A Case Report of a Soeumin Patient with Dyspnea Induced by Neuromuscular Disease (신경근육질환으로 인한 호흡곤란을 호소하는 소음인환자 치험 1례 보고)

  • Objectives Dyspnea due to neuromuscular disease is very rare in the complementary medicine field. The purpose of this study is to report a case which showed sypmtoms improvement after treatment with Sohaphyang-won. Methods To evaluate the results of this treatment, number of days which the patient had dyspnea, results of an Arterial blood gas analysis, and the patient's lab reports were measured. Results The patient who suffered from severe sudden dyspnea and applying mechanical ventilator, was treated by Sohaphyang-won and acupuncture therapy for about 2 months. He had been diagnosed as dyspnea caused by neuromuscular disease, which in Western medicine there was no specific treatment. But after treated with Sohaphyang-won and acupuncture, his frequency of dyspnea decreased. Conclusions This study suggests that using Sasang constitutuional medical treatment is effective for Soeumin patient with dyspnea induced by neuromuscular disease.

  • The tremor which has been showed most frequently in the clinic has various reasons, which is common in the dyskinesias and has rhythmic vibration involuntarily. The purpose of this study is to give an account of the effects of the Korean herbal medicine therapy which is based on the constitutional medicine for the patient who has suffered from tremor which is accompanied with diarrhea in the lacunar infarction. The subject is a 70-year-old woman who has had the symptoms caused by lacunar infarction, which are tremor, right side weakness, right facial palsy, dizziness, chest discomfort, and chronic diarrhea, dipsia, abdominal discomfort, andominal pain and so forth. We have diagnosed her as Shaoyangin(少陽人) Mangyin(亡陰證) and have prescribed Huashi Kushen Tang(滑石苦蔘湯) in accordance with the principle of Pyoeumganggi(表陰降氣). The consequence is that the accompanied tremor and diarrhea and general depressed condition have improved.

A Systematic Review on the Treatment of Post-Stroke Patients Based on Sasang Constitutional Medicine (사상의학적 중풍 후유증 치료에 대한 체계적 문헌 고찰)

  • Objectives The purpose of this study was to review and evaluate the clinical evidence of the efficacy and safety of treatment based on Sasang constitutional medicine (SCM) for post-stroke patients by systematic review and meta-analysis. Methods Randomized controlled trials (RCTs), published in 10 electronic databases up to December 2020, were searched. For the included studies, Cochrane's risk of bias assessment was performed to analyze the methodological quality. The strength of evidence was evaluated using the grading of recommendations assessment, development, and evaluation system based on the results of analyses. All review processes were performed by two independent researchers. Results Five RCTs were finally included. All included RCTs were conducted for one month on post-stroke patients in 60-80s, four studies on Tae-Eum patients and one study on So-Yang patients. Four types of constitution-specific herbal medicine (Chungpyesagan-tang, Cheongsimsanyak-tang, Yeoldahanso-tang, and Yangkyuksanhwa-tang) and constitution-specific acupuncture therapy were identified as interventions. More than half of the included studies were evaluated as low quality due to the high-risk of bias in selection, performance, and detection. The combination of constitution-specific herbal medicine, acupuncture, and conventional treatment was more effective in improving the patients' motor impairment, dysphagia, aphasia, and depression than conventional treatment alone. No serious adverse events by SCM treatment were reported. Conclusions SCM treatment may improve the sequelae of post-stroke patients safely in combination with conventional treatment. Since the quality of clinical evidence included in this study was low, higher quality clinical evidence obtained in well-designed clinical studies will be needed.

A Review on Principles and Access Methods to Sasang Constitutional Medicine of Music Therapy (음악치료(音樂治療)의 원리(原理)와 체질의학적(體質醫學的) 접근을 위한 검토)

Objectives The present study purposed to examine the contents and the principles of music therapy according to Oriental medicine theories in order to prove that music therapy is not a new research area but its principle is found in the long tradition of Oriental medicine. 2. Methods We investigate the possibility of music therapy based on Oriental medicine theories and examine the meanings of music therapy from the viewpoint of Oriental medicine. 3. Conclusions and discussions (1) The principles of music therapy are the principle of homogeneity, catharsis and balance. (2) When one’s mind changes, there are naturally occurred sounds, which are called Oseong (五聲: the oriental five voices exhalation, laughing, singing, wailing and groaning), and the notes defined by arranging the Oseong according to the principle of Ohaeng (五行: the oriental five phases wood, fire, earth, metal, water) are Oheum (五音: the oriental five musical notes Gakeum, Chieum, Gungeum, Sangeum and Wooeum.). If Eum (musical notes) is classified into Ohaeng, it can be divided into Gakeum, Chieum, Gungeum, Sangeum and Wooeum. (3) Change of Sinji (神志: consciousness) induces change of Gigi (氣機: function of Gi), which can change the character of voices. Oseong controls the functions of Ojang (五臟: the oriental five viscera) by ruling one’s Jeongji (情志: emotion). It can reduce the damage of the viscera caused by excessive vent of emotion resulted from unconscious expression of Oseong - Hoseong (呼聲: exhalation), Soseong (笑聲: laughing), Gaseong (歌聲: singing), Gokseong (哭聲: wailing) and Sinseong (呻聲: groaning). (4) Yijeongseungjeong (以情勝情: Control emotion with emotion) therapies, which suppresses an emotion by stimulating another, include Noseungsabeop (怒勝思法: Control anxiety with anger), Heeseungbibeop (喜勝悲法: Control sorrow with joyfulness), (思勝恐法: Control fear with anxiety), Biseungnobeop (悲勝愁法: Control anger with sorrow) and Gongseungheebeop (恐勝喜法: Control joyfulness with fear). (5) Seongeum (聲音: voices and musical notes) can be applied to a stimulation method that not only harmonizes the rhythm of living organs but also controls the occurrence of diseases caused by mutual Pyeonseongpyeonsoi (偏盛偏衰: relative preponderance and weakness) through direct induction of the strength and weakness of Gi function of the oriental five viscera in a human body according to the individual character. Sounds preferred by the patient, the material of an instrument selected by the patient, the character of rhythm and music expressed by the patient and the sound or voice uttered frequently by the patient can be considered in diagnosis and treatments for the patient’s body and mind.

A Case Report of Soyangin Burning Feet Syndrome Treated with Sukjiwhanggosam-tang (소양인 족번열에 대한 숙지황고삼탕 치험 1례)

  • Introduction This is the first case report of a soyangin suffering from burning feet syndrome who reported a significant improvement after taking Sukjiwhanggosam-tang. Case presentation The patient complained of hot feet, pain and numbness that occurred during the night. The patient was diagnosed with 'Yin deficiency diurnal fever disease' and treated with Sukjiwhanggosam-tang as well as acupuncture and cupping therapy. The degree of improvement in heat sensation, pain and numbness was expressed in numeric rating scale compared to the first visit. Conclusion After treatment, the heat sensation, pain and numbness of feet was decreased significantly, and the condition improved much at night.

  • 1. Objectives: The purpose of this case study is to evaluate the effects of Sasang constitutional therapy and treatment of an adult atopic dermatitis patient, who was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病). 2. Methods: The adult Atopic Dermatitis patient was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病) which was based on his Nature & Emotion(性情), physical characteristics, symptoms. He was medicated with Prescriptions of Soyangin. 3. Results and Conclusions: The clinical evaluation was made by SCORAD index, Total eosinophil count and Total IgE. After treatment, his SCORAD results significantly decreased from 88.4 to 21.9, Total eosinophil count decreased from 1320 to 120 and Total IgE also decreased from 1406.6 to 897.3. And the patient's subjective symptom and gerneral condition considerably improved after treatment. This case study describe the effectiveness on adult Atopic Aermatitis symptom by using Prescriptions of Soyangin.

  • If the proximal portion of the fourth segment of the vertebral onery is occluded before the posterior inferior cerebellar artery branch, ischemia usually affects the lateral medulla oblongata or the ipsilateral side of the cerebellum. The lateral medullary syndrome(wallenberg syndrome) resulting from occlusion in this area is common. 62 years old male who has swallowing difficulty, dizziness and diagnosed as the lateral medullary syndrome(wallenberg syndrome) is admitted in Wonkwang university oriental medical hospital Kwangju. We have diagnosed him as Taeumin and treated by east integrated therapy. We classified this as Taenmin-liyoiljung(太陰人 裏熱症) and prescribed chungsimyoinjatang(淸心蓮子湯) to the principle of cheongganjoyoil(淸肝燥熱). In the result, we had the improvement of the symptoms. This report described the process and contents about the way the patient was cured.

  • The etiology of spontaneous intracerebral hematoma is aneurysmal rupture, arteriovenous malfomation, brain rumor, blood dyscrasia, cerebral infaction, cerebra inflammatory vascular disease, hypertension, and unknown causes. And the primary intracerebral hematoma is caused by unknown causes including hypertension. The clinical diagnosis of intracerebral hematoma, size, location, ventricular penetration, and evolution of brain parenchyme by hemorrhage are confirmed by CT scan. The authors have experienced 1 case of spontaneous intracerebral hematoma confirmed by CT scan. We have diagnosed the patient as Soyangin and treated by east integrated therapy. We classified this case as Soyangin-liyoiljung(少陽人裏熱證) and prescribed Yangkyuksanhwatang' Jihwangbaekhotang Hyungbangsabaeksan to the principle of Cheongyangsangsung(淸陽上升). In the result, we had the improvement of the symptoms. This report discribed the process and contents about the way the patient was cured.

The Study on The Validity of "Questionnaire of Sasang Constitution Classification(I)" (사상변증내용(四象辯證內容) 설문조사지(設問調査紙)(I)의 타당화(妥當化) 연구(硏究))

  • This study was conducted for the purpose of finding out an objective classification method for Sasang Constitutional medicine, which divides people into 4 groups of constitution and presents comprehensively physiology, pathology, diagnosis, therapy and recuperation regarding each constitution. Questionnaire of Sasang Constitution Classification(I) was administered to 328 inpatients at Kyung Hee Oriental Medicine Hospital. Data was collected during 10 months from June 1994 to Mar. 11, 1995. For the purposes of this study, the collected data was analyzed by crosstabs, variation analysis, and discrimination analysis. The analyzing program was SPSS PC+V4.0. For the purposes of this study, the collected data was analyzed by crosstabs, cariation analysis, and discrimination analysis. The analyzing program was SPSS PC+V4.0. The results were as follows : 1. There was significant differences of each group scales through variation analysis. The questions of each group had Sasang constitutional diagnostic discrimination abilities 2. The diagnostic discrimination abilities(Hit-ratio=56.10%) of the Questionnaire for Sasang Constitution Classification(I) were found to have over 20% improvement than the propotional chance criteria(33.3%), Especially, Hit-ratio for $So{\breve{u}}m$-In(63%) was higher than that of SoYang-In(55%) and $Ta{\breve{e}}um$-In(56.3%). 3. Through discrimination analysis on good questions of each group, the diagnostic discrimination abilities of the Questionnaire for Sasang Constitution Classification(I) was 57.93%. 4. This would be on the ground that the Questionnaire for Sasang Constitution Classification(I) could be used as a tool for Sasang constitution classification.

Analysis on Intervention Studies of Sasang Constitutional Diet : Participant, Intervention, Comparison, and Outcome (PICO) (사상체질 식이중재연구 현황분석 : Participant, Intervention, Comparison, Outcome (PICO)를 중심으로)

  • Objectives The purpose of this study was to evaluate intervention studies on Sasang Constitutional diet (SCD) through the checks of Participant, Intervention, Comparison, and Outcome (PICO) Methods Randomized controlled trial (RCT) and non-randomized study for intervention (NRSI) about SCD were searched in 4 Korean core databases and other sources, and then PICO was checked. Results 1. Total 10 studies were conducted with 1 RCT and 9 NRSIs. 2. Participants were people with no specific disease, or patients with essential hypertension, hyperlipidemia, obesity, or stroke with diabetes or hyperlipidemia. Most studies were conducted on groups of various Sasang Constitutional types except Taeyangin. 3. Two studies provided participants with meals and exercise. Three studies, instead of providing meals directly, taught participants how to eat SCD on their own. 4. NRSIs have tested the effectiveness of various outcome measures without the presentation of primary outcome, and then concluded that all outcomes were ineffective or some are effective. 5. There was no mention of adverse events. In most studies, a single doctor of Korean medicine diagnosed Sasang Constitution the QSCC II questionnaire. The intervention period ranged from three weeks to three months, and recent studies have conducted interventions for 12 weeks. Conclusions Intervention studies about SCD which were conducted so far have shown problems on the study design of PICO items. The study design and implementation that carefully consider how to maintain similarity between groups, minimize the risk of bias, set primary outcome measure, and control the diet are required.
