Factors associated with patient satisfaction after arthroscopic rotator cuff repair: do they differ by age?

  • Background: Several studies have investigated factors affecting patient satisfaction after arthroscopic rotator cuff repair (ARCR); however, it is unknown if these factors vary according to age. Therefore, this study aimed to evaluate the factors associated with satisfaction of ARCR in individuals 70 years and older versus younger patients. Methods: Among 319 consecutive patients who underwent ARCR, 173 were included. Patients were divided into an old age group (≥70 years) and a young age group (<70 years), and the two age groups were further divided into satisfied and unsatisfied subgroups. Patient satisfaction was evaluated at the final follow-up visit using a binary question (yes or no). Clinical outcomes were assessed preoperatively and at the final follow-up. Results: Satisfaction rates in the older and younger age groups were 75.41% and 79.47%, respectively. Mean changes in Constant and American Shoulder and Elbow Surgeons scores were significantly different between the satisfied and unsatisfied subgroups (P=0.031 and P=0.012, respectively) in the young patients. In the old patients, there was a significant difference in the mean change in depression subscale of the Hospital Anxiety and Depression Scale (P=0.031) and anxiety subscale of the Hospital Anxiety and Depression Scale (P=0.044) scores between the satisfied and unsatisfied subgroups. Conclusions: Factors affecting patient satisfaction after ARCR differed according to age. Psychological improvement was more important to elderly patients, whereas restoration of function was more important to younger patients. Pain relief was important for both age groups. 일 도시 대학병원 환자의 서비스 만족도 (Patients' Satisfaction with the Services of University Hospitals in Local Cities)

  • Introduction: This study was conducted to investigate patients' satisfaction with hospital services at a university hospital in G city. Methods: The subjects of this study were 92 inpatients and data were collected during the period from March to April 2003. The research tool used was a 24-item scale(Cronbach' a= .768) on patients' satisfaction with two subcategories of services: 18 items on hospital human resources(a= .722) and 6 items on hospital environment(a= .700). Data were analyzed through $x^2$-test, t-test and ANOVA using SPSS/PC. Results: The level of patients' satisfaction was $38.4{\pm}3.77$ on hospital services $27.6{\pm}6.14$ on hospital human resources, and $10.8{\pm}2.39$ on hospital environment on the average. Satisfaction related to hospital human resources was higher in male subjects (t = 4.15, p = .003) and in those who stayed longer than 15 days (t= 4.404, p= .039) than the others, and also higher in those who replied that all items related to hospital facilities are satisfactory except the parking lot. Satisfaction related to hospital environment was significantly higher in more educated subjects(F = 2.945, p= .037) and in those who replied that all items related to hospital facilities are satisfactory or appropriate except the parking lot and admission procedure. Conclusions: Length of stay and the level of accessibility and appropriateness of hospital facilities were found to be factors that have significant effects on patients' satisfaction.

비.부비동염으로 내원한 소아환자들의 치료 유형별 만족도와 치료 평가 (The Effects of Simultaneous Treatment with Supplementary Therapy and Treatment Interval for Improving Symptoms and Satisfaction Rate by Treating Child Rhinitis or Paranasal Sinusitis Patients)

  • Objectives The purpose of this study is to investigate the effects of simultaneous treatment with supplementary therapy and treatment interval for improving symptoms and satisfaction rate by treating child rhinitis or paranasal sinusitis patients. Methods 41 rhinitis or paranasal sinusitis patients who visited the clinic between April 2004 and April 2006 were involved for this study. The patients were classified into Group A(2 sessions per week, simultaneous supplementary therapy), Group B(1 session in 2 weeks, simultaneous supplementary therapy) and Group C(1 session in 2 weeks, no supplementary therapy). After the experiment, the improvement rates of symptoms and satisfaction rate were surveyed by the questionnaires. Supplementary therapies used in Group A and B were aroma spray, Saengbit-patch, cutaneous acupuncture, nebulizer and Chuna manipulation. Results From 41 of the rhinitis or paranasal sinusitis patients, the Group A(simultaneous treatment group, 2 sessions of intensive care per week with supplementary therapy) showed the remaining symptoms score of $12.1{\pm}10.0$ in average. This score was the lowest comparing with the average of $25.7{\pm}12.9$ in Group B(1 session in 2 weeks, simultaneous treatment group with supplementary therapy) and the average of $21.0{\pm}20.1$ in Group C(1 session in 2 weeks, no supplementary therapy). Nose symptoms and sleep scores were significantly lower in Group A(p<0.01). The satisfaction score was the highest in Group B that had the most remaining symptoms and it had no significant relationship with the symptom improvement. Additional analysis showed that among many factors such as treatment interval, treatment method, cost, accessibility of the clinic, family history and satisfaction with the doctor; the satisfaction with their doctor was the most closely related to the satisfaction rate of the patients. Conclusions In this study with 41 of rhinitis or paranasal sinusitis patients, the 2 sessions of intensive care group showed the best results in improving symptoms. Supplementary treatments were also used simultaneously; aroma spray, cutaneous acupuncture, Saengbit-patch, Chuna manipulation and nebulizer. The questionnaires showed that the satisfaction rate of the patients(care taker) was not related to the symptom improvement. Additional analysis of the factors that influence the satisfaction rate showed that it was more closely related to the satisfaction with their doctors.

개인맞춤형 주거환경개선이 작업수행만족도 및 일상생활활동에 미치는 효과 : 뇌졸중 환자를 대상으로 한 사례연구 (The Effects of Personalized Residential Environment Improvement on Occupational Performance Satisfaction and Activities of Daily Living : Case Studies in Stroke Patients)

  • Purpose: The purpose of this study was to investigate the effects of personalized residential environment improvement on occupational performance satisfaction and activities of daily living(ADL) in stroke patients, and desire to use as the basis for presenting an effective method for improving the residential environment of the disabled patients. Method: This study has been carried out with 3 stroke patients undergoing therapy for rehabilitation at the S hospital from August 2014 to January 2015. Residential environment improvement was conducted based on the desired space. Occupational performance, satisfaction and ADL assessed by modified COPM, K-MBI. Intervention has provided grab bar and aids fit to the environment of each person. Result: After residential environment improvement, ADL score was improved, but improved scores for specific items only. In occupational performance and satisfaction, there was a significant difference. Conclusion: The results of this study were to find out that there is a positive effect of personalized residential environment improvement on occupational performance satisfaction and activities of daily living in stroke patients, could be used as a basis for presenting an effective way to residential environment improvement of the disabled patients.

가려움증 중재 프로그램이 혈액투석환자의 가려움증과 수면에 미치는 효과 (Effects of a Pruritus Intervention Program on Pruritus and Sleep Satisfaction for Hemodialysis Patients)

  • Purpose: The purpose of this study was to analyze the effects of a pruritus intervention program on pruritus and sleep satisfaction in patients undergoing hemodialysis. Method: This study was designed for a nonequivalent control group repeated measures quasi-experimental study. Hemodialysis patients were recruited from a kidney center at a university hospital located in G city, Korea. The subjects consisted of 43 patients 21 for the experimental group and 22 for the control group. The 2-week pruritus intervention program was given to the experimental group only. An analytical survey was done before and after the pruritus intervention program. Result: There was a significant decrease in pruritus and increase in sleep satisfaction in the experimental group compared to the control group. There was significant improvement in clinical symptoms and sleep satisfaction after completing the pruritus intervention program in the experimental group. Conclusion: Pruritus intervention program in hemodialysis patients decreased pruritus severity and increased sleep satisfaction, which can be used as an efficient nursing intervention for hemodialysis patients.

중소 종합병원 입원환자와 간호사의 간호서비스에 대한 기대와 지각, 질 평가와 만족도 차이 (The Differences in Quality Perceptions, Expectations, Evaluation, and Satisfaction for Nursing Service between Patients and Nurses: Small-medium Sized General Hospitals)

  • Purpose: This study was performed to give direction to quality improvement strategies of nursing services by comparing the differences in quality perceptions and satisfaction for nursing services between patients and nurses in small-medium sized general hospitals with 200 beds. Method: The subjects, who were 150 inpatients and 162 nurses of 4 general hospitals in a community, answered a self-report questionnaire with a SERVQUAL scale. Result: There were differences between patients' and nurses' expectations and perceptions of nursing service and satisfaction. In the service expectation, the highest factor was 'the responsiveness', and in the perceived performance, the highest was the 'assurance'. In addition, overall patients' perceptions on nursing services showed higher than nurses'. There were positive correlations among the expectations and perceptions on nursing service, and satisfaction. The correlation between perception and satisfaction was higher than the correlation between expectations and satisfaction. Conclusion: To improve the nursing service quality at small-medium hospitals, strengthening the 'assurance' factor and improving the nursing service support system is needed. Also, this study on nurses' perceived nursing service at small-medium sized hospitals should be duplicated.

Botulinum Toxin Therapy versus Anterior Belly of Digastric Transfer in the Management of Marginal Mandibular Branch of the Facial Nerve Palsy: A Patient Satisfaction Survey

  • Background Botulinum toxin (BT) chemodenervation and anterior belly of digastric muscle (ABD) transfer are both treatment options in the management of an isolated marginal mandibular branch of the facial nerve (MMB) palsy. We compare the patient satisfaction following either BT injections or ABD transfer in the management of their isolated MMB palsy. Methods Patients in the ABD-arm of the study were identified retrospectively from September 2007 to July 2014. The patients in the BT-arm of the study were identified prospectively from those attending the clinic. Both groups of patients completed a validated patient satisfaction survey. Statistical analysis was performed and a P-value <0.05 was considered statistically significant. Results Seven patients were in the ABD-arm and 11 patients in the BT-arm of the study. The patient satisfaction in both groups was high with 45% of ABD-arm patients and 40% of BT-arm patients rating their overall outcome as 'better' or 'much better', which was significantly more than the proportion rating their outcome as 'worse' or 'much worse' (P<0.001), although there was a significant trend towards those in the ABD-arm being more likely to be dissatisfied with their outcome (P=0.01). Conclusions BT therapy is a good first-line intervention in the management of isolated MMB palsy. We have, however, shown that the overall satisfaction in both groups is high. Therefore, in patients who would prefer a more permanent solution to manage their facial asymmetry, ABD transfer remains a satisfactory treatment option with a good level of patient satisfaction.

Plate waste study among hospitalised patients receiving texture-modified diet

  • BACKGROUND/OBJECTIVES: While plate waste has been widely investigated in hospitals, there have been minimal studies specific to the texture-modified diet (TMD). This study aims to determine the percentage of plate waste among patients prescribed with TMD and its contributory factors. SUBJECTS/METHODS: This was a single-centre study conducted in the university hospital on three types of TMD (blended diet, mixed porridge, minced diet) during lunch and dinner meals. Weighing method and visual estimation method assisted by digital photograph were adopted in this study. Face to face interview was carried out to investigate on 1) the food/food service quality factors in terms of patients' satisfaction level towards sensorial quality of food and food services provided and 2) the clinical/external factors including appetite, the provision of oral nutrition support, time taking the diet, the need for feeding assistance and the length of hospital stay. RESULTS: The mean percentage of overall plate waste of 95 patients receiving TMD was high (47.5%). Blended diet was identified as the most wasted diet (65%) followed by minced diet (56%) and mixed porridge (35%). Satisfaction level among patients was moderate. Patients on TMD in general had higher satisfaction level on the aspect of food service as compared to food quality. Substantial association between sensorial qualities of food and plate waste were varied according to individual TMD type. A multiple linear regression showed that only the satisfaction level toward the aspects of appearance and variety of foods were the predictors of TMD plate waste (R2 = 0.254, P < 0.05). CONCLUSIONS: A significant relationship between the percentage of plate waste and the overall satisfaction level of patients receiving TMD suggests that vigorous strategies are needed to reduce the food waste of TMD which will lead to a better nutritional status and clinical outcomes among the patients.

대학병원 암환자의 의료서비스 만족도에 영향을 미치는 요인 (Factors Affecting Health Services Satisfaction of Cancer Patients in University Hospitals)

  • 본 연구는 우리나라 대학병원 암환자의 의료서비스 만족도에 영향을 미치는 요인을 분석하기 위해 수행하였다. 자료는 2011년 3월 21일부터 3월 30일까지 수도권과 부산지역 대학병원을 이용한 암환자 420명을 대상으로 설문조사를 통해 수집하였으며, 회귀분석을 통해 환자만족도에 영향을 미치는 요인을 분석하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 대학병원을 이용하는 암환자에게는 의료진의 실력, 친절, 상세한 설명 등과 같은 의료의 질적 요인이 환자 만족도에 가장 큰 유의한 양 (+)의 영향력이 있는 것으로 나타났다. 둘째, 대학병원의 최신의 시설 및 환경요인은 서비스 만족도에 유의한 양 (+)의 영향력이 있는 것으로 분석되었다. 셋째 대학병원을 이용하는 암환자에게 이용의 접근성 요인은 만족도 요인에 유의한 영향력이 없는 것으로 나타났다. 상기의 연구 결과를 통해 대부분이 생명과 직결되는 중환자인 암환자의 경우 의료진의 실력, 병원의 세심한 배려 및 친절, 상세한 설명, 최신 장비, 쾌적한 주변 환경, 적절한 편의시설 제공이 만족도에 중요한 요인임을 시사해 주고 있어 향후 이를 충족시키기 위한 대학병원 차원의 다각적인 방안이 강구되어야 할 것이다.

임플란트 수술용 가이드를 이용하여 임플란트를 식립한 환자의 만족도 (Satisfaction of patients with implants placed using the implant surgical guide)

  • Background: This study was aimed at improving the satisfaction of patients with implants placed using an implant surgical guide by analyzing patient satisfaction with the implant procedure carried out using the guide . Methods: We extracted convenient samples from patients with implants who had visited dental hospitals and clinics in Mokpo from March 2 to June 30, 2022. We used 228 copies of the self-report survey for analyses. Results: All items of implant satisfaction were positively correlated with the use of an implant surgical guide. Conclusions:The results of this study showed that implant satisfaction can be increased with the use of implant surgical guides.
