A study on the ground reaction forces and plantar pressure variables in different safety shoes and applying insole during walking (안전화 형태와 Insole 착용 유무에 따른 보행동작시 하지부위에 대한 지면반발력과 압력분포 부하)

  • The purpose of this study was to compare the ground reaction forces and plantar pressure variables among three different safety shoes (Type 1: ergonomically designed and high quality shoes, 2: curved and cushioned safety hoes, and 3: regular safety shoes) and to find the effect of insole during walking. Ten healthy subjects were recruited for this study. The ground reaction force was measured using a 3 dimensional motion analysis system. Plantar pressures were measured Pedar Mobile foot pressure scan system. The ground reaction force variables were not significantly different among three different shoe types and insole conditions. After insertion insole, plantar pressure distributions were improved. These results suggest that the type 1 safety shoes was superior than other safety shoes in the statistics, and applying insole could be a possible method to prevent fatigue of lower extremity and musculoskeletal disorders. Further studies are needed to find the effect of ergonomically designed safety shoes design and insole on practical value prevention of musculoskeletal disorder, fatigue and satisfaction of workers.

A Study of Event-Related Brain Potentials in Children's Korean Sentence Comprehension (아동의 한국어 문장이해과정에 나타난 사건관련전위 연구)

  • The present study investigated the semantic and syntactic processes in Korean children's sentence comprehension by measuring event-related brain potentials(ERPs). The subjects were 18 right-handed, healthy native Korean children(1st graders), who were free from any hearing problems. While the children listened to the auditory sentence stimuli, ERPs were recorded with the electrodes mounted in a 'Quik-Cap' on the subject's head. The ERP data were recorded and analyzed using the NeuroScan 4.3 and EEGLABv6.03b programs. For statistical analyses, descriptive statistics, three-way repeated measures ANOVA, and t-tests were performed using a SPSS 15.0 PC program. The results indicated that semantic violations elicited a negativity (N400) ranging from 300 to 500 msec. For syntactic violations, children displayed a positivity (P600) ranging from 900 to 1,100 msec. The discovery of N400 and P600 in semantic and syntactic processes respectively, confirmed the suggestion that Indo-European and Korean languages share a common mechanism for sentence comprehension.

Effects of Korean Medicine Treatments on Pain Reduction of Postoperative Meniscal Injury Patients : A Retrospective Observational Study (슬관절 반월상 연골 파열 후 수술 환자의 통증 감소에 대한 한의학적 치료 효과: 후향적 관찰연구)

  • The objective of this study is to observe the effectiveness of complex Korean Medicine treatments on pain reduction of patients after meniscal surgery. We analyzed 16 patients who have been diagnosed as Meniscal injury on Magnetic Resonance Imaging(MRI) scan and had admitted to Dunsan Korean Medical hospital from January 30, 2014 to April 30, 2019. The analysis was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 25 program. We used Numeric Rating Scale to evaluate pain reduction, Five-point likert scale to asses treatment satisfaction. The result showed that there were statistically significant pain reduction on the postoperative meniscal injury patients treated with complex Korean Medicine treatments. In conclusion, we found that the complex Korean Medicine treatments showed a positive effect on pain reduction of postoperative meniscal injury patients. Further research is required to confirm the effectiveness of Korean Medicine treatment.

A Comparative Study on the CT Effective Dose by the Position of Patient's Arm (전신 PET/CT 검사에서 환자의 팔 위치에 따른 CT 유효선량의 비교 연구)

  • Purpose: In the whole body PET/CT scan, it is natural to lift the patient's arm for its quality improvement. However, when the lesion is located in head and neck, the arms should be located lower. This study was designed to compare the CT effective dose for each arm position applying Automatic Exposure Control (AEC). Materials and Methods: 45 patients who had $^{18}F$-FDG whole body PET/CT scan were studied with Biograph Truepoint 40 (SIEMENS, GERMANY), Biograph Sensation 16 (SIEMENS, GERMANY), Discovery STe 8 (GE healthcare, USA). The CT effective dose of 15 patients for each equipment was measured and comparatively analyzed in both arm-lifted position and lower-arm position. ImPACT v1.0 program was used as the method of measurement for CT effective dose. For the statistics analysis, Paired t-test which paired with SPSS 18.0 statistic program was applied. Results: In the case of arm-lifted, it was measured as $6.33{\pm}0.93mSv$ for Biograph Sensation 16, $8.01{\pm}1.34mSv$ for Biograph Truepoint 40, and $9.69{\pm}2.32mSv$ for Discovery STe 8. When arms are located lower position, it was measure as $6.97{\pm}0.76mSv$, $8.95{\pm}1.85mSv$, $13.07{\pm}2.87mSv$ for each. CT effective dose according to the arm position was 9.2% for Biograph Truepoint 40, 10.5% for Biograph Sensation 16, and 25.9% for Discovery Ste 8. The statistics analysis showed the meaningful difference ($p$<0.05). Conclusion: For the whole body PET/CT case, CT effective dose applying AEC was decreased the radiation exposure of the patients when the arm was lifted for 15.2% of average value. The patient who has no lesion in head and neck would decrease the artifact occurrence in objective part and lower the CT effective dose. Also, for the patient who had lesion in head and neck, the artifact in objective part can be lower by putting the arms down, the fact that CT effective dose increases should be concerned in its whole body PET/CT scan.

Evaluation of the Usefulness of Virtual Reality Equipment for Relieving Patients' Anxiety during Whole-Body Bone Scan (전신 뼈 검사 환자의 불안감 해소를 위한 가상현실 장비의 유용성 평가)

  • Purpose When performing a whole-body bone scan, many patients are experiencing psychological difficulties due to the close distance to the detector. Recently, in the medical field, there is a report that using virtual reality (VR) equipment can give pain relief to pediatric patients with weak concentration or patients receiving severe treatment through a distraction method. Therefore, in this paper, VR equipment was used to provide psychological stability to patients during nuclear medicine tests, and it is intended to evaluate whether it can be used in clinical practice. Materials and Methods As VR equipment, ALLIP Z6 VR (ALLIP, Korea) was used and the experiment was conducted after connecting to a mobile phone. The subjects were 30 patients who underwent whole-body bone examination from September 1, 2021 to September 30, 2021. After intravenous injection of 99mTc-HDP, 3 to 6 hours later, VR equipment was put on and whole body images were obtained. After the test, a survey was conducted, and a Likert scale of 5 points was used for psychological anxiety and satisfaction with VR equipment. Hypothesis verification and reliability of the survey were analyzed using SPSS Statistics 25 (IBM, Corp., Armonk, NY, USA). Results Anxiety about the existing whole-body bone test was 3.03±1.53, whereas that of anxiety after wearing VR equipment was 2.0±1.21, indicating that anxiety decreased to 34%. When regression analysis of the effect of the patient's concentration on VR equipment on anxiety about the test, the B value was 0.750 (P<0.01) and the t value was 6.181 (P<0.01). decreased and showed an influence of 75%. In addition, overall satisfaction with VR equipment was 3.76±1.28, and the intention to reuse was 66%. The Cronbach α value of the reliability coefficient of the questionnaire was 0.901. Conclusion When using VR equipment, patients' attention was dispersed, anxiety was reduced, and psychological stability was found. In the future, as VR equipment technology develops, it is thought that if the equipment can be miniaturized and the resolution of VR content images is increased, it can be used in various clinical settings if it provides more realistic stability to the patient.

The Quantitative Assessment of Renal Function and Size by Differences of Acquisition Counts in $^{99m}Tc$-DMSA Renal Scan ($^{99m}Tc$-DMSA 신장검사에서 획득 계수의 차이에 따른 기능 및 형태 평가)

  • Purpose: In nuclear medicine study, there are two methods, preset count method and preset time method, to acquire static images. We usually use preset count method for static image in $^{99m}Tc$-DMSA renal scan, but occasionally use preset time method. In case of using preset count method, we always acquire same counts but it causes a difference of scan time. In case of using preset time method, it takes same scan time to acquire images but it causes different counts. Therefore, the purpose of this study is to investigate any differences of function and formal information in both kidney by acquisition counts Materials and Methods: From January 11, 2010 to March 31, 2010, we analyzed the 30 patients (M: 11, W: 19). who were examined by $^{99m}Tc$-DMSA scan and have one side of functioning kidney relatively between 40~60%. And the patients who have cold and hot region in image were analyzed but we did not accept images of patients when it was hard to divide kidney into cortex. There was no division between subjects and age of subjects is $14.83{\pm}22.07$ old. We used the BrightView gamma camera from PHILIPS. To analyze function and formal of kidney, we used JET stream release 3.0 version from PHILIPS. Using SPSS 12.0 program, we compared descriptive statistics and paired T-test. Images were acquired sequentially in the same parameters, but there are three methods which different from acquisition time and scan time, 100 kcounts, 300 kcounts and 7 minutes method (exceed 300 kcounts). To assess function and formal information of kidney, we measured renal relative function, geometric mean and size of kidney and analyzed each difference. Results: In case of renal relative function in both kidney, 100 kcounts method was $50.52{\pm}3.64%$. 300 kcounts method was $50.38{\pm}3.66%$ and 7 minutes method was $49.91{\pm}3.40%$ and there were no statistical significant differences between each method. In case of geometric mean, 100 kcounts method was $50.08{\pm}3.25%$. 300 kcounts method was $49.89{\pm}3.40%$ and 7 minutes method was $49.91{\pm}3.24%$. And also, there were no statistical significant differences. When comparing size of kidney, 100 kcounts method was $8.23{\pm}1.96$ cm. 300 kcounts method was $8.12{\pm}1.90$ cm and 7 minutes method was $8.35{\pm}1.97$ cm. In case of right kidney, 100 kcounts method was $7.91{\pm}1.88$ cm. 300 kcounts method was $8.12{\pm}1.90$ cm and 7 minutes method was $8.25{\pm}1.96$ cm. From those values, we recognized that there were significant differences each method (p<0.05). Conclusion: From results of this study, there were no statistical differences in renal relative function and geometric mean by acquisition counts. However, in shape of kidney, the more acquisition counts are increasing, the more size of kidney is getting big. And there were statistical significant differences. Therefore, to perform reliable quantitative result, preset count method is more desirable than preset time method. Especially, in case of a follow-up test, if we use preset time method, it will cause differences of formal results in kidney due to acquisition counts each time we examine patients.

Study on the Difference in Intake Rate by Kidney in Accordance with whether the Bladder is Shielded and Injection method in 99mTc-DMSA Renal Scan for Infants (소아 99mTc-DMSA renal scan에서 방광차폐유무와 방사성동위원소 주입방법에 따른 콩팥섭취율 차이에 관한 연구)

  • Purpose $^{99m}Tc-DMSA$ renal scan is a test for the comparison of the function by imaging the parenchyma of the kidneys by the cortex of a kidney and by computing the intake ratio of radiation by the left and right kidney. Since the distance between the kidneys and the bladder is not far given the bodily structure of an infant, the bladder is included in the examination domain. Research was carried out with the presumption that counts of bladder would impart an influence on the kidneys at the time of this renal scan. In consideration of the special feature that only a trace amount of a RI is injected in a pediatric examination, research on the method of injection was also carried out concurrently. Materials and Methods With 34 infants aged between 1 month to 12 months for whom a $^{99m}Tc-DMSA$ renal scan was implemented on the subjects, a Post IMAGE was acquired in accordance with the test time after having injected the same quantity of DMSA of 0.5mCi. Then, after having acquired an additional image by shielding the bladder by using a circular lead plate for comparison purposes, a comparison was made by illustrating the percentile of (Lt. Kidney counts + Rt. Kidney counts)/ Total counts, by drawing the same sized ROI (length of 55.2mm X width of 70.0mm). In addition, in the format of a 3-way stopcock, a Heparin cap and direct injection into the patient were performed in accordance with RI injection methods. The differences in the count changes in accordance with each of the methods were compared by injecting an additional 2cc of saline into the 3-way stopcock and Heparin cap. Results The image prior to shielding of the bladder displayed a kidney intake rate with a deviation of $70.9{\pm}3.18%$ while the image after the shielding of the bladder displayed a kidney intake rate with a deviation of $79.4{\pm}5.19%$, thereby showing approximately 6.5~8.5% of difference. In terms of the injection method, the method that used the 3-way form, a deviation of $68.9{\pm}2.80%$ prior to the shielding and a deviation of $78.1{\pm}5.14%$ after the shielding were displayed. In the method of using a Heparin cap, a deviation of $71.3{\pm}5.14%$ prior to the shielding and a deviation of $79.8{\pm}3.26%$ after the shielding were displayed. Lastly, in the method of direct injection into the patient, a deviation of $75.1{\pm}4.30%$ prior to the shielding and a deviation of $82.1{\pm}2.35%$ after the shielding were displayed, thereby illustrating differences in the kidney intake rates in the order of direct injection, a Heparin cap and the 3-way methods. Conclusion Since a substantially minute quantity of radiopharmaceuticals is injected for infants in comparison to adults, the cases of having shielded the bladder by removing radiation of the bladder displayed kidney intake rates that are improved from those of the cases of not having shielded the bladder. Although there are difficulties in securing blood vessels, it is deemed that the method of direct injection would be more helpful in acquisition of better images since it displays improved kidney intake rate in comparison to other methods.

Study on the Contrast Media Volume according to Scan Time during Brain Angiography Examination (Brain Angiography 검사 시 Scan Time에 따른 Contrast Media Volume에 대한 연구)

  • The purpose of this study was to investigate whether there is a difference in image quality before and after reducing the amount of contrast medium in cerebral hemorrhagic patients who periodically follow up CT, and after reducing the amount of contrast medium, the image before and after reduction was examined, and the image quality was assessed by setting ROI at each vessel location using MMWP program. First Rt. CCA, Lt.CCA. Rt.MCA, Lt.MCA and Basilar Artery each got an ROI. Second, the mean and standard deviation values for the ROI were obtained. Third, SNR and CNR were obtained through the average and standard deviation values obtained. T-test statistics show that the SNR and CNR values obtained show that the result values for SNR are Rt.CCA 0.765, Ltd.CCA 0.871, Rt.MCA 0.343 LT.MCA 0.235, Basilar alternative 0.916, result value for CNR Rt.CCA 0.088, Ltd.CCA 0.069, Rt.MCA 0.818, Let's...MCA 0.579 and Basilar Artery 0.878. The results obtained through the obtained SNR and CNR values showed no difference in the quality of the images before and after reducing contrast medium. Therefore, we hope that this study will be an indicator that reduces the burden on contrast agents on patients who do CT examinations periodically.

Breast Cancer Clustering in Kanagawa, Japan: A Geographic Analysis

  • Background: The purpose of the present study was to determine geographic clustering of breast cancer incidence in Kanagawa Prefecture, using cancer registry data. The study also aimed at examining the association between socio-economic factors and any identified cluster. Materials and Methods: Incidence data were collected for women who were first diagnosed with breast cancer during the period from January to December 2006 in Kanagawa. The data consisted of 2,326 incidence cases extracted from the total of 34,323 Kanagawa Cancer Registration data issued in 2011. To adjust for differences in age distribution, the standardized mortality ratio (SMR) and the standardized incidence ratio (SIR) of breast cancer were calculated for each of 56 municipalities (e.g., city, special ward, town, and village) in Kanagawa by an indirect method using Kanagawa female population data. Spatial scan statistics were used to detect any area of elevated risk as a cluster for breast cancer deaths and/or incidences. The Student t-test was performed to examine differences in socio-economic variables, viz, persons per household, total fertility rate, age at first marriage for women, and marriage rate, between cluster and other regions. Results: There was a statistically significant cluster of breast cancer incidence (p=0.001) composed of 11 municipalities in southeastern area of Kanagawa Prefecture, whose SIR was 35 percent higher than that of the remainder of Kanagawa Prefecture. In this cluster, average value of age at first-marriage for women was significantly higher than in the rest of Kanagawa (p=0.017). No statistically significant clusters of breast cancer deaths were detected (p=0.53). Conclusions: There was a statistically significant cluster of high breast cancer incidence in southeastern area of Kanagawa Prefecture. It was suggested that the cluster region was related to the tendency to marry later. This study methodology will be helpful in the analysis of geographical disparities in cancer deaths and incidence.

A Study on the Relation between Occlusal Wear Area and Occlusal Contact Patterns (교모면적과 교합접촉양태 간의 관계에 대한 연구)

  • The aim of the present study was to investigate a relation between occlusal wear area and occlusal contact patterns. For the purpose, occlusal wear area were measured in 58 dental students and in 129 patients with temporomandibular disorders(TMDs) from dental casts. Teeth used in this study were from canine to second molar on both sides in upper arch, totally ten. Occlusal wear area on casts was marked by pencil and photocopies, and then, the area was measured with planimeter. Occlusal relation was clinically examined with regard to Angle's classification, chewing side preference, lateral guidance pattern and bruxing and/or clenching habit. T-Scan, electronic occlusal contact analyzer, was used to record occlusal contact number, contact force, contact time and occlusal balance that is TLR(total left-right statistics) during tooth contact. All measurement were repeated 3 times and the average value was used for data processing. The obtained results were as follows : 1. Mean value of occlusal wear area did not differ significantly between dental students and patients. 2. There ws not significant difference in wear area between chewing side and non-chewing side in both groups. 3. Occlusal wear area was significantly increased with age in both groups. 4. Three subgroups divided by Angle's classification did not show any difference in occlusal wear area among them, but three subgroups divided by lateral guidance pattern showed slightly significant difference between canine guide subgroup and group function subgroup in patients. Occlusal wear ares\a in group function subgroup wear larger than canine guide subgroup. 5. Mean value of wear area in patients with bruxing and/or clenching habit did not differ from those in patients without such habit. 6. Correlationship among items related to occlusal contact pattern were highly consistent and significant in dental students and only one item significantly correlated with occlusal wear area was tooth contact time.
