Analytical Evaluation of Residual Stresses in Dissimilar Metal Weld for Cast Stainless Steel Pipe and Low-Alloy Steel Component Nozzle (스테인리스주강 배관과 저합금강 기기노즐 이종금속용접부 잔류응력의 해석적 평가)

  • This paper is concerned with numerical analyses of residual stresses in welds and material's susceptibility to stress corrosion cracking (SCC) for the primary piping system in nuclear power plants: Both the dissimilar metal weld (DMW) for stainless steel to low alloy steel joints and the similar metal weld (SMW) for forged stainless steel to cast stainless steel joints are considered. Thermal elasto-plastic analyses using the finite element method (FEM) are performed to predict residual stresses generated in fabrication welding and its related processes for both the DMW and SMW, including effects of quenching for cast stainless steel piping, machining of the DMW root, and grinding of the SMW root. As a result, the effect of quenching should be included in the evaluation of residual stresses in the SMW for the cast stainless steel piping. It is deemed that residual stresses in both the DMW and SMW would not affect the SCC susceptibility of the welds providing that the welding processes are completed without any weld repair on the inside wall of the joint. However, the grinding process if performed on the safe-end to piping weld, would produce a high level of residual stresses in the inner surface region and thus a stress improvement process (e.g. buffing) should be considered to reduce susceptibilities to SCC.

  • Sens of insecutity of the public and professionals about harmful effects of radiation is increasing in an accident at the Chernobyl and Fkushima nuclear power plant.Anxiety was amplified to lack of information about radiation majority of people. To target the middle and high school in the region of Daegu and Gyeongsangbuk-do, to investigate the radiation recognition of the next generation, it is intended to present a model of education for the safe use of radiation. Fine needle aspiration biopsy for the diagnosis of fatty liver in cattle (Fine needle aspiration biopsy에 의한 소의 지방간진단(脂肪肝診斷))

  • This study was carried out to investigate the morbidity of fatty liver in cattle at the abattoir and on the farm, and to cytodiagnose fatty liver in cattle by fine needle aspiration biopsy. Incidence rates of fatty liver in cattle, detected macroscopically or based on hepatic lipid content by buoyancy, were 0.30% in Korean native cows, 4.70% in dairy cows, and 0.15% in dairy bull. Fatty liver was enlarged, swollen with round edges, light weight, and pale to yellow-orange color, but its color was not always correlated to the severity of fatty liver. The findings of fat infiltration of the hepatic lobule were large droplets around central vein, fine droplets in the periphery, and fat infiltration in the perivascular region execpt for most of normal liver and severe fatty liver. The sensitivty, specificity, and accuracy of cytological finding compared with hepatic lipid content by buoyancy were 94.4%, 95.2%, and 94.9% in normal cases, 64.3%, 100%, and 87.2% in mild cases, 100%, 83.3%, and 87.2% in moderate cases, and 100%, 100%, and 100% in sesvere cases, respectively. Cytological findings were well correlated with histological findings. Complications of fine needle aspiration biopsy were not recognized clinically. Consequently, the cytodiagnosis by fine needle aspiration biopsy is simple, rapid, safe, and economical method compared with histological techniques in the diagnosis of fatty liver in cattle.

  • This study examined the effects of socket flexion angle in trans-tibial prosthesis on stump/socket interface pressure. Ten trans-tibial amputees voluntarily participated in this study. F-socket system was used to measure static and dynamic pressure in stump/socket interface. The pressure was measured at anterior area (proximal, middle, and distal) and posterior area (proximal, middle, and distal) in different socket flexion angles ($5^{\circ}$, $0^{\circ}$, and $10^{\circ}$). Paired t-test was used to compare pressure differences in conventional socket flexion angle of $5^{\circ}$ with pressures in socket flexion angles of $0^{\circ}$ and $10^{\circ}$ (${\alpha}$=.05). Mean pressure during standing in socket flexion angle of $10^{\circ}$ decreased significantly in anterior middle area (19.7%), posterior proximal area (10.4%), and posterior distal area (16.3%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (19.3%) and decreased significantly in anterior distal area (19.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (19.6%) and increased significantly in anterior distal area (8.2%) compared with socket flexion angle of $5^{\circ}$. Peak pressure during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.0%) compared with socket flexion angle of $5^{\circ}$ and peak pressure during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (22.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure over 80% of peak pressure ($MP_{80+}$) during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.9%) and decreased significantly in anterior distal area (22.5%) compared with socket flexion angle of $5^{\circ}$. $MP_{80+}$ during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior distal area (34.1%) compared with socket flexion angle of $5^{\circ}$. Asymmetrical pressure change patterns in socket flexion angle of $0^{\circ}$ and $10^{\circ}$ were revealed in anterior proximal and distal region compared with socket flexion angle of $5^{\circ}$. To provide comfortable and safe socket for trans-tibial amputee, socket flexion angle must be considered.

  • Finite element analyses were performed to study effects on stress distribution generated in jaw bone for various shapes of dental implants: plateau type, plateau with small radius of curvature, triangular thread screw type in accordance with ISO regulations and square thread screw filleted with small radius partially. It was found that square thread screw filleted with small radius was more effective on stress distribution than other dental implants used in analyses. Additional analyses were performed on the implant with square thread screw filleted with small radius for very-ing design parameters, such as the width of thread end, the height of the thread of the implant and load direction, to determine the optimum dimensions of the implant. The highest stress concentration occurred at the region in jaw Pone adjacent to the first thread of the implant. The maximum effective stress induced by a 15 degree oblique load of 100 N was twice as high as the maximum effective stress caused by an equal amount of vertical load. Stress distribution was more effective in the case when the width of thread end and the height of thread were p/2 and 0.46p, respectively, where p is the pitch of thread. At last, using tensile force calculated from the possible insert torque without breading bone thread, finite element analysis was performed on the implant to calculate pre-stress when the primary fixation of the implant was operated in jaw bone. The maximum effective stress was 136.8 MPa which was proven to be safe.

THE USE OF BISPECTRAL ANALYSIS AND HEMODYNAMIC MONITORING IN PATIENTS UNDERGOING INTRAVENOUS SEDATION FOR MINOR ORAL SURGERY (구강악안면외과 소수술시 정맥 의식하 진정법에서의 Bispectral Index and Hemodynamics monitoring을 이용한 진정 깊이에 관한 연구)

  • Purpose: The purpose was to assess the usefulness of midazolam in patients undergoing minor oral surgery under conscious sedation. Materials and methods: Bispectral index was examined in 20 patients receiving oral minor surgery with conscious sedation supplemented with local anesthesia. All patients included were ASA I and had no contraindications to the study medications. The patients were escorted to the day surgery operation room where, before the commencement of the sedation and surgical procedures, routine monitoring was applied, including the noninvasive monitoring of arterial blood pressure, arterial oxygen saturation, and 3-lead electrocardiogram (Electrocardiogram). Bispectral index electrodes were applied on the frontotemporal region after cleansing the skin with alcohol. Bispectral index was calculated with the Electrocardiogram monitor (A-2000; Aspect Co.). Midazolam was then titrated (initially 3mg wait 2min and 2mg). Vital sign and Bispectral index checked every 5 minute until the end of the procedure. The results were then compared. Results: The Bispectral index index values throughout the sedation study period alter many level. The index was dropped at 5 minutes after administration, but raised at injection and odontomy procedure. During the operation, mean Bispectral index index was higher than conscious sedation index range($60{\sim}80$). The amnesic effect was shown 17 cases out of 20 cases(85%). Conclusion: Conscious sedation technique using midazolma is a safe and effective method of controlling behavior in oral and maxillofacial surgery.

Detection and Distribution of Bacterial Pathogens in Raw Water and During Water Treatment Process by Polymerase Chain Reaction (유전자 검색법을 이용한 상수원수와 정수처리 공정중의 병원성 미생물 검출 및 분포특성)

  • The development of polymerase chain reaction (PCR) technology has the potential to solve for isolating pathogenic microorganisms from environmental samples than traditional plate counting methods. We have been detected pathogenic bacteria from raw water and water treatment process in Busan metropolitan city by PCR method. According to the result of survey from July 2004 to October 2005, 80 out of 92(87.0%) were positive for bacterial pathogens in raw water samples and positive rate of Shigella spp., Yersinia spp., Salmonella spp. and Legionella spp. were 46.2%, 40.7%, 17.6% and 9.9%, respectively. Pathogenic bacteria in raw water was mainly distributed through the lately Autumn to the winter and more highly detected Maeri than Mulgum region. During the period of survey in water treatment process, Shigella spp. was highly detected but all of bacterial pathogens were entirely removed after in post-ozonation step. These suggest that waters supplied in Busan metropolitan city may be safe against the pathogenic bacteria.

  • A drug transport carrier could be used for safe send of drugs to the affected region in a human body. The chitosan is adequate for the drug delivery carrier because of adaptable to living body. The gold, a metallic nanoparticles, tends to form a nano complex at rapidly when it combined with chitosan because of its negative charge. having energy from the other, outer gold nano-complex make heat due to its property to release the contained drugs to the target area. Silver could be also formed an useful biocompatible nano-composites with chitosan which should be used as an useful drug transfer carrier because its special ability to protect microbial contamination. Being one of the oxidized nano metals, $Fe_3O_4$ is nontoxic and has been used for its magnetic characteristics. In this study, the control of catalyst, reducing agent, and solvent amount. The chitosan-$Fe_3O_4$-gold & silver nanoshell have been changed to form about 100 nm size by ionic bond between the amine group, an end group of chitosan, and the metal. It was observed the change in order to seek for its optimum reaction condition as a drug transfer carrier.

Gluteal Perforator Flaps for Coverage of Sacral Pressure Sores (둔부 천공지피판을 이용한 천골부 욕창의 재건)

  • Purpose: Gluteal perforator is easily identified in the gluteal region and gluteal perforator flap is a very versatile flap in sacral sore reconstruction. We obtained satisfying results using the gluteal perforator flap, so we report this clinical experiences with a review of the literature. Methods: Between November of 2003 and April 2006, the authors used 16 gluteal perforator flaps in 16 consecutive patients for coverage of sacral pressure sores. The mean age of the patients was 47.4 years (range, 14 to 78 years), and there were 9 male and 7 female patients. All flaps in the series were supplied by musculocutaneous arteries and its venae comitantes penetrating the gluteus maximus muscle and reaching the intrafascial and suprafascial planes, and the overlying skin forming a rich vascular plexus arising from gluteal muscles. Patients were followed up for a mean period of 11.5 months. Results: All flaps survived except one that had undergone total necrosis by patient's negligence. Wound dehiscence was observed in three patients and treated by secondary closure. There was no recurrence during the follow-up period. Conclusion: Gluteal perforator flaps allow safe and reliable options for coverage of sacral pressure sores with minimal donor site morbidity, and do not sacrifice the gluteus maximus muscle and rarely lead to post-operative complications. Freedom in flap design and easy-to perform make gluteal perforator flap an excellent choice for selected patients.

Bone-Preserving Decompression Procedures Have a Minor Effect on the Flexibility of the Lumbar Spine

  • Objective : To mitigate the risk of iatrogenic instability, new posterior decompression techniques able to preserve musculoskeletal structures have been introduced but never extensively investigated from a biomechanical point of view. This study was aimed to investigate the impact on spinal flexibility caused by a unilateral laminotomy for bilateral decompression, in comparison to the intact condition and a laminectomy with preservation of a bony bridge at the vertebral arch. Secondary aims were to investigate the biomechanical effects of two-level decompression and the quantification of the restoration of stability after posterior fixation. Methods : A universal spine tester was used to measure the flexibility of six L2-L5 human spine specimens in intact conditions and after decompression and fixation surgeries. An incremental damage protocol was applied : 1) unilateral laminotomy for bilateral decompression at L3-L4; 2) on three specimens, the unilateral laminotomy was extended to L4-L5; 3) laminectomy with preservation of a bony bridge at the vertebral arch (at L3-L4 in the first three specimens and at L4-L5 in the rest); and 4) pedicle screw fixation at the involved levels. Results : Unilateral laminotomy for bilateral decompression had a minor influence on the lumbar flexibility. In flexion-extension, the median range of motion increased by 8%. The bone-preserving laminectomy did not cause major changes in spinal flexibility. Two-level decompression approximately induced a twofold destabilization compared to the single-level treatment, with greater effect on the lower level. Posterior fixation reduced the flexibility to values lower than in the intact conditions in all cases. Conclusion : In vitro testing of human lumbar specimens revealed that unilateral laminotomy for bilateral decompression and bone-preserving laminectomy induced a minor destabilization at the operated level. In absence of other pathological factors (e.g., clinical instability, spondylolisthesis), both techniques appear to be safe from a biomechanical point of view.
