Taxonomical approach to scabies mites of human and animals and their prevalence in Korea (사람과 동물 옴진드기의 형태학적 분류와 국내에서의 발생빈도)

  • Three types, hitherto commonly known as varieties of Solcoptes scobiei (Linnaeus, 1758) were collected from scabietic patients, pigs and dogs to couirm their taxonomic status. As a result, we consider that the mites of human and pig scabies are different subspecies [S. scobiei scohiei (Linnaeus, 1758) and S. scobiei skis Megnin, 1880, respectively], and that of dog scabies is a different species, S. canis Gerlach, 1857. Infestation rate of human scabies in Korea peaked in 1981-1982 and showed nearly 10% of prevalence rate among outpatients of the dermatology clinic, but decreased dramatically during the past ten years and now lowered below 1% since 1990. On the other hand, dog scabies seems to be increasing recently. Key words: Human scabies, dog scabies, pig scabies, taxonomy, prevalence.

Epidemiologic Trends and Seasonality of Scabies in South Korea, 2010-2017

  • Scabies is a parasitic skin infection with intense itching. Scabies infection seriously impairs quality of life, while outbreaks in medical institutions cause financial losses. This study aimed to present the annual and seasonal trend of prevalence of scabies in the national population. Scabies cases were extracted from National Health Insurance Service database and its epidemiologic characteristics were assessed. To analyze the seasonality of scabies occurrence, temperature and humidity were included in the model as weather factors, and the per capita gross national income index was adjusted. The annual prevalence by age group was 0.56-0.69 per 1,000 persons until the age of 40 years and peaked at 3.0-4.1 per 1,000 persons in the age group over 80 years. The number of women diagnosed with scabies has been consistently higher compared to that of men since 2010. Mean number of cases diagnosed as scabies was lowest in spring, approximately 4,000 cases, when the average temperature was less than $5^{\circ}C$ at 2 months prior, whereas more than 6,000 scabies cases occurred in autumn when temperatures exceeded $25^{\circ}C$ at 2 months prior. This study presents the epidemiological characteristics and seasonality of all cases nationwide over 8 years and will help to establish control policies.

A Study of Clinical Aspects of Scabies in the Jeonbuk Province (전북지역 옴 감염의 임상양상에 대한 연구)

  • Background: Scabies is a contagious skin infestation primarily observed in poor or overcrowded environments. However, an individual may be affected regardless of hygiene and/or socioeconomic status, and/or age. Objective: To investigate the characteristics and epidemiology of scabies in patients visiting a single tertiary hospital in Jeonbuk province. Methods: We retrospectively reviewed the medical records of 194 patients with scabies who visited the Dermatology clinic at Wonkwang University Hospital between June 2015 and May 2018. Results: Of the 194 patients investigated, 82 (42.3%) were men and 112 (57.7%) were women. The mean age of men was 46.3 years and that of women was 60.1 years. Scabies was diagnosed in patients most commonly in autumn. The most common symptom of scabies was the occurrence of papules (83.5%) followed by the presence of burrows (23.2%). The most common route of infection was a nursing hospital (26.3%), except unknown (29.4%). Medications used to treat scabies included 5% permethrin cream, 10% crotamiton ointment, and 1% gamma benzene hexachloride lotion. The mean duration of treatment was 33.9 days using 5% permethrin cream, 34.8 days using 10% crotamiton ointment, and 34.6 days using 1% gamma benzene hexachloride lotion. Conclusion: This study could help in the prevention and management of scabies, by guiding clinicians in choosing optimal therapeutic agents based on patients' condition.

Prevalence of canine scabies in the Korean stray dogs (국내 유기견의 개 옴 감염 상황)

  • Recently, the number of stray dogs is proportionate to the increase of the number of the companion dogs. Sarcoptic scabiei var canis, causing scabies, is one of the most important canine zoonotic arthropods in Korea and around the world. Thus, we have tried to know the prevalence of canine scabies in the stray dogs in Korea. A total of 565 stray dogs were collected from the rescue centers all over the country from Jan. 2006 to Dec. 2006. They were grouped with euthanasia or natural death and examined for the gender and age estimated by dental formula. To identify the lesions, the whole body was grossly examined and tested pathologically. Thirty two (5.66%) of 565 dogs were diagnosed as canine scabies. Dogs from urban areas had fewer scabies (0.62%) than those of rural areas (12.5%). Prevalence of scabies in male and female dogs was no difference as 5.96% and 5.25%. Euthanasia group showed higher prevalence (6.48%) than natural death group (2.44%) in scabies. Old dogs over five years showed lower infestration (1.82%) in scabies. In histopathological findings, there were mites in the burrows formed in the subcorneal space. Acanthosis, hyperkeratosis with crust, and vascular dilatation were main findings. One hundred thirteen (20%) of 565 stray dogs were diagnosed to have skin disease. Among them, canine scabies is the most prominent ectoparasite as 5.66 %. With previous reports on human infection in Korea, canine scabies must be regarded as the important zoonotic canine skin disease. Accordingly, for the human and canine hygiene it is imperative that stray dogs with skin problems are segregated and tested for the parasites to treat properly as soon as arriving at rescue shelter.

A Case of Diagnosis and Treatment of Scabies Using Mineral Oil Examination (광유(Mineral oil)법을 통한 옴(Scabies) 진단 및 치료 증례)

  • Objectives : The purpose of this study is to report the diagnosis and treatment of scabies using mineral oil examination and sulfur. Methods : Mineral oil is dropped on the skin lesion area and the burrow, and then scrape 6-7 times with blade 11. Collect the contained mineral oil, place it on a slide, and observe it under an optical microscope. After the scabies were confirmed, sulfur was applied to the whole body once a day. Results : When examined 2 weeks after treatment, scabies and feces were not observed and symptoms were relieved. Conclusions : Mineral oil examination is recommended to confirm scabies, and sulfur can be used to treat scabies.

Diagnostic Accuracy of Dermoscopy for Scabies

  • The diagnostic accuracy of dermoscopy (DS) for scabies, a highly contagious parasitic disease, remains disputed. This study aimed to assess the diagnostic accuracy of DS in scabies, analyze the factors influencing DS, and explore its role in post-treatment evaluation. Patients with suspected scabies were randomly divided into 2 groups: 71 patients in the skin scraping (SS) group and 73 patients in the DS group. The diagnostic efficiencies of SS and DS in these groups were calculated. We also analyzed the influence of body part and investigator competence on the accuracy of DS. Then 16 body parts with typical signs of scabies were monitored by DS 2 and 4 day after sulfur ointment treatment. The sensitivity and specificity of DS were 98.3% and 88.5%, respectively. Hands, arms, and the abdomen had higher positivity rates than other body parts (P<0.001). The accuracy of dermatologists' interpretations of images negative for scabies in the intermediate- and high-level groups was higher than that in the low-level group (P<0.001). At follow-up, the mites were still visible on 43.8% to 62.5% of the skin lesions 2 and 4 day after sulfur ointment treatment. These results showed that DS could significantly increase the accuracy of diagnosing scabies owing to its high sensitivity and specificity. Therefore, it may be useful for monitoring clinical responses to anti-parasitic treatment.

Prevalence of pediculosis and scabies in preschool nursery children of Afyon, Turkey

  • Scabies and pediculosis are ubiquitous, contagious, and debilitating parasitic dermatoses. The tendency of high prevalence of pediculosis and scabies among school and preschool age children has prompted us to conduct a head louse and scabies prevalence survey among preschool nursery children in our district. A school-based, crosssectional study was performed, with 1,134 children chosen for evaluation. All cases were evaluated by physical examination and a detailed, structured questionnaire. The infestation was found in 14 $(1.2\%)$ of 1,134 children; 9 $(0.8\%)$ with pediculosis capitis and 5 $(0.4\%)$ with scabies. We found that infestations were more frequent in children with mothers whose education levels were low. This indicates the necessity of an improvement in the economic and sociocultural status of the community and the promotion of hygiene concepts and practices in order to improve health of preschool age children.

Scabies mimicking graft versus host disease in a hematopoietic cell transplant recipient

  • Scabies is a highly contagious skin infestation caused by the mite, Sarcoptes scabiei var. hominis. Complex responses to scabies mites in the innate, humoral, and cellular immune systems can cause skin inflammation and pruritus. Diagnosis can be challenging because scabies resembles other common skin conditions. We report the first Korean case of scabies in a hematopoietic cell transplant (HCT) recipient, initially suspected of skin graft versus host disease (GVHD). A T-cell acute lymphocytic leukemia patient underwent a sibling-matched allogeneic HCT and developed pruritus after cell engraftment. Treatment for GVHD did not improve the symptoms. He was diagnosed with scabies 30 days after the onset of symptoms.

Efficacy of Application of Selamectin for Canine Scabies Infestation in a Dog (개에서 개선충(Canine Scabies) 감염에 대한 월 2회 Selamectin 적용 일례)

  • A 3-year-old, male, mongrel dog was presented with pruritus, alopecia to the Veterinary medical Teaching Hospital, College of Veterinary Medicine of Kyungpook National University. On physical examination, generalized erythema, papules, scales and crusts were observed. The canine scabies infestation was diagnosed by deep skin scraping and Malassezia spp. and Staphylococcus spp. were found on skin cytology. He was treated with selamectin at 6 mg/kg applied to the skin in a single spot every two weeks for one month in order to investigate the effect of selamectin on canine scabies and was treated with amoxicillin trihydrate/clavulanate pattassium, ketoconazole and chlorhexidine shampoo for Malassezia sup. and Staphylococcus spp. simultaneously. When he was rechecked after 7, 14, 21, and 28 days after the first application of selamectin, scabies werent seen. The dermatological signs including pruritus, papules, scales, and crusts were remarkably resolved at 14 days. The alopecia was almost disappeared and few Malassezia spp. and Staphylococcus spp. were seen after 28 days. The efficacy of selamectin by this protocol is excellent in the dog infestated canine scabies.

Efficacy of Twice a Week Selamectin Application as a Treatment for Mixed Canine Scabies and Ear Mite Infestation in a Dog (개에서 피부개선충(Canine Scabies)과 귀진드기(Ear Mite) 혼합감염에 대한 주 2회 selamectin 적용 일례)

  • A 6-month-old, male, American cocker spaniel was presented with severe pruritus, alopecia and head shaking. In physical examination generalized erythema, papules, scales and crust were observed. Otic discharge was dark-brown colored in both ear canals. After extensive dermatologic diagnostic evaluations, the infestation of scabies, ear mites, Malassezia spp, cocci were proved. Therefore, he was treated with selamectin 6 ㎎/㎏ which was applied to the skin in a single spot twice a week for 2 weeks in order to investigate the effect of selamectin on canine scabies and ear mites by this treatment protocol. When he was rechecked at 14th day after fourth being administered, scabies and ear mites wasn't seen. The dermatological signs, pruritus, papules were remarkably decreased and only slight scales were observed. After treatment of scabies and ear mites, the clavamox and koto-conazole was administered for cocci and Malassezia spp, respectively, for 1 week and topical therapy was initiated. with sebolytic and chlorhexidine shampoo once a week for 3 weeks. Two months later skin lesions were normal dermatologically and the hair was fully recovered. Despite of the expensiveness the selamectin is very effective in the dogs that infected by scabies and ear mites as this treatment protocol.
