  • 국내외 건설현장에서 사용빈도가 급격히 증가하고 있는 슬래브용 알루미늄 폼은 탈형시 바닥으로 그대로 떨어뜨리는 경우가 많아 소음으로 인한 주변 민원이 발생하고, 높은 위치에서 떨어지면서 형틀이 변형되거나 바닥에서 튀어 오로면서 주변 인부들에게 상해를 입히기도 하여 중대재해처벌법이 강화되는 현실에서 개선이 절실한 상황이다. 이에 본 연구에서는 공동주택의 1세대 슬래브 거푸집 전체가 동시에 작업자가 펼친 손이 닿는 위치까지 안전하게 낙하되면 자유롭게 분해하여 다음 층으로 인양할 수 있게 함으로써 안전성, 품질, 경제성, 공정이 개선된 필러서포터 존치형 공법으로써 슬래브의 처짐문제 해결 등의 장점을 확인되고 건설신기술을 획득하였으므로 적극적인 시장개척을 통한 적용 활성화가 가능할 것으로 판단된다.

Ten technical aspects of baseplate fixation in reverse total shoulder arthroplasty for patients without glenoid bone loss: a systematic review

  • The aim of this systematic review was to collect evidence on the following 10 technical aspects of glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA): screw insertion angles; screw orientation; screw quantity; screw length; screw type; baseplate tilt; baseplate position; baseplate version and rotation; baseplate design; and anatomical safe zones. Five literature libraries were searched for eligible clinical, cadaver, biomechanical, virtual planning, and finite element analysis studies. Studies including patients >16 years old in which at least one of the ten abovementioned technical aspects was assessed were suitable for analysis. We excluded studies of patients with: glenoid bone loss; bony increased offset-reversed shoulder arthroplasty; rTSA with bone grafts; and augmented baseplates. Quality assessment was performed for each included study. Sixty-two studies were included, of which 41 were experimental studies (13 cadaver, 10 virtual planning, 11 biomechanical, and 7 finite element studies) and 21 were clinical studies (12 retrospective cohorts and 9 case-control studies). Overall, the quality of included studies was moderate or high. The majority of studies agreed upon the use of a divergent screw fixation pattern, fixation with four screws (to reduce micromotions), and inferior positioning in neutral or anteversion. A general consensus was not reached on the other technical aspects. Most surgical aspects of baseplate fixation can be decided without affecting fixation strength. There is not a single strategy that provides the best outcome. Therefore, guidelines should cover multiple surgical options that can achieve adequate baseplate fixation.

Numerical study on the influence of embedment footing and vertical load on lateral load sharing in piled raft foundations

  • Piled raft foundation has become widely used in the recent years because it can increase bearing capacity of foundation with control settlement. The design for a piled raft in terms vertical load and lateral load need to understands contribution load behavior to raft and pile in piled raft foundation system. The load-bearing behavior of the piled raft, especially concerning lateral loads, is highly complex and challenge to analyze. The complex mechanism of piled rafts can be clarified by using three dimensional (3-D) Finite Element Method (FEM). Therefore, this paper focuses on free-standing head pile group, on-ground piled raft, and embedded raft for the piled raft foundation systems. The lateral resistant of piled raft foundation was investigated in terms of relationship between vertical load, lateral load and displacement, as well as the lateral load sharing of the raft. The results show that both vertical load and raft position significantly impact the lateral load capacity of the piled raft, especially when the vertical load increases and the raft embeds into the soil. On the same condition of vertical settlement and lateral displacement, piled raft experiences a substantial demonstrates a higher capacity for lateral load sharing compared to the on-ground raft. Ultimately, regarding design considerations, the piled raft can reliably support lateral loads while exhibiting behavior within the elastic range, in which it is safe to use.

Clinical Midterm Results of Surgical Aortic Valve Replacement with Sutureless Valves

  • Background: Sutureless aortic valves may enable shorter procedure times, which benefits patients with elevated surgical risk. We describe the outcomes of patients with aortic stenosis who underwent aortic valve replacement (AVR) using the sutureless Perceval aortic bioprosthesis. Methods: Data from a retrospective cohort were obtained from a clinical database. The study enrolled patients with symptomatic severe aortic stenosis who underwent surgical AVR with a sutureless bioprosthesis between August 2015 and December 2020. In total, 113 patients were included (mean age, 75.3±8.4 years; 57.5% women; median Society of Thoracic Surgeons score, 9.7%; mean follow-up period, 51.19±20.6 months). Of these patients, 41 were octogenarians (36.2%) and 3 were nonagenarians (2.6%). Transthoracic echocardiography was employed to assess changes in ejection fraction (EF), left ventricular mass index (LVMI), and mean pressure gradient (MPG). Results: The in-hospital mortality rate was 2.6%, and 13 patients developed new-onset atrial fibrillation. A permanent pacemaker was implanted in 3 patients (2.6%). The median intensive care unit stay was 1 day (interquartile range [IQR], 1-2 days), and the median hospital stay was 12 days (IQR, 9.5-15 days). The overall survival rate at 5 years was 95.9%. LVMI and MPG were reduced postoperatively, while EF increased over the follow-up period. No structural valve deterioration was observed, and no meaningful paravalvular leakage developed during follow-up. Conclusion: The use of a sutureless valve in the aortic position is safe and feasible, even for high-risk elderly patients requiring surgical AVR. LVMI and MPG decreased postoperatively, while EF increased over the follow-up period.

Robotic-assisted Total Hip Arthroplasty and Spinopelvic Parameters: A Review

  • Total hip arthroplasty (THA) is an effective treatment for osteoarthritis, and the popularity of the direct anterior approach has increased due to more rapid recovery and increased stability. Instability, commonly caused by component malposition, remains a significant concern. The dynamic relationship between the pelvis and lumbar spine, deemed spinopelvic motion, is considered an important factor in stability. Various parameters are used in evaluating spinopelvic motion. Understanding spinopelvic motion is critical, and executing a precise plan for positioning the implant can be difficult with manual instrumentation. Robotic and/or navigation systems have been developed in the effort to enhance THA outcomes and for implementing spinopelvic parameters. These systems can be classified into three categories: X-ray/fluoroscopy-based, imageless, and computed tomography (CT)-based. Each system has advantages and limitations. When using CT-based systems, preoperative CT scans are used to assist with preoperative planning and intraoperative execution, providing feedback on implant position and restoration of hip biomechanics within a functional safe zone developed according to each patient's specific spinopelvic parameters. Several studies have demonstrated the accuracy and reproducibility of robotic systems with regard to implant positioning and leg length discrepancy. Some studies have reported better radiographic and clinical outcomes with use of robotic-assisted THA. However, clinical outcomes comparable to those for manual THA have also been reported. Robotic systems offer advantages in terms of accuracy, precision, and potentially reduced rates of dislocation. Additional research, including conduct of randomized controlled trials, will be required in order to evaluate the long-term outcomes and cost-effectiveness of robotic-assisted THA.

선박 거주구역의 높이가 피난안전에 미치는 영향에 대한 연구 (A Study on the Effects of an Increase in the Height of Ship's Accommodation Area on Safe Evacuation in Emergency Situation)

  • 선박화재는 육상화재와 달리 숙련된 인원과 다양한 장비에 의한 소화활동이 곤란하므로 거의 자체적으로 소화되어야 하므로 소화가 쉽지 않다. 화재발생시 온도 상승에 의한 사망보다 연기에 의해 질식사의 경우가 더 많다. 그 이유는 화재현장에서 충분한 가시거리를 확보하지 못하여 신속하게 피난하지 못하기 때문이다. 이 연구에서는 피난에 필요한 시간을 좀 더 확보하기 위해 선박 거주구역의 높이를 기존의 2.0m에서 공동주택(아파트)의 높이에 해당하는 2.3m로 상향하여 연기거동을 상호 비교하였다. 비교 방법은 기존의 실습선 한바다호의 도면을 바탕으로 30cm 상향조정된 도면을 추가 제작하여 미국의 NIST에서 제작 운용중인 FDS를 이용하여 시뮬레이션을 실시하고 결과를 예측하였다. 온도에 의한 피난 안전시간을 예측한 시뮬레이션 결과, 화재구역에서 10m 떨어진 지점에서 피난 안전시간은 55.8초 증가하였다. 가시거리에 의한 피난 안전시간을 예측한 시뮬레이션 결과, (1) 화재구역에서 10m 떨어진 지점에서 피난 안전시간은 27.1초 증가하였고, (2) 화재구역에서 20m 지점에서는 피난 안전시간이 109.2초 증가하였으며, (3) 화재구역에서 30m지점에서는 피난 안전시간이 73.3초 증가하였다. 즉, 선박의 거주구역 높이를 육상건축물과 동일하게 할 경우 승무원의 피난안전성이 증가하는 것으로 예측되었다.

일반 수술 침대와 짧은 대퇴 주대를 이용한 인공 고관절 전 치환술의 직접 전방 도달법과 변형된 전 측방 도달법에 따른 결과 비교 (Comparison of the Outcomes after Primary Total Hip Arthroplasty Using a Short Stem between the Modified Anterolateral Approach and Direct Anterior Approach with a Standard Operation Table)

  • 목적: 일반 수술용 침대와 길이가 짧은 대퇴 주대(short stem)를 이용하여 인공 고관절 전 치환술을 직접 전방 도달법(direct anterior approach, DAA)을 이용하여 시행하고, 임상적 영상의학적 결과를 변형된 전 측방 도달법(modified Hardinge; anterolateral approach, ALA)을 시행한 경우와 비교하여 알아보고자 하였다. 대상 및 방법: 2013년 1월부터 2015년 11월까지 단일 술자가 DAA로 인공 고관절 전 치환술을 시행한(DAA group) 102명(102예)과 동 수의 ALA를 사용한 환자(ALA group)를 후향적으로 비교 분석하였다. 수술시간과 출혈량을 비교하였고, 수술 후 통증의 개선 정도와 보행 능력 및 고관절의 기능 회복을 알아보았다. 영상의학적으로 비구 컵과 대퇴 주대의 삽입 위치를 평가하였고, 두 군 간에 발생한 합병증을 알아보았다. 결과: 출혈량은 DAA group에서 유의하게 적었다(p=0.018). 수술 후 3주까지는 하지 근력의 회복이 DAA group에서 유의하게 높았다(굴곡/신전력 p=0.023, 외전력 p=0.031). Harris hip score를 이용한 기능 평가는 3개월까지(p≤0.001), Koval score를 이용한 보행 능력 평가는 6주까지(p≤0.001) DAA group에서 유의하게 나은 결과를 보였고, visual analogue scale score를 이용한 통증의 개선은 7일까지는 DAA group에서 유의하게 높았다(p=0.035). 비구 컵의 경사각(p≤0.001)과 전염각(p≤0.001)은 DAA group에서 ALA group보다 더 안정 범위에 위치하였고, 대퇴 주대의 위치와 하지 길이는 통계적 유의한 차이를 보이지 않았다. DAA group에서 수술 중 2예의 대전자 골절이 발생하였다(p=0.155). 결론: 길이가 짧은 대퇴 주대를 이용하여 일반 수술 침대에서 시행하는 DAA는 수술 후 조기 기능 회복을 보이며, 술자에게 친숙한 해부학적 자세로 수술 중 영상 증폭기 사용이 간편하여 원하는 위치에 인공 관절물을 삽입하는 것과 하지 부동 방지에 유용한 도달법으로 생각된다.

SERVQUAL 모델을 이용한 간호 서비스 질 측정 (Measurement of Nursing Service Quality using SERVQUAL Model)

  • This study is a descriptive analytic research measuring nursing service quality, using SERVQUAL model, to make fundamental data and strategies for nursing service improvement. Data were collected by self-reported questionnaire from 202 patients and 142 nurses, from June 7 to 14, 1999. The reliability of instrument were adequate(Cronbach ${\alpha}=.94$). SAS program was utilized for statistical analysis of collected data. The results were as follows; 1. There was a gab between patient's expectation and perception on nursing service(Gap B). Gap D was indicated an affecting factor to decide nursing service quality. Gap C was indicated an indirect affecting factor of nursing service quality. Because it was not statistically significant in total item analysis, but in individual item analysis, 7 items were appeared statistically significant. Gap A was not a gap occurrence factor of nursing service quality. 2. Focuses of nursing service quality improvement strategies were; (1) to direct qualitative improvement of nursing service in order to correspondence patient's nursing service expectation. (2) to make nurse's service activity modified because nurse's practice were not reached patient's expectation level. (3) to need internal, external factor analysis affecting nurse's service activity. 3. Nursing service quality was decided by rather environmental inappropriateness provided nursing service than itself. Therefore, to make nursing service quality improvement, it is required to improve nursing service environment. For this, followings are required; (1) to strengthen nurse's education on lower part of nursing service satisfaction and QI activities. (2) to balance demand and supply of nursing personnel. 3) to fix computerized system for reducing other duties weight except nursing care through analysis of nursing activity. (4) to construct rational cooperating system among related departments. 4. The important parts for nursing service quality improvement were indicated as follows: (1) Gap B: 'prompt reaction', 'examination symptom before patient's complaint', 'hearted nursing service reducing patient's dissatisfaction', 'explanation goals of nursing activities', 'having special Knowledge enough', 'maintenance position comfortably', 'management of patient's physical hygiene'. (2) Gap C: 'maintenance physical safety', 'explanation about hospital rules and facilities'. (3) Gap D: 'tender, safe injection and wound care'. Because above items are mostly improved through nurse's attitude change and quality improvement, it is required to establish nursing standardization and to strengthen nurse's clinical education. As the based on above results, followings are suggested; 1. SERVQUAL model is very useful to make strategies for nursing service quality improvement because it indicates multiple factors affecting hap occurrence. 2. At individual items analysis of Gap C, statistically significant 7 items appeared higher nurse's perception level than patient's perception level on nursing service were trouble perception level on nursing service quality improvement. So. it need further research to analysis about these difference occurring factors. 3. At analysis of Gap D, it is indicated that in nursing service performance process, multiple factors lowing nursing service quality were intruded. So it needs further research to analysis what these factors are and how each factors affect on nursing performance process. 4. nursing service quality measurement is changeable according to sample select time or sampled subject's characteristics. So to develope strategy for nursing service quality improvement is based on the results of periodical analysis.

초음파 영상을 이용한 고강도 집중 초음파 빔 시각화 (High-intensity focused ultrasound beam path visualization using ultrasound imaging)

  • 고강도 집중 초음파(High-Intensity Focused Ultrasound, HIFU) 치료에서 HIFU 초점의 효과적인 위치 파악은 안전한 치료 계획을 개발하는 데 중요하다. 자기 공명 영상 유도 HIFU(Magnetic Resonance Imaging guided HIFU, MRIgHIFU)는 HIFU 초점을 영상화하여 치료 중에 초음파 경로를 시각화 할 수 있지만 초음파 이미징 유도 HIFU(Ultrasound imaging guided HIFU, USIgHIFU)에서는 어려움이 있다. 본 연구에서는 USIgHIFU에 대해 HIFU 초점을 영상화할 수 있는 실시간 초음파 빔 시각화 기법을 제시 하였다. 제안 된 방법에서, 음향 강도(Ispta < 720 mW/㎠) 아래의 이미징 초음파 변환자의 동일한 중심 주파수를 갖는 짧은 펄스가 HIFU 변환기를 통해 전송되고, HIFU 빔 경로를 시각화하기 위해 수신 신호는 동적 수신 포커싱 및 후속 에코 처리를 거쳤다. 소 혈청 알부민 젤 팬텀을 이용한 생체외 실험으로부터, HIFU 빔 경로는 낮은 음향 강도 (Ispta = 94.8 mW/㎠)에서도 명확히 영상화 할 수 있었고 HIFU 초점은 손상이 생성되기 전에 성공적으로 시각화하였다. 이 결과는 제안 된 초음파 빔 경로 시각화 방법이 USIgHIFU 치료에서 원치 않는 조직 손상을 최소화하면서 실시간으로 HIFU 초점을 영상화하는 데 사용될 수 있음을 나타낸다.

GEANT4를 이용한 치료용 I-131 캡슐의 안정성 시뮬레이션 (Safety Simulation of Therapeutic I-131 Capsule Using GEANT4)

  • 국내 갑상선암 환자의 발병률이 증가함에 따라 방사성요오드 치료가 필요한 환자가 증가하고 있다. 방사성 요오드 치료에 이용되고 있는 캡슐용기들 중 1.1 GBq과 5.5 GBq의 국산제품에 설계도면을 바탕으로 차폐체를 구현한 후 366 keV 이상에 감마선들에 대하여 캡슐용기의 방사선 누설선량을 GEANT4 전사모사를 이용하여 평가하였다. 치료용 I-131 캡슐용기 누설선량을 측정하기 위해 각각의 용기에 대하여 용기 표면에서 10 cm 거리 및 100 cm 거리에서의 누설선량을 측정하였다. 용기표면에서 10 cm 거리와 100 cm 거리에서의 누설선량 측정은 방사형으로 발생되는 방사선을 위치별로 측정하기 위해 $10{\times}10{\times}10cm^3$ 부피의 정육면체 형태의 물 팬텀(phantom)을 상부, 상부측면, 측면, 하부측면, 하부 다섯 방향에 설치하여 누설선량을 계산하였다. 용기별로 5개 방향에서 용기표면으로 부터 10 cm, 100 cm 거리에서 전산모사를 수행한 결과 법적 허용기준인 10 cm 거리에서 2.0 mSv/h, 100 cm 거리에서 0.02 mSv/h 이하의 선량 규정과 비교하였을 때 법적기준치보다 현저히 낮은 누설선량이 방출되는 것을 확인하였다.
