고령화사회를 대비한 소방방재정책에 관한 연구 (A Study on Safety Policies Toward Aging Society)

  • 우리나라는 2000년에 65세 이상의 노인인구가 전체 인구의 $7.2\%$를 차지하여 고령화사회가 되었고, 2019년에는 2배인 $14.4\%$에 달하여 고령사회가 될 것으로 예상하고 있다. 고령자는 신체기능의 저하 등에 의해 재해발생 시에 정확하고 신속한 판단과 행동을 하지 못할 가능성이 높으므로 고령화가 진전될수록 화재 등 각종 재해로 인한 고령자의 인명피해가 증가할 것이다. 고령자의 화재 등 재해로 인한 희생을 줄이기 위해서 고령자 등 재해약자관련 소방방재대책 추진, 화재로 인한 인명피해 최소화대책 추진, 농촌지역 소방력 보강 및 소방서비스확대, 소방방재 정보제공 및 홍보기능의 강화, 새로운 연계활동 발굴(우편집배원 및 사회복지사의 119요원화), 소방공무원 보강 및 장비의 경량화 등의 정책을 제언한다.

농촌노인의 삶의 만족도 결정요인에 대한 융합 연구: 주관적 건강상태의 매개효과를 중심으로 (A Convergence Study on Determinants of Life Satisfaction among Rural Older Adults: Focusing on Mediating Effect of Self-rated Health)

  • 본 연구는 농촌노인의 어떠한 특성들이 주관적 건강상태, 삶의 만족도에 직접적으로 영향을 미치는지, 그리고 주관적 건강상태를 매개로 삶의 만족도에 영향을 미치는지 고찰하는 것을 목적으로 한다. 연구목적을 달성하기 위하여 G군에서 2015년 조사한 60세 이상 169명을 대상으로 다중회귀분석을 실시하였다. 연구결과는 첫째, 질병수는 부(-)적으로, 주관적 경제상태와 운동시간은 정(+)적으로 주관적 건강상태에 영향을 미쳤다. 둘째, 주관적 경제상태, 운동시간, 규칙적인 식사, 하루 끼니 수가 삶의 만족도에 정(+)적 영향을 미친 것으로 나타났다. 셋째, 주관적 경제상태, 운동시간이 주관적 건강상태를 부분매개로, 질병 수는 완전매개로 삶의 만족도에 영향을 미치는 것으로 나타났다. 이러한 결과를 토대로 보건 및 식사지원을 기초로 융합된 정책지원을 제언하였다.

한국과 일본 도농복합 지역 거주 노인의 건강관리 및 여가활동 실태 비교 (Comparison of Health Management and Leisure Activities between Korean and Japanese Elderly in Urban-Rural Composite Areas)

  • Purpose: The purpose of this study was to compare health management and leisure activities of the Korean and Japanese elderly. Methods: This study used a cross-sectional descriptive research design. A total of 149 Korean and 140 Japanese older adults living in two urban-rural composite areas participated in this research project. Data were collected using interviews with the participants. t-test and $x^2$ test were used to analyze the data. Results: Eighty eight point two percent of the Japanese perceived that their health status was good whereas 27.3% of the Koreans responded that their health status was poor. There were significant differences in types of leisure activities in art, intellectual, events, and volunteer activities between the two groups. A majority of the Japanese elderly replied that they spent some time and money for leisure activities; however, 20.2% and 27.2% of the Koreans reported that they did not spend time or money at all for leisure activities. Conclusion: The number of the Korean elderly participating leisure activities was significantly lower than Japanese participants. Developing nursing strategies using leisure programs would improve the quality of life of older adults and the quality of nursing care.

지역사회 노인들의 사전의료의향서 작성의향과 불안과의 상관관계 (Correlations between Intention to use Advance Directives and Death Anxiety in the Community-dwelling Elderly)

  • Purpose: The purpose of this study was to identify the relationship between intention to use advance directives and level of death anxiety in community-dwelling elders. Method: The participants were 200 older adults who were aged 60 or over and attended the community welfare centers. The level intention to use advance directive was measured by a questionnaire that was developed by the authors for the study. The measure by Kraus and Ellisond was administered to evaluate the level of death anxiety. Result: The average score for intention to use advance directives was $2.05{\pm}0.88$ and that of death anxiety was $6.2{\pm}2.28$. There were significant differences in the intention to use advance directives according to education levels and individual properties. Although there was no significant relationship between the intention to use advance directives and the level of death anxiety, they were negatively associated. Therefore, older adults who had lower death anxiety would tend to use advance directives. Conclusion: It would be necessary to screening the level of death anxiety to promote use of the advance directives. In addition, education programs for advance directives would be essential to consider about advance directives for their end-of-life especially for the community-dwelling elderly in Korea.

Regional disparities in healthy eating and nutritional status in South Korea: Korea National Health and Nutrition Examination Survey 2017

  • BACKGROUND/OBJECTIVES: Concerns about regional disparities in heathy eating and nutritional status among South Korean adults are increasing. This study aims to identify the magnitude of regional disparities in diet and nutritional status among Korean adults who completed the 2017 Korea National Health and Nutrition Examination Survey (KNHANES). SUBJECTS/METHODS: The participants were a nationally representative sample of Korean adults aged 19 years and older from the 2017 KNHANES (n = 6,126). We employed the svy commands in STATA to accommodate the complex survey design. The relative concentration index (RCI), absolute concentration index (ACI) and index of disparity were used to measure regional nutritional inequalities. RESULTS: Overweight and obese adults were more prevalent among the poor than among the rich in urban areas (RCI = -0.041; P < 0.05), while overweight and obese adults were more prevalent among the rich than among the poor in rural areas of South Korea (RCI = 0.084; P < 0.05). Economic inequality in fruit and vegetable intake ≥ 500 g per day was greater in rural areas than in urban areas in both relative size (RCI = 0.228 vs. 0.091, difference in equality = 0.137; P < 0.05) and absolute size (ACI = 0.055 vs. 0.023, difference in equality = 0.032; P < 0.05). CONCLUSIONS: This study provides useful information identifying opposite directions in the relative concentration curves between urban and rural areas. Adult overweight/obesity was more prevalent among the poor in urban areas, while adult overweight/obesity was more prevalent among the rich in rural areas. Public health nutrition systems should be implemented to identify nutritional inequalities that should be targeted across regions in South Korea.

지역사회 거주 노인의 건강증진행위에 영향을 미치는 요인 (Factors Influencing on Health Promoting Behavior of Community-dwelling Older Adults)

  • 본 연구는 지역사회 거주하는 65세 이상 노인의 건강증진행위에 영향을 미치는 요인을 확인하기 위해 시도되었으며, Pender의 건강증진모형에 근거한 가설적 모형을 검증하기 위해 Theoretical substruction을 구성하였다. 연구대상자는 G시에 거주하는 65세 이상 노인 199명을 대상으로, 구조화된 설문지를 통해 개인적 특성, 건강정보이해능력, 자기효능감 및 건강증진행위를 조사하였다. 자료 분석은 SPSS 20.0프로그램을 이용하여 t-test, ANOVA, 사후검증으로 Scheffé test를 실시하였고, 건강정보이해능력, 자기효능감 및 건강증진행위 간의 관계를 확인하기 위해 Pearson's correlation coefficients로 분석하였으며, 건강증진행위에 영향을 미치는 요인을 확인하기 위해 위계적 회귀분석을 실시하였다. 연구 결과, 노인의 건강증진행위는 4점 기준으로 평균 2.56±0.50점이었으며, 하위영역 중 대인관계 영역이 가장 높았고, 신체활동 영역이 가장 낮았다. 건강증진행위에 영향을 미치는 요인은 건강정보수집 경로의 수(β=.16, p=.031)와 자기효능감(β=.53, p<.001)인 것으로 확인되었으며, 건강증진행위를 30.8% 설명하였다(F=14.19, p<.001). 본 연구 결과를 통해 노인 대상자의 건강증진행위를 높이기 위해서는 개인의 자기효능감 수준과 건강정보의 특성을 고려한 중재가 필요함을 확인하였다. 추후 노인의 건강증진행위 수준과 다양한 영향 요인을 확인하기 위한 지속적인 연구가 필요할 것으로 생각된다.

The Impact of the Financial Crisis on Lifestyle Health Determinants Among Older Adults Living in the Mediterranean Region: The Multinational MEDIS Study (2005-2015)

  • Objectives: By the end of the 2000s, the economic situation in many European countries started to deteriorate, generating financial uncertainty, social insecurity and worse health status. The aim of the present study was to investigate how the recent financial crisis has affected the lifestyle health determinants and behaviours of older adults living in the Mediterranean islands. Methods: From 2005 to 2015, a population-based, multi-stage convenience sampling method was used to voluntarily enrol 2749 older adults (50% men) from 20 Mediterranean islands and the rural area of the Mani peninsula. Lifestyle status was evaluated as the cumulative score of four components (range, 0 to 6), that is, smoking habits, diet quality (MedDietScore), depression status (Geriatric Depression Scale) and physical activity. Results: Older Mediterranean people enrolled in the study from 2009 onwards showed social isolation and increased smoking, were more prone to depressive symptoms, and adopted less healthy dietary habits, as compared to their counterparts participating earlier in the study (p<0.05), irrespective of age, gender, several clinical characteristics, or socioeconomic status of the participants (an almost 50% adjusted increase in the lifestyle score from before 2009 to after 2009, p<0.001). Conclusions: A shift towards less healthy behaviours was noticeable after the economic crisis had commenced. Public health interventions should focus on older adults, particularly of lower socioeconomic levels, in order to effectively reduce the burden of cardiometabolic disease at the population level.

Regional comparison of dietary intakes and health related behaviors among residents in Asan

  • Inadequate dietary intakes and poor health behaviors are of concern among rural residents in Korea. This study is conducted to compare dietary intakes, dietary diversity score (DDS), mean nutrient adequacy ratio (MAR) and health related behaviors by rural, factory and urban areas in Asan. A total of 930 adults (351 men and 579 women) were interviewed to assess social economic status (SES), health related behaviors and food intakes by a 24-hour recall method. Mean age was 61.5 years with men being older (64.8 years) than women (59.3 years, p<0.001). Men in the factory area were older than rural or urban men while urban women were the youngest. Education and income of urban residents were higher than other area residents. There were more current drinkers in urban area while smoking status was not different by regions. Physical activity was significantly higher in rural or factory areas, whilst urban residents exercised more often (p<0.05). Rural or factory area residents considered themselves less healthy than others while perceived stress was lower than urban residents. Energy intakes were higher in urban residents or in men, however, after SES was controlled, energy intake did not show any differences. Energy-adjusted nutrient intakes were significantly higher in the urban area (p<0.05) for most nutrients except for carbohydrate, niacin, folic acid, vitamin $B_6$, iron and fiber. Sodium intake was higher in factory area than in other areas after SES was controlled. DDS of rural men and MAR of both men and women in the rural area were significantly lower when SES was controlled. In conclusion, dietary intakes, diversity, adequacy and perceived health were poor in the rural area, although other health behaviors such as drinking and perceived stress were better than in the urban area. In order to improve perceived health of rural residents, good nutrition and exercise education programs are recommended.

전라도 구곡순담 장수벨트지역에 거주하는 중노년층의 연령군에 따른 여름철 식품 섭취량과 식품 섭취 다양성 및 식이 패턴 평가 (Assessment of Food Consumption, Dietary Diversity and Dietary Pattern during the Summer in Middle Aged Adults and Older Adults Living in Gugoksoondam Logevity Area, Korea)

  • The purpose of this study was to investigate food intake, dietary diversity and dietary pattern during the summer in middle aged and older adults living in Gugoksundam (Gurye, Goksung, Sunchang and Damyang counties), Jeonla Province located in southern part of Korea and known as one of the representative Korean longevity areas. Food intake and dietary diversity were assessed by using the data from 2 day-24 hour recall of 1,051 subjects (394 males and 711 females) aged 45 years and older (45-93 years in male, 45-105 years in female). The average ages of males and females were 70.7 years and 71.1 years, respectively. Average total daily food intake was significantly decreased with aging in both genders, and the average animal food intake ratio to total food intake was ranged 10~14% in 3 different age groups, 45~64 years, 65~74 years and 75 years and older. The contribution of each food group, in weight, to total food intake showed the descending order of grains, fruits, vegetables and alcohols in males, and grains, fruits, potatoes and meats in females. On daily intake amount of each food item, rice, watermelon, soju, kimchi, and potato were ranked on top 5 in descending order in males, and rice, watermelon, potato, kimchi and ylmukimchi in females. On intake frequency, rice, kimchi, onion, green pepper and potato were ranked on top 5 in descending order in both genders. Dietary variety score (DVS) and dietary diversity score (DDS) for the assessment of dietary diversity and balance were significantly decreased with aging in both genders. In food group intake pattern (DMGFV), 01101 type without consumption of dairy and fruits was the most prevalent, and only 3.6% of male and 3.9% of female subjects showed 11111 type, consumed all the 5 food groups a day. Dietary pattern of subjects was analyzed by cluster analysis with 18 food groups intake. Overall dietary pattern was classified into two clusters, one was more desirable and the other was less desirable. The percentage of subjects with more desirable dietary pattern was about 15% in male and 32% in female. In conclusion, most of our subjects living in Gugoksundam area were taking very simple diet with low amount of dairy products and fruits. These results indicate that nutrition intervention and education for older people living in rural areas should be focused on various food intake including dairy products and fruits.

Cross-sectional study on the prevalence of anemia among rural elderly in Asan

  • The objective of this study was to assess the prevalence and clinical characteristics of anemia and to define the risk factors for anemia in older Koreans in Asan. From January to February 2002, five hundred sixty two community-dwelling older adults aged over 60 years were selected from a cross sectional study. All subjects underwent laboratory tests, which included a complete blood cell count, reticulocyte, liver and renal functional tests, lipid profiles and iron status tests. The median age was 68.6 years (range, 60-92 years). The mean levels of hemoglobin were $14.4{\pm}1.3\;g/dL$ in men and $12.9{\pm}1.0\;g/dL$ in women, and the overall prevalence of anemia was 12.5% in all subjects, 10.8% in men and 13.6% in women. The prevalence of anemia was the lowest among age group of 60-69 (10.0%) followed by 70-79 (15.5%), and the highest among age over 80 (20.7%), but the difference was significant only for men. The age difference was more distinct in men than in women (p<0.05). The mean hemoglobin level was significantly lower in the subjects aged over 80 y than those in the 60-69 y group (p<0.05). According to a logistic regression analysis, lower albumin and higher creatinine levels were identified as independent risk factors of anemia among older adults in Asan. In conclusion, the overall prevalence of anemia in our study group was 12.5% and the highest (20.7%) among those aged over 80 y.
