Effects of Tai Chi Self-help Program on Glucose Control, Cardiovascular Risks, and Quality of Life in Type II Diabetic Patients (제 2유형 당뇨환자를 위한 타이치 자조프로그램이 혈당조절, 심혈관 위험효소, 삶의 질에 미치는 효과)

  • Purpose: The purpose of the study was to identify the effects of Tai Chi self-help program on glucose control, cardiovascular risks, and quality of life in patients with type II diabetes. Method: The study subjects were 95 adults with type II diabetes recruited from five primary health centers and one outpatient clinic. The Tai chi self-help program was provided twice a week for 6 months. Outcome measures included blood glucose(HbA1c), cardiovascular risk checklist, and quality of life(SF36). Result: At the completion of 6-month program, HbA1c for Tai Chi group dropped from 6.79 to 6.59 without significant group differences. However, cardiovascular risk scores for Tai Chi group has been significantly reduced compared to the control group. In 6 months, quality of life (SF36) was significantly better for Tai Chi group especially in the areas of physical and social functioning. Conclusion: Tai Chi self-help program applied for type 2 diabetes patients has been partially effective to control cardiovascular risk factors, and some dimensions of quality of life. Tai Chi self-help program can be safely and easily applicable to primary care centers for diabetic patients and other people with chronic disease for their symptom management, and health promotion.

A Clinical Study for Patients with Lumbar Disc Herniation: Association Between Clinical Outcomes of Conservative Treatment and Cigarette Smoking (요추 추간판 탈출증 환자의 보존적 치료결과와 흡연의 상관성에 대한 임상적 고찰)

  • Objectives : To investigate the association between cigarette smoking and clinical outcomes of conservative treatment for lumbar herniated disc. Methods : We reviewed 74 male patients with lumbar herniated disc who were hospitalized at the Jaseng Oriental Medicine Hospital. 74 patients were divided into 2 groups(smokers, non-smokers) and we measured age, ODI score, SF-36 score, VAS. Results : Between smokers and non-smokers, statically significant differences were found in clinical symptoms on the time of 6 months later, effective scores of treatments, value of ODI and value of VAS on the time of 6 months later. And in these values of measurements, non-smokers were higher than those of smokers. Conclusion : These results suggest that possibility of the association between cigarette smoking and clinical outcomes of conservative treatment for lumbar herniated disc.

Effect of E10 on Relief of Cervical Myalgia; One Group Pre-Post, Pilot Trial Study (E10의 경항부 근육통 완화에 대한 효과; 전-후 비교, 예비 임상 연구)

  • Objectives The purpose of this study is to investigate the efficacy of E10 on the relief of cervical myalgia. Methods This study is a one group pre-post, pilot trial test. After the approval of institutional review board (IRB), we have recruited 25 patients suffering from cervical myalgia. 23 patients (2 patients dropped out) was treated by using E10 on the cervical muscle and acupuncture points, three times a week for a total of two weeks from February 2016 to April 2016. Evaluations of VAS for bothersomeness of neck pain (VASB), VAS for pain intensity (VASP), NDI, SF-36, X-ray, thermography, etc. before and 1 week after treatment were carried out. The primary outcome measure was the VASB, measured 1 week after the end of the therapy. Results VASB scores was significantly decreased from $72.5{\pm}9.9$ to $50.4{\pm}9.9$. VASP scores was significantly decreased from $71.5{\pm}10.5$ to $48.5{\pm}18.3$. Results showed significant improvements in VASB, VASP, NDI, SF-36 from the baseline. However, in X-ray, thermography, there was no singnificant difference. Conclusions This study demonstrates the effectiveness of E10 treatment for relieving cervical myalgia.

Effectiveness of Oriental Medical Treatments to Symptoms and Quality of Life in the Patients with Herniated Lumbar Disc being Suggested Operation (한의학적 치료가 수술 권유받은 요추간판탈출증 환자의 임상 증상 및 삶의 질에 미치는 효과)

  • Objectives : The purpose of this study was to report effectiveness of oriental medical treatments to symptoms and quality of life in the patients with herniated lumbar disc being suggested operation. Methods : Eighteen patients diagnosed with herniated lumbar disc by MRI in hospital were treated by oriental medical treatments. Symptoms of the patients were assessed by visual analogue scale(VAS) and SF-36. Results : After the treatments, the VAS of symptoms of herniated lumbar disc and the score of role-physical scale in SF-36 were improved but the other scores of scales were not significantly different. Conclusions : It could be suggested that oriental medical treatments were effective for the patients with herniated lumbar disc being suggested operation. Further study of the effect of oriental medical treatments on herniated lumbar disc is needed.

Health-related Quality of Life Instrument With 8 Items for Use in Patients With Type 2 Diabetes Mellitus: A Validation Study in Korea

  • Objectives: This study assessed the validity and reliability of the Health-related Quality of Life Instrument with 8 Items (HINT-8) in patients with diabetes. HINT-8 is a newly-developed, generic health-related quality of life (HRQoL) instrument. Methods: Three HRQoL instruments-HINT-8, EuroQoL 5-Dimension 5-Level (EQ-5D-5L), and Short Form Health Survey version 2.0 (SF-36v2)-were provided to 300 patients with diabetes visiting a tertiary hospital for follow-up visits in Korea. The HRQoL scores obtained using the HINT-8 were evaluated for subgroups with known differences based on demographics and diabetes-related characteristics (known-group validity). The mean scores of the instruments were compared between groups segmented by their responses to the HINT-8 (discriminatory ability). Correlation coefficients of the HINT-8 with other instruments were calculated (convergent and divergent validity). The Cohen kappa and intra-class correlation coefficient (ICC) were also evaluated (test-retest reliability). Results: The average HINT-8 indexes were lower among women, older, and less-educated subjects. Subjects who did not list any problems on the HINT-8 had significantly higher HRQoL scores than those who did. The correlation coefficients of the HINT-8 with the EQ-5D-5L index and EuroQoL visual analogue scale were 0.715 (p<0.001) and 0.517 (p<0.001), respectively. The correlation coefficients between the HINT-8 index and the scores of 8 domains of the SF-36v2 ranged from 0.478 (p<0.001) to 0.669 (p<0.001). The Cohen kappa values for the HINT-8 ranged from 0.268 to 0.601, and the ICC of the HINT-8 index was 0.800 (95% confidence interval [CI], 0.720 to 0.860). Conclusions: This study showed that the HINT-8 is a valid and reliable HRQoL instrument for patients with diabetes.

  • Valid evaluation of self-perceived health status is important for the promotion of individual health and quality of life, In advanced countries, many types of health profile have been developed, and currently, the SF-36, NHP, and EuroQol, etc, are widely used. However, the outcomes of these profiles may vary according to regional, cultural or emotional backgrounds. For these reasons a Korean Health Profile should be developed. In this study, we reviewed the concept of self-perceived health status, and its significance to public health, and reviewed some of the differences between the available profiles in 139 related publications. Based on this review, we are trying to develop a Korean Health Profile in order to measure the self-perceived health stati of Koreans.

Study of Selective Etching of GaAs over AlGaAs and InGaP Semiconductors in High Density Planar Inductively Coupled BCl3/SF6 Plasmas (고밀도 평판형 유도결합 BCl3/SF6 플라즈마를 이용한 GaAs/AlGaAs와 InGaP 반도체의 선택적 식각에 관한 연구)

  • We investigated selective dry etching of GaAs over AlGaAs and InGaP in high density planar inductively coupled $BCl_3/SF_6$ plasmas. The process parameters were ICP source power (0-500 W), RE chuck power (0-30W) and gas composition $(60-100\%\;BCl_3\;in\;BCl_3/SF_6)$. The process results were characterized in terms of etch rate, selectivities of GaAs over AlGaAs and InGaP, surface morphology, surface roughness and residues after etching. $BCl_3/SF_6$ selective etching of GaAs showed quite good results in this study. Selectivities of GaAs $(GaAs:AlGaAs\~36:1,\;GaAs:InGaP\~45:1)$ were superior at $18BCl_3/2SF_6$, 20 W RF chuck power, 300 W ICP source power and 7.5 mTorr. Addition of $(5-15\%)SF_6\;to\;BCl_3$ produced relatively high selectivities of GaAs over AlGaAs and InGaP during etching due to decrease of etch rates of AlGaAs and InGaP (boiling points of etch products: $AlF_3\~1300^{\circ}C,\;InF_3>1200^{\circ}C$ at atmosphere) at the condition. SEM and AFM data showed slightly sloped sidewall and somewhat rough surface$(RMS\~9nm)$. XPS study on the surface of processed GaAs proved a very clean surface after dry etching. It shows that planar inductively coupled $BCl_3/SF_6$ plasmas could be a good candidate for selective dry etching of GaAs over AlGaAs and InGaP.

A Trial for Development of Health Profile (KHP 1.0) to Measure the Self-Perceived Health Status of Korean (한국인의 자가평가 건강수준 측정도구(KHP 1.0) 개발)

  • Objectives : The 1990s has seen advances in the conceptualization of self-perceived health status which has important roles for individual health and the quality of life. Many types of standardized questionnaires have been developed with the current wide use of SF-36, NHP, andEuroQol. However, the outcomes of these tools may be different with regard to regional, cultural and emotional backgrounds. The purpose of this study was to trial the development of a Korean Health Profile (KHP 1.0) to measure the self-perceived health stati of Koreans. Methods : The KHP 1.0 was designed on the basis of the Medical Outcome Study Form 36 (SF-36), the Nottingham Health Profile (NHP), and the EuroQOL. It was composed of 9 scales; physical functioning, role limitation-physical, pain, general health, energy, social isolation, sleep, role limitation-emotional, and e-motional health. Self-reported chronic disease conditions, and the Zung's Self-Rating Depression Scale (SDS), were also checked for the evaluation of clinical validity. This study was conducted, from December 2000 to January 2001, on 800 middle-aged parents, with four high school students, with 100 retest sets being conducted two weeks later. From the 800 subjects there were 588 complete responses (effective response 73.5%). The reliability of the test-retest results, and the factor analysis on the validity of the KHP 1.0 components, were evaluated using the SPSS (ver 10.0) software. Results : The reliability of the KHP 1.0 was good with Cronbach's alpha (>0.6), test-retest correlation coefficients (>0.5), but with no significant differences from the paired t-test. From the psychometric validity tests, the 9 scales of the KHP 1.0 were divided into two components; physical and mental, and trimmed to the established model with 55% of the total variance, with the exception of role limitation-emotional. The clinical validity on the basis of the comparison for the four characteristic groups; healthy, physical conditions only, mental conditions only, and physical and mental conditions were also good. Conclusions : The KHP 1.0 appears to be a valid measurement tool of self-perceived health stati of Koreans, although there are limitations, i.e. sample size was too small, a limited number of middle-aged subjects, and it was based on unconfirmed diagnoses, etc. Therefore, further study is required to standardize the assessment.

Exercise Effect Applied to Patients Who Underwent Lung Resection with Lung Cancer: Systematic Review and Meta-Analysis (폐암으로 폐 절제술을 시행한 환자에게 적용한 운동 효과: 체계적 문헌고찰과 메타분석)

  • The purpose of this study was to investigate systematic review and meta-analysisto identify exercise intervention effect applied to patients who underwent lung resection with lung cancer. A total of 1,322 publications were searched from 1990 to 2016 through domestic and foreign electronic databases, and the final 13 publications were selected through literature selection process. Finally, eight RCT (randomized controlled trial) and five NRCT (non-randomized controlled trial) were identified, including 925 participants. The results of the meta-analysis of the effect sizes of the intervention on the outcome variables showed that the health-related quality of life EORTC QOL-C13/30 (the European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire) (MD-0.50 95 % CI -0.83-0.18) and SF-36 PCS (the Medical Outcomes 36-Item Short Form Health Survey PCS) (MD 0.75, 95 % CI 0.41-1.10) in the intervention group were significantly higher than the control group. The results of this study suggest that exercise interventions can potentially improve the quality of life of patients who underwent lung resection with lung cancer.

A Study of Walking, Viewing and Fragrance-based Forest Therapy Programs Effect on Living Alone Adults' Dementia Prevention (걷기, 경관감상 그리고 향기를 이용한 산림치유프로그램이 독거노인의 치매예방에 미치는 효과 연구)

  • The purpose of this study is to examine the effect of forest healing programs with walking, sightseeing, and forest scent among six healing forest treatments using forest environment on the cognitive function, depression, and quality of life of elderly living alone. The subjects of the study were 17 senior citizens living in Samcheok and complaining of severe depression and forgetfulness. They were divided into two groups, and each group participated in the six sessions of the weekly forest healing program. All of them were surveyed of a Korean version of the MMSE-K, BDI (Beck Depression Inventory Korean Version), and SF-36 (Short Form 36 health survey questionnaire) with the questionnaire designed for the one-group pre-post test to examine changes in cognitive function, depression, and quality of life. The results showed that the mental status, depression level, and quality of life of respondents were significantly higher after healing programs. These results suggest that participation in healing program activities using forest environment can improve cognitive function, relieve depression and improve quality of life for senior citizens living alone. Therefore, inducing older adults who live alone and avoid outside activities to participate in the regular forest healing program will contribute to the prevention of dementia due to age and environment.
