A Case of Amniotic Band Syndrome Associated with Aplasia Cutis of the Scalp (두피 피부 무형성증을 동반한 양막대증후군 1례)

  • The amniotic band syndrome is an uncommon congenital fetal abnormality, presumably due to fetal entanglement in strands of ruptured amniotic sac. The defects caused by this syndrome vary from simple limb defects to major visceral and craniofacial defects. We cared for a newborn infant with this syndrome, who showed constriction rings of the right leg and right axilla, right club foot, thoracic scoliosis, polydactyly, absence of the right thumb and aplasia cutis of the scalp. We report this case with a brief review of the literature.

Effect of SOD on Ultrastructural Changes of Gastric Parietal Cells in the Cisplatin Treated Rats (흰쥐에서 cisplatin에 의한 위벽세포의 미세구조변화에 미치는 SOD의 영향)

  • This study aims to demonstrate the effect of SOD (superoxide dismutase), one of the antioxidant enzymes, on the ultrastructural changes in the parietal cells caused by the administration of cisplatin in the rat. A total of 60 healthy Sprague-Dawley rats weighing about 200 gm were used as experimental animals. Cisplatin (6 mg/kg) was administered intraperitoneally to rats pretreated with 15,000 unit/kg of SOD or rats without the pretreatment. The experimental animals were sacrificed at 6 hours, 12 hours, 24 hours and 3 days after the administration of cisplatin. The results were as follows: 1. SOD alone did not affect the ultrastructural changes in the gastric parietal cells in the rat. 2. Irregular shaped mitochondria, mitochondria with dim cristae, dilated cristae, ruptured outer membrane, electron lucent matrix and degenerative mitochondria were seen in cisplatin treated rat. Whorled membranous body, many lysosomes and large vacuole were observed in the gastric parietal cells in cisplatin treated rat. 3. Mitochondria with dilated cristae and electron lucent matrix and irregular shaped mitochondria were observed in the gastric parietal cells of the cisplatin treated rat with pretreatment of SOD. These results suggest that SOD attenuates the toxic effect of the cisplatin in the gastric parietal cells of the rat.

Severe Hematoma in the Neck Following the Stellate Ganglion Block -A case report- (성상신경절 차단 후 발생한 심한 경부혈종 -증례 보고-)

  • The technique of the stellate ganglion block is widely used as it is relatively simple and safe. But it can cause severe complications because there are major blood vessels and nerves around the stellate ganglion. We practiced CPR because of the respiratory failure caused by severe hematoma in the neck following the stellate ganglion block. A 46-year-old male patient admitted to ENT department because of the both sudden sensorineural hearing loss that happened after URI. He was referred to Pain Clinic for further evaluation and treatment. We decided to block the stellate ganglion. We injected 6ml of 0.5% mepivacaine on both sides of the stellate ganglion. There were no blood aspiration and abnormal vital signs during the 30 minute observation, either. Three hours after he went to the private room, he had pain and edema in his neck, but no respiratory defficulty. But later, respiratory failure was suddenly followed. So we practiced CPR. We confirmed severe hematomas in the neck through CT scanning. Hematomas is removed and the ruptured blood vessels which is supposed to be muscular branch of vertebral artery is ligated under general anesthesia. The patient was discharged from hospital after the treatment of pneumonia and duodenal ulcer as complications. We recommand you to compress the block site more than five minutes and not to prick with the needle several times at one point to prevent the formation of hematomas.

Mechanical Behavior of Potato and Sweet Potato under Impact and Compression Loading (감자와 고구마의 충격 및 압축 특성에 관한 연구)

  • Mechanical properties of potato and sweet potato were measured under impact and compression loading. The test apparatus consisted of disgital storage oscilloscope and simple mechanisms which can apply compression and impact forces to potatoes and sweet potatoes. The mechanical properties could be measured while the tissues were ruptured in a very short period time less than 10 ms by impact loading. Rupture force, energy, and deformation were measured as mechanical properties of potatoes and sweet potatoes under impact and compression loading. Rupture forces under impact and compression loading were in the range of 84.1 to 93.7N and 128.9 to 132.2N for external tissues and 60.1 to 64.8N and 158.9 to 171.1N for internal tissues of potato and sweet potato, respectively. Compression speeds and drop heights for each test were in the range of 1.25 to 62.5mm/min and 8 to 24cm.

Deep Intracerebral Hemorrhage Caused by Rupture of Distal Lenticulostriate Artery Aneurysm : A Report of Two Cases and a Literature Review

  • Intracerebral hemorrhage (ICH) is common among various types of storkes; however, it is rare in young patients and patients who do not have any risk factors. In such cases, ICH is generally caused by vascular malformations, tumors, vasculitis, or drug abuse. Basal ganglia ICH is rarely related with distal lenticulostriate artery (LSA) aneurysm. Since the 1960s, a total of 29 distal LSA aneurysm cases causing ICH have been reported in the English literature. Despite of the small number of cases, various treatment methods have been attempted : surgical clipping, endovascular treatment, conservative treatment, superficial temporal artery-middle cerebral artery anastomosis, and gamma-knife radiosurgery. Here, we report two additional cases and review the literature. Thereupon, we discerned that young patients with deep ICH are in need of conventional cerebral angiography. Moreover, initial conservative treatment with follow-up cerebral angiography might be a good treatment option except for cases with a large amount of hematoma that necessitates emergency evacuation. If the LSA aneurysm still persists or enlarges on follow-up angiography, it should be treated surgically or endovascularly.

Endovascular Treatment of a Ruptured Posterior Inferior Cerebellar Artery Aneurysm during Pregnancy

  • Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is quite rare, however it has a high maternal mortality rate. A pregnant woman in the 16th gestational week was admitted to our hospital with a drowsy level of consciousness. A brain magnetic resonance (MR) image showed hemorrhage on the prepontine cistern, and both sylvian fissures, and MR angiography and cerebral digital subtraction angiography demonstrated an aneurysm at the left posterior inferior cerebellar artery (PICA). We performed endovascular coil embolization attempting to minimize radiation exposure. She was discharged with no neurologic deficit and delivered a healthy baby by cesarean section at the 38th week of gestation. This case study reported the shortest gestational period and this is the first report on an aneurysmal rupture arising from PICA which was treated using an endovascular method. Using an appropriate technique for reduced radiation exposure to the fetus and limited alterations in maternal-fetal physiology, endovascular coil embolization could guarantee good results in treatment of aneurysmal SAH in pregnant women.

Overview of the PCL Reconstruction (후방 십자 인대 손상 치료의 개관(over view))

  • The PCL reconstruction in chronic isolate PCL reconstruction was still controversy. 1) In isolate PCL deficient knee, functionally not so bad as like ACL deficient knee. 2) The result of the PCL reconstruction was not as good as ACL reconstruction. Therefore, isolate PCL injuries has been treated as nonoperatively. Hey Grovere, who was the first to attempt an intra-articular reconstruction of the PCL, utilized the semi-tendinous tendon other static procedures have been described in only a few cases with very limited follow-up. Dynamic procedures utilizing the medial head of the gastrocnemius has been reported by Hugston and Degenhardt, Kennedy and Grainger, and Insall and Hood. These procedures did not improve static stability. Dr Clancy, who was introduce the use of BPTB for the PCL reconstruction transtibial and femoral tunnel. From 1995, untill early 1990 PCL reconstruction was done as tend as placement of the isometric point. Physiometic placement of Anatomical placement of the femoral tunnel in PCL reconstruction were introduced in 1995. Tibial Inlay Technique was reported by Dr Berg in 1995. The main advantage of the tibial Inlay Technique was to avoid fraying of the graft at the posterior tibial tunnel orifice. In complete PCL ruptured and severely posterior unstable knee, dual femoral tunnel technique will be to get better result than one bundle technique. To achieve restoration of normal posterior laxity, it is critical to address the posterior as well as the posterolateral structures. Futher research is necessary to evaluate new surgical approches such as double-bundle reconstructions and tibial inlay techniques as well as improved techniques for capsular and collateral ligament injuries.

Fundamental Study on Geotechnical Properties of Sand-Bentonite Mixtures (모래-벤토나이트 혼합물의 지반 공학적특성에 관한 기초 연구)

  • The study was conducted in order to investigate the basic geotechnical properties of sand-bentonite mixtures with the various bentonite contents. The results obtained are as follows : 1. Optimum moisture content of sand-bentonite mixtures was approximately 17.10~18.52% corresponding to the maximum dry density of 1.58~1 .64gf/$cm^3$. As the bentonite contents and curing peroid increased, both the maximum dry density and optimum moisture content of sand-bentonite mixtures increased. 2. The unconfined compressive strength of sand-bentonite mixtures increased as the increase of bentonite content, but it did not change along the curing period. 3. The sand-bentonite mixtures ruptured at 8~15% of the axial strain and the maxi-mum shearing stress was about O.7Okgf/$cm^2$. 4. According to the increase of bentonite content, the cohesion intercept and internal friction of the sand-bentonite mixtures increased slightly in the shear test, while the cohesion intercept increased largely, and the internal friction angle decreased largely in the triaxial test. 5. Both the initial void ratio and swelling of the sand-bentonite mixtures were very low with respect to the consolidation pressure increase. 6. The swelling and shrinkage of sand-bentonite mixtures increased slightly according to the increment of bentonite content.

Surgical Clipping of Intracranial Aneurysm Regrown after Endovascular Coiling

  • Operative clipping after previous endovascular coiling in an aneurysm is a different problem from primary clipping procedure for neurosurgeons. With the increasing use of coil embolization, neurosurgeons will more and more face the similar situation. We report surgical clipping cases of intracranial aneurysm regrown after endovascular coiling. Three patients with a history of subarachnoid hemorrhage due to ruptured aneurysm underwent endovascular treatment (EVT) with detachable coils. The aneurysms were in the posterior communicating artery, the middle cerebral artery and distal anterior cerebral artery (DACA). Two near-total occlusions and one partial occlusion were achieved by EVT. After several months, angiographic follow-up revealed regrowth of the aneurysm requiring surgical clipping. Here, we report three cases in which surgical clipping was more difficult than a usual clipping procedure performed several months after EVT, because of adhesion and coil bulging into the aneurysmal neck. The difficulty of the treatment of the residual aneurysm after coiling is discussed, as are the surgical complications and limitations of clipping.

Ruptured Total Intrameatal Anterior Inferior Cerebellar Artery Aneurysm

  • Among the distal anterior inferior cerebellar artery (AICA) aneurysms, a unique aneurysm at the meatal loop inside the internal auditory meatus is extremely rare. The authors report a case of surgically treated total intrameatal AICA aneurysm. A 62-year-old female patient presenting with sudden bursting headache and neck pain was transferred to our department. Computed tomography and digital subtraction angiography showed subarachnoid hemorrhage at the basal, prepontine cistern and an aneurysm of the distal anterior inferior cerebellar artery inside the internal auditory meatus. Surgery was performed by retrosigmoid craniotomy with unroofing of the internal auditory meatus. The aneurysm was identified between the seventh and eighth cranial nerve in the meatus and was removed from the canal and clipped with a small straight Sugita clip. After operation the patient experienced transient facial paresis and tinnitus but improved during follow up.
