  • Purpose: In this research, the properties of the shoulder joint were measured through eccentric resistive exercise with the patients who have rotator cuff tear of shoulder joints as the targets. Methods: 23 patients who have pain due to the rupture of rotator cuff on shoulder joints were selected and divided into Exercise group (12) and Control group (11). Two groups executed exercise for 30 minutes 3 days a week. Exercise group had executed eccentric resistive exercise, and the Control group executed Complex exercise. Results: In external rotation $180^{\circ}/s$, $240^{\circ}/s$, the Exercise group showed to be an average of 8% higher than the Control group. For Internal rotation $90^{\circ}/s$, $180^{\circ}/s$, $240^{\circ}/s$ the Exercise group showed to be an average of 30% higher that the Control group. Conclusion: To lessen the rupture of rotator cuff on shoulder joints, muscles strengthening is very important across various methods of eccentric exercise programs which are external/internal rotators of the shoulder joint needed for throwing. This is effective in preventing injury and improving rotation.

  • With increased development of patch graft, the number of repair using patch graft in massive rotator cuff tear has increased. Understanding characteristics of various patch graft might be helpful for selection of type of patch, and to improve the outcomes for the treatment of massive rotator cuff tears using patch grafts. Therefore, this paper reviews various studies dealing with clinical outcomes of rotator cuff repair using diverse patch grafts in massive rotator cuff tears.

  • Objectives: To lay the foundation for future research into Korean Medicine treatment for Rotator Cuff repair surgery patients by analyzing Korean Medical and Western Medical service utilization and treatment duration. Methods: Data sampling was performed on 2015's HIRA patient data (confidence level of 97%) to analyze patients' Korean Medical and Western Medical service usage tendency. Sampled groups were divided into two groups: i) Patients who completed their treatment within five months after the rotator cuff surgery (termination group), ii) Patients who were treated for more than five months after the surgery (continuation group). Then the patients' Korean Medical and Western Medical service usage tendency was investigated and information of these patients are arranged. Results: Out of 1,453,486 patients who were gathered for sampling, 2,461 patients in total had gone through rotator cuff repair surgery. The termination group had 517 patients and the continuation group had 541 patients. The proportion of patients who visited a Korean Medicine clinic was lower in the termination group than the continuation group. Conclusion: The continuation group received more treatments (both in Western Medicine and Korean Medicine) and spent more on medical expenses compared to the termination group. Further research is highly recommended for more efficient Western Medicine and Korean Medicine treatments and reduced medical expenditure.

  • This scoping review was conducted to identify the trends of cost-effectiveness studies on treatment of Korean medicine after rotator cuff surgery and indicators used in those studies. In researching stage, "patients after rotator cuff surgery", "cost-effectiveness evaluation" and "treatment of Korean medicine" were set as the keywords in "Pubmed", "Google scholar" and Korean databases (RISS, OASIS, KCI). There were no studies containing all three keywords, and after searching separately for "cost-effectiveness evaluation" and "treatment of Korean medicine", each 7 studies were reviewed. The indicators used in the study were identified such as study design, characteristics of subjects, outcome measurement and others. This study identified the need for cost-effectiveness evaluation on the treatment of Korean medicine after rotator cuff surgery. We expect that those indicators will be used to implement other studies afterwards.

  • Purpose: The study aims to determine the effect of a breathing exercise on shoulder pain, range of motion (ROM), and forward head posture in patients with rotator cuff repair. Methods: In total, 25 patients with rotator cuff repair were included in this study. The experimental group (n = 13) underwent a breathing exercise, while the control group (n = 12) received traditional physical therapy. The visual analogue scales (VASs) for pain, flexion and abduction ROM, and the craniovertebral angle (CVA) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with the pre-intervention period, and independent t-tests were used to analyze differences in dependent variables between the two groups. Results: After the two-week intervention, the experimental group experienced a significantly decreased VAS (p < 0.05) and significantly increased ROM and CVA (p < 0.05), while the control group experienced a significantly decreased VAS (p < 0.05). Further, the experimental group that underwent the breathing exercise showed greater improvements in flexion and abduction ROM and in the CVA than the control group (p < 0.05). Conclusion: The results suggested that a breathing exercise can reduce shoulder pain and enhance ROM and posture in patients with rotator cuff repair.

  • Purpose: The purpose of this study is to analyze the correlation between the degree of torn rotator cuff as recorded by MRI and degenerative change of acromion and greater tuberosity of humerus determined by simple radiographs. Materials and Methods: Of the 518 cases included in this study, a group of 234 cases had a chronic rotator cuff tear and a control group of 284 cases had an intact rotator cuff in shoulder MRI. The degree of degenerative changes was classified according to the length of spur and morphological change of acromion and greater tuberosity through the true anteroposterior simple radiograph in supraspinatus outlet view. The degree of tear (partial-thickness or full-thickness tear) and the size of complete rotator cuff tear were analyzed according to the MRI findings of shoulder. The authors also evaluated the correlation between the degree and size of torn rotator cuff and the degenerative change of acromion and greater tuberosity. Results: There were significant differences in the size and extent of torn rotator cuff according to the age, sex and degenerative change of acromion and greater tuberosity of humerus (p<0.001). More degenerative changes of acromion and greater tuberosity in simple shoulder radiographs showed the increased degree and size of torn rotator cuff (p<0.001). In addition, the higher degree and larger size of torn rotator cuff were noted in older age subjects (p<0.001) and in the female group (p<0.001). Conclusion: More degenerative changes of acromion and greater tuberosity in simple shoulder radiographs showed the increased degree and size of torn rotator cuff. Therefore, if a high degree of degenerative change of the acromion and greater tuberosity on simple radiograph is noted, the possibility of degenerative rotator cuff tear should be considered. In addition, the age and sex could be associative factors for larger size of torn rotator cuff.

  • With the advancement of shoulder arthroscopy, use of biodegradable suture anchors in the surgical repair of rotator cuff tears has increased. Because of the radiolucency of these anchors, radiography is not appropriate for early detection of anchor failure. Ultrasonography is an advantageous modality in visualizing biodegradable, radiolucent anchors on a real-time basis without risk of radiation exposure. We report on two cases of displacement of a biodegradable suture anchor diagnosed on ultrasonography during the postoperative follow- up, which has not been previously reported. Because this displacement could be missed in the postoperative follow up ultrasonography, we describe the ultrasonographic features of the displaced biodegradable anchors. Surgeons and radiologists should pay special attention to the possibility of displacement of the suture anchor in patients who underwent rotator cuff repairs using suture anchors.
