The Effect of Botulinum Toxin on an Iatrogenic Sialo-Cutaneous Fistula

  • A sialo-cutaneous fistula is a communication between the skin and a salivary gland or duct discharging saliva. Trauma and iatrogenic complications are the most common causes of this condition. Treatments include aspiration, compression, and the administration of systemic anticholinergics; however, their effects are transient and unsatisfactory in most cases. We had a case of a patient who developed an iatrogenic sialo-cutaneous fistula after wide excision of squamous cell carcinoma in the parotid region that was not treated with conventional management, but instead completely resolved with the injection of botulinum toxin. Based on our experience, we recommend the injection of botulinum toxin into the salivary glands, especially the parotid gland, as a conservative treatment option for sialo-cutaneous fistula.

Expression of Type IV Collagen and Fibronectin in Salivary Gland Tumors (타액선 종양에서 제4형 교원질과 Fibronectin 발현)

  • Objectives: Salivary gland tumors pose considerable difficulty in diagnostic and prognostic assessment based on the histopathologic features alone. We studied the expression of type IV collagen and fibronectin in salivary gland tumors with special emphasis on the differential diagnostic significance. Materials and Methods: We did immunohistochemical stain on paraffin embedded tissues of 33 benign and 24 malignant salivary gland tumors using monoclonal antibody for type IV collagen and polyclonal antibody for fibronectin. Results: 1) Well preserved linear basement membrane-like staining of type IV collagen was detected in duct-cell-derived benign salivary gland tumors. But pleomorphic adenoma exhibited a heterogeneous pattern as focal augmentation and interruption. 2) In malignant tumors, type IV collagen was distributed in an irregular, interrupted manner or completely absent. Adenoid cystic carcinomas displayed a marked staining of the basal membrane associated substances in the pseudocysts. 3) The staining pattern of fibronectin was similar to that of type IV collagen execpt more dense in the stroma. 4) Salivary gland tumors which have a prominent myoepithelial cell component revealed a particular deposition of basement membrane materials adjacent to the myoepithelial cells. Conclusion: The study of the basal membrane substances may be helpful for differential diagnosis of benign and malignant salivary gland tumors and identifying special features of salivary gland tumors such as pseudocystic pattern of adenoid cystic carcinoma. Also we think that the myoepithelial cells contribute to the formation of basement membrane materials.

  • The 2019 American Society of Clinical Oncology (ASCO) Annual Meeting, which took place May 31-June 4 in Chicago, drew more than 32,000 oncology specialists from around the world. The theme of 2019 ASCO conference was "Caring for Every Patient, Learning from Every Patient". Among the topics of interest covered were new approaches to surmount limited access to cancer care and the latest advances in targeted therapies for pancreatic, prostate cancers and soft tissue sarcomas. In the field of head and neck cancer, 8 oral abstracts and 75 poster abstracts were presented at this meeting. In this review, we are going to summarize the eight studies that have been presented orally. The topics are recurrent and/or metastatic head and neck squamous cell carcinoma for two abstracts (#6000, #6002), salivary duct carcinoma for one abstract (#6001), locally advanced nasopharyngeal carcinoma for two abstracts (#6003, #6004), oropharyngeal carcinoma for two abstracts (#6006, #6008), and oral cavity cancer for one abstract (#6007).

Immunohistochemistry of CK7, CK19, CK20, SMA and Ki-67 Expression in Adenoid Cystic Carcinoma of the Head and Neck (두경부 영역에 발생한 선양낭성암종에서 CK7, CK19, CK20, SMA 및 Ki-67의 발현에 관한 면역조직화학적 연구)

  • Objectives : The aim of this study was to investigate immunohistochemical expression of CK7, CK19, CK20, SMA and Ki-67 in Adenoid cystic carcinoma(ACC) of the Head and Neck. Material and Methods : Sixteen patients who were treated in Chungbuk National University Hospital from 1992 to 2004, were included in this study. Ten ACCs, 3 MECs, 1 Salivary duct carcinoma, 1 Adenocarcinoma(NOS), and 1 cacinoma ex pleomorphic adenoma were analyzed immunohistochemically for CK7, CK19, CK20, SMA, and Ki-67. Results : CK7 was expressed in 100% of the adenoid cystic carcinoma and 75% of the other tumors. CK19 was expressed in 75% of the adenoid cystic carcinoma and 100% of the other tumors. CK20 was not expressed in all tumors. SMA was expressed in 88.9% of the adenoid cystic carcinoma and not expressed in the other tumors. Ki-67 was expressed in low level in the adenoid cystic carcinoma. Conclusion : The Ki-67 index could explain the natural course of tumor. Immunohistochemistry of CK7, CK19, CK20, SMA and Ki-67 expression in Adenoid cystic carcinoma may provide useful information to diagnosis.

THE STUDY OF EGF EXPRESSION BETWEEN HUMAN PLEOMORPHIC ADENOMA AND ADENOID CYSTIC CARCINOMA (다형성 선종과 선양낭성 암종에서 상피성장인자 발현에 관한 연구)

  • Epidermal growth factor is a single-chain polypeptide consisting of 53 amino acids and has a potent mitogenic activity that stimulates proliferation of various normal and neoplastic cells through the interaction with its specific receptor(epidermal growth factor receptor, EGFR). Pleomorphic adenoma is the most common salivary benign tumor and histologically, it contains the epithelial cell, the myo-epithelial cell and mesenchymal ingredient, which is various aspect. Adenoid cystic carcinoma is an infiltrative malignant salivary gland tumor with three different histological patterns: cribriform, tubular or solid. The tumor cell structure composed of modified myoepithelial cell, and basaloid cell. In this study, we used an immunohistochemical technique to investigate the expression of EGF in 6 specimens of adenoid cystic carcinoma and 10 specimens of pleomorphic adenoma taken from patients treated at Dept. of Oral and Maxillofacial Surgery, Dankook University. The results were as follows. 1. In pleomorphic adenoma, ductal structure and scattered spindle cells in hyalinized stroma, disclosing myxoid stroma and hyalin, cartilage formation were observed. Immunohistologically, weak EGF expression in ductal structure and negative in stromal area were observed. 2. Cribriform type of adenoid cystic carcinoma showed numerous pseudocyst surrounded by dark small neoplastic cells in the back-ground of fibrous connective tissue and moderate EGF expression of dark cells adjacent to pseudo lumen in cribriform pattern, while weak expression in other most cells. 3. Tubular type of adenoid cystic carcinoma showed numerous ductal pattern surrounded by two layered neoplastic cells in the back-ground of fibrous connective tissue and strong EGF expression in luminal cells of ductal structure, while weak expression in outer cells. From the results obtained, we suggest that EGF is mainly biosynthesized in cells forming duct like structures of tubulo-ductal type or cribriform adenoid cystic carcinoma and it may play a role, as a cell mitogen in adenoid cystic carcinoma growth.

Polymorphous low-grade adenocarcinoma on hard palate: case report (구개부에 발생한 다형성 저등급 선암종의 치험례)

  • Polymorphous low-grade adenocarcinomas (PLGA) are distinctive salivary gland neoplasms with a propensity to arise from the minor salivary glands. The most frequent location of PLGA is the palate, even though other locations have been described. Previously used terms for PLGA include lobular carcinoma and terminal duct carcinoma. Although the frequency of the tumor is unknown, the recognition of PLGA as an individual tumor has increased with the establishment of specific histopathological criteria characterizing the PLGA. The first choice of treatment is a wide surgical excision including the subjacent bone if necessary. The prognosis is generally good and the recurrence rate ranges from 17% and 22%. Distant metastases is unusual (9%) but occur mainly in the regional lymph nodes. This is a case report of a 67 year old female patient with PLGA who was treated with a wide excision by layers (2 stage) of the lesion including the surrounding bone. We present this case with a review of the relevant literature.

A Case of Mucosa Associated Lymphoid Tissue Lymphoma in Accessory Parotid Gland (부이하선에 발생한 점막관련 림프조직 림프종 1예)

  • The tumor in accessory parotid gland (APG) is rarely occurred and its incidence is about 7.7% of all parotid gland neoplasms, but has a higher frequency of malignancy than major salivary glands. The mucoepidermoid carcinoma is the most common malignancy in APG, while B-cell lymphoma is less than 2%. It is often appeared as superficial mass in mid-cheek area. This lesion requires differential diagnosis with epidermoid cyst, lipoma, neurogenic tumors, Stensen's duct stone, lymphadenopathy and hemangioma etc. The mucosa associated lymphoid tissue (MALT) lymphoma, which is also termed extra-nodal marginal zone B-cell lymphoma tends to be localized disease for long time and has a relatively indolent course. We recently encountered a 69-year-old man with superficial solitary mass on the right cheek area that finally diagnosed as MALT lymphoma in APG. We report the rare and unique case with brief literature review.
