MRI Findings of a Malignant Solitary Fibrous Tumor of the Diaphragmatic Pleura: a Case Report

  • Solitary fibrous tumors (SFT) are rare mesenchymal tumors that most commonly develop in the pleura; they rarely involve the diaphragm. MRI has not been widely used to evaluate SFTs of the thoracic cavity, though it may be highly useful in assessing local invasion, predicting malignant potential, and helping in the differential diagnosis. However, MRI findings of malignant SFTs of the diaphragmatic pleura have been described in only two cases. We report a rare case of a malignant solitary fibrous tumor of the diaphragmatic pleura in an 82-year-old man. We describe the clinical and characteristic imaging features, including computed tomography, conventional MRI, and diffusion-weighted imaging. Contrast-enhanced MRI is more accurate than is CT in identifying the origin of SFTs, predicting whether they ae benign or malignant, and assessing local invasion. This imaging modality proved helpful in deciding on the treatment strategy for these rare tumors.

  • 고립성 섬유종은 흉막에서 흔히 발생하는 양성 종양으로 알려져 있으나, 신체 어느 곳에서나 발생할 수 있고 10%-30%에서는 악성이다. 전형적으로, 고립성 섬유종은 단일성의 조영증강되는 종괴로 나타나지만, 척추 주위의 양측성 종괴로 나타나는 경우는 보고되어 있지 않다. 이 증례에서는, 등 통증과 만성 농흉의 병력이 있던 88세 남자 환자의 영상 검사에서 침습적인 양측성 척추 주위 종괴가 발견되었고, 만성 농흉과 연관된 흉부 악성 종양을 먼저 의심하였다. CT 유도하 이중구조 바늘 생검을 통해 진단된 양측성의 척추 주위 악성 고립성 섬유종의 증례를 보고한다.

북한산 둘레길에서 참진드기의 계절적 발생과 중증열성혈소판감소증후군 바이러스 검출 (Seasonal Prevalence of Ticks at Bukhansan Dullegil and Detection of Severe Fever with Thrombocytopenia Syndrome Virus)

  • 2019년 4월부터 10월까지 북한산 둘레길 12구간(지축동), 14구간(울대리), 18구간(호원동)에서 참진드기를 채집해서 계절적 분포 조사와 매개체의 SFTS 바이러스 감염 여부를 확인하였다. 천젓기와 천끌기 방법으로 개피참진드기, 일본참진드기와 중증열성혈소판감소증후군의 주된 매개체로 알려진 작은소피참진드기 3종이 채집되었다. 작은소피참진드기는 채집비율이 지축동 91%, 울대리 94%, 호원동 98%를 차지하여 우점종이었다. 작은소피참진드기의 성숙단계에 따른 계절별 채집비율을 보면 성충의 경우 세 지역 모두 9월에 최고치를 보였고 약충의 경우에는 4월에서 6월 사이에 주로 채집되었다. 유충의 경우에는 9월과 10월에 최고치를 보였다. 채집된 작은소피참진드기 1,158개체를 성숙단계별로 2× OneStep RT-PCR과 nested PCR로 SFTS 바이러스 검사를 한 결과 양성은 나타나지 않았다. 2010년 개장 이후 이용객이 늘어나는 북한산 둘레길에서 기후 온난화로 증가가 예상되는 참진드기 매개 질환에 대한 위험분석과 효율적인 예방에 본 조사 결과가 활용될 수 있다.

관악산에서 참진드기 조사 및 중증열성혈소판감소증후군 바이러스 검출 (Prevalence of severe fever with thrombocytopenia syndrome virus among ticks surveyed at Mt. Gwanak, Korea)

  • This study was performed to investigate the distribution of ticks and the rate of infection with severe fever with thrombocytopenia syndrome (SFTS) virus in ticks collected at Mt. Gwanak and the Seoul National University campus, Korea. Ticks (n = 273) were collected from May to October and included 76 Haemaphysalis longicornis (4 adult females, 72 nymphs), 49 Haemaphysalis flava (9 adult females, 3 adult males, 37 nymphs), and 148 Haemaphysalis spp. larvae. SFTS virus detection was performed by using one-step RT PCR and nested PCR. The SFTS virus was detected in 7 samples (1 Haemaphysalis longicornis nymph, 3 Haemaphysalis flava nymphs, and 3 Haemaphysalis spp. larva). The overall minimum field infection rate was 2.6%, whereas the minimum field infection rates of adult, nymphal, and larval ticks were 0%, 3.2%, and 2.0%, respectively. For a more accurate indication of the prevalence of SFTS virus in Korea, further in-depth investigations of tick species and SFTS virus occurrence over a larger area and longer period are needed.

Feasibility Study of Submerged Floating Tunnels Moored by an Inclined Tendon System

  • Concepts of submerged floating tunnels (SFTs) for land connection have been continuously suggested and developed by several researchers and institutes. To maintain their predefined positions under various dynamic environmental loading conditions, the submerged floating tunnels should be effectively moored by reasonable mooring systems. With rational mooring systems, the design of SFTs should be confirmed to satisfy the structural safety, fatigue, and operability design criteria related to tunnel motion, internal forces, structural stresses, and the fatigue life of the main structural members. This paper presents a feasibility study of a submerged floating tunnel moored by an inclined tendon system. The basic structural concept was developed based on the concept of conventional cable-stayed bridges to minimize the seabed excavation, penetration, and anchoring work by applying tower-inclined tendon systems instead of conventional tendons with individual seabed anchors. To evaluate the structural performance of the new type of SFT, a hydrodynamic analysis was performed in the time domain using the commercial nonlinear finite element code ABAQUS-AQUA. For the main dynamic environmental loading condition, an irregular wave load was examined. A JONSWAP wave spectrum was used to generate a time-series wave-induced hydrodynamic load considering the specific significant wave height and peak period for predetermined wave conditions. By performing a time-domain hydrodynamic analysis on the submerged floating structure under irregular waves, the motional characteristics, structural stresses, and fatigue damage of the floating tunnel and mooring members were analyzed to evaluate the structural safety and fatigue performance. According to the analytical study, the suggested conceptual model for SFTs shows very good hydrodynamic structural performance. It can be concluded that the concept can be considered as a reasonable structural type of SFT.

Four Year Surveillance of the Vector Hard Ticks for SFTS, Ganghwa-do, Republic of Korea

  • The seasonal abundance of hard ticks that transmit severe fever with thrombocytopenia syndrome virus was monitored with a collection trap method every April to November during 2015-2018 and with a flagging method every July and August during 2015-2018 in Ganghwa-do (island) of Incheon Metropolitan City, Republic of Korea. This monitoring was performed in a copse, a short grass field, coniferous forest and broad-leaved forest. A total of 17,457 ticks (8,277 larvae, 4,137 nymphs, 3,389 females, and 1,654 males) of the ixodid ticks comprising 3 species (Haemaphysalis longicornis, H. flava, and Ixodes nipponensis) were collected with collection traps. Of the identified ticks, H. longicornis was the most frequently collected ticks (except larval ticks) (94.26%, 8,653/9,180 ticks (nymphs and adults)), followed by H. flava (5.71%, 524/9,180) and Ix. nipponensis (less than 0.04%, 3/9,180). The ticks collected with collecting traps were pooled and assayed for the presence of SFTS virus with negative results. In addition, for monitoring the prevalence of hard ticks, a total of 7,461 ticks (5,529 larvae, 1,272 nymphs, 469 females, and 191 males) of the ixodid ticks comprising 3 species (H. longicornis, H. flava, and Ix. nipponensis) were collected with flagging method. H. longicornis was the highest collected ticks (except larval ticks) (99.53%, 1,908/1,917 ticks (nymphs and adults)), followed by H. flava (1.15%, 22/1,917).

충북 보은지역의 참진드기류 분포조사 (2016-2020) (Seasonal Distribution of Ticks in Boeun, Chungbuk during 2016-2020)

  • 본 연구는 2016년부터 2020년까지 충북 보은지역의 참진드기류의 시기별 분포를 조사하였다. 참진드기류는 다양한 환경(매년 4지점)에서 채집하였고, 5년 동안 충북 보은지역에서 채집된 참진드기류는 2속 3종으로 작은소피참진드기(Haemaphysalis longicornis), 개피참진드기(Haemaphysalis flava), 일본참진드기(Ixodes nipponensis)로 총 17,704개체의 참진드기가 채집되었다. 작은소피참진드기는 가장 많은 28.83 T.I. (68.40%) 값을 보였고, 개피참진드기 1.49 T.I. (3.53%), 일본참진드기 0.02 T.I. (0.06%)순으로 채집되었고 종 동정이 되지 않는 유충은 11.81 T.I. (28.01%)로 조사되었다. 우점을 차지하는 작은소피참진드기는 봄철 (5-6월)에, 유충은 8월과 9월에 가장 많은 수가 채집되었다. 대부분의 채집지에서 작은소피참진드기가 가장 많이 채집되었고 무덤과 초지에서만 유충이 많이 채집되었다. 개피참진드기는 충북 보은지역에서 채집된 개체수는 적지만 모든 채집지에서 분포하는 것으로 나타났다. 5년간 채집된 3종의 참진드기류에 대하여 총 828 pools의 SFTS 감염률을 조사한 결과 모두 음성으로 병원체 감염이 확인되지 않았다.

  • Severe fever with thrombocytopenia syndrome (SFTS) is an emerging disease characterized by fever and thrombocytopenia. Haemaphysalis longicornis ticks comprise the major population of ticks in the environment and have been considered as the main vector for SFTS virus (SFTSV). Here we investigated the distribution of ticks carrying SFTSV collected from the environment using the dragging or sweeping methods during April~October 2015 in Jeollanam-do, Korea. Sampling was taken from Songjeong, Omi, Bangkwang, Sandong areas in Jiri walking trails. Among the total 3,869 ticks collected, 3,823 ticks (98.8%) were H. longicornis, 41 (1.1%) were Amblyomma testudinarium, and 5 (0.1%) were Ixodes nipponensis. Classification results by regional groups of H. longicornis indicated that 1,613 ticks were collected in Sandong, 1,190 ticks in Omi, 603 ticks in Bangkwang, and 417 ticks in Songjeong. In monthly distributional studies of H. longicornis based on the developmental stages, nymph (325 ticks) was collected from May to October, 94% of larvae from April to June, and 94% of adult from June to August. These results showed the different dominant stage of ticks according to seasons. However, no SFTSV-specific gene was detected in 3,823 ticks of H. longicornis, 41 of A. testudinarium and 5 of I. nipponensis.

Numerical study on the resonance behavior of submerged floating tunnels with elastic joint

  • In submerged floating tunnels (SFTs), a next-generation maritime transportation infrastructure, the tunnel module floats in water due to buoyancy. For the effective and economical use of SFTs, connection with the ground is inevitable, but the stability of the shore connection is weak due to stress concentration caused by the displacement difference between the subsea bored tunnel and the SFT. The use of an elastic joint has been proposed as a solution to solve the stability problem, but it changes the dynamic characteristics of the SFT, such as natural frequency and mode shape. In this study, the finite element method (FEM) was used to simulate the elastic joints in shore connections, assuming that the ground is a hard rock without displacement. In addition, a small-scale model test was performed for FEM model validation. A parametric study was conducted on the resonance behavior such as the natural frequency change and velocity, stress, and reaction force distribution change of the SFT system by varying the joint stiffness under loading conditions of various frequencies and directions. The results indicated that the natural frequency of the SFT system increased as the stiffness of the elastic joint increased, and the risk of resonance was the highest in the low-frequency environment. Moreover, stress concentration was observed in both the SFT and the shore connection when resonance occurred in the vertical mode. The results of this study are expected to be utilized in the process of quantitative research such as designing elastic joints to prevent resonance in the future.
