Arthroscopic Decompression of an Inferior Paralabral Cyst of the Shoulder in an Elderly Patient: A Case Report

  • The widespread use of diagnostic radiography, especially using magnetic resonance imaging, has helped to increase the diagnosis of paralabral cysts in patients with chronic shoulder pain. These paralabral cysts are frequent in the anterior, the superior, and the posterior compartment of the shoulder joint but are rare in the inferior compartment. Paralabral cysts in the shoulder appear particularly in men in their third and fourth decades but rarely in elderly patients. We report a case of an inferior paralabral cyst in an elderly patient whom we treated through arthroscopic decompression.

Reverse Total Shoulder Arthroplasty in the Massive Rotator Cuff Tear

  • In the patients of retracted massive rotator cuff tears, there are much of difficulty to functional recovery and pain relief. Nevertheless the development of treatment, there are still debates of the best treatments in the massive rotator cuff tears. Recenlty various of treatments are introduced; these are acromioplasty with debridement, biceps tenotomy, great tuberoplasty with biceps tenotomy, partial repair, mini-open rotator cuff repair, arthroscopic rotator cuff repair, soft tissue augmentation, tendon transfer, flap, hemiarthroplasty, and reverse total shoulder arthroplasty. That there is no difference of result for reverse total shoulder arthroplasty between patients who have massive rotator cuff tear without arthritis and patients who have cuff tear arthropathy. Reverse total shoulder arthroplasty is one of reliable and successful treatment options for massive rotator cuff tear. Especially it is more effective for patients who have a pseudoparalysis.

Arthroscopic Treatment for Calcific Tendinitis of Origin of Long Head of Triceps

  • A 55-year-old female experienced acute left shoulder pain without specific trauma. Radiography showed calcific deposits in the inferior part of the glenoid fossa. Magnetic resonance arthrography showed calcific deposits in the origin of the long head of triceps brachii muscle. Conservative treatment failed to resolve the symptoms; therefore, arthroscopic surgery was performed. The patient experienced immediate and dramatic pain relief, and normal shoulder motion was demonstrated 1 year after surgery. In conclusion, although rare, calcific tendinitis of the triceps brachii muscle, which causes shoulder pain, should be included in the differential diagnosis of acute shoulder pain. Arthroscopic surgery is a treatment option for chronic cases and those resistant to conservative treatment.

테이핑이 편마비 환자의 결관절 아탈구에 미치는 효과 (The effect of taping therapy on shoulder in hemiplegic patients)

  • The purpose of this case study was to evaluate the taping therapy program designed to improve shoulder joint subluxation in hemiplegic patients. This study was 9 males who had hemiparesis with limitation of range of motion (ROM) and subluxation on affected side. Patients received proprioreptive facilitation using shoulder weight shifting and additional A type and B type taping. As a result of this study, Patients showed reduction of the subluxation on hemiplegic shoulder joint. but it was difficult to generalize. We concluded that proprioceptive facilitation using shoulder weight shifting and taping therapy was effective in reducing severe shoulder subluxation. Following studies must be evaluated its statistical significance.

  • The purpose of study is aimed at analysing the body deformity types through movement pattern of shoulder pain patients without considering of upper extremity radiating pain. This study was proceeded in a time-series method through the fifty shoulder pain patients visited the hospital for the first time. The results were as follows. 1. Body type I: Deformity style of Lt shoulder & pelvis forward tilt was 8 patirnts (16%) 2. Body type II: Deformity style of Rt shoulder & pelvis forward tilt style was 22 patirnts (44%) 3. Body type III: Deformity style of Lt shoulder & Rt pelvis forward tilt style was 9 patirnts (18%) 4. Body type IV: Deformity style of Rt shoulder & Lt pelvis forward tilt style was118 patirnts (22%).

  • Synovial chondromatosis is a rare disease. The knee is the most commonly affected joint but the shoulder joint is very rarely affected. We report typical synovial chondromatosis of the shoulder joint which was managed by arthroscopic removal of multiple loose bodies, synovectomy, good objective and functional outcome was obtained until postoperative 7-year follow-up. Arthroscopy can be used as an effective and useful therapeutic method for synovial chondromatosis of the shoulder joint.

  • Disorders of the shoulder in which conservative management is indicated as a primary treatment include idiopathic frozen shoulder, atraumatic instability. acute calcific tendinitis , subacromial impingement syndrome, partial thickness or small full thickness tear of the rotator cuff, and chronic rotator cuff tear in elderly population. Aside from medication and various physical modalities used for relief of pain, the mainstay of orthopaedic rehabilitation consists of stretching of the tendons and capsulologamentous complex, and strengtnening of rotator cuff muscles as well as surrounding muscles. However, orthopaedic rehabilitation in chronically painful conditions of the shoulder differs in detail according to the specific diseases or injuries, And it is important to make the rehabilitation program that can be undertaken by the patients rather than adhering to a fixed schedule. Also the program should be changed as the condition of the patient improves or deteriorates. Nevertheless there are basic rules of stretching and strengthening , which should always be observed in applying orthopaedic rehabilitation treatment.

  • There are few case reports of glenoid labral cysts related to the labral tear in the shoulder. Glenoid labral cyst is often overlooked in the diagnosis of shoulder pain. We are reporting a case of a glenoid labral cyst accompanying with anterior labral tear in the right shoulder of a 42 years old woman with the history of trauma. The cyst and anterior labral tear was successfully treated with arthroscopic excision and repair.

Surgical Treatment for the Shoulder Joint in Rheumatoid Patients

  • Rheumatoid arthritis (RA) is a systemic disease with medication as the treatment of choice. However, surgical treatment is recommended when no improvement is noted despite aggressive conservative treatment. Synovectomy provides desirable outcomes for RA patients in the early stage with a glenohumeral joint of Larsen grade II or less; conversely, arthroplasty is recommended for patients with a glenohumeral joint of grade III or higher. RA patients often have attenuation and dysfunction of the rotator cuff, and reverse shoulder arthroplasty has been proven to provide favorable outcomes in some patients. RA is often complicated with osteoporosis and bony deformity; therefore, close attention is necessary to prevent fractures during shoulder arthroplasty.

  • Objective: There are some case reports on treatment of shoulder dislocation in Patients Undergoing Stroke. We report the electronic needle and the Bee venom Acupunure Theraphy about under using shoulder dislocation in patients Undergoing Stroke got a good remedial value. Methods : The patients diagnosed as shoulder dislocation and treated mainly with Bee venom Acupunture Therapy and electronic needle Results & Conclusions : Symptoms of the patientes such as shoulder pain, Range Of Motion(ROM) were improved after above treatments. The interval in X-ray film comes to be narrow. So, it is suggested that oriental medical treatment are effective on post stroke with shoulder dislocation.
