The purpose of this study is to investigate reasearch trend of acupuncture treatment on shoulder impingement syndrome. This study investigated the recent studies about acupuncture treatment for shoulder impingement syndrome via searching Korean and foreign electronic databases(Research Information Services, National Digital Science Library, Oriental Medicine Advanced Searching Integated Services, PubMed, China National Knowledge Infrastructure). The search term was searched by combining 'shoulder impingement sydrome', 'subacrominal pain', 'acupuncture', 'needle therapy', and was limited to the articles published from 2005 to 2017. 14 studies were found to be analyzed according to the type of study, the number of cases, the type of treatment, the instruments for assessment and published year. 14 studies were published since 2005, 6 case reports, 8 randomized controlled trials. In the treatment of shoulder impingement syndrome, treatments such as acupuncture, acupotomy, pharmacopuncture, laser acupuncture, combined treatment of exercise and rehabilitation were performed. Visual analogue scale(VAS), range of motion(ROM) and various questionnaires were used as primary assessments. In each study, acupuncture treatment was reported to have a significant effect in the pain, range of motion and disability of shoulder impingement syndrome. In order to ensure objective evidence on acupuncture treatment, large scale case reports and randomized controlled trials should be continued.
The purpose of this single subject care study was to evaluate the effectiveness of a functional electrical stimulation(FES) treatment program designed to prevent glenohumeral pint stretching and subsequent subluxation and shoulder pain in hemiplegic patients. The subject was a 43-year-old male who had left side hemiparesis with shoulder pain and subluxation on affected side. He received conventional physical therapy and additional FES therapy where two flaccid shoulder muscles, supraspinatus and posterior deltoid. were induced to contract repetitively up to 20 minutes for 2 weeks. As a result. the patient showed improvements in hemiplegic arm function, reduction in subluxation(as indicated by Jig test) and range of motion(SLROM). But it was difficult to generalize. We concluded that the FES program was effective in reducing the severity of shoulder subluxation and pain. Further study must be evaluated its statistical significance.
The purpose of this studied was the comparison with shoulder girdle muscles of isokinetic evaluation in the spinal cord injury group, and adults normal group. Data was collected from 20 cases spinal cord injury from January 10. 1994 to February 10, 1995 in a Chungnam University hospital and adults normal group to 20 cases. The results were as follows; 1. Low speeds peak torque was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, external rotator showed statistically significant in spinal cord injury group(p<0.05). Against normal groups shoulder girdle muscles of internal rotator showed statistically significant in normal group(p<0.05). 2. High speeds peak torque was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, showed statistically significant in spinal cord injury group(p<0.05). 3. High speeds total work was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, external rotator showed statistically significant in spinal cord injury group(p<0.05).(p<0.01). Against normal group shoulder girdle muscles of internal rotator showed statistically significant in normal group(p<0.05).

Objectives : There are little reports on treatment of shoulder impingement syndrome with acute bursitis and supraspinatus tendinopathy in traditional Korean Medicine. We suggest oriental treatment for pain relief and better movement of shoulder impingement. Methods : A -Shi Point, Sa-am acupuncture, ohjuksan-gamipang, seokyongtang-gamipang were used to treat shoulder pain. we evaluated the patient through VAS(Visual Analogue Scale) daily and Physical Examinations. Results : After 5 days of treatment, shoulder pain was decreased from VAS 7.5 to VAS 2 and After 3 weeks, the patient showed nearly full ROM(range of movement). Conclusion : In shoulder impingement syndrome, oriental treatment is good method for pain relief and better movement.
Objective : Assuming That the Characteristic of Meridian System Has been Similar to This of Electric Potentials in Human Body And that Measurements of Electric Potential at Well(井穴) and Sea(合穴) Points in Branches of the Twelve Meridians(WSBTM) Will Be Representative of Measurements of the Twelve Meridians, to Measure the Electric Potentials of 21 Patients with Shoulder Lesions, And Aching of Shoulder and Arm(肩臂痛)(ASA), to Be Differences in Electric Potentials of Patients Between Before And After Acupuncture Treatment, And Then to Find out the Characteristic of Meridian System in Patients with Shoulder Lesions. Methods : Electric Potentials of Well And Sea Points in the Meridians in Twenty one Patients with ASA Diagnosed by Both x-ray And Self View on Pain at Shoulder And Arm were Repeatedly Measured by Physiograph(PowerLab) at Before And after Acupuncture Treatment, Respectively. Measurements of Those Electrical Potentials Were Analyzed by Factor Analysis. Results : The Electric Potentials of WSBTM At the Left Side Before Acupuncture Treatment Were Divided Into Five Factors, But Those After Acupuncture Treatment Were Done Into Four Factors. On The Other Hand Those At the Right Side Before Acupuncture Treatment Were Divided Into Four Factors, The Number of Factors After Acupuncture Treatment Was Five. Conclusions : In Conclusions, The Number of Factors Before Acupuncture Treatment Are Different from Those After Acupuncture Treatment. In the Results The Effect of Acupuncture Treatment May be Reflected in Measurements of Electric Potential at WSBTM.
A single subject experimental design (alternating treatment design) was used to compare the effects of only ultrasound and ultrasound combined with stretching of the joint capsule on the ROM increase and pain reduction in patients with frozen shoulder. Two subjects were included in each group. In the only ultrasound treatment sessions, ultrasound was applied at the pain point of the shoulder joint in supine position. In the ultrasound combined with stretching treatment sessions, ultrasound was applied at the pain point of the shoulder joint positioned in external rotation and abduction in sitting position. Only ultrasound treatment and ultrasound combined with stretching treatment were alternately performed on each patient. Pain and disability was measured by shoulder pain and disability index (SPADI), and range of motion (ROM) was measured by scratch test. The results of this study showed that ultrasound combined with stretching treatment were more effective than only ultrasound treatment in ROM increase and pain reduction. However, disability score was not significantly different.

Purpose : This study was performed to comparison of distance change of subacromial space according to shoulder abduction angle in loaded and unloaded conditions. Methods : The subjects were 20 male college students, and the subacromial space was measured using an ultrasound diagnostic apparatus. The participants shoulder abduction was measured during each shoulder resting position and 45 ° abduction with loads of 1 kg, 2 kg, 3 kg. The collected data were analyzed by a paired t-test and repeated measure ANOVA with the SPSS (Ver. 22) program. Results : The subacromial space showed statistically significant decreased in 45 ° abduction than resting position (p<.001). All the subacromial space increased during the according to loads (1 kg, 2 kg, 3 kg) at the resting position but at 45 ° abduction showed statistically significant decreased as the load increases (p<.05). Also, subacromial space showed significant changes in the 2kg, 3kg compared with the 0kg loads. Conclusion : These results identified that shoulder abduction angle and load were related to subacromial space. When resting position, subacromial space is larger at loads than unload. As the load and shoulder abduction angle increase, subacromial space is reduced compared to resting.
Purpose : To analyze the outcome after barbotage and subacromial corticosteroid injection in acute pain attack with calcific tendinitis of the shoulder. Materials and Methods : Twenty-two patients with acute calcific tendinitis were analyzed with average follow-up of Twenty-two months. The shape and size of calcific deposits were classified. The outcome was assessed by UCLA shoulder score and pain in visual analogue scale(VAS). Results : Sixteen patients(73%) had permanent relief of pain. The results assessed by UCLA score were excellent in fourteen patients and good in two patients. Average VAS for pain was 0.6 ± 0.73 (range:0-2). No complication was enccpuntered, but six patients underwent surgical treatment due to lack of improvement or recurrence. Conclusion : Barbotage as a primary treatment in acute calcific tendinits of the shoulder is simple and effective, and therefore should be tried before surgical intervention.
Purpose: To evaluate the results of arthroscopic repair of traumatic anterior shoulder instability with glenoid bone defect. Materials and Methods: Nineteen patients who had underwent arthroscopic repair for the shoulder with traumatic anterior instability and glenoid bone defect were retrospectively reviewed. Mean age was 24.6 years(range, 20 to 39) and mean follow-up was 23 months(range, 19 to 55). No glenoid bone defect was greater than 7mm in length and 20% of the glenoid. The results were evaluated according to stability, range of motion and function. Results: All patients obtained excellent-good results according to Rowe scoring system. Two patients(10.5%) had instability. The mean loss of external rotation was 15 degrees (range, 0 to 25). Functionally, 17 patients could participate in preinjured work or sports to the same level with or without mild discomfort. The remained 2 patients who had 25 degree loss of external rotation could not play sports. Conclusion: Though arthroscopic repair is a good treatment for traumatic anterior shoulder instability with small glenoid bone defect, it is possible to cause loss of external rotation
Background: In a previous study undertaken to quantify capsular volume in rotator cuff interval or axillary pouch, significant differences were found between controls and patients with instability. However, the results obtained were derived from two-dimensional cross sectional areas. In our study, we sought correlation between three-dimensional (3D) capsular volumes, as measured by magnetic resonance arthrography (MRA), and multidirectional instability (MDI) of the shoulder. Methods: The MRAs of 21 patients with MDI of the shoulder and 16 control cases with no instability were retrospectively reviewed. Capsular areas determined by MRA were translated into 3D volumes using 3D software Mimics ver. 16 (Materilise, Leuven, Belgium), and glenoid surface area was measured in axial and coronal MRA views. Then, the ratio between capsular volume and glenoid surface area was calculated, and evaluated with control group. Results: The ratio between 3D capsular volume and glenoid surface area was significantly increased in the MDI group ($3.59{\pm}0.83cm^3/cm^2$) compared to the control group ($2.53{\pm}0.62cm^3/cm^2$) (p<0.01). Conclusions: From these results, we could support that capsular volume enlargement play an important role in MDI of the shoulder using volume measurement.
