새만금호의 상괭이 대량 폐사 후 상괭이(Neophocaena asiaeorientalis) 재출현과 분포 경향 (Reappearance and Distribution Tendency of Finless Porpoises Neophocaena asiaeorientalis after their Mass Mortality in the Saemangeum Dyke)

  • A mass mortality of 249 finless porpoises Neophocaena asiaeorientalis occurred in the Saemangeum Dyke in February 2011. It was an extraordinary event, notable due to the death toll and the location of the occurrence, a semi-isolated lake enclosed by a man-made structure. We conducted sighting surveys that consisted of a land-based sighting survey recorded from three different platforms, and a ship-based sighting survey in the lake. The land-based survey was dedicated to clarifying the distribution of finless porpoises and whether they passed through two water gates (Shinsi and Garyek) of the dyke from 2011 to 2013. No finless porpoises were observed in the 2011 or January 2012 surveys. In April 2012, two months and one year after the mass mortality, one finless porpoise, swimming 400 m from the Shinsi water gate, was observed by a land-based survey. The number of observed individuals increased to nine in 2012 and reached 10 by May 2013 at the time of the surveys. Most of the porpoises were detected near the Garyek water gate. The density of the animals was $0.075/km^2$ in 2012 and $0.083/km^2$ in 2013. The density of porpoises was $2.063/km^2$ at the time of the mass mortality.

  • Evaluation in both an art and a science. The art of evaluation involves working with the management to agree upon purpose and users of results, creating design and gathering information that are appropriate for a specific situation and a particular policy making context. The value of evaluating extension p개grams has received a lot of attentions recently, and many extension educations see evaluation as an integral part of their work. The science of evaluation involves determining standards and developing indicatiors, selecting methods appropriate to gather information in a systematic way, analyzing information to assist in determining the value of the program in an objective manner. First, extension specialists have to consider relative merits about methods of gathering evaluation data. Selection of method should be influenced by the type of information desired, time availability, and cost of using the method. Second, good evaluations involve stakeholders at all stages including planning, implementation, and utilization of results. Third, far from being an "add-on ," evaluation begins with the initial planning of an educational program. Fourth, it is important for extension specialists that although evaluation is valuable and essential in any effective program, one of the biggest mistakes of extension program evaluators in to promise results that cannot possibly solve all the problems of project.

기계학습모델을 이용한 저수지 수위 예측 (Reservoir Water Level Forecasting Using Machine Learning Models)

  • This study investigates the efficiencies of machine learning models, including artificial neural network (ANN), generalized regression neural network (GRNN), adaptive neuro-fuzzy inference system (ANFIS) and random forest (RF), for reservoir water level forecasting in the Chungju Dam, South Korea. The models' efficiencies are assessed based on model efficiency indices and graphical comparison. The forecasting results of the models are dependent on lead times and the combination of input variables. For lead time t = 1 day, ANFIS1 and ANN6 models yield superior forecasting results to RF6 and GRNN6 models. For lead time t = 5 days, ANN1 and RF6 models produce better forecasting results than ANFIS1 and GRNN3 models. For lead time t = 10 days, ANN3 and RF1 models perform better than ANFIS3 and GRNN3 models. It is found that ANN model yields the best performance for all lead times, in terms of model efficiency and graphical comparison. These results indicate that the optimal combination of input variables and forecasting models depending on lead times should be applied in reservoir water level forecasting, instead of the single combination of input variables and forecasting models for all lead times.

소규모 농산물 가공사업장 작업자의 직무스트레스 평가 및 근골격계부담작업 유해요인 조사 (The Research of Job Stress and MSDs Symptoms of Small Plants with Agricultural Products)

  • The purpose of this study is to survey job stress and Musculoskeletal Disorder(MSDs) of workers in small plants with agricultural products. The subjects were comprised of 69 workers participated in "Helping the rural women to get the small businesses" project. The questionnaire surveys were Korean Occupational Stress Scale(KOSS), and Legal Risk Assessment of Musculoskeletal Disorders(KOSHA CODE H-30-2003). As a result of the survey, scores of KOSS 8 sub-items except for job demand were similar to that of Korea workers. Prevalence of MSDs symptoms of small plants workers was high percent (44.9%). And the rate according to kinds of product was significant difference at neck (p<0.01), lumbar (p<0.01), and leg (p<0.01). There was no significant difference between view person sign of MSDs and non MSDs according to sex, year, the number of workers, working years, working time per one day, rest time per one day, and subjective working intensity, but there were significant difference between high stress group and low stress group about MSDs at interpersonal conflict(p<0.05). Job stress and MSDs were important factor to workers of small plants producted agricultural products. Therefore, starting a business with small plats, workers consider interpersonal conflict and body part showing MSDs.

농촌지역사회 보건요원의 교육을 통한 주민의 보건복지향상에 관한 사회의학적 연구 (Socio-Medical Approach to the Welfare of Rural Residents Through the Education of Community Health Personnel)

  • In this county, the gap between the urban 'haves' and the rural 'have-nots' continues to be an increasing problem. WHO and UNICEF see primary health care(PHC) as the key to achieving an acceptable level of health throughout the world as a community development. PHC is essential health care made accessible to individuals and families in the community by means acceptable to them. It is the first level of contact of individual, the family, and community with the national health system. It includes at least education on health system. It includes at least education on health problems, promotion of food supply, MCH including family planning, immunization against infectious diseases, control of endemic diseases, treatment of common diseases and injuries, promotion of mental health, and provision of essential drugs. However, of the aboves, education concerning of mental health problems and the methods to identify, prevent, and control them is the principal step of establishment. In Korea, the category of PHC worker includes the physician as public doctor and nurse as primary health care practitioner and community health leader as village health worker. PHC workers of the aboves will thus function best if they are appropriately trained to respond to the health needs of the community. However in this country, since the national PHC service project launched in 1980, the government has not developed and performed appropriate and enough education and training activities. In light of above reasons, several categories of health education activities had been planned and performed being aimed at above specific target groups and the main focus was on the village health workers for about one year from July 1991 to July 1992 in Yeoju Kun of Kyonki Province. At the end of the period, evaluation of education input was carried out to measure the improvement of healthful life of people in terms of awareness, attitude, and practice. The totals of 80 village health workers, 13 public health practitioners and 9 public docters took in the course of health education for a few hours at every month and the evaluation works of educational effect were taken. The results the study were as follows. 1) Number of persons who realized the maxim "health care of the people is a duty of the government" increased after the education course, On the other hand, the rate of satisfaction on the effort of government for health promotion of the people decreased. 2) Public doctors and primary health care practitioners(nurses) liked and enjoyed the education schedule as a meeting of peer group. It provided chances of communication with staffs of Korea University Hospital. It was said that lectures covered great deal of knowledge and technic they urgently needed in the field. 3) After finishing the education course, more of village health workers(VHW) thought they adapted themselves to their roles and functions showing increased number of home visit and contact with primary health care practitioners by month. 4) In case of patient refer, VHW preferred primary health care practitioners to public doctors. 5) Capability of VHWs in most of their functions increased dramatically after when the education course finished except tuberculosis control.

  • Objectives: The purpose of this manuscript was to propose the policy and perspectives of prevention and management for hypertension and diabetes in Korea. Methods: Authors reviewed the chronic disease prevention and management projects and models were executed in Korea until now, and analyzed and evaluated their performances. Results: In the circumstances of Korea, the following several requisites should be improved ; Specific Korean strategy for development and pursuing of national level policy agenda for chronic disease management must be established. There are a need to establish several means of supplementing the weaknesses of the current chronic disease management policies and programs. Firstly, development and distribution of contents of guidelines on the systematic project execution regime (regarding systematization of local community, subjects and contents of the projects) with guarantee for the quality of chronic disease prevention and management are necessary. Secondly, there is a need for development of information system that can lead the chronic disease management programs currently being implemented. Thirdly, there is urgent need to develop resources such as cultivation of manpower and facilities for provision of education and consultation for the patients and holders of risk factors of chronic disease. Fourthly, there is a need for means of securing management system and financial resources for operation of policies and programs. Conclusions: The results can be able to use as a road map, models, and direction and strategies of policies for chronic disease prevention and management of Korea.

일부 도시지역 주민의 약물 장기복용에 관한 사회의학적 연구 (A Study on the Long-Term Use of Drugs Among Some Urban Residents)

  • One of the familiar medical facility that most people reach easily in Korea is the drug store. In Korea, it is possible to purchase all kinds of common drugs without physician's prescriptions, which caused some problems. In other words, such treatment without professional supervision has led to medical, social and economical problems. In view of the above, this study is aimed at revealing the actual status of long-term use of drugs in some urban residents. Long-term use of drugs is operationally defined as using certain drugs at least once a week for more than 3 months. This survey took the residents of Guro 6-Dong where was one of the target areas for Community Health Development Project managed by Korea University as a target population. A sample of 1,517 residents was selected by the multistage sampling method. The interview was conducted on September 21st and 22nd in 1985. The object of this study was to compare the result with that of the rural area which was obtained by the same method, tools and research team, prior to this study in 1984. The results were as follows; 1) The age-standardization of the study showed that 97 per 1,000 urban residents were actually on long-term drug use. The prevalance of long-term use is high in accordance with aging and low with education level. 2) Out of 1,000 urban samples the most popular item involved in the long-term drug use was antipyretic-analgesic-antiinflammatory drug (26), and next in order was vitamin (23), antibiotics (13), digestives (10) and antacids (7). In the rural samples as for compare, that was antipyretic-analgesic-antiinflammatory drug (100), antacids (36), digestives (23), adrenocortical hormones (12) etc. 3) With antipyrctic-analgesic-antiinflammatory drugs, 50% of the urban samples were taking for more than a year, whereas such were 82.7% of the rural samples. Using such a high percentage of antipyretic-analgesic-antiinflamatory drugs in the rural residents is probably due to the high prevalence rate of musculo-skeletal diseases. 4) The urban long-term drug users of antipyretic-analgesic-antiinflammatory drugs were influenced mostly by the mass media (43.6%), next in order was pharmacist (35.9%) and physician (10.3%). Comparing with the result from the rural areas the role of mass media was much more influencial in the urban areas. 60% of them consulted with pharmacists, 14.3% with physicians and 25.7% had no history of consultation in the urban samples. 5) Considering the incidence of knowing the possible side-effects of each drug, 28.2% of the urban residents had no recognition about side-effects prior to use antipyretic-analgesic-antiinflammatory drugs. In the rural residents, 29.67o had no knowledge about the side-effects before using the drug. 6) For the solution of the above problems, it is necessary to limit the advertisement of some drugs by the parmaceutical company. And therapeutic drugs which may bring on side effects in case of long-term use should not be sold at drug stores without physician's prescription.

마을공동체 아카이브의 거버넌스 모델 연구 (The Politics and Governance of 'Maeul' Community Archives in South Korea)

  • 이 논문은 국내 마을공동체 아카이브의 역사적 사례들을 통해 주민 주도형 마을아카이브 거버넌스 구축과 이의 지속성을 확보하기 위한 기록학적 방안을 구체화하는데 연구 목적이 있다. 먼저 역사적으로 한국 사회에서 마을과 마을공동체의 역사적 형성과 쇠락의 과정 속에서 자생적으로 출현했던 성미산과 풀무 마을공동체 아카이브의 출현을 살핀다. 본 연구는 풀뿌리 운동의 일환으로 구축된 이들 '독립' 공동체 아카이브를 마을 공동체의 내적 동인에 의해 추동된 자율적 자생적 산물로 바라본다. 반면 2000년대 접어들면서 이러한 독립 마을 공동체아카이브가 민 관 협업에 의해 새로운 공동체아카이브의 발전 국면에 돌입했다고 본다. 이 글은 서울시가 마을아카이브 시범사업을 통해서 과거 중앙정부 주도의 전시행정적 컬렉션 구축을 넘어서서 마을주민 스스로 아카이브를 구축할 수 있는 역량 강화에 일부 성공했다고 판단한다. 하지만 서울시의 거버넌스 모델은 궁극적으로 정책 사업화하면서 오히려 마을아카이브가 지니는 풀뿌리적 성격과 아카이브의 지속적 발전 개념을 확보하기에 여전히 한계가 있다고 보고 있다. 이 글은 실제 서울시 마을공동체 아카이브 시범사업에 대한 분석을 통해 그 명암을 살펴보고, 마을공동체 아카이브의 발전적 대안을 마련한다.

심뇌혈관질환 고위험군을 위한 지역사회 단계별 교육프로그램 개발 및 효과 평가 (Development and Evaluation of a Community Staged Education Program for the Cardiocerebrovascular Disease High-risk Patients)

  • 본 연구는 대구광역시 심뇌혈관질환 고위험군 등록관리 시범사업단 고혈압 당뇨병 교육정보센터를 방문한 고위험군환자와 교육자의 교육요구도를 반영하여 질환, 영양, 운동 3영역의 기본, 심화, 개별 3단계로 단계별 교육프로그램을 개발하였다. 이 프로그램의 효과평가를 위하여 교육정보센터에 방문한 기본교육군 32명, 단계교육군 37명을 대상으로 교육 전후 임상적 지표, 행동변화단계, 자아효능감을 측정하였고 주요 연구결과는 다음과 같다. 교육요구도조사결과 고위험군과 교육자의 요구도가 높은 소그룹형태, 30분-1시간이내 수업 및 이론 50%와 실습 50%의 구성과 형태로 이루어지며 3개월의 교육과정인 질환, 영양, 운동영역의 단계 교육프로그램을 개발하였다. 교육요구도조사 결과 교육영역별 교육내용은 고위험군과 교육자의 필요도와 지식정도의 통계적으로 유의한 차이에 따라 5가지 범주로 분류하여 프로그램 구성에 적용할 수 있었다. 첫째, 고위험군과 교육자간의 요구도에 유의한 차이가 없는 항목은 기본과정내용으로 반복하게 하였고 둘째, 고위험군의 인지도가 평균점수 이하인 항목은 기본교육과정내용을 조정하여 효과적인 실습방법으로 모든 단계에서 반복할 수 있도록 하였다. 셋째, 고위험군의 지식정도 평균 편차가 큰 항목은 개별교육과정에 포함시켜 문제를 해결할 수 있는 기술습득을 목표로 이루어지도록 하였다. 넷째, 고위험군과 교육자간 지식정도의 유의한 차이가 있는 항목과 다섯째, 고위험군과 교육자간 요구도에서 유의한 차이가 있는 항목은 고위험군의 참여도가 높은 실습 중심의 심화교육으로 구성하며 교육자도 표준 매뉴얼에 따라 교육방법, 교육환경을 일관성 있게 유지하도록 교육훈련을 강화하여 교육자 간 격차를 줄이도록 하였다. 교육자와 고위험군과의 격차를 줄이기 위하여 자가관리 목표, 단기 계획 설정을 고위험군과 교육자 상호약속에 의해서 계획하여 실천동기와 문제해결능력을 향상시키도록 하는 과정을 단계마다 필수적으로 포함시켰다. 교육프로그램의 평가는 기본교육군, 단계교육군을 대상으로 임상적 지표, 행동변화단계 자기 효능감을 측정하여 비교하였다. 임상지표 중 허리둘레, 수축기혈압, 이완기혈압(p<0.05), 자기효능감 중 혈압/혈당조절 불량시 병원방문, 의사의 지시에 따라 약 복용, 식사량 일정유지(p<0.05), 정기적으로 합병증 검사, 정상적인 혈압/혈당을 유지, 허리둘레 유지, 체중 유지(p<0.01), 행동변화단계에서는 싱겁게 먹기(p<0.05)에서 유의적인 차이가 있었다. 환자와 교육자의 교육요구도를 반영하여 개발된 실습 중심의 단계별 교육프로그램은 이론중심의 기본교육프로그램과 비교한 결과 임상지표, 자기효능감, 행동변화단계에서 유의한 차이가 있었다. 대상자 수가 적고 추구관리기간이 짧았으며 임상지표측정이 부족했던 한계점은 있으나 환자가 노인이며 만성질환환자로서 유병기간이 긴 점을 감안하면 이 프로그램을 지속적으로 시행한다면 환자의 심뇌혈관질환 예방에 기여할 수 있을 것이다.

〈사례보고〉 경상남도 지역 간 건강불평등 완화사업: 건강플러스 행복플러스 사업 (〈 Field Action Report 〉 The Strategies to Address Regional Health Inequalities in Gyeongsangnam-Do: Health Plus Happiness Plus Projects)

  • '건강 플러스 행복 플러스 사업'은 경상남도의 높은 표준화사망률을 감소시키고 지역 간 건강불평등을 완화하기 위하여 표준화사망비가 구조적으로 높은 40개 읍면동을 대상으로 수행되는 사업이다. 본 사업은 영국의 헬스 액션 존 사업의 방법론을 기반으로 하여 참여, 파트너쉽, 자원의 집중을 사업의 기본 원칙으로 설정하고 2010년부터 시작되었으며 2017년까지 진행될 예정이다. 총 사업비는 56억원이며, 2010년에는 17개 읍면동, 2011년에는 12개 읍면동, 2012년에는 11개 읍면동에 사업이 개시되었거나 개시될 예정이다. 2010년 사업의 경우, 사업대상지역인 17개 읍면동에 예산이 배정되었고, 관련 수단 및 지역 차원의 사업진행 틀을 만드는 과정을 거쳤다. 또한 사업대상지역의 건강결정요인을 파악하기 위하여 지역 사회조사를 실시하였으며 사업에 대한 교육훈련지원 및 기술지원을 위하여 교육훈련지원단과 사업운영지원단이 설치되었다. 그 결과 사업 인력들과 코디네이터에 대한 교육이 실시되었으며, 사업대상지역에는 사업팀과 사업추진위원회가 결성되었다. 지역사회조사 이후에는 지역사회조사결과 보고회를 통하여 지역의 건강결정요인과 건강수준을 지역주민들과 함께 공유하였으며 최종적으로 사업 관련 주민자치조직인 건강위원회가 17개 읍면동에서 출범하였다. 본 사업은 지역사회 참여 및 파트너쉽의 구축과 관련하여 아직 해결해야 될 과제들이 많으나 원칙에 근거한 사업이 지속적으로 진행될 수 있다면 경상남도의 건강불평등 완화를 통한 표준화사망률 감소 및 사업대상지역 주민들의 안녕 수준 향상에 기여할 수 있을 것으로 판단된다.
