행동변화단계 중 유지단계가 가장 많이 증가하였다. 교육내용 전달 방법은 인쇄된 교육자료를 활용한 우편 서비스가 인터넷 서비스보다 효과적인 것으로 나타났고, 행동변화단계별 전환 양상에서도 우편 서비스 중재군이 인터넷 서비스 중재 군보다 긍정적인 변화를 나타내었다. 한편 행동변화단계별 교육 프로그램에 관한 만족도 평가에서는 전달방법에 따른 유의한 차이가 없는 것으로 나타났다. 결론: 이 연구는 지역사회 여성 주민의 자궁경부암 조기검진에 관한 수검 행동을 증진시키기 위해 행동변화단계에 따른 맞춤형 교육 프로그램을 개발하여 적용하였고, 그 결과, 행동변화단계별 교육 프로그램이 자궁경부암 조기검진의 수검 행동을 증진시키는데 효과적인 것으로 나타났다.

민간종합검진 유소견자들의 치료기관 선택에 미치는 영향 (The Influence on Selecting the Medical Institute for Treatment by Patients Who Had Abnormal Findings through the Private Health Screening)

  • The purpose of this study is to analyze the medical care utilization behavior of patients to whom treatment (surgery) is recommended after they are diagnosed with abnormal findings on health screening and factors affecting the selection of the medical institute for treatment. The data was collected from 291 patients who need treatment or surgery, according to the abnormal findings on the additional examination such as cardiac CT, brain MRI, Gastroscopy and Colonoscopy since four diseases are suspected among of 2,752 people who receive health screening. The results are as follows. First, the most common disease of patients who have abnormal findings by the diagnosis through the results of first testing is colon disease based on through the additional examination. The most common disease of patients who will get treatment (surgery) based on final diagnosis by a doctor who determines the result of health screening on the basis of diagnosis from the first testing is cardiovascular disease. Second, in terms of diseases, patients with cardiovascular disease select the medical institute where they get the health screenings as a place for treatment. Patients with cerebrovascular disease select another medical institute for treatment. Finally, the affective factors of selectivity treatment facility on health screening satisfaction were human, facility, health screening and revisit factors.

Screening Colonoscopy Participation in Turkish Colorectal Cancer Patients and their First Degree Relatives

  • Background: This study aimed to research the awareness of screening colonoscopy (SC) among patients with colorectal cancer (CRC) and their relatives. Methodology: A questionnaire form including information and behavior about colonoscopic screening for CRCs of patients and their first-degree relatives (FDRs) was prepared. Results: A total of 406 CRC patients were enrolled into the study, with 1534 FDRs (siblings n: 1381 and parents n: 153). Positive family history for CRC was found in 12% of the study population. Previous SC was performed in 11% of patients with CRC. Mean age of the patients whose FDRs underwent SC was lower than the patients whose FDRs did not (52 vs 57 years; p<0,001). The frequency of SC in FDRs was 64% in patients diagnosed CRC under 35 years of age. Persons having a positive family history of CRC had SC more often (51 vs 22%, p<0,001). FDRs of patients having a higher educational level and income had SC more frequently. Conclusions: When screening for CRC is planned, elderly subjects, those with family history for CRC, and those with low educational and lower income should be given esspecial attention in order that they be convinced to undergo screening for CRC.

  • Development of the 3 scales comprising the present research was based on review of literature, existing screening scales, and empirical research pertaining to (Scale I) the development of social and emotional problems of children, (Scale II) parent-child relations, and (Scale III) assessment of children's environment. Professionals in each area approved a draft of the new screening scale. The clinical group was classified into normal and at-risk groups based on the Denver II scale and the Child Behavior Check List(ages 1.5-5). The clinical groups were administered the newly developed screening scale to see whether the same classification pertained. Results proved the cross-validity of the new scale.

Factors Affecting Preferences of Iranian Women for Breast Cancer Screening Based on Marketing Mix Components

  • Background: According to recent statistics, the breast cancer rate is growing fast in developing countries. In North West of Iran, the incidence of breast cancer after esophageal and gastric cancers has the highest rate. Previous studies have also indicated that women in this region show reluctance to do breast cancer screening. There is a great need for change to promote breast cancer screening among women. Social marketing is a discipline that uses the systematic application of commercial marketing techniques to promote the adoption of behavior by the target audience. Materials and Methods: In the present qualitative study, thirty-two women with breast cancer were interviewed about their experiences of breast cancer screening. A semi-structured interview guide was designed to elicit information specific to the 4 P's in social marketing. Results: Three main categories emerged from the analysis: price, service and promotion. Subcategories related to these main categories included factors effective in increasing and decreasing cost of screening, current and desirable features of screening services, and weakness of promotion. Conclusions: Screening programs should be designed to be of low cost, to meet patients' needs and should be provided in suitable places. Furthermore, it is essential that the cultural beliefs of society be improved through education. It seems necessary to design an executive protocol for breast cancer screening at different levels of primary health care to increase the women's willingness to undergo screening.

Predictors of Progress in the Stage of Adoption of Breast Cancer Screening for Korean Women

  • Background: It has been proven that an individuals health behavior is determined through a series of processes. This study aimed to assess the stages of adoption of breast cancer screening, and to identify the factors relating to progress through these stages. Materials and Methods: There were 202 female participants aged 20-59 years who were living in Chungbuk, South Korea. They were informed of the study purpose and agreed to participate. Data were collected from October 2010 to January 2011 by assessing the breast cancer screening stage, health beliefs, socio-demographic factors, and other facilitating factors. The participant current stage of adoption of breast cancer screening was classified using the Precaution Adoption Process Model (PAPM), and the various PAPM stages were compared with each other to identify factors likely to determine progress between stages. The data were analyzed using the ${\chi}^2$-test, ANOVA, Duncan test, and multiple logistic regression. Results: Approximately half of all participants were not on-schedule for breast self-examination and mammography (unaware, 9.4% and 11.4%, unengaged, 8.4% and 5.0%, undecided, 20.3% and 17.8%, decided not to act, 1.5% and 1.0%, decided to act, 13.4% and 15.3%, respectively). The factors likely to determine the progress from one stage to another were age, marital status, exposure to media information about breast cancer, self-efficacy, and perceived severity. Conclusions: These results suggest that it is necessary to develop a tailored message for breast cancer screening behavior.

Factors related to the Performance of Mammography Screening among Women with a Family History of Breast Cancer in Korea

  • Purpose: Early diagnosis is the primary method aimed at controlling breast cancer. The purpose of this study was to analyze some factors affecting the performance of mammography screening among women with a family history of breast cancer in Korea. Methods: This study applied a descriptive design method through structured self-report questionnaires. The Care Seeking Behavior Theory provided a theoretical framework for the study. Factors measured in this study represent demographic, clinical, and psychosocial variables including anxiety, barriers, utility, habits, perception, and facilitators. A total of 212 participants, of at least 20 years old, were sampled from April 8, 2010 to March 31, 2011. The data was analyzed by logistic regression method using the Statistical Package for the Social Science 18.0 software. Results: Of the 212 participants, 122 women (57.5%) went through mammography screening. The results of the analysis showed that (a) age (Odds Ratio [OR] =1.10, p<.001), (b) facilitating influences (OR=1.83, p=.008), (c) perception of mammography importance (OR=1.92, p=.011), (d) barriers to mammography (OR=0.60, p=.031), and (e) utility of mammography (OR=2.01, p=.050) significantly affect mammography screening. Conclusion: The results underscore the impact that psychosocial variables in obtaining mammography have on adherence to screening. Women with a family history of breast cancer should be given accurate information and recommendation about mammography by healthcare provider and a regular source of healthcare.

일부 보건소 내소 여성의 유방자가검진과 유방암 검진의 관련요인 (The Behavioral and Educational Factors Affecting the Breast Self-examination and Breast Cancer Screening)

  • The purpose of this research was to find out plans to promote the fulfillment of the breast self-examination and breast cancer screening, which is preventive health behavior about women's having breast cancer exam in some part of our country with searching for the primary causes which have an effect on their fulfillment. The subjects of the study were 540 literate volunteers who were of age from 20 to 69 and who visited 8 health centers in Taegu. The survey research was made through questionares in the 8 health centers in Taegu from the 15th to the 30th of September, 1998. Based on Becker's health belief model asking medical staff's opinions with bibliographies, the major findings from the survey was as follow. The fulfillment rate of the breast self-examination and the breast cancer screening was 37.8%(204 subjects), 27.6% (149 subjects) each. Getting breast self-exam varied significantly depending on subject's age, income of family, religion, occupation and the number of children. As far as the breast cancer screening was concerned, there was remarkably significant difference in accordance with their age, marital status, religion, occupation, income of family, the number of children, ways of nursing and parturition age of the first baby. The subjects in their forties accounted for the highest rate. Based on the fulfillment of the breast self-examination and the breast cancer screening, the average points of all the variables presented from the health belief as well as the points of the susceptibility, seriousness, benefits, knowledge and level of health concern of interest in health were significantly higher than the points of non-fulfillment of them in the statistical analysis whereas the barriers was lower than them. In consequence of the multiple-logistic regression analysis putting the breast self-examination and the breast cancer screening in dependent variables, occupation, knowledge, seriousness, benefits, barriers and cues to action had significant effects on breast self-examination against breast cancer. Looking more attainments, thinking diseases more seriously, feeling more usefulness and there being any patient around with breast cancer led to the higher rate of the breast self-examination against it. On the other hand, feeling more impedition as well as housewives caused the lower rate of it. The breast cancer screening was significantly affected by age, sensitivity, usefulness, impedition and behavior momentum as well. The cases who are older, feeling more sensitivity against breast cancer, more usefulness of the examination and where any patient around, the fulfillment rate was higher. Contrarily the more impedition they felt, the lower the fulfillment rate was. Accordingly, the study of the causes, which have significant effects on the fulfillment of the self-examination and the breast cancer screening in order to find it in an early stage, and the development of health education programs as well as proper education for public health through carrying on campaigns and stepping up public relations must be indispensable.

여성 결혼이민자의 유방암과 자궁경부암 검진행위에 영향을 미치는 요인 (Factors of Breast and Cervical Cancer Screening Behaviors in Married Female Immigrants)

  • 본 연구는 여성 결혼이민자의 유방암과 자궁경부암 검진행위를 파악하고 관련 요인을 규명함으로써 여성결혼이민자의 유방암과 자궁경부암 검진행위 증진 프로그램을 위한 기초 자료를 제공하려는 목적으로 시도되었다. 연구의 주요 변수로는 Pender의 건강증진모형에 포함된 변수를 사용하였으며, 이들의 측정을 위해 총 7개의 측정도구가 사용되었다. 연구대상자는 한국인 남자와 결혼하여 대구, 경북지역에 거주하고 있는 결혼이민 여성 157명이었다. 연구결과, 대상자의 유방암 검진 행위율은 유방촬영술 33.1%, 유방 자가검진 29.9%, 자궁경부암 검진 51%로 나타났다. 유방암 검진행위는 사회적지지가 1점 높아짐에 따라 1.25배, 지각된 유익성이 1점 높아짐에 따라 1.13배, 유방암 및 자궁경부암 교육경험이 있는 경우가 없는 경우보다 3.58배, 행동계획 수립이 1점 높아짐에 따라 1.24배 높아졌다. 자궁경부암 검진행위는 유방암 및 자궁경부암 교육경험이 있는 경우가 없는 경우보다 2.89배, 사회적지지가 1점 높아짐에 따라 1.23배, 지각된 장애성이 1점 높아짐에 따라 0.82배 낮아졌다.
