회전근 개 파열 봉합술에서의 최신 지견 (What's New in Rotator Cuff Repair)

  • 회전근 개 파열의 봉합술은 최근 들어 괄목할만한 발전을 보여주고 있다. 회전근 개 파열의 자연사를 관찰하면 대부분의 경우에서 시간이 지남에 따라 크기가 증가하는 양상이 관찰되었다. 회전근 개 파열의 후기에 오는 지방 침윤 및 광범위 회전근 개 파열 후 발생하는 회전근 개 파열 관절증은 회전근 개 파열의 봉합술에서 예후를 예측하거나 수술 시기를 결정하는데 중요한 요소라 할 수 있다. 회전근 개 파열시 증상 발생 후 3년 뒤에 중등도의 지방 침윤이 발생하는 것으로 관찰되어 그 이전에 회전근 개 파열의 봉합술을 시행하는 것이 추천되고 있으며, 또한 회전근 개 파열 관절증에서도 견봉상완골 간격이 감소하기 전에 봉합술을 하는 것이 권장되고 있다. 회전근 개 파열의 관절경하 봉합술은 크게 일열 봉합술 및 이열 봉합술로 나눌 수 있으며, 시간과 비용 면에서는 전자가 장점이 있으나, 생역학적이 관점에서는 후자가 더 우수하다. 이열 봉합술의 생역학적인 장점을 유지하면서 보다 적은 시간에 회전근 개의 봉합을 가능하게 한 교량형 봉합술이 개발되어 최근 널리 사용되고 있으며, 교량형 봉합술 내에서도 파열의 특성에 따라 몇 가지 다른 술식이 개발되어 사용되고 있다.

Acute Rotator Cuff Tears due to Low Voltage Electrical Injury: A Case Report

  • Since shoulder have a higher proportion of muscle which would have low electrical resistance, there could be more electrical damage to the rotator cuff muscles. We present a patient with acute rotator cuff tear by sudden uncontrolled jerking contractions caused by an electrical shock. A case of 42-year-old man with acute rotator cuff tear due to electrical injury to the shoulder was presented. Magnetic resonance imaging showed a full thickness tear and an undulating appearance of the peripheral end of the torn supraspinatus and infraspinatus muscle, suggesting an acute complete rupture. By arthroscopic surgery, the torn rotator cuff tendons were repaired with a suture bridge technique. At the final follow-up, the patient had a full, pain-free range of motion and had fully recovered shoulder muscle power.

  • Full-thickness rotator cuff tears are relatively uncommon in the young adults. One of the pathogenesis of such tear is thought to be closely related to the specific trauma event. Favorable outcome is expected in young patient rotator cuff tears when it is diagnosed early following prompt surgical repair. However, early detection is sometimes difficult when the acute rotator cuff tear is combined with other injuries especially around the shoulder joints such as ipsilateral humerus fractures. Authors report an uncommon case of acute traumatic rotator cuff tear accompanied by the midhumerus shaft fracture in young adult.

  • Although the prevalence of rotator cuff tears is dependent on the size, 11% to 94% of patients experience retear or healing failure after rotator cuff repair. Treatment of patients with failed rotator cuff repair ranges widely, from conservative treatment to arthroplasty. This review article attempts to summarize the most recent and relevant surgical options for failed rotator cuff repair patients, and the outcomes of each treatment, except arthroplasty.

Arthroscopic Partial Repair of Massive Contracted Rotator Cuff Tears

  • Typically, massive rotator cuff tears have stiff and retracted tendon with poor muscle quality, in such cases orthopaedic surgeons are confronted with big challenging to restore the cuff to its native footprint. Furthermore, even with some restoration of the footprint, it is related with a high re-tear rate due to less tension free repair and less tendon coverage. In this tough circumstance, the partial repair has yielded satisfactory outcomes at relatively short follow-up by re-creating the transverse force couple of the rotator cuff. Through this partial repair, the massive rotator cuff tear can be converted to the "functional rotator cuff tear" and provide improvement in pain and functional outcomes in patient's shoulder.

  • 대부분의 환자는 관절경적 회전근 개 봉합술 후 통증이 완화되고 기능적 호전을 보이지만 일부 환자는 수술 후에도 증상이 지속된다. 재파열은 생물학적, 기술적, 외상적 요인이 복합적으로 작용하여 발생한다. 회전근 개 봉합술 후 재파열이 지속되는 통증의 원인일 수도 원인이 아닐 수도 있다. 따라서 이런 환자들의 평가 및 치료는 난해하여 철저한 문진, 이학적 검사, 적절한 영상 검사를 통해서 통증의 원인에 대한 분석이 이루어져야 한다. 재봉합술의 시행 여부는 환자의 나이, 기능적 요구도 회전근 개의 상태, 수술 전 관절 운동 범위, 삼각근의 상태, 관절과 상완 관절의 관절염의 유무에 따라 결정되어야 한다. 성공적인 재봉합술은 술기뿐만 아니라 수술 전 환자 교육 또한 중요하다.

  • Rotator cuff tear is a common cause of shoulder pain and disability in adults. Due to the various nature of progression of rotator cuff tears and the complex biomechanics of the shoulder joint, repair and treatment of large-to-massive tears are challenging for many surgeons. Despite the recent popularity of reverse total shoulder arthroplasty as a treatment option for large-to-massive irreparable rotator cuff tears, biological and mechanical repair augmentation has also shown promise as a viable treatment option. The purpose of this study was to briefly summarize and review current studies on the assessment and arthroscopic treatment of large-to-massive rotator cuff tears, whether repairable or irreparable, to aid in developing a consensus on future treatment directions.

Prevalence of rotator cuff diseases in adults older than 40 years in or near Chuncheon city, Korea

  • Background: To determine the prevalence of rotator cuff diseases in a population older than 40 years in or nearby Chuncheon city, Republic of Korea. Methods: Sixty shoulders of 30 people older than 40 years who participated in a health lecture were examined for free by an orthopedic surgeon. Visual analog scale of pain and American Shoulder and Elbow Surgeons scores were assigned, and routine physical examination was performed. Ultrasonography was performed on the shoulder. Results: On ultrasonographic examination, there were one shoulder with full thickness rotator cuff tear, 20 of 60 (33%) with partial thickness rotator cuff tear, five of 60 (8%) with calcific tendinitis, one of 60 (2%) with tear of the long head of the biceps, and five of 60 (8%) with tendinitis of the long head of the biceps. Participants older than 60 years showed significantly high proportions of lesion of the long head of the biceps and rotator cuff diseases (P=0.019 and P=0.015, respectively). Participants who performed physical labor had high proportions of rotator cuff tear and rotator cuff disease (P=0.001 and P<0.001, respectively). Conclusions: Rotator cuff diseases showed a high prevalence in aged persons and resulted in a decrease in shoulder function.

  • 목적: 회전근 개의 질환은 견관절 질환 중 가장 흔하며, 퇴행성 변화와 관련한 회전근 개 파열은 임상에서 가장 흔히 경험하는 질환이다. 대상 및 방법: 회전근 개의 정상적인 생물학적 특성을 이해하는 것은 회전근 개의 퇴행성 변화를 이해하고, 병태생리를 이해하는 것에 도움이 되며 나아가서 새로운 치료방법이나 예방법을 발전시키는 기초 지식으로 중요하다 하겠다. 결과 및 결론: 이에 저자들은 회전근 개의 생물학적 정상소견과, 회전근 개 파열의 병리를 설명하는 이론에 대한 문헌 고찰과 최근 활발히 연구되고 있는 회전근 개 건섬유모세포의 세포자멸 기전등에 관한 문헌고찰 내용을 보고하고자 한다.

Early and Delayed Postoperative Rehabilitation after Arthroscopic Rotator Cuff Repair: A Comparative Study of Clinical Outcomes

  • Background: The duration of immobilization after arthroscopic rotator cuff repair and the optimal time to commence rehabilitation are still the subject of ongoing debates. This study was undertaken to evaluate the functional outcome and rotator cuff healing status after arthroscopic rotator cuff repair by comparing early and delayed rehabilitation. Methods: Totally, 76 patients with small, medium, and large sized rotator cuff tears underwent arthroscopic repair using the suturebridge technique. In early rehabilitation group, 38 patients commenced passive range of motion at postoperative day 2 whereas 38 patients assigned to the delayed rehabilitation group commenced passive range of motion at postoperative week 3. At the end of the study period, clinical and functional evaluations (Constant score, the University of California, Los Angeles [UCLA] shoulder score) were carried out, subsequent to measuring the range of motion, visual analogue scale for pain, and isokinetic dynamometer test. Rotator cuff healing was confirmed by magnetic resonance imaging at least 6 months after surgery. Results: No significant difference was obtained in range of motion and visual analogue scale between both groups. Functional outcomes showed similar improvements in the Constant score (early: 67.0-88.0; delayed: 66.9-91.0; p<0.001) and the UCLA shoulder score (early: 20.3-32.3; delayed: 20.4-32.4; p<0.001). Furthermore, rotator cuff healing showed no significant differences between the groups (range, 6-15 months; average, 10.4 months). Conclusions: Delayed passive rehabilitation does not bring about superior outcomes. Therefore, early rehabilitation would be useful to help patients resume their daily lives.
