학습성과를 기반으로 한 기초간호학 교과목의 강의계획서 개발: 인체 구조와 기능, 병원미생물학, 병태생리학, 약물의 기전과 효과 (Development of Syllabuses for Biological Nursing Science Subjects based on Learning Outcomes: Structure and Function of Human Body, Pathogenic Microbiology, Pathophysiology, and Mechanisms and Effects of Drugs)

  • Purpose: The purpose of this study was to develop syllabuses of basic nursing science subjects (Structure and Function of Human Body, Pathogenic Microbiology, Pathophysiology, and Mechanisms and Effects of Drug) based on learning outcomes. Methods: We developed a 3-phase plan to develop the syllabuses. In the first phase, The Korean Society of Biological Nursing Science (KSBNS) held a workshop in May, 2013 in which professors who are in charge of basic nursing science subjects shared opinions about learning outcomes. As a result, initial prototype syllabuses came out. In the second phase, revised syllabuses based on learning outcomes were presented and discussed in the conference held by KSBNS in November, 2013. In the last phase, a research team who taught basic nursing science subjects finalized the syllabuses during the meetings. Results: Syllabuses of 4 basic nursing science subjects were developed. Conclusion: These syllabuses of 4 subjects need to be disseminated throughout nursing colleges. Further revision needs to be made according to the circumstances and context of each school.

뇌졸중 환자의 식이지식 및 식이교육 요구도 관련 요인 (The Factors Associated with Dietary Knowledge and Educational Needs of Stroke Patients)

  • Purpose: The purpose of the this study was to examine the dietary knowledge and educational needs of stroke patients and analyze the related factors that are associated with dietary knowledge and educational needs. Methods: Our study analyzed data from 146 stroke patients. The survey was conducted by individual interviews using questionnaires and data collected from participants' medical records. Dietary knowledge and educational needs were measured by tools developed by the author. Results: The mean scores for the dietary knowledge and educational needs were 19.09±3.48 and 36.17±7.18, respectively. Many stroke patients had misconceptions about soy sauce, cholesterol, and fruits. The most needed items in dietary education was first identifying foods that are beneficial or harmful, and second, food interaction with medications. As for the results of multiple regressions, dietary knowledge was significantly associated with gender, educational attainment, monthly income, exercise, body mass index, and level of compliance to dietary therapy. Additionally, the educational needs were significantly related with educational attainment and employment. Conclusion: For secondary prevention of stroke patients, education strategy considering dietary knowledge and educational needs of stroke patients should be established. Special attention is needed for stroke patients with a lower level of dietary knowledge and educational needs.

교대근무 간호사의 월경 전 증상 영향 요인 2차자료 분석: 수면, 직무 스트레스를 중심으로 (Secondary Data Analysis on the Factors Influencing Premenstrual Symptoms of Shift Work Nurses: Focused on the Sleep and Occupational Stress)

  • Purpose: This study aimed to examine premenstrual symptoms (PMS) of shift nurses and identify the association between PMS, sleep, and occupational stress. Methods: This study was conducted with a secondary data analysis that used data from the Shift Work Nurse's Health and Turnover study. The participants were 258 nurses who were working in shifts including night shifts. PMS, sleep patterns (sleep time and sleep time variability), sleep quality, and the occupational stress of each participant were measured using the Moos Menstrual Distress Questionnaire, a sleep diary, an actigraph, the Insomnia Severity Index, and the Korean Occupational Stress Scale, respectively. Data were analyzed using SPSS 23 and STATA 15.1 to obtain descriptive statistics, Pearson's correlation coefficients, multiple linear regression with generalized estimating equations (GEE) and Baron and Kenny's mediating analysis. Results: The average PMS score, average sleep time, average sleep time variability, average sleep quality score, and average occupational stress score of the participants was 53.95 ± 40.45, 7.52 ± 0.89 hours, 32.84 ± 8.43%, 12.34 ± 5.95, and 49.89 ± 8.98, respectively. A multiple linear regression analysis with GEE indicated that sleep time variability (B = 0.86, p = .001), and sleep quality (B = 2.36, p < .001) had negative effects on nurses' PMS. We also found that sleep quality had a complete mediating effect in the relationship between occupational stress and PMS. Conclusion: These findings indicate that both sleep time variability and sleep quality are important factors associated with PMS among shift work nurses. To improve shift nurses' PMS status, strategies are urgently needed to decrease sleep time variability and increase sleep quality.

타이어 고무 애쉬 치환에 따른 플라이애쉬 혼입 콘크리트의 내구성능 성능 평가 (Evaluation of Durability Performance of Fly Ash Blended Concrete due to Fly Ash Replacement with Tire Derived Fuel Ash)

  • 본 연구에서는 FA (Fly Ash)를 20%치환한 배합에 대하여 TDFA (Tire Derived Fuel Ash)를 3.0~12.0%까지 중량 치환하면서 내구성 평가를 수행하였다. TDFA는 열병합발전소에서 열효율을 높이기 위해 폐타이어를 혼소시킨 뒤 발생한 산업부산물로서 국내에서 콘크리트에 적용한 연구는 없는 상태이다. 이를 위해 물-결합재를 50%, FA를 20% 치환한 Control 콘크리트를 제조하였으며, TDFA를 치환하면서 압축강도, 촉진 탄산화 시험, 촉진염해 시험, 공극구조평가를 수행하였다. 압축강도, 탄산화, 공극구조에서는 12%까지 TDFA를 FA와 치환해도 동등이상의 성능을 확보하였다. 특히 염해에 대해서는 TDFA의 치환률의 증가에 따라 뚜렷한 염화물 확산계수의 감소를 나타내어 최종적으로 75.3~70.9%까지 염화물 확산계수가 감소하였다. TDFA를 혼입한 콘크리트 배합시, 워커빌리티의 확보가 가능하다면 TDFA를 혼입한 콘크리트는 내구성 개선에 효과적일 것으로 판단된다.

내과 중환자실 환자의 혈당조절 프로토콜 적용 및 효과 (Application and Effects of a Blood Glucose Control Protocol for Medical Intensive Care Unit Patients)

  • Purpose: The purpose of this study was to examine the effect of a blood glucose control protocol for medical intensive care unit (ICU) patients. Methods: The subjects were recruited from medical ICU adult patients whose blood glucose levels exceeded 200 mg/dL in two consecutive tests. The experimental group (n=62) received the modified Yale (MY) insulin protocol, whereas the control group (n=64) was treated with the conventional insulin therapy methods. Results: In the experimental group, the mean blood glucose levels (p<.001) and the time to reach the target range of glucose (p<.001) decreased significantly while the incidence rates of a target range of glucose of 100-140 mg/dL (p<.001) increased significantly as compared to the control group. However, no statistically significant differences were found in the incidence of hypoglycemia(p=.644), or the number of glucose tests (p=.236) between the groups. The length of stays in the ICU (p=.001), ventilator care days (p=.038), and the Sequential Organ Failure Assessment (SOFA) score (p=.029) in the experimental group were significantly lower than those of the control group. Conclusion: Application of the protocol was effective in improving the state of blood glucose control in medical ICU patients. Therefore, this protocol is expected to be used as a part of nursing intervention in critical care nursing.

바이오휘드백을 이용한 복식호흡훈련이 유방암절제술 환자의 스트레스, 면역반응 및 삶의 질에 미치는 효과 (Effects of Abdominal Breathing Training Using Biofeedback on Stress, Immune Response and Quality of Life in Patients with a Mastectomy for Breast Cancer)

  • Purpose: This study was to determine the effects of abdominal breathing training using biofeedback on stress, immune response, and quality of life. Method: The study design was a nonequivalent control group pretest-posttest, quasi-experimental design. Twenty-five breast cancer patients who had completed adjuvant chemotherapy were enrolled. The experimental group(n=12) was provided with abdominal breathing training using biofeedback once a week for 4 weeks. State anxiety, cancer physical symptoms, serum cortisol, T cell subsets(T3, T4, T8), NK cell and quality of life were measured both before and after the intervention. Result: Though state anxiety, cancer physical symptoms, and serum cortisol were reduced after 4 weeks of abdominal breathing training using biofeedback, there was no statistical significance. It showed, however, improvement in quality of life (p=.02), and T3(p=.04). Conclusion: Abdominal breathing training using biofeedback improves quality of life in breast cancer patients after a mastectomy. However, the mechanism of this beneficial effect and stress response requires further investigation with special consideration in subject selection and frequency of measurement. Nurses should consider this strategy as a standard nursing intervention for people living with cancer.

부인암 환자의 항암화학요법으로 인한 오심과 구토, 비약물적 대처방법과 영양상태간의 관계 (The Relationships among Chemotherapy-Induced Nausea and Vomiting (CINV), Non-Pharmacological Coping Methods, and Nutritional Status in Patients with Gynecologic Cancer)

  • Purpose: Chemotherapy-induced nausea and vomiting (CINV) can cause severe malnutrition. However, relationships between CINV levels, non-pharmacological coping methods, and nutritional status of female cancer patients have rarely been investigated. Therefore, this study aimed to analyze their relationships in gynecologic cancer patients. Methods: Participants receiving a highly and moderately emetogenic chemotherapy were recruited. The level of CINV was assessed using a numeric rating scale. Coping methods were determined using multiple-choice self-report questionnaires and categorized into seven types for statistical analysis. Nutritional status was evaluated using biochemical and anthropometric parameters. Results: Among all the 485 patients, 200 eligible inpatients were included. Despite the administration of prophylactic antiemetics, 157 patients (78.5%) still experienced CINV, and several used nonmedically recommended coping methods, such as just enduring the symptom or rejecting food intake. A total of 181 patients (90.5%) had nutritional disorders. Although the level of CINV was indirectly related to the occurrence of nutritional disorders, patients who rejected food (${\beta}=1.57$, p=.023) and did not use physical measures (${\beta}=-1.23$, p=.041) as coping methods were under the high risk of nutritional disorders. Conclusion: Korean gynecologic cancer patients had high levels of CINV and were at high risk of nutritional disorders, which may be related to the use of nonscientific coping methods, possibly due to cultural backgrounds and lack of proper nutritional program. Therefore, developing a culturally appropriate educational program for the cancer patients with CINV is urgently needed.

연소 · 고령 노인의 성별에 따른 건강 및 영양상태 비교 조사 연구 (Nutritional Status, Nutrients Intakes, and Health Status of Young-old and Old-old Homebound Elderly in Korea)

  • Purpose: Number of the old-old elderly is rapidly increasing in Korea, but it is unclear whether there are differences between old-old and young-old elderly in nutritional status, nutrient intakes and health status. The gender differences in Korean elderly in these conditions also remains unknown. This study, therefore, investigated gender-associated differences in nutritional, health status and nutrient intakes and how they are related among young-old and old-old homebound elderly. Methods: Two hundred and eighty elderly who were attending a local elderly welfare center were recruited. Evaluation included demographic, nutritional and health status related data, nutritional intakes, and life style related factors including physical activity. Results: Of the 280 subjects, old-old were 147 (52.5%) and young-old were 133 (47.5%). Male old-old elderly had more often abdominal obesity than female old-old, but male old-old more often had malnutrition than female old-old. There were few differences in nutrient intakes between old-old male elderly and female elderly after energy intakes were controlled. Male old-old more often had less intake of beta carotene and Vitamin A than female old-old. On the other hand, male old-old elderly more often had hypercholesterolemia and hypertension than male young-old. Conclusion: Male old-old may be at a greater health and nutritional risk than female old-old. Targeted nutritional intervention for male old-old emphasizing antioxidant nutritional intakes may be warranted.

종합병원 임상간호사 실무교육에서의 기초간호학 교육현황 (Current Status of Biological Nursing Science Education for Clinical Nurses in General Hospital)

  • Purpose: The purpose of this study was to investigate the extent and the nature of biological nursing science education for clinical nurses in general hospital. Methods: Five advanced general hospitals located in Seoul and Gyeonggi province were conveniently sampled. Contents of education for nurses conducted by the hospital nursing department from January 1 to December 31 of 2015 were collected. Contents of education included biological nursing science and the scope of inclusion and time of assignment were analyzed. Results: A total of 271 cases of nursing education data were collected and 223 cases were analyzed after excluding 48 cases whose contents were not confirmed. Biological nursing science was included in the contents of education for 117 cases (52.5%), but not for 106 cases (47.5%). Regarding the frequency of the biological nursing science education contents, 'pathophysiology'was the most frequently included (n= 286), followed by 'structure and function of the human body' (n= 191), 'mechanisms and effects of drugs' (n= 114) and 'clinical microbiology' (n= 43). Conclusion: Results of this study confirmed that the education for clinical nurses included a lot of biological nursing science related contents. These results can be used as basis for the development of curriculum and training course for nurses.

교사 참여형 교육프로그램(TPEP)을 경험한 초등교사의 과학 수업 전문성 변화 사례 - 시각적 주의를 중심으로 - (The Case Study of Elementary School Teachers Who Have Experienced Teacher Participation-oriented Education Program (TPEP) for Elementary School Teachers to Improve Class Expertise in Science Classes - Focusing on Visual Attention -)

  • The purpose of this study is to identify the effect of Teacher Participation-oriented Education Program (TPEP) for Elementary School Teachers to Improve Class Expertise in Science Classes with a focus on visual attention. The participants were two elementary school teachers in Seoul and taught science subjects. The lesson topic applied to this study were 'Structure and Function of Our Body' in the second semester of fifth grade and 'Volcano and Earthquake' in the second semester of fourth grade. The mobile eye tracker SMI's ETG 2w, which is a binocular tracking system was used in this study. In this study, the actual practice time, participant's visual attention, visual intake time average, and visual intake time average were analyzed by class phase. The results of the study are as follows. First, as a result of analyzing the actual class execution time, the actual class execution time was almost in line with the lesson plan after the TPEP application. Second, visual attention in the areas related to teaching and learning activities was high after applying TPEP. Factors affecting the progress of the class and cognitive burdens were identified quantitatively and objectively through visual attention. Third, as a result of analyzing the visual intake time average of participants, there was a statistically significant difference in all classes. Fourth, as a result of analyzing the visual intake time average of participants, the results were statistically significant in the introduction(video), activity 1, activity 2, and activity 3 stages in the lecture type class. The Teacher Participation-oriented Education Program (TPEP) for Elementary School Teachers to Improve Class Expertise in Science Classes can extend elementary science class expertise such as self-class analysis, eye tracking, linguistic, gesture, and class design beyond traditional class analysis and consulting.
