급성 아킬레스건 파열 - Krackow 봉합술 후 등속성 족저 굴곡력의 분석 - (Acute Achilles Tendon Rupture : - Isokinetic plantarflexion torque evaluation after Krackow suture technique -)

  • Purpose: We try to evaluate the functional outcomes of the ankle with isokinetic plantaflexion torque for acute achilles tendon rupture cases those treated by primary repair with the Krackow suture technique and early rehabilization. Materials and Method: The authors studied retrospectively, 15 patients of acute achilles tendon rupture treated and followed over six months, from July 1997 to May 2001. There were 12 men and 3 women, and mean age was 39.6year. The repair method of ruptured tendon was single or double Krackow suture technique. One week(5days-2weeks) after operation, early ROM with ankle-foot orthosis was started. We used Arner-Lindholm Scale for the clinical evaluation and analyzed patient's satisfaction and subjective strength deficit. We analyzed the dorsiflexion peak torque and plantarflexion peak torque of the ankle statistically with strength test with Cybex dynamometer. Result: In clinical results, we had 11 excellent cases(73%) and 3 good cases(20%). In patients satisfaction degree, 11 excellent cases(66.6%), 3 good cases(20%) and 2 fair cases(13.4%). And in strength deficit, 3 none deficit (20%), 9 minimal deficit(60%). We evaluated the isokinetic plantar flexion torque in 3months or 6months after operation. After 3 months, isokinetic test showed the mean functional deficits, 32% and 25% at $30^{\circ}$ and $120^{\circ}/sec$, in 7 cases(46.6%) of 15 cases respectively. After 6 months, the mean deficits were in 21%, 24% at 30. and $120^{\circ}/sec$, respectively. At 3 and 6 months' follow up, absolute value of isokinetic test showed increase of 25. 31bs to 421bs and 19.61bs to 271bs at $30^{\circ}$ and $120^{\circ}/sec$, respectively. Conclusion: We had good result for acute achilles tendon rupture treated by Krakow suture technique and early range of motion exercise of the ankle. After 6 months, strength deficit was 21% in all of cases but were able to return pre-injured state. This study shows Krakow suture technique was recommended method for primary repair and early rehabilization of achilles tendon ruptue.

  • 최근 화학 산업이 발전함에 따라 고압설비가 널리 이용되고 있다. 이러한 고압 설비는 편리성과 함께 많은 위험을 내포하고 있다. 파열판 안전장치는 고압설비의 안전사고로부터 설비와 생명을 보호하는 장치이다. 파열판 안전장치는 홀더, 파열판, 버큠서포트로 구성되어져 있으며, 다양한 환경 조건에서 부식되거나 파괴되는 단점이 있다. 본 논문에서는 부식 저항성이 우수하여 파열판 안전장치의 소재로 많이 사용되고 있는 STS 316L stainless steel 소재를 이용해서 제작한 파열판 안전장치를 CATIA V5를 이용하여 3차원 모델링을 수행하고, 유한요소해석을 통하여 소재의 두께와 높이에 따라 파열판 안전장치의 파열에 영향을 미치는 요인을 분석하였다.

  • 본 연구에서는 주로 보일러의 과열기 및 재열기등의 관재료로 널리 사용되고 있는 304 스테인레스강에 대하여 실제 고온부식현상을 재현한 부식환경하인 630.deg. C, 690.deg. C 및 750.deg. C 3가지 온도범위에서 크리프시험을 실시하고 고온부식이 크리프파단 특성에 미치는 영향을 부식조건이 없는 고온대기중의 실험결과와 비교 검토하였다.

  • Acute Achilles tendon rupture is a frequent injury during sports and recreational activities. Treatments for Achilles tendon rupture have been controversial in recent decades. Traditionally the surgical treatment had benefit over nonsurgical treatment in terms of low rerupture rate and early functional restoration. Recently, nonsurgical treatment was found to show no statistically significant inferiority in re-rupture rate, functional outcome, and calf strength. Whereas, surgical treatment had some complications including adhesion, nerve injury, and infection. Nonsurgical treatment has been increasing due to functional rehabilitation with early weight bearing and restricted early motion. It focuses more attention on the course of caring for patients with deep discussion. There are open repair and minimally invasive repair in terms of surgical treatment. There are various techniques for minimally invasive repair of Achilles tendon, which has some advantages over the open repair. However, the optimal technique for minimally invasive repair has not been established. The number of suture strands is important regardless of suture technique.

Prediction of the Rupture of Circular Sections of Reinforced Concrete and Fiber Reinforced Concrete

  • As part of this study, has been developed a numerical method which allows to establish abacuses connecting the normal force with bending moment for a circular section and therefore to predict the rupture of this type of section. This may be for reinforced concrete (traditional steel) or concrete reinforced with steel fibers. The numerical simulation was performed in nonlinear elasticity up to exhaustion of the bearing capacity of the section. The rupture modes considered occur by plasticization of the steel or rupture of the concrete (under compressive stresses or tensile stresses). Regarding the fiber-reinforced concrete, the rupture occurs, usually, by tearing of the fibers. The behavior laws of the different materials (concrete and steel) correspond to the real behavior. The influence of several parameters was investigated, namely; diameter of the section, concrete strength, type of steel, percentage of reinforcement and contribution of concrete in tension between two successive cracks of bending. A comparison was made with the behavior of a section considering the conventional diagrams of materials; provided by the BAEL rules. A second comparative study was performed for fibers reinforced section.

블리스터 시험법을 이용한 열증착 금박막의 기계적 성질 측정 (Measurement of Mechanical Properties of a Thermally Evaporated Gold Film Using Blister Test)

  • Mechanical properties, including Young's modulus, residual stress and rupture strength, of a thermally evaporated gold film have been measured form a blister test. In a theoretical study, the priniple of minimum potential energy and that of virtual work have been applied to the pressurized circular membrane problem, and load-deflection relations have been derived for typical membrane deflection mode of spheroidal shape. In an experimental study, circular gold membranes of 4800 A-thickness and 3.5mm diameter were fabricated by the silicon electropolishing technique. Mecahnical properties of the thin gold films were deduced from the load-deflection curves obtained by the blister test, Young's moduli, obtianed from blister test, have been in the range of 45-70 GPa, while those of bulk gold have been in the range of 78-80 GPa. Residual stresses in the evaporated gold films have been measured as 28-110MPa in tension, The rupture strength of the gold film has turned out to be almost equal to that of dental gold alloy (310-380MPa). It has been demonstrated that the present specimen fabrication method and blister test apparatus have been effective for simultaneous measurement of Young's modulus, residual stress and repture strength of thin solid films. Especially, the electropolishing technique employed here has provided a simple and practical way to fabricate thin membranes in a circular or an arbitrary shape, which could not be obtained by the conventional anisotropic silicon mecromachining technique.

연속섬유에 의하여 보강된 철근콘크리트 보의 전단강도 평가 (Evaluation of the Shear Strength of Reinforced Concrete Beams Strengthened with Continuous fiber Reinforced Polymer)

  • 섬유에 의하여 보강된 철근콘크리트 보의 파괴모드는 섬유가 파단하여 전단강도에 도달하는 경우와 섬유가 파단하기 이전에 전단강도에 도달하는 경우로 나누어진다. 이와 같이 섬유가 파단강도에 도달하지 않는 경우에 두 파괴모드의 전단강도는 상이하다. 섬유가 파단하지 않고 전단강도에 도달할 경우에 보의 전단강도를 예측하기 위하여 섬유의 파단강도를 사용할 수 없으므로 기존의 제안식에서는 섬유강도저감계수를 사용하여 섬유보강 보의 전단강도를 예측하였다. 그러나 기존 제안식의 대부분은 실험결과에 근거한 회귀분석을 통하여 일정한 상수 값에 의하여 섬유강도저감계수를 평가하였으며, 섬유의 도달강도와 밀접한 관계가 있는 콘크리트의 압축강도를 고려하지 않고 있다. 이 연구에서는 변형률의 적합조건을 이용한 트러스모델에 근거하여 섬유보강 보의 파괴모드의 경계점을 구별하고, 이를 이용하여 섬유보강 보의 전단강도를 예측하였다.

아킬레스건 파열에 대한 수술 후 조기 재활 치료 (Early Mobilization after Operative Treatment of a ruptured Achilles tendon)

  • Purpose: The purpose of the study is to evaluate the clinical outcomes of acute achilles tendon rupture patients treated by early mobilization after operative repair. Materials and Methods: In this retrospective study, 12 patients of acute Achilles tendon rupture were surgically treated from May 2001 to November 2002, with follow-up period of at least 6 months. There were 10 men and 2 women, and mean age was 35.9 year. The repair method was single Krackow suture technique. Two to three weeks after the operation, early ROM with removable ankle-foot orthosis(AFO) was started. We used Amer-Lindholm scale for the clinical evaluation and analyzed patient's satisfaction and subjective strength deficit. Results: We had 11 excellent results(92%), and 1 good results(8%). One-leg heel raising was possible in all patients. All the patients returned to the preinjury occupations and 7 patients(58%) reached the preoperative level of sports activities. There was no significant difference in ankle ROM and ankle plantar flexion isokinetic strength compared to contralateral side. There was, no reruptures. Conclusion: Early mobilization proved to be a well tolerated, safe, and effective means for the compliant patient.

A Study on the Corrosion and Degradation of Boiler Tubes Steel in Fossil Power Plant

  • It was analyzed the causes of boiler tube rupture due to a degradation and corrosion on the boiler tubes in fossil power plant. The experiments were carried out among samples taken from the operating facilities. The result were analyzed based on experimental results from mechanical strength, microstructure observation, and hardness measurement in order to determine the cause of local rupture on boiler tubes. In general, 2.25Cr-1Mo steel generates carbides, it is coarsened, its ductility and strength abruptly decreased as degradation is in progress, In order to confirm this phenomenon, we observed changes of the mount of Cr and Mo of carbide by carrying out EDX chemical composition analysis. The amount of Cr and Mo in the degraded material or service exposed material gradually increased the amount of Mo but initially they were almost maintained at the same amount. Furthermore, we observed that the carbide become coarsened both in the grain and at the grain boundary. Tensile test was carried out to measure a material hardness and to recognize a drop-off of hardness. Overall result for tensile strength and hardness turned out to be lower than new material and mechanical strength and hardness was degraded as the material degradation was in progress.

철도사면파괴 원인 및 대책공법 적용을 위한 강도정수 결정 (Cause of Rall Road Slope Failure and Determination of Soil Strength for Remedy)

  • 철도사면은 예상치 못한 연약지반의 존재로 인해 붕괴될 수 있다. 이 연구는 철도사면의 파괴원인을 규명하고 대책공법 적용을 위한 강도정수를 결정하는데 그 목적이 있다. 사면파괴의 원인을 파악하기 위하여 시추조사, 콘관입시험, 현장베인시험 등이 수행되었다. 또한 연약지반의 강도정수를 구하기 위해 불교란상태로 시료를 채취하여 실내시험을 수행하였다. 현장원위치시험 및 실내시험 결과 사면 파괴의 원인은 과압밀점토의 점진적 파괴에 따른 연약지반 강도저하영역의 전파에 기인한 것으로 밝혀졌다.
