Comparison of Scapular Kinematics During Active Shoulder Horizontal Adduction Between Subjects With and Without Limited Range of Motion of Shoulder Horizontal Adduction

  • Background: Shoulder horizontal adduction (HA) is performed in many activities of daily living. The limited range of motion (LROM) of HA is affected by the tightness of the posterior deltoid, infraspinatus, teres major, and posterior capsule of glenohumeral joint. The LROM of shoulder HA contributes to excessive scapular abduction. Objects: The aim of this study is to compare the scapular abduction distance and three-dimensional displacement of the scapula during shoulder horizontal adduction between subjects with and without the LROM of shoulder HA. Methods: 24 subjects (12 people in LROM group and 12 people in normal ROM group) participated. Subjects with less than $115^{\circ}$ of HA ROM were included in LROM group. Shoulder HA was performed 3 times for measuring scapular abduction distance and three-dimensional displacement of the scapula. Tape measure was used for measuring scapular abduction distance. Scapular abduction distance was normalized by dividing the scapular size. Polhemus Liberty was used for measuring the three-dimensional displacement of the scapula. Results: Normalized scapular abduction distance was significantly greater in LROM group than normal ROM group (p<.001). Three-dimensional displacement of the scapula during shoulder HA was greater in LROM group than normal ROM group (p<.05). Conclusion: LROM group had a greater scapular abduction and three-dimensional displacement of the scapula during shoulder HA compared to normal ROM group.

Effect of Shoulder Protraction Exercise on the Supine position for Selective Activation of the Serratus Anterior according to Weight in Winging Scapula

  • Purpose: This study investigated the effects of shoulder protraction exercise according to weight by examining the surface electromyography (EMG) amplitude in the serratus anterior (SA), upper trapezius (UT), and pectoralis major (PM) as well as the activity ratio of each muscle. Methods: Twenty three winging scapula subjects participated in the study. The subjects performed scapula protraction at shoulder $90^{\circ}$ flexion and $60^{\circ}$ horizontal abduction with up to four (none, 1kg, 1.5kg, and 2kg) dumbbells in the supine position. The EMG data were collected from the dominant side muscles during a shoulder protraction exercise according to weight in the supine position. One way repeated measures analysis of variance (ANOVA) was used to compare the normalized activities of the SA, UT, and PM and the ratios of PM/SA and UT/SA. Results: The results showed that the activities of both the SA and UT were highest for the shoulder protraction exercise at 2kg in the supine position. The UT/SA ratio also was the lowest for exercise at 2kg. On the other hand, the activities of both the UT and PM/SA ratio were similar under all conditions. Conclusion: These results show that there is a need to selectively strengthen the SA muscle in the case of patients with the shoulder dysfunction. In particular, it is necessary to weigh 2kg when performing shoulder protraction exercises in the supine position to activate the SA muscle in patients with a winging scapula.

  • The effect of serratus anterior dysfunction is seen in the scapular winging. We report 2 cases with winging scapula clinically but do not reveal denervation potentials in needle electromyography. Surface myoelectric measurements of median frequency and root mean square values show difference in fatigability and amplitude in affected side.

The Characteristics of Muscles around Scapula in High-Level Athletes of Fencing

  • In fencing, athletes must hit opponents by the sword of fencing, so they need long reach between their foot and their tip of the sword. We thought the motion of shoulder joint, mainly scapula, is one of the important roles in this action and noticed the difference of right and left side in muscles around scapula caused by this action. We evaluated these muscles with MRI imaging in 5 high-level athletes of fencing who aims at the next Olympics (3 men and 2 women, their average age is 21 years). Besides we observed their physical measurement related with these muscles. We reported the characteristics of these results.

  • Traumatic anterior shoulder dislocation combined with scapular fracture in elderly patients is relatively rare. In this case, a patient visited Emergency Room of Dong-A University Hospital for shoulder pain after falling off a ladder. Radiographs demonstrated anterior shoulder dislocation with displaced Ideberg type IIb scapula (glenoid fossa) fracture combined with a large rotator cuff tear on magnetic resonance imaging. We performed arthroscopic rotator cuff repair, but a large fragment in the inferior glenoid was left untreated. At the 1 year follow-up visit, the pain visual analogue scale of the patient was 2, the American Shoulder and Elbow Society score was 88 and the patient had gained nearly full range of motion without any apprehension.

  • 투명세포연골육종은 모든 연골육종의 2%를 차지하는 매우 드문 저등급성 종양이다. 주로 긴뼈의 뼈끝에서 발생하며 대퇴골과 상완골의 근위부에서 가장 흔하고, 견갑골을 포함한 납작뼈에는 드물다. 25-50세 사이의 연령에서 호발하며, 20세 이전의 발생은 흔치 않다. 조직학적으로 종양 세포는 소엽상 무리를 지어 관찰되며, 투명하고 풍부한 세포질을 특징적으로 가진다. 저자들은 8세 여아의 견갑골에서 발생된 투명세포연골육종을 보고하고자 한다.

테니스 선수의 어깨 통증과 플랫서브 동작의 견갑골 움직임 분석 (Analysis shoulder pain of tennis players and the movement of the scapula in flat serve)

  • 본 연구는 테니스 1st 플랫서브 동작에서 어깨 통증 유무에 따른 견갑골의 움직임 차이를 확인하기 위해 테니스 선수 15명(남8, 여7)을 대상으로 3차원 영상분석시스템을 활용하여 견갑골의 앞쪽/뒤쪽 기울임, 아래쪽/위쪽 회전 및 안쪽/바깥쪽 회전에 대한 각속도와 모멘트를 산출하여 이를 집단 간 비교 분석을 하였다. 그 결과, 앞쪽/뒤쪽 기울임에 대한 최대 각속도는 실험군인 어깨통증집단(SPG)보다 대조군 집단(CG)이 빠른 경향을 보였으며, 팔로우 스로우 구간을 제외하고 모든 국면에서 안쪽/바깥쪽 회전 최대 각속도가 SPG가 CG보다 빠른 것으로 나타났다. 후기 코킹 구간에서의 앞쪽/뒤쪽 기울임 최대 모멘트가 SPG보다 CG가 더 크게 나타났고 통계적으로 유의한 차이를 보였다. 코킹 구간의 위쪽/아래쪽 회전 최대 모멘트 또한 SPG가 CG보다 작게 나타나면서 통계적으로 유의한 차이를 보였다. 안쪽/바깥쪽 회전 모멘트는 모든 국면에서 CG가 SPG보다 큰 것으로 나타났다.

  • This study was performed to evaluate the influences of the passive glenohumeral rotation on the scapulothoracic motion. We took anteroposterior radiograms of the right shoulders including the thoracic vertebrae with supine position in 10 normal male adults, at 0 degree abduction, 45 degrees abduction and 90 degrees abduction in scapular plane and in neutral rotation, maximal internal rotation and maximal external rotation in each abduction view. The scapulothoracic motion was measured as the distances between the vertical line drawn from the spinous process of the 7th cervical vertebra and the inferior and superior angles of the right scapula respectively. At 0 degree abduction, the distances were not changed in internal rotation relative to neutral rotation, but decreased significantly in external rotation, that is, the scapula shifts medially on external rotation. At 45 degrees abduction, the distances were increased significantly only in internal rotation, that is, the scapula shifts laterally on internal rotation. At 90 degrees abduction, the scapula rotated laterally on internal rotation and medially on external rotation. In conclusion, when a physician examines the rotation of the shoulder joint, he cannot exclude the scapulothoracic motion just by examining the patient with supine position. And we concluded that the rotatory movement of the shoulder is not solely contributed to the glenohumeral motion.

수정된 크로스-바디 스트레칭이 오목위팔관절의 관절가동범위에 미치는 영향 (Effects of Modified Cross-body Stretching on Range of Motion in Glenohumeral Joint)

  • PURPOSE: The purpose of this study was to compare the effects of cross-body stretching (CBS) and modified cross-body stretching (MCBS) on the range of motion (ROM) of glenohumeral joint (GHJ) in healthy subjects. METHODS: Thirty subjects were randomly assigned to 1 of 2 groups: CBS group without stabilization of scapula (n=15) and MCBS group with stabilization of scapula (n=15). We measured horizontal adduction and internal rotation ROM of GHJ in pre- and post-intervention. The data were analyzed using the analyses of covariate (ANCOVA) and least significant difference (LSD) post hoc tests (p=.05). RESULTS: In the post-intervention, the MCBS group had a significantly greater increase in horizontal adduction ROM (mean ${\pm}$ SD, $11.46^{\circ}{\pm}0.83^{\circ}$) compared to the CBS group ($7.81^{\circ}{\pm}0.83^{\circ}$) (p=.007) and a greater increase in internal rotation ROM ($62.27^{\circ}{\pm}0.74^{\circ}$) compared to the CBS group ($59.20^{\circ}{\pm}0.74^{\circ}$) (p=.004). CONCLUSION: A single session application of an MCBS provides immediate more improvements in both horizontal adduction and internal rotation ROM than CBS. These results suggest that application of MCBS with stabilization of scapula may be a more useful to gain ROM of GHJ than CBS without stabilization of scapula.

치과용 임프란트 매식을 위한 견갑골외연의 형태학적 연구 (MORPHOMETRIC STUDY OF SCAPULAR LATERAL BORDER FOR INSTALLATION OF DENTAL IMPLANT.)

  • The scapular flap, described by dos Santos in 1986, has been used successfully for the reconstruction of a variety of defects of oro-mandible. Some have defined the gross and vascular anatomy of the lateral border of the scapula, yet useful anatomical information and a complete description of area and contour of each cut surface of lateral border of scapula, which is very important for esthetic and functional reconstruction using dental implants, are missing. These prompted us to clarify the cross-sectional area of lateral border of scapula. Twenty three scapulas of 15 fixed adult Caucasian cadavers were sectioned in every 1cm interval along the lateral border of scapular, and the metric relations and the shape of cut surface were assessed. The lateral border of the scapula, consisting of cortico-cancellous bone measuring $7.86{\pm}0.97mm$ in width, $19.6{\pm}2.86mm$ in height and $12{\pm}1.78cm$ in length, could be harvested as an osteocutaneous scapular flap or as a single vascularized bone flap. The mean thickness of cortical bone of lateral, medial, dorsal and costal surface was $0.46{\pm}1.48mm$, $1.78{\pm}1.34mm$, $1.54{\pm}1.11mm\;and\;1.35{\pm}0.87mm$, respectively. So we have thought that all scapular transplants could be supported osseointegrated implants for fixation of dental prosthesis.
